  • Painful, ulcerative lesions of various systemic disease can affect the oral mucosa membrane at first. If you don't consider the skin lesion, followed or accompanied by oral mucosa, you are likely to fail in differential diagnosis. In this cases, we introduced erythema multiforme and Stevens-Johnson syndrome(SJS) patients with painful, ulcerative lesions on oral mucosa and skin. Also we review oral mucosal diseases come with the skin lesions.

  • Erythema multiforme(EM), a blistering and ulcerative inflammatory disorder, affect skins or mucosa, and is thought to be triggered mainly by preceding infection, such as Herpes simplex virus, or exposure to drugs and medication, particular antibiotics or analgesics. Symptoms include typical cutaneous target lesions on skins and in case of oral manifestation, erosive and ulcerative lesions on lips, buccal mucosa, and tongue are known to occur, which needs differential diagnosis with other intraoral lesions. In this case, EM assumed that it is occurred by giving Trichomonas infection or Metronidazole in oral region is introduced with a review of diagnosis and treatment of EM.

  • Echinostomiasis is an endemic intestinal trematodiasis of humans in Korea We observed a human case of Echinostomn honense infection who had ulcerations on the duodenal mucosa. A 55-year old man living in Hamyang-gun, Kyongnam, complained of epigastric pain with hematemesis In April 1994. Endoscopy revealed lesions of early gastric cancer and duodenal ulcerations. A penetrating parasite into the duodenal mucosa was picked out, and identified as E. honense. As the patient was treated 10 praziquantel 10 mg/kg single dose,3 more E. hofene and 7 Metogonimw worms were recovered. This case demonstrates that echinostomiasis causes gross ulcerations in the duodenum.

  • Purpose: Pyoderma gangrenosum is a rare inflammatory, reactive dermatosis marked by painful cutaneous ulcer. The causes of pyoderma gangrenosum remain unclear. Gastrointestinal, hematological, rheumatological, and immmune disorders may be associated with pyoderma gangrenosum. The appearance of this disease may range from mild skin ulcers to life-threatening conditions. Generalized multiple ulcerative pyoderma gangrenosum is very rare. Here we report our experience with a case of multiple ulcerative pyoderma gangrenosum accompanied by ulcerative colitis. Methods: A 67-year-old man had cutaneous ulcers at multiple sites including the scalp, face, chest, abdomen, hands, and buttocks. He also developed gastrointestinal symptoms such as intermittent dyspepsia and bloody excrement. Debridement and irritation aggravated the disease progress. We gave a diagnosis of pyoderma gangrenosum with ulcerative colitis based on the clinical appearance and biopsy. The patient was treated with systemic intravenous steroid therapies and careful wound cares. Ulcers of the scalp and buttocks were treated with split thickness skin grafts. Results: Most of the multiple cutaneous ulcers were treated by systemic intravenous steroid therapies and wound cares. The rest of the ulcers were treated with skin grafts. Systemic intravenous steroid therapy was used to treat the ulcerative colitis. Conclusion: Generalized multiple ulcerative pyoderma gangrenosum is very rare. Without making an accurate diagnosis, hasty surgical treatments could aggravate the progression of the disease. Additionally, care should be taken to systemically treat underlying disease as well as administrating local treatments for the skin lesions. Intravenous systemic steroid therapy and skin grafts are useful treatments for generalized pyoderma gangrenosum.

Gastric Protective Effects of Banhasasim-tang on Indomethacin-treated Rats

  • Purpose : Banhasasim-tang (BHSST) has been applied for treating the symptom of gastric stuffiness, which is similar to dyspepsia. The object of this study was to observe the healing effect of BHSST on the indomethacin (IND)-induced gastric ulcer in rats. Methods : Three different dosages of BHSST (400, 200 and 100 mg/kg) were orally administered 30 min before IND treatment; 6 hrs after IND treatment, the changes on the gross lesion scores, fundic histopathology, myeloperoxidase (MPO) activity, lipid peroxidation and antioxidant defense system (glutathione contents, catalase (CAT) and superoxide dismutase (SOD) activities) were observed, and compared with the activity of the synthetic anti-ulcer drug, a representative proton pump inhibitor omeprazole (OME) 10 mg/kg. Results : All three different dosages of BHSST treatment in the IND-induced gastric ulcer rats, significant and dose dependent decreased gastric damages - hemorrhagic gross lesions, gastric mucosa MPO levels and histopathological gastric ulcerative lesions - were detected as compared with the IND treated control rats. BHSST also strengthened the antioxidant defense systems - decreased the level of lipid peroxidation and CAT activity but increased the level of GSH and SOD activity, and BHSST 200 mg/kg showed similar anti-ulcerative effect as compared with OME 10 mg/kg. Conclusions : The results obtained in this study suggest that BHSST has favorable effects against IND-induced gastric damages, through significant and dose-dependent decreasing gastric damages and the strengthening of the body's antioxidant defense systems with direct anti-inflammatory effects.

Cutaneous Sterile Pyogranuloma/Granuloma Syndrome in an Old English Bulldog: Case Report

  • A four-year old, indoor-living neutered male Old English Bulldog was presented for generalized ulcerative dermatitis. Generalized alopecia and multifocal papules and ulcers with crusting were observed mainly in the dorsal trunk. Cytology of the skin lesions revealed a pyogranulomatous inflammation comprising macrophages and nondegenerate neutrophils. Histopathology also revealed a nodular dermatitis characterized by mixed infiltration of monocytes and neutrophils involving the superficial and deep dermis. Neither of bacteria nor fungus was identified in microscopic exam and culture. From those findings, a diagnosis of cutaneous sterile pyogranuloma/granuloma syndrome (SPGS) was made. Treatment with immunosuppressive drugs of prednisone and cyclosporine was performed and visible ulcerative skin lesions were resolved after 4 weeks of initiation of therapy. Treatment with combination of cyclosporine and prednisone may be effective for the case of SPGS.

  • Hemangioma is the most common benign tumor of infancy. Greater than 60% of hemangiomas occur on the head and neck, and have an uncomplicated course. In contrast, most complicated hemangiomas develop in the urogenital or anogenital areas. These lesions are frequently associated with pain, bleeding, recurring infections, and ulcerations. Sometimes, perianal ulcerative hemangiomas are difficult to treat with multiple therapies, such as laser and steroid therapy. We managed a case of a severe perianal ulcerative hemangioma in a male newborn who did not respond to conservative management. He was successfully treated after a colostomy.

  • Herpes virus family is highly infectious to patients, their families and dentists. The diagnosis of herpes infection is based on the characteristic clinical appearance and the location of the lesions. Herpes Simplex Virus(HSV) usually acquired through direct contact with infected lesions or body fluids, and the prevalence of HSV infection increases progressively from childhood. Primary infections provoke herpetic gingivostomatis typically affects the tongue, lips, gingival, buccal mucosa and palate. Recurrent infections give rise to vesiculo-ulcerative lesions at vermilion border of lip(herpes labialis). In the form of chickenpox, Varicella Zoster Virus(VZV) usually is infected in childhood. VZV spreads in the affected primary afferent nerve to the skin and produces a vesicular rash and pain. Epstein-Barr Virus(EBV) infects B cells and cause infectious mononucleosis. Latent EBV infection has also been implicated in Burkitt lymphoma, nasopharyngeal carcinoma. Cytomegalovirus(CMV) is associated with immune-compromised patient such as organ transplantation and AIDS patients.

Feline Herpes Virus-1 Associated Facial and Perianal Dermatitis in a Cat (고양이 헤르페스 바이러스-1 감염과 관련된 고양이의 안면 및 회음부 피부염)

  • We herein describe a feline case of facial dermatitis whose histopathological features resembled to those of FHV-associated ulcerative dermatitis. A 3-year-old, intact male domestic short-haired cat was presented with 2-years history of pruritic dermatitis that initially appeared on periocular area and extended toward the entire face. The cat had ocular discharge and conjunctivitis from 2-month of age. Clinically, skin lesions were characterized as erythema, erosions and ulcers covered with crusts on the facial and perianal area. Histopathologically, the facial lesion was characterized as interface dermatitis with hydropic degeneration at the basal layer, and single cell necrosis of keratinocytes. In addition, the epidermal and dermal necrosis infiltrated with eosinophils, and intranuclear inclusion bodies in keratinocytes were also recognized. Moreover, feline herpesvirus-1 gene was detected by a PCR analysis using a swab obtained from the crusted lesions. Based upon these findings, the present case was considered as having FHV-associated ulcerative dermatitis. Therapy including oral acyclovir and topical recombinant feline interferon omega resulted in marked improvement of the skin and mucosal lesions.

  • Four water monitors at Zoological Garden, Chang Gyeong Won, Seoul, died within a week after signs of anorexia, lethargy, and discharge from eyes, nasal and oral cavities. The autopsy findings of the four animals were similar. As a main lesion, the liver was congested and diffuse necrosis was observed. The terminal portions of the rectum were studded with numerous small ulcers causing rectal stenosis. Histopathologically, massive hepatic necrosis preceded by fatty changes were evident. The rectal lesions manifested coagulative necrosis and thrombosis in the mucosa and submucosa.
