음악 감상이 만성기 뇌졸중 환자의 편측무시에 미치는 영향 : 단일대상 연구 (The Effect of Listening to Music for the Reduction of Unilateral Neglect in Chronic Stroke: A Single Subject Study)

  • 목적 : 본 연구의 목적은 편측무시 현상이 나타나는 뇌졸중 환자에게 본인이 선호하는 음악을 감상하게 하여, 편측무시 현상이 지속적으로 감소되는지를 알아보기 위한 것이다. 연구방법 : 본 연구는 편측무시 현상이 나타나는 한명의 대상에 대해 A-B 단일설계를 사용하여 2013년 6월에 4주간 주 3회씩 총 12회기를 실시하였다. 편측무시 현상은 선 이등분 검사와 캐서린 버지고 척도(Catherine Bergego Scale; CBS)를 사용하여 결과를 측정하였다. 결과 : 선 이등분 검사에서는 평균오차 범위가 기초선에서는 19.00%, 실험처치에서 9.67%로 감소되었다. 캐서린 버지고 검사는 총점이 기초선에서 17점, 실험처치에서 15점으로 약간의 감소를 보였다. 결론 : 본 연구의 결과를 볼 때, 음악 감상이 편측무시 현상의 감소에 영향을 미친다고 볼 수 있다.

A damage model formulation: unilateral effect and RC structures analysis

  • This work deals with a damage model formulation taking into account the unilateral effect of the mechanical behaviour of brittle materials such as concrete. The material is assumed as an initial elastic isotropic medium presenting anisotropy, permanent strains and bimodularity induced by damage evolution. Two damage tensors governing the stiffness in tension or compression regimes are introduced. A new damage tensor in tension regimes is proposed in order to model the diffuse damage originated in prevails compression regimes. Accordingly with micromechanical theory, the constitutive model is validate when dealing with unilateral effect of brittle materials, Finally, the proposed model is applied in the analyses of reinforced concrete framed structures submitted to reversal loading. The numerical results have shown the good performance of the modelling and its potentialities to simulate practical problems in structural engineering.

Angora 토끼에 있어서 과배란처리후 편측난소척출 수정란의 생존율 및 난소 Steroid 홀몬수준에 미치는 영향 (Effect of Unilateral Ovariectomy on Survival Rate of Embryo and Ovarian Steroid Hormone Levels in the Superovulated Angora Rabbits)

  • This study was conducted to investigate the effect of unilateral ovariectomy on the embryo survival and changes of plasma progesterone and estradiol concentration. Mature Angora rabbits were divided into 2 groups (Superovulation and unilateral ovariectomy after superovulation). Unilateral ovariectomied rabilts were subdivided into two groups according to the time of ovariectomy (24 hours and 96 hours after mating). The results obtained were summarized as follows; 1. After unilateral ovariectomy, survival rate and collecting rate of embryos recovered from contralateral and ipsilateral oviduct and uterus were not different, and were lower than intact rabbits. 2. Plasma progesterone concentration at 93, 99, 102 and 114 hours after HCG injection in superovulated rabbits were 12.9 $\pm$ 0.5, 34.8 $\pm$ 5.1, 12.2 $\pm$ 2.7 and 43.4 $\pm$ 5.8ng/ml, respectively. Mean progesterone concentrations were significantly higher at 99 and 114 hours than at 93 and 102 hours (p <0.05). But plasma estradiol concentration was not different. 3. Plasma progesterone concentration in unilateral ovariectomized rabbits was somewhat decreased after unilateral ovariectomy. Plasma estradiol concentration was not different.

  • Unilateral Fin-Line 구조와 I-평면 구조를 Rayleigh-Ritt의 변분법으로 해석하는 방법을 제시하고, Cohn의 여파기 설계 이론에 의해 대역통과 멸파기를 설계하는 CAD프로그램을 개각했다. 관내 재역폭이 5%∼24.6%인 Unilateral Fin-Line 여파기를 제작하여 수험과 이론이 일치함을보였다. 시험 결과, 삽입손실이 0.17∼0.2545의 저손실 여파특성을 얻었으며 중심주파교에 대한 오차는 0.2%이 내였다.

Effect of Unilateral Pneumonectomy on the Secretory Function of Type II Pneumocyte and Compensatory Growing Pattern of the Residual Lung in Growing Rabbits

  • At the fifth day after right lung pneumonectomy in New-Zealand white rabbits $(0.8{\sim}1.1\;kg\;B.W.)$, phospholipid and protein concentration in the left lung lavage fluid were measured for clarification of the effect of unilateral pneumonectomy on the secretory function of the type II pneumocytes in growing rabbits. In an attempt to evaluate the effect of unilateral pneumonectomy on the compensatory growth of the residual lung, left lung weight and left lung weight-body weight ratio and DNA concentration, RNA/DNA and total DNA content in the left lung tissue were measured in pneumonectomized and in sham operated control rabbits. The lung weight of pneumonectomized rabbit was approximately two times heavier than that of the control rabbits. DNA concentration and RNA/DNA of the lung tissue were not changed but total DNA content was increased significantly. Phospholipid concentration in the lung lavage fluid of the pneumonectomized rabbits was over two times higher than that of control rabbits. from these experimental results, It is concluded that unilateral pneumonectomy in growing rabbits might cause to increase the secretion of pulmonary surfactant from type II pneumocyte of the residual lung. The cellular hyperplasia seems to be the primary response of the compensatory growing lung in unilateral pneumonectomized growing rabbits.

Metabolic Abnormalities in Idiopathic Parkinson's Diseases with Unilateral Symptoms

  • Authors investigated whether there is a lateral effect of $^1$H MRS observable metabolite ratios between the symptomatic and the nonsymptomatic side in the early stage of Parkinson's disease with unilateral symptom. Localized in vivo $^1$H MR spectroscopy (MRS) was used to measure the metabolite levels in the symptomatic and the nonsymptomatic sides of the lentiform nucleus in Parkinson"s disease with unilateral symptom (N=25). The metabolite ratios of NAA/Cr and Cho/Cr in the symptomatic side were compared with those in the nonsymptomatic side. Significant metabolic lateral effect of NAA/(Cho+Cr) ratio was established between the symptomatic and the nonsymptomatic side of lentiform nucleus in Parkinson's disease with unilateral symptom (p<0.05). The ratio of NAA/(Cho+Cr) homolateral to the sympatomatic side of the patient is also lower than that of the control (P<0.05). On the basis of NAA/Cr ratios of lentiform nucleus between the symptomatic and the nonsymptomatic side, the present $^1$H MRS study shows a significant neuronal laterality in Parkinson's disease with unilateral symptom. In vivo $^1$H MRS may provide a diagnostic marker for neuronal dysfunction in Parkinson's disease with unilateral symptom.

편측무시 유무에 따른 거울치료가 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과 비교 (Effect of Upper Extremity Function and Activities of Daily Living of Mirror Therapy on Patients Who Have a Stroke, with or without Unilateral Neglect)

  • 목적 : 본 연구에서는 뇌졸중 환자 중에서 편측무시가 있는 군과 없는 군 각각에 대해 4주간의 거울치료를 적용하여 그 효과를 알아보고자 하였다. 또한 거울치료를 통하여 상지의 기능 향상과 일상생활동작에 어떠한 효과가 있는지에 대해서도 알아보고자 한다. 연구방법 : 창원시 C병원에 입원하여 작업치료를 받는 성인 편마비 환자로써 편측무시가 있는 환자 10명, 편측무시가 없는 환자 10명, 총20명을 대상으로 실시하였고 치료사와 1:1로 30분씩 주5회 총 4주간 걸쳐 거울치료 프로그램을 실시하였다. 편측무시 유무를 확인하고 거울치료 중재 전, 후 비교를 위해 알버트 검사를 사용하였다. 상지기능평가로써 Fugl-Meyer motor function Assessment(FMA), Box & Block Test(BBT)을 사용하였으며 일상생활활동평가는 Functional Independence Measure(FIM)를 사용하였다. 결과 : 거울치료는 편측무시를 가진 뇌졸중 환자에게 효과가 있었다(p<.05). 또한 뇌졸중환자의 편측무시 유무에 관계없이 상지기능과 일상생활활동에 효과가 있었다(p<.05). 결론 : 거울치료가 뇌졸중 환자의 마비측 상지기능과 일상생활활동 능력을 향상시킬 뿐만 아니라 편측무시 증상을 가지고 있는 뇌졸중 환자에게서도 편측무시 감소를 보였다. 결론적으로 거울치료는 편측무시 감소를 보일 뿐만 아니라 뇌졸중 환자의 기능적인 회복과 일상생활활동 수행능력 향상을 위한 치료방법으로써 임상에서 유용하게 적용될 수 있을 것이라 사료된다.

Modeling of unilateral effect in brittle materials by a mesoscopic scale approach

  • This work deals with unilateral effect of quasi-brittle materials, such as concrete. For this propose, a two-dimensional meso-scale model is presented. The material is considered as a three-phase material consisting of interface zone, matrix and inclusions - each constituent modeled by an appropriate constitutive model. The Representative Volume Element (RVE) consists of inclusions idealized as circular shapes randomly placed into the specimen. The interface zone is modeled by means of cohesive contact finite elements developed here in order to capture the effects of phase debonding and interface crack closure/opening. As an initial approximation, the inclusion is modeled as linear elastic as well as the matrix. Our main goal here is to show a computational homogenization-based approach as an alternative to complex macroscopic constitutive models for the mechanical behavior of the quasi-brittle materials using a finite element procedure within a purely kinematical multi-scale framework. A set of numerical examples, involving the microcracking processes, is provided. It illustrates the performance of the proposed model. In summary, the proposed homogenization-based model is found to be a suitable tool for the identification of macroscopic mechanical behavior of quasi-brittle materials dealing with unilateral effect.

Immediate Effect of Fabric Ankle Foot Orthosis on Balance in Children With Unilateral Cerebral Palsy

  • Balance control deficits have been indicated to be a primary problem among cerebral palsy (CP) patients. Fabric ankle foot orthosis (AFO) can allow more efficient balance control by facilitating proprioception. The purpose of this study was to investigate the immediate effect of fabric AFO on balance, compared to a barefoot condition in children with unilateral CP. Twelve children with unilateral CP participated in this study. Their balance ability was evaluated using pediatric balance scale and bubble test. Both pediatric balance scale and bubble test showed significant improvement with the use of the fabirc AFO (p<.05). The fabric AFO could improve functional balance ability, and promote better balance among children with unilateral CP. We demonstrated that fabric AFO contributed to improving balance among children with unilateral CP, classified as Gross Motor Function Classification System level I and II. Consequently, fabric AFO might be an assistive device leading to the improvement of balance instead of the typical AFOs.
