Accuracy Urinalysis Discrimination Method based on high performance CNN (고성능 CNN 기반 정밀 요검사 판별 기법)

  • There are three types of urinalysis: physical test, chemical test, and microscopic test. Among these, the chemical urinalysis is an easily accessible method of the general public to compare the chemical reaction of urinalysis strip with a standard colorimetric table by sight or purchase the portable urinalysis machine separately. Currently, with the popularization of smartphone, research on the urinalysis service using smartphone is increasing. The urinalysis screening application is one of the urinalysis services using a smartphone. However, the RGB values of the urinalysis pad taken by the urinalysis screening application have large deviations due to the effect of lighting. Deviation of RGB value debases the accuracy of urinalysis discrimination. Therefore, in this paper, the accuracy of urinaylsis pad image discrimination is improved through CNN after classifying urinalysis strips taken by the urinalysis screening application based on smartphone by urinalysis pad items. Urinalysis strip was taken from various backgrounds to generate CNN image, and urinalysis discrimination was analyzed using the ResNet-50 CNN model.

  • Routine urinalysis is a simple, economical, and useful test that facilitates the detection of urinary system diseases and monitoring of renal disease progression. It consists of 4 parts of specimen evaluation, gross examination, a dipstick urinalysis, and a sediment microscopic urinalysis. Urine specimens should first be evaluated in terms of acceptability, and thereafter, the gross appearance is examined for color, turbidity, and odor. In particular, a dipstick urinalysis is an easy and rapid test that provides information on the multiple physicochemical properties of the urine sample. Moreover, although a sediment microscopic urinalysis is time-consuming, it provides information on the cells, microorganisms, casts, and crystals. In the present report, the clinical significance of the routine urinalysis and the problems concerning interpretation are summarized.

  • The urinalysis, which is universally accessible to the general public, has disadvantages of being less objective using sight and purchasing a separate portable urinalysis machine. However, due to the high penetration rate and performance improvement of smartphone created by the development of mobile communication technology, research on urinalysis services using smartphone has been conducted. In this paper, a new urinalysis screening application based on smartphone was developed by supplementing the limitations of the previously studied urinalysis services. The key technology of the application is urinalysis recognition algorithm and urinalysis pad color determination algorithm through image-processing and contour detection. In order to confirm the performance of the developed application, urinalysis strip was photographed and analyzed from various backgrounds and angles.

  • This study focuses on the contents about the Urinalysis which is the most representative one of the Tibetan medicine. Also, this study compares it with the Urinalysis of Traditional Korean medicine. This study focuses on the simple history, the theoretical system, the diagnosis and the urinalysis mainly based on the documents related to the Tibetan medicine. Also, through the comparison with the contents of textbook of Traditional Korean Medicine and the urinalysis shown in the Dongeuibogam(東醫寶鑑) which is the representative Korean medical book, the difference between Tibetan Medicine and Traditional Korean Medicine will be observed. The contents related about the urinalysis of the Tibetan medicine are more specific than those of Traditional Korean Medicine. By observing the color, scent, vapor and sediment of one's urine together with periodic changes more thoroughly, it is possible to find out the cause of a certain disease. However, Traditional Korean Medicine examines the medical conditions mainly based on the state of discharge and fever, showing a clear difference. The Tibetan medicine has an extremely specific type of urinalysis in comparison with that of Traditional Korean Medicine. It is the only characteristic of the Tibet medicine, which cannot be found in any other traditional medicine. By applying the viewpoint of the doctor who diagnoses the patient by measuring his or her pulse, it is possible to make the diagnosing process more specific and accurate. It is expected that the follow-up study will be continuously executed with the introduction of the system for the urinalysis of the Tibetan medicine to Traditional Korean medicine.

Diagnostic Value of Dipstick Urinalysis (Leukocyte Esterase and Nitrite) as a Screening Test for Urinary Tract Infection in Acute Febrile Children (급성 발열 환아에서 요로감염 선별검사로서 Dipstick 요분석(Leukocyte esterase와 Nitrite)의 진단적 가치)

  • Purpose : To evaluate the diagnostic value of dipstick urinalysis($Multistix^{(R)}$) as a screening test for urinary tract infection(UTI) in acute febrile children. Methods : We compared dipstick uinalysis (leukocyte estease(LE) and nitrite) with microscopic urinalysis (WBC and bacteria) and urine culture in 80 children with UTI and 605 children without UTI, who were admitted due to acute fever at EWHA Womans University Hospital between Oct. 1994 and Feb. 1996. Results : 1) In dipstick urinalysis, LE and nitrite were positive in 38.7%, and 13.8% of 80 children with UTI and in 2.1%. and 1.2% of 605 children without UTI. 2) In microscopic urinalysis, WBC and bacteria were positive in 31.2%, and 17.5% of 80 children with UTI and in 4.1%, and 0.6% of 605 children without UTI. 3) Positive rate of LE on dipstick urinalysis was 38.7%, which was significantly higher than 31.5% of WBC on microscopic urinalysis (P<0.05). Positive rate of nitrite was not significantly different to that of bacteria. 4) Sensitivity, specificity, positive predictive value, negative predictive value of dipstick urinalysis were 43.7%, 98.3%, 63.6%, 92.9% which were not significant different to 40.0%, 99.0%, 54.0%, 92.3% of microscopic urinalysis. The sensitivity of combined dipstick and microscopic urinalysis were still low(50%). 5) Additional positive rate of microscopic urinalysis to negative dipstick urinalysis was 6.3%(5/80) in 80 children with UTI. Conclusion : The sensitivity of dipstick urinalysis was low and additional value of microscopic urinalysis was minimal in febrile UTI. So urine culture is mandatory in acute febrile children even though dipstick and microscopic urinalysis are negative.

Ultrasonographic and Clinical Findings in Cats with Feline Lower Urinary Tract Disease

  • Urethral obstruction is a life-threatening feline lower urinary tract disease (FLUTD). The rate of recurring urethral obstruction was 14.8-58.1% after the first occurrence. Ultrasonographic findings associated with reobstruction had been rarely reported although ultrasonography was a valuable technique for diagnosing urinary bladder calculi and distinguishing different FLUTD causes. This retrospective study aims to describe the ultrasonographic findings, urinalysis, and serum chemistry profile in cats with FLUTD and determine the associations of reobstruction with ultrasonographic findings, urinalysis, and serum chemistry profile. The present study included 141 cats that were followed up for more than 1 year. The ultrasonographic criteria included the presence of cystolithiasis, urine echogenicity, sediment, suspended linear strand, pericystic effusion, hyperechoic pericystic fat, ureteral dilation, pyelectasia, and perirenal effusion. The urinalysis criteria included hematuria, urine-specific gravity, pH, sediment, and proteinuria. The most common ultrasonographic findings in cats with FLUTD were echogenic urine and sediment. However, this study did not find an association between reobstruction and ultrasonographic findings, urinalysis, and serum chemistry profiles. Thus, an ultrasonographic examination may be insufficient to predict the risk of reobstruction although it is a useful modality for diagnosing FLUTD and making treatment direction.

  • Improving validity and reliability is the important components of clinical laboratory tests. And the quality control of the test should be started with the accurate collection of specimen. Urinalysis is one of the useful and common tests in diseases diagnosis and determining the process of medical treatment. Since urinalysis is requested routinely in hospital setting, the importance of the quality control for urine specimen is often ignored. To improve the validity of urinalysis, a clinical trial was done on the method of collecting urine specimen. The result was as follows : 1. The rate of presumtive UTI(urinary tract infection) was decreased in 21.6% with experiment method for collecting urine specimen. 2. The rate of presumtive UTI in female patients was decreased in 43.2% with the experiment method. 3. The rate of negative urine culture was decreased in 6.6% with the experiment method.

  • The spectroscopic characteristic of 9 items in the urinalysis strip are measured to develop the urine analysis system. From experimental results, we determined the wavelength range which could accurately distinguish the degrees of 9 test items. Simulation is performed to distinguish the primary color reaction in the urinalysis strips using reflectance of urine strips, luminous intensity of LEDs, and spectral sensitivity of photodiodes. The simulation results agree well with experimental results by using UV-Visible spectrophotometer.

  • To evaluate the effects on health by exposure to mixed organic solvents, the author analyzed some labortory findings of blood (hemoglobin, hematocrit RBC, WBC, SGOT, SGPT) and urine (protein, blood, sugar) that were obtained from 722 workers (male:446, female:276) who worked in plywood, painting and shoes factories in Pusan area from January to December, 1990. The result were as follows ; 1. The mean values of each hematologic findings were all within normal limit but there were significant differences at hematocrit, RBC, WBC, SGOT in male and at WBC, SGPT in female at the comparison of the mean values and the distribution according to the normal criteria by type of work. 2. There were no differences in urinalysis(protein, blood, sugar) by type of work. 3. In stepwise multiple regression, analysis of hematologic findings on age, working duration and type of work, regression coefficients on age and type of work were relatively high in male and female sub1, respectively. As a result of this study, it is considered that the measurement of WBC with morphology is significant in hematologic findings and instead of spot urine and urinstix, 24 hours urine and quantitative analysis is required in urinalysis in screening test for healthy status of workers who deal with the organic solvents.

The Difference in Circadian Rhythm and Level of Fatigue Between ER and General Ward Nurse (응급실과 일반병동 간호사의 밤번 근무시 Circadian rhythm과 피로도 비교)

  • The purpose of this study is to find out the difference in circadian rhythm and level of fatigue between ER and General Ward Nurse. This study was done through analyzing body temperature, heart rate and major electrolytes such as $Na^+$ and $K^+$ concentration on urinalysis, which are related to factors of circadian rhythm and level of fatigue. The subjects, 17 nurses from Emergency Room and 25 nurses from general ward, were selected from a General Hospital in Taegu City on August 8 to 23, 1998. And the data were analyzed by proportion, t-test, chi-square and Pearson's Correlation, ANCOVA, using SAS program. The results of this study were shown as follows: First, The difference of Maximal and minimal heart rate was significant between ER group and general ward group. However, body temperature difference wasn't notable. Second, Urine $Na^+$ concentration on urinalysis was lower and $K^+$ concentration on urinalysis was higher in ER group than general ward group significantly. Third, Fatigue level was higher in ER group than general ward group significantly. Fourth, through urine concentration, significant relations in circadian rhythm of ER nurse group and general ward group and their fatigue level were found. Urine $Na^+$ concentration on urinalysis of ER nurse was significant related with their fatigue level. In conclusion, the effect of fatigue on circadian rhythm is greater in ER group than general ward group during night shift. This study could be a help in improvement of nurse's health and understanding the effect of fatigue on health.
