Pathological Studies on the Esopha-gastric Ulcers in Swine (돼지 위궤양 발생에 관한 병리학적 조사)

  • Based on the gross lesion, a survey of ulcer prevalence was carried out on pigs (3,161 heads) at a slaughterhouse in seoul from April to December in 1989. The pigs stomachs were observed anatomically and classified, according to the presence or absence of foreign bodies. Ulcer prevalence is usually classified according to size of pig farm, ages, and supplied month. The results in the studies were as follows, 1. Fundus followed the highest rate with 28.2%, esophagus with 24.5% each of cardiac and pyloric part with 7.8% and 4.1% in cases of gastric mucosa pathological studies examed total 3,161. In the acute ulcer prevalence, fundus follwed with 5.4%, esophagus with 2.6%, each of cardiac and pyloric part with 0.8% and 0.3%. When prevalent frequencies of fundus are compared with cardiac and pyloric part, they have meaning.(P <0.05). 2. In the survey pig of total 3,161, pathological changes of epithelium tissue foliowed 44.0%, erosion(11.5%), chronic ulcer(14.8%), acute ulcer(1.6%), scar formation (1.7%) and perforation (0.9%). 3. In general monthly prevalence rates of gastric lesion are higher in spring and fall than in summer. 4. In the size of pig farms, ulcer showed higher rate at professional farms than raising on the side. 5. When there are more fluid condition in stomach and they are old, the ulcer appeared for more. 6. Foreign bodies in stomach were sand, hairballs, and intestinal worms were detected (1%).

  • We'd performed the upper gastrointestinal study for a total of 10,047 insureds-male 5,919 female 4,128 persons-who were examined at medical department of Dae Han Kyoyuk Insurance Co., from August, 1986 to December, 1989. The results on duodenal ulcer were as follows; 1) Duodenal ulcer was found in 138 cases(1.37%) among the 10,047 insureds. 2) In all 138 cases who have duodenal ulcer, duodenal ulcer was found in 112 cases(1.89%) among the 5,919 insureds of male and in 26 cases(0.63%) among the 4,128 insureds of female. The incidence of duodenal ulcer in male was higher than in female. 3) The possessing rate of duodenal ulcer is 1.28% in 20 yrs, 1.24% in 30 yrs, 1.72% in 40 yrs, 1.21% in 50 yrs, 0.48% in 60 yrs. 4) The possessing rate of duodenal ulcer in male is 2.24% in 20 yrs, 1.78% in 30 yrs, 1.99% in 40 yrs, 2.05% in 50 yrs, 0.91% in 60 yrs and in female, 0.52% in 20 yrs, 0.48% in 30 yrs, 1.20% in 40 yrs, 0.2% in 50 yrs. 5) Coexistent lesion with duodenal ulcer were found in 19 cases; one case of esophageal diverticulum, one case of gastric diverticulum, one case of duodenal diverticulum, tow cases of gastric ulcer, six cases of gastritis, five cases of duodenitis, two cases of gastroduodenitis, one case of pseudodiverticulum.

Adult Patients with Peptic Ulcer in Korea-related Factors, Flexible Workers (한국 성인 변형근로자의 소화성궤양 환자 관련요인)

  • To identify health behavior of Peptic ulcer patients among all employees in Korea and examine relevant factors to present basic data for improving Korean adult employees' health conditions and quality of living, 3,515 employees aged 20 to 69 were sampled from the group that responded to the question concerning the presence of Peptic ulcer in the fourth Korea National Health and Nutrition Examination Survey for the third year (2009). After logistic regression analysis was carried out to see the effects of working conditions, harmful factors, shift work, stress perception, and experience of depression on the presence of Peptic ulcer, it was found that unsafe or unpleasant working conditions led to higher risk of getting Peptic ulcer, that flexible workers were more likely to get Peptic ulcer, and that depression or stress perception exerted almost no effect on Peptic ulcer. With the increase in the number of flexible workers, increasingly more interest is taken in the effects of shift work on health; occupational Peptic ulcer tends to become chronic as compared with the condition in the general population and reappear even after recovery in many cases; therefore, it is necessary to take special management measures by estimating the number of those at a risk of getting Peptic ulcer and by finding out its risk factors.

Stability and Effectiveness of Laser and Liquiband Treatments in the Rat Model of Oral Ulcer

  • Background and Objectives This study was undertaken to assess the stability and efficacy of laser therapy (808 nm), Liquiband (a commercial topical skin adhesive product), and a combination treatment, for application in oral ulcers. Materials and Methods The oral ulcer rat animal model was used to determine the efficacy of photobiomodulation, Liquiband, or combination therapy. Ulcers were induced by injecting 60% acetic acid in the oral mucosa. Three days after ulcer confirmation, the chemically induced ulcers were treated with either laser (808 nm), Liquiband, or a combination of both, every two days for 8 days (4 sessions). Combination therapy was performed by first treating with laser followed by application of the Liquiband. Reduction in ulcer area size was subsequently determined, and animals were sacrificed at 5 and 10 days after the last administration, for histological observation of the extracted oral ulcer tissue samples. Results In this study, treatment with either laser (808 nm) or commercial Liquiband product as well as the combined treatment significantly enhanced oral ulcer healing in the rat animal model. The effect of laser treatment is mainly attributed to collagen synthesis, whereas application of the Liquiband promotes vascularization and formation of granulation tissue. Our results indicate that further optimization of the combined therapy could synergistically and significantly enhance oral ulcer healing. Conclusion Laser or Liquiband treatment of oral ulcer in the rat animal model exert different outcomes, but both methods confirm significant effects relative to the untreated group. The combined treatment group with laser and Liquiband showed marked oral ulcer healing, but further optimization is required to get highly significant results and establish the potential synergistic effect of the combination therapy, as compared to the individual enhancing effects of healing oral ulcers.

The Control System of a Robot Bed for Caring Pressure Ulcer

  • The medical bed developed in this study is an electrically driven segmental keyboard. First describe the instrument of the segmental bed specially designed for pressure ulcer prevention, then the motor control system and pressure ulcer prevention operation of the bed. The main factor of pressure ulcer generation is displayed as body pressure x time, and when the keyboard falls, the body pressure becomes zero, and the pressure becomes higher than the threshold even if the body pressure is above the threshold, the pressure control algorithm has been developed. Therefore, using the proposed pressure control method, it has no particular ulcer occurred theoretically.

  • Decubitus ulcer is a pressure-induced ulceration of the skin occurring in persons confined to bed for long periods of time. In order to prevent decubitus ulcer, the medical bed mechanism to minimize pressure, friction, and shear forces acting between bed and patient had been developed. Air mattress also can be effective for reducing pressure over bony prominence. In this study, motion analysis was performed to examine whether this bed mechanism functioned properly in case of using air mattress, which was much thicker than common hospital mattress. We found that the patient on the air mattress above the medical bed for preventing decubitus ulcer slipped upward and downward excessively as the general motored-bed. New bed mechanisms were synthesized kinematically using simple 4-bar and 6-bar linkages so as to reduce sliding between the bed and the patient on the air mattress for preventing decubitus ulcer.

  • The ethanol extract of Sida acuta Burm. (ALSA) whole plant was studied for its anti-ulcer activity against aspirin plus pylorous ligation induced gastric ulcer, HCl-ethanol induced ulcer, and water immersion stress induced ulcer (WISIU) in rats. We found that ALSA at a dose of 300 mg/kg, (orally) markedly decrease the incidence of ulcers in the first two models. ALSA showed reduction in gastric volume, free acidity, and ulcer index (53.69%). It has not reduced the total acidity significantly and no significant change in pH. It also showed 55.14% gastro protective activity, wheres standard drug sucralfate showed 94.85%. WISIU showed protection index 24.4%, whereas standard drug omeprazole (OMEZ) showed protection index 100%.

Effect of Recombinant Human Epidermal Growth Factor(DWP 401) on Gastric Secretion and Ulcers in Rats (재조합 인간 상피세포 성장인자(DWP 401)의 흰쥐 위액분비 및 궤양에의 작용)

  • The effects of human epidermal growth factor(EGF) which was produced by recombinant DNA technique was investigated on gastric secretion, gastric lesion and ulcer models in rats. The EGF showed significant inhibition of secretion of gastric juice and total acid output, at 0.4mg/kg, id and also inhibited Shay ulceration at 0.4mg/kg, id in rats. The lesion induced by absolute ethanol was significantly reduced by oral administration of EGF at 0.4mg/kg. Likewise, EGF caused significant inhibition of indomethacin induced gastric ulcer at oral doses of 0.2 and 0.4mg/kg. The EGF produced dose-dependent inhibition of gastric ulcer induced by acidified aspirin, but showed no significant inhibition at oral doses of 0.1, 0.2 and 0.4mg/kg. The chronic gastric ulcer induced by injection of 20% acetic acid solution was significantly reduced by oral doses of 0.1 and 0.4mg/kg of EGF. Duodenal ulcer induced by mepirizole was dose-dependently inhibited by oral doses of 0.1, 0.2 and 0.4mg/kg of EGF. These data suggest that EGF possesses pronounced inhibitory action in gastric ulcer and duodenal ulcer of rats.

Accuracy of Nurses' Assessments of Pressure Ulcers in Hospitalized Patients (간호사의 입원환자 욕창 사정에 대한 정확성)

  • Purpose: The purpose of this study was to examine the accuracy of general nurses' assessments of pressure ulcers in hospitalized patients. Methods: A total of 129 nursing records of assessments on pressure ulcers were analyzed. Assessment records of pressure ulcers by general nurses were compared to those by Wound, Ostomy, Continence Nurses (WOCN) on the same pressure ulcers. A WOCN of a nursing unit was a nurse certified by the hospital after completion of a formal WOCN course and passing a cyber education course, both offered by the hospital. The formal WOCN course was taught by an internationally certified WOCN. The inter-rater reliability among WOCNs was 98.2%. General nurses in this study did not receive a structured pressure ulcer education. Results: The accuracy for nursing assessment of pressure ulcers by general nurses compared to WOCNs' were evaluated in various ways and resulted in as follows; the existence of pressure ulcer 91.4%, site of pressure ulcer 85.3%, stage of pressure ulcer 85.3%, Braden scale 36.3%, size of pressure ulcer 51.9~64.3%, details of pressure ulcer 0~100%. Conclusion: The structured education about pressure ulcer assessment is important to enhance the accuracy of pressure ulcer assessment of hospitalized patients by general nurses.

A Case Report on Pressure Ulcer Patient Treated with Fractional Laser Therapy and Korean Medicine (프락셔널 레이저 치료와 한의치료를 병행한 욕창치료 1례)

  • Objectives : The purpose of this report to represent the efficacy of pressure ulcer treatment with fractional laser therapy and Korean medicine. Methods : We treated Parkinsonism patients with pressure ulcer using herbal medicine, fractional mode laser therapy, acupuncture and simple dressing for 3 weeks. Laser was set on fractional mode (on time $100{\mu}s$, dot pitch 0.6 mm, shape RECT) and irradiated around pressure ulcer site and center of pressure ulcer site once a day. The treatment effect was evaluated by The National Pressure Ulcer Advisory Panel(NPUAP) stage and the The Pressure Ulcer Scale for Healing(PUSH) tool(3.0). Results & Conclusions : Pressure ulcer site have improved markedly. NPUAP stage was improved from 2 to 1 and total score of the PUSH tool decreased from 12 to 8. Even though the wound was not cured completely, this shows that fractional laser therapy and Korean medicine could be effective treatment for improving pressure ulcers.
