A Study on the Nursing Organizational Change and Outcome in a General Hospital (일 종합병원 간호부서의 조직구조 재설계 전후 조직성과 비교)

  • Purpose: The purpose of this study was to confirm the change of nursing organizational characteristics and to define the difference of organizational outcome by measuring the organizational commitment and job satisfaction perceived by nursing staffs and patient satisfaction before and after organizational redesign. Method: The structured questionnaires of which contained organizational characteristics, organizational commitment and job satisfaction were developed from previous published studies after modification by researcher. The questionnaires were distributed to 1,200 and 679 nurses before and after nursing organizational change. patient satisfaction were measured before and after organizational change by direct interview with structured questionnaire from 1,566 and 1,291 patients. Results: 1. The organizational characteristics were significantly increased and the leadership of team leader and head nurse was significantly increased among the factors of organizational characteristics. 2. Organizational commitment was showed decreased tendency, but the difference was not significant. Job satisfaction was increased significantly and administration, autonomy, and job tasks were increased significantly in the factors of job satisfaction. Patient satisfaction was also increased significantly. 3. There was significant positive correlation between organizational commitment and job satisfaction and also job satisfaction and nursing organizational characteristics. All factors of organizational characteristics and satisfaction in present working unit were major factors to influence organizational commitment and explained 43.8% in a significant level by multiple regression analysis. The major factor which affect job satisfaction were the factors of nursing organizational characteristic except 'interaction with team leader' and satisfaction in present working unit among general characteristics and these factors explained 58.8% of job satisfaction. Conclusion: In conclusion, the nursing organizational change was very effective to bring about desirable change in nursing organizational characteristics and also to improve job satisfaction and patient satisfaction. Therefore the nursing productivity and comparability would be increased when the nursing organizational change is accomplished by the way of improving the factors of organizational characteristics and it would be followed by the promotion of job satisfaction and organizational commitment.

Perception of Clinical Ladder System, Professional Self-Concept, Self-Efficacy, and Organizational Commitment by the Level of Clinical ladder among Intensive Care Unit Nurses (중환자실 간호사의 경력개발제도에 따른 경력개발제도의 인식, 전문직 자아개념, 자기효능감, 조직몰입)

  • Purpose : This study aimed to verify the effectiveness of the clinical ladder system (CLS) by identifying the perception of the CLS, professional self-concept, self-efficacy, and organizational commitment among intensive care unit (ICU) nurses. Methods : Data were collected through a questionnaire survey of 173 nurses working in 11 ICUs at a tertiary hospital in Seoul. Results : The perception of the CLS, self-efficacy, and organizational commitment showed significant difference according to the CLS level. As a result of the partial Spearman correlation analysis after adjusting for demographic difference according to CLS (age, marital status, education, and clinical career), CLS positively correlated with the perception of the CLS (r=.16, p=.045), professional self-concept (r=.24, p=.001), and self-efficacy (r=.21, p=.007). On the contrary, organizational commitment negatively correlated with the perception of the CLS (r=-.43, p<.001) and self-efficacy (r=-.32, p<.001). Conclusion : The CLS is effective for professional self-concept and self-efficacy. However, specialists 2 nurses, who had high professional self-concept and self-efficacy, showed low level of organizational commitment. Thus, it is necessary to examine the factors of work burden and career plateau that are imposed after promotion and improve experience and planning according to the growth needs. Moreover, regular evaluation of the CLS will require hospital support to ensure that ICU nurses appreciate and actively participate in the support.

A Study on the Relationship between Nursing Organizational Culture of Military Hospital and Organizational Performance (군병원 간호조직문화와 조직성과에 관한 연구)

  • Purpose: This research was, by investigating the relations between the cultural types of a nursing organization in military hospital and performance, to ascertain the cultural types to be able to improve performance. Method: The date were gathered from 255 nurse officers who were in 19 military hospitals by using the self-report type of questionnaire. The period of data collection was from April 20, 2003 to July 15, 2003. For this research, the following tools were used: the tools for measuring the organizational characteristics, organizational culture, and job satisfaction, the tool for measuring organizational commitment. For data analysis the SPSS Win 12.0 program was used. Result: 1) Most of the cultural types of a nursing organization in military hospital is Relation-oriented. 2) In the relation between general the characteristics of subject and the organizational cultural type, there was a difference in the innovation-oriented, relation-oriented, hierarchy-oriented, and task-oriented culture according to nurse officers careers, hospital types, year in hospital, marital status, and unit(p<0.05). 3) In the relation between general characteristics of subject and organizational performance, there was a difference in the job satisfaction, affective commitment, transactional commitment, and normative commitment according to nurse officers careers, education, madrigal status and unit(p<0.05). 4) In the relationship between the of a military hospital were correlated with the type of each culture(p=0.00), 5) In the relation between the organizational culture type of military hospital and its performance, there was a positive correlation among job satisfaction and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with job satisfaction(p<0.05). There was a positive correlation among affective commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with affective commitment(p<0.05). There was no culture type significantly related to continuance commitment and there was a weak positive correlation among normative commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture. The types to have an influence upon nurse officers' job satisfaction were relation-oriented culture and innovation-oriented culture(p=0.00). And relation-oriented culture and innovation-oriented culture were major variances for affective commitment and only relation-oriented culture was influential variance for normative commitment(p=0.00). Conclusion: The organizational culture type was found which had an influence upon nurse officers' job satisfaction and organizational commitment. These result are very significant in having showed the persons in charge of nursing administration a basic data for creation of an effective organizational culture.

  • This paper describes the problem of the shortange-economic-scheduling for unit commitment and load dispatching in thermal power system. For economic operation of thermal system, the optimum time of startup and shoutdown of the generating unit must be determined so as to minimize the sum of generating and starting fuel cost over a given period. The above problems are analyzed for the purpose of the application of Dynamic Programing Method. Also the technique of Dynamic Programming is applied to the problems. For the illustative purpose, a case study was made on a model system composed of eight units and the computing time was about 190 seconds by IBM 360-40 system. Therefore, one can utilize this suggested method on any of the practical power systems.

  • This paper proposes a unit commitment scheduling method based on hybrid genetic algorithm(GA). When the systems are scaled up, conventional genetic algorithms suffer from computational time limitations because of the growth of the search space. So greatly reduce the search space of the GA and to efficiently deal with the constraints of the problem, priority list unit ordering scheme are incorporated as the initial solution and the minimum up and down time constraints of the units are included. The violations of other constraints are handled by integrating penalty factors. To show the effectiveness of the proposed method. test results for system of 10 units is compared with results obtained using other methods.

Organizational Commitment of Hospital Employees -Testing a Causal Model in Korean Hospitals- (병원근무자의 직장애착에 관한 연구 -한 인과모형의 검증을 중심으로-)

  • A causal model of organizational commitment on the basis of Western literature was tested with a sample of 1,164 employees from two university hospitals in Korea. The model contains three groups of determinants : environmental variables(job opportunity, spouse support, and parent support), psychological variables(met expectations, work involvement, positive affectivity, and negative affectivity), and structural variables(job autonomy, work unit control, routinization, supervisor support, coworker support, role ambiguity, role conflict, workload, resource inadequacy, distributive justice, promotional chances, job security, job hazarda, and pay). The data were colleted with questionnaires and analyzed with the LISREL maximum likelihood method. It is found that (1) the following variables, listed in order of size, have significant total effects on organizational commitment : job satisfaction, met expectations, supervisor support, job security, routinization, job opportunity, negative affectivity, work involvement, distributive justice, and promotional opportunity, (2) the model explains fifty-nine percent of the variance in organizational commitment, and (3) the link with expectancy theory is justified by the results for met expectations. Two conclusions can be drawn from these findings. First, the model of organizational commitment appears to be generalizable to Korean hospitals. Second, the model of organizational commitment should include such theoretical variables as environmental, psychological, and structural factors.

The Relationship among Nurse-Doctor Collaboration, Job Autonomy and Organizational Commitment (간호사-의사 협력, 직무자율성과 조직몰입의 관계)

  • Purpose: This study aimed to investigate and analyze the state of the relationship among nurse-doctor collaboration, job autonomy and organizational commitment. Method: The 304 participants were obtained who were working at a General ward, Intensive care unit and Operation room in three university hospitals located in Seoul and Kyunggi-do. The data were collected using a structured questionnaire from March 2d to April 10th, 2009. The collected data were analyzed with t-test, ANOVA, Scheff$\acute{e}$ test and Pearson's correlation on SPSS Win 16.0. Result: There was a significant relationship among nurse-doctor collaboration, job autonomy and organizational commitment. The level of appointment, clinical experience and current hospital experience of nurses affected significantly nurse-doctor collaboration, job autonomy and organizational commitment. The age of nurse had the relation nurse-doctor collaboration and organizational commitment. The relationship between the nurse's working area and job autonomy had positive correlation. Conclusions: The findings of study suggest that the program enhancing the collaborated relationship between nurses and doctors is important to improve nurse's job autonomy and organizational commitment under the situation of citizen's demanding more advanced medical service.

Job Satisfaction and Organizational Commitment of Medical Insurance Review Nurses (보험심사간호사의 직무만족과 직장애착에 관한 연구)

  • This study purports to Investigate the determinant of job satisfaction and the organizational commitment of medical insurance review nurses working at Korean hospitals. The independent variables contain three groups of determinants: organizational characteristics variables(job autonomy, work unit control, role variety, role ambiguity, role conflict, workload, resource inadequacy, coworker support, supervisor support, distributive justice, promotional chances, job security, and job hazard), environmental variables(job opportunity, spouse support, and parent support), and psychological variables (met expectation, work involvement, positive affectivity, and negative affectivity). The sample used in this study consisted of 445 medical insurance review nurses from 89 hospitals nationwide. Data were collected with self-administered questionnaires and analyzed using multiple regression analysis. The results of the study are as follows : 1) the following variables, listed in order of size, have significant effects on job satisfaction : role ambiguity(-), distributive justice(+), work involvement(+), role variety(+), met expectation(+), negative affectivity(-), job autonomy(+), and positive affectivity(+). 2) the following variables, listed in order of size, have significant effects on organizational commitment: met expectation(+), work involvement(.+), distributive justice(+), job security(+), role variety(+), positive affectivity(+), negative affectivity(-), resource inadequacy(+), and tenure(-). 3) the variance of job satisfaction and organizational commitment explained by the variables used in the study are 30.0% and 39.1% respectively. 4) In comparison to the results of other studies on the determinants of job satisfaction and organizational commitment of clinical nursing staff working at hospitals, the results of this study indicate that three variables of distributive justice, work involvement, and role variety are especially important for improving the level of job satisfaction and organizational commitment of medical insurance review nurses.

Prediction Factors on the Organizational Commitment in Registered Nurses (간호사의 조직몰입 예측요인)

  • Purpose: This research has been conducted in order to confirm the major factors that prediction organizational commitment in registered nurses. Method: The subjects were 350 registered nurses from 3 hospitals in Seoul. The sample for data collection consisted of 329 useable questionnaires (94% overall return rate) for 2 weeks. The Instrument tools utilized in this study were organizational commitment scale, empowerment scale, job stress scale and job satisfaction scale and thoroughly modified to verify validity and reliability. The collected data have been analyzed using SPSS 11.0 program. Three outliers which were bigger than 3 in absolute value were found, so after taking them off, Multiple Regression was used for further analysis. Result: The major factors that prediction organizational commitment in registered nurses were job satisfaction, empowerment, age and unit experience, which explained 51.9% of organizational commitment. Conclusion: It has been confirmed that the regression equation model of this research may serve as a organizational commitment prediction factors in Registered Nurses.

Role Clarity and Organizational Commitment in Food Manufacturing and Distribution Firms: The Mediating Role of Creativity (식품 제조 및 유통 기업의 역할명확성이 조직몰입에 미치는 영향: 창의성의 매개효과)

  • Purpose - Although ensuring that all the organizational members know their roles exactly is an important part of managerial work, few studies address the role clarity in food manufacturing and distribution firms. Role clarity refers to the extent to which individuals clearly understand the tasks, duties, responsibilities, and expectations of their work roles (Hinkin & Schriesheim, 2008). The purpose of this study is to examine the effect of the role clarity on organizational commitment and the mediating role of creativity in the role clarity-organizational commitment relationship. Research design, data, and methodology - Data were collected from 297 employees in food manufacturing and distribution firms of Korea by self-evaluations questionnaires. The unit of the analysis for testing is an individual. We proposed four hypotheses: (1) is about the relationship between role clarity and organizational commitment, (2) is about the relationship between role clarity and creativity, (3) is about the relationship between creativity and organizational commitment, and (4) is about the mediating role of creativity. We conducted the hierarchical regression analysis for testing the main and mediating effects. Results - The results of the study indicated that the role clarity was positively associated with organizational commitment and creativity. Results also indicated that the creativity was positively associated with organizational commitment. Especially, the impact of the role clarity on organizational commitment was mediated by creativity. Conclusions - An important contribution of this study is extending prior theory on the relationship between role clarity and organizational commitment by shedding new light on assumptions regarding the mediating effect of creativity. The results demonstrate that the role clarity has an indirect effect as well as a direct effects on organizational commitment. Our research suggests important implications for organizations that pursue to stimulate their employees' creativity, emphasizing the importance of the role clarity. This study has several potential limitations. Maybe the most serious limitation is its cross-sectional study design. Another limitation is the use of employees' self-reports to measure the variables. In the future, researchers should employ a longitudinal study design and use diverse sources of data.
