The Stereomicroscope and SPM Study on the Marginal Change of Porcelain Crown in Various Repeated Instrumentations for Periodontal Therapy (치주처치를 위한 기구의 반복 사용시 도재관 변연부 변화에 관한 입체 및 주사탐침현미경적 연구)

  • Instrumentation for periodontal therapy may induce marginal damage which increases plaque accumulation and result in periodontal disease. But there have not been many reports of instrumentations on the artificial crown so far. Therefore this study is conducted to evaluate the effects of various repeated instrumentations on the porcelain crown marginal portion. Of the 10 extracted periodontally diseased maxillary first premolars, were 12 proximal surface used in this study. The finishing line of the preparation was placed on the root surface below CEJ and then the crown was cast and cemented in usual manner. Every 4 surfaces of the 3 instruments-curet, ultrasonic scaler, and ultrasonic curet- is used. and four samples used in each instruments. The relevant procedures and measurements were repeated 3 times in each surfaces. Marginal gap is measured by the microscope and surface roughness, Scannig Probe Microscope. Measurements are made at 5 points in each surfaces, making 20 points in each instrument. The results evaluated statistically were as follows 1. As instrumentation was repeated, both marginal gap and roughness were increased in all group 2. In the hand curet, marginal gap was increased every instrumentation and roughness was increased after second. 3. In the ultrasonic scaler, both marginal gap and roughness were increased every instrumentation. 4. In the ultrasonic curet, marginal gap was increased after third instrumentation and roughness was increased after second. 5. Marginal gap and roughness used by the ultrasonic curet were lower than the others and no difference was seen between the hand curet and ultrasonic scaler From the results of this study, the ultrasonic curet was useful in some aspect, but careful instrumentation was needed. Furthermore it was important to minimize the instrumentation through complete periodontal therapy before setting and adequate plaque control.

Clinical Features and Survival Analysis of Very Young (Age<35) Breast Cancer Patients

  • Objectives: To compare the clinicalpathological features and prognosis between premenopausal breast cancer patients aged of <35 and ${\geq}35$ years old. Methods: The clinical data and survival status of 1498 cases premenopausal operable breast cancer treated in our hospital from 2002.1 to 2004. 12 were collected, 118 cases were aged <35. They were divided into 4 groups: Luminal A, Luminal B, HER2-positive, Triple-negative. The disease free survival (DFS) and overall survival (OS) were identified. Results: The 5-year DFS and OS rates were significantly lower in age<35 than in $age{\geq}35$ patients. In the Luminal B, HER2-positive, Triple-negative group, the 5-year recurrence risk was higher in age<35 than in $age{\geq}35$ patients, and age<35 patients' 5-year death risk was higher only in Luminal B, Triple-negative group. Regardless of whether lymph node involved, age<35 patients had a bad prognosis in both DFS and OS. Conclusions: Compared with premenopausal age ${\geq}35$ breast cancer, age<35 patients had a worse outcome.

A Study on the Actual Output and Thermal Effect in Tissue Mimicking Phantom by the Material of the Ultrasonic Transducer (초음파트랜스듀서의 재질에 따른 실출력과 인체모사조직의 온열효과에 관한 연구)

  • PURPOSE: In this study investigated the thermal effect in tissue mimicking phantom by the material of the ultrasonic transducer in low intensity sonication. METHODS: The material of the ultrasonic transducer was made of ceramic, stainless steel, aluminum. Korea Testing Laboratory was measured of the three kinds of materials the total output of the ultrasonic transducer. Each material was measured core temperature and the actual output depending on the type of transducer. Agarose tissue mimicking phantom and silicone tissue mimicking phantom was made. Transducers made of three kinds of materials were emitted in the phantom. It is shown as a graph about time and temperature and the surface temperature rising speed and deep temperature rise rate was investigated. RESULTS: Ceramic transducers were highest output. Higher than the stainless steel transducer, aluminum had the lowest total output. Deep temperature was the highest in the ceramic transducer, and the surface temperature was the highest in the stainless steel transducer. Thermal images of ceramic transducer showed that a valid output is formed deeper wider than the metal. CONCLUSION: Ceramic transducer is confirmed the excellence than the metal transducer in deep thermal effect and the actual output of the ultrasound.

  • Objectives The purpose of this study is to investigate pain reduction and treatment satisfaction of patients with rib fracrture treated with Soyeom pharmacopuncture therapy under ultrasonic guided - thoracic paravertebral space (TPVS), and also compare to the patients with non-guided pharmacoacupuncture. Methods We studied medical records of 6 patients hospitalized with rib fractures were analyzed retrospectively at department of Korean Medicine Rehabilitation from March 1, 2019 to May 31, 2019, The 3 patients had performed the treatment of Soyeom pharmacopuncture therapy on TPVS under ultrasonic guidance with Korean Medicine treatment (such as herbal medicine, acupuncture, moxabustion, etc) and the other 3 patients had performed the treatment of Soyeom pharmacopuncture therapy on TPVS through non-guided. We used numeric rating scale to assess improvements in pain and the five-point likert scale was used to assess treatment satisfaction. Statistical analysis was carried out using the IBM SPSS Statistics 24 program. Results The purpose of this study is to investigate pain reduction and treatment satisfaction of patients with rib fracrture treated with Soyeom pharmacopuncture therapy under ultrasonic guided - TPVS, and also compare to the patients with non-guided pharmacoacupuncture. Conclusions After analyzing 6 studies, we could imply that the treatment of Soyeom pharmacopuncture therapy on TPVS under ultrasonic guidance could be more effective on pain reduction and improvement of treatment satisfaction with rib fracture compared to those who were treated with non-guided pharmacoacupuncture.

The Effect of Ozonized Water Irrigation in the Circuits of Professional Ultrasonic Scalers for Causal Therapy of Stage I Periodontitis: A Randomized Clinical Study

  • Background: Periodontitis is a chronic inflammatory condition associated with dysbiosis of the oral microbiota. The aim of the present clinical study was to explore the adjunctive effect of ozonized water irrigation in the circuits of ultrasonic scalers for the full-mouth decontamination of patients with periodontitis Stage I or II. Methods: The study was a randomized, single-blinded, parallel-group clinical trial. The test group (n=25) was treated with ultrasonic scalers irrigated with ozonized water, whereas the control group (n=25) received normal tap water irrigation within the ultrasonic scalers used during the professional mechanical debridement. Full mouth plaque score, bleeding score, probing pocket depth, and the gingival index were evaluated at baseline, two, and 4 weeks after treatment. The pain perceived and dental anxiety were also assessed after treatment by means of the visual analog scale (VAS). Results: All periodontal parameters resulted in significant improvement for both study groups. The effect of the treatment group on the gingival index was significant, in particular, patients in the test group experienced a greater reduction in this score. No significant differences could be observed with regards to the average probing depth, full mouth plaque index and bleeding score. Patients treated with ozonized water running in the circuits of ultrasonic scalers displayed also lower scores for pain and dental anxiety. Conclusion: The present study showed a significant clinical effect on gingival inflammation attributable to adjunctive ozone irrigation during nonsurgical periodontal therapy. Further studies, including patients with severe periodontitis and greater sample sizes, are recommended to test the clinical effect of ozonized water in the circuits of ultrasonic scalers.

  • ■ Objectives The purpose of the this study was to investigate organization change(pennation angle) of lower extremity muscle in elderly with knee osteoarthritis through ultrasonic image. Also, we examined organizational change of muscle was by influenced after total knee replacement(TKR). ■ Methods Rectus femoris image was taken at 50% of the distance between the anterior superior iliac spine and the superior border of the patella. Vastus medialis image was taken at the most distal insertion level of vastus medialis on the medial border of the patella. measurements included the most distal insertion level of the vastus medialis on the medial border of the patella. ■ Results Change of pennation angle in rectus femoris showed decrease from 5.11° in preoperation to 4.29° in 1 week after operation. Change of pennation angle in vastus medialis showed decrease from 6.94° in preoperation to 5.76° in 1 week after operation. ■ Conclusion We consider decrease of pennation angle after total knee replacement in this study was casued by organizational change of muscle and this pennation angle was influenced force created by muscle. Therefore, we thinks that this pennation angle through ultrasonic image will be available to plane individual muscle exercise program and to confirm muscle mass by muscle strength training.

Partial Pancreatectomy Using Ultrasonic Scalpel for Exocrine Pancreatic Ductal Papillary Carcinoma in a Cat

  • This report presents a rare case of feline exocrine pancreatic ductal carcinoma including treatment and prognosis. A 4.6-year-old castrated male American Shorthair cat, weighing 5 kg was presented with intermittent vomiting, diarrhea and loss of appetite. Through physical examination and radiography, a cranial abdominal mass was identified. Exploratory laparotomy was carried out. The largest mass was connected to the left lobe of the pancreas, and there were several small, nodular masses scattered along the abdominal lining. The pancreatic mass was removed using an ultrasonic scalpel, and the patient recovered favorably. Histopathologically, the resected lesion was diagnosed as an exocrine pancreatic ductal papillary carcinoma. The patient was maintained with conservative therapy and euthanized on post-operative day 262. Partial pancreatectomy using an ultrasonic scalpel was accomplished safely without evidence of pancreatic leakage.

The Effects of Abdominal Draw-in Maneuver With Shoulder Isometric Contractions on Abdominal Muscles Thickness in Healthy Person (견관절 등척성 수축을 동반한 복부 드로우-인 기법이 젊은 성인의 복근 두께에 미치는 효과)

  • This study was conducted in order to examine whether abdominal draw-in maneuver (ADIM) with isometric shoulder flexion, extension, adduction, and abduction selectively contracted deep abdominal muscles. This study's subjects were 13 males 17 females. In order to evaluate the comparison of effects of ADIM and ADIM with isometric shoulder flexion, extension, adduction and abduction, measurements were made on transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) using a real-time ultrasonic diagnostic imaging system. Each position was repetitively measured three times with a real-time ultrasonic diagnostic imaging system and their mean values were used for analysis. The ADIM with isometric shoulder flexion, extension, adduction and abduction significantly increased the thickness of TrA relative to the ADIM only (p<.05). The ADIM with isometric shoulder abduction significantly increased the thickness of IO compared to the ADIM only (p<.05). The ADIM with isometric shoulder extension and abduction significantly decreased the thickness of EO compared to the ADIM only and the ADIM with isometric shoulder extension significantly decreased the thickness of EO relative to the ADIM with isometric shoulder adduction (p<.05). ADIM with isometric shoulder abduction is an effective method to selectively strengthen deep abdominal muscles and therefore may be employed as an intervention for trunk stabilization.

Effects of Repeated Instrumentation for Periodontal Therapy on the Marginal Portion of Artificial Crown (치주처치를 위한 기구들의 반복조작이 인공치관 변연부에 미치는 영향)

  • The aim of periodontal therapy is a removal of a bacterial plaque butthe instrumentation for plaque control has two nature : removal of a bacterial plaque and increase of surface roughness. Complication of instrumentation is enable to damage to the root surface and artificial crown. Therefore this study was conducted to evaluate the effects of repeated instrumentation on the marginal portion of artificial crown. Fifteen proximal surfaces of ten extracted periodontally diseased maxillary first molars were used. The finish line was placed on the root surface, and then the crown was casted and cemented in usual manner. Three kinds of instruments: hand curet, ultrasonic scaler, and ultrasonic curet were used. After instrumentation, final polishing was done with rubber cup and pumice. And surface changes were evaluated by stereomicroscope and scannig probe microscope. Roughness was increased after instrumentation in all groups, and was decreased after polishing except ultrasonic scaler group. Roughness in the ultrasonic scaler group was lower than others, and roughness after polishing in the hand curet group was lower than others. These results indicate that polishing procedure is recommended, because periodontalinstruments increase the surface roughness and induce the irreversible damage to the marginal portion.
