인공지능 세대 제 1형 당뇨 학생 인슐린 투약 지원 경험 (A study on the experiences of insulin medication support for the type 1 diabetes mellitus AI-generation students)

  • 본 연구의 목적은 인공지능세대인 제 1형 당뇨 학생들을 관리하는 간호사의 인슐린 투약지원 경험을 확인하는데 있다. 본 연구는 현상학적 질적 연구로, 자료는 제 1형 당뇨 학생의 투약지원을 맡고 있는 당뇨교실 간호사와 보건교사 5명을 대상으로 2018년 6월 17일~24일까지 인터뷰를 통해 수집하였으며, 자료의 의미는 Colaizzi(1978) 방법을 사용하여 분석하였다. 연구 결과 '첫걸음마를 응원', '치료적 관계유지', '도약을 위한 발판 마련'과 같은 3개의 범주와 그에 따른 7개의 주제를 도출하였다. 본 연구의 결과는 제 1형 당뇨 학생들의 투약지원과 관련된 학교보건법 개정에 따른 보건교사의 업무지침을 인공지능 세대 학생-보호자의 요구에 맞춰 실질적으로 구성하는데 기초자료를 제공할 것이다.

Increase in blood glucose level and incidence of diabetic ketoacidosis in children with type 1 diabetes mellitus in the Daegu-Gyeongbuk area during the coronavirus disease 2019 (COVID-19) pandemic: a retrospective cross-sectional study

  • Background: The coronavirus disease 2019 (COVID-19) outbreak in the Daegu-Gyeongbuk area in 2020 has caused difficulties in the daily life and hospital care of children with type 1 diabetes mellitus (T1DM). We detected an increase in blood sugar levels in these children and the number of patients hospitalized with more severe diabetic ketoacidosis (DKA) compared to those before COVID-19. Methods: This single-center study was conducted at Kyungpook National University Children's Hospital. The following patient groups were included; 45 returning patients diagnosed with T1DM and undergoing insulin treatment for more than 2 years and 20 patients newly diagnosed with T1DM before and after COVID-19 were selected by age matching. Returning patients before and after the outbreak were selected, and changes in hemoglobin A1c (HbA1c) levels were retrospectively reviewed. The HbA1c levels and severity of symptoms in newly diagnosed patients during hospitalization were examined. Results: HbA1c levels in returning patients with T1DM were significantly increased after COVID-19 (before, 7.70%±1.38% vs. after, 8.30%±2.05%; p=0.012). There were 10 and 10 newly diagnosed patients before and after COVID-19, respectively. The proportion of patients with drowsiness and dyspnea at the time of admission was higher after COVID-19 than before (before, 2 of 10 vs. after, 4 of 10). The HbA1c levels were higher in newly diagnosed patients hospitalized after COVID-19 than before (before, 11.15% vs. after, 13.60%; p=0.036). Conclusion: Due to COVID-19 in the Daegu-Gyeongbuk area, there was an increase in blood glucose levels in children with T1DM and in the incidence of severe DKA in newly diagnosed diabetes mellitus patients.

Type 2 Diabetes Mellitus as a Risk Factor for Female Breast Cancer in the Population of Northern Pakistan

  • Background: There has been much research work in the past to ascertain the association between type 2 diabetes mellitus and breast cancer, but definitive evidence has been scanty. The present study was carried out to determine the association of type 2 diabetes mellitus with breast cancer in the female population of Northern Pakistan. Materials and Methods: This case-control study was carried out in the Oncology Department of NORI Hospital. A total of 400 patients were included. Data were entered into PSPP 0.8.1. Two-tailed significance tests were used and a p-value of 0.05 was considered significant. Results: There were a higher percentage of postmenopausal women in the diabetic breast cancer patients' group as compared to the non-diabetic subset. The odds ratio for the association between diabetes and risk of developing breast cancer was elevated with statistical significance (OR = 2.96; 95 % CI =1.3-6.3; p-value=0.004). The results of our study showed that diabetes is associated with a risk of developing breast cancer, especially in postmenopausal women (OR = 4.928; 95 % CI = 2.1-11.3; p-value=0.001). The association was particularly marked in obese subjects (OR = 31.49; 95 % CI = 1.8-536; p- value=0.01), as compared to non-obese subjects (OR = 0.642; 95 % CI = 0.2-1.7). Conclusions: Diabetes is strongly associated with obesity and it tends to increase the risk of breast Cancer, especially in postmenopausal women. A high-risk subset for breast cancer comprised postmenopausal, diabetic and overweight women.

고지방식이와 STZ 유도 제2형 당뇨병 마우스에서 상엽 추출물의 항당뇨 효과 (Anti-Diabetic Effects of Mori Folium Extract on High-Fat Diet and Streptozotocin-Induced Type II Diabetes Mellitus in Mice)

  • Objectives : The present study was designed to investigate the anti-diabetic effects of Mori Folium (Morus alba L. of Moraceae) extract (MFE) on high fat diet (HFD) and streptozotocin (STZ)-induced type II diabetes mellitus in mice. Methods : The mice (C57BL/6J) were fed HFD for 8 weeks and then was induced with a single injection of STZ (75 mg/kg). The diabetic mice were divided into four groups [(STD, HFD, HFD + MFE and HFD + quercetin (QUR)] and administered with MFE or OUR for 4 weeks. Fasting blood glucose, lipid profile (triglycerides and cholesterol etc.), glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT), insulin and leptin were measured every 2 weeks. Results : Body weight gain was lower in the MFE and QUR groups than HFD group. The fasting blood glucose was lower in the MFE and QUR groups. Oral glucose and insulin tolerance were decreased in the MFE and QUR groups. The levels of serum total cholesterol, triglycerides, and LDL cholesterol were reduced in the MFE and QUR groups. The HDL cholesterol was much higher in the MFE and QUR groups than HFD group. The levels of GOT, GPT and atherogenic index were decreased in the MFE and QUR groups. The serum insulin and leptin concentrations were reduced in the MFE and QUR groups. Conclusions : These results showed that MFE could decrease blood glucose level and lead to an amelioration in dyslipidemia states on HFD/STZ-induced type II diabetes mellitus in mice.

Group B Streptococcal Renal Abscess in a 17-Year-Old Girl with Type 1 Diabetes Mellitus

  • Streptococcus agalactiae or group B streptococcus (GBS) is associated with infections in neonates and pregnant women. Herein, we describe a rare case of GBS renal abscess with peritonitis and pleural effusion in a 17-year-old girl with type 1 diabetes mellitus. The girl was admitted due to fever and right flank pain. Laboratory findings included leukocytosis and increased C-reactive protein level and erythrocyte sedimentation rate. Her serum glucose level was 484 mg/dL. Urinalysis showed no pyuria. Renal sonography revealed parenchymal swelling in the right kidney. The patient was administered intravenous cefotaxime. Urine and blood cultures were negative. Fever seemed to improve, but the following day, she complained of abdominal pain and fever. Antibiotic was switched to imipenem, and abdominal and pelvic CT revealed a ruptured right renal abscess, peritonitis, and bilateral pleural effusion with atelectasis. Pigtail catheter drainage of the abscess was performed. Culture from the abscess was positive for GBS, and fever subsided 2 days after the drainage. She was discharged with oral cefixime. The clinical course of urinary tract infections (UTIs) can be atypical in patients with diabetes, and GBS can be a cause of UTIs. Prompt diagnosis and management are necessary to prevent complications in patients showing atypical courses.

Comparison of male reproductive parameters in mice with type 1 and type 2 diabetes

  • Objective: The differences between type 1 and type 2 diabetes mellitus (T1DM and T2DM) in terms of their adverse effects on male reproductive parameters have never been elucidated. This study aimed to distinguish between the effects of the DM types in mice treated with multiple low doses of streptozotocin (STZ) to mimic human T1DM and coadministered a high-fat diet (HFD) to mimic human T2DM. Methods: The T1DM mice were intraperitoneally injected with STZ (40 mg/kg body weight) for 5 days. The T2DM mice received an HFD for 14 days prior to STZ injection (85 mg/kg body weight), followed by continuous feeding of an HFD. Male reproductive parameters were evaluated. Results: The reproductive organs of the DM mice weighed significantly less than those of controls, and the seminal vesicles plus prostates of the T1DM mice weighed less than those of the T2DM mice. Increased sperm abnormalities and incomplete DNA packaging were observed in the DM groups. Sperm concentration and the proportion of normal sperm were significantly lower in the T1DM group. The seminiferous histopathology of DM mice was classified into seven types. The penises of the DM mice were smaller than those of the controls; however, tunica albuginea thickness and the amount of penile collagen fibers were increased in these mice. Round germ cells were abundant in the epididymal lumens of the mice with DM. Conclusion: T1DM adversely affected reproductive parameters to a greater extent than T2DM.

제 2형 당뇨병 여성 노인 환자의 혈당조절을 위한 국내 운동 프로그램의 효과분석 (The Effect of Exercise Program in Korea for Controlling Blood Glucose Levels in elderly women patients undergoing type II diabetes mellitus)

  • 본 연구의 목적은 제2형 당뇨병 여성 노인 환자의 혈당 조절을 위한 국내 운동 프로그램 효과에 대한 주요 결과를 고찰하고 중재의 효과를 알아보기 위함이다. 연구 방법은 국내 데이터베이스로 한국 교육학술정보원, 한국 학술정보, 대한의학 편집인 협의회, 한국 의학논문 데이터베이스를 통해 검색하였으며, 2010년부터 2017년 5월까지 출판된 국내 논문을 분석하였다. 검색어로는 당뇨, 여성 당뇨, 노인 당뇨, 제2형 당뇨병을 사용하였으며, 중재 관련 검색어로는 프로그램, 중재, 운동, 혈당, 혈당장애, 혈당조절, 공복 시 혈당, 당화 혈색소를 사용하였다. 최종 12개의 논문이 분석에 사용되었으며, 연구결과 운동프로그램의 중재 기간은 12주(50%), 주당 3회(58.3%), 1회당 50-69분(83.4%)이 가장 많았다. 또한, 유산소와 무산소복합운동은 혈당뿐 만 아니라 당화 혈색소도 조절할 수 있는 효과적인 운동방법으로 나타났다. 따라서 제2형 당뇨병 여성 노인 환자의 체력 수준과 건강 상태를 고려하여 집단교육보다는 개별교육의 운동 프로그램 중재를 적용해야 하며 일회성 운동보다는 지속적인 운동을 통해 효과적인 혈당조절을 유지하는 것이 필요하다.

당뇨병 근로자의 우울 영향요인: 국민건강영양조사를 바탕으로 (Factors Influencing Depression in Workers with Diabetes Mellitus-using the Korean National Health and Nutrition Examination Survey-)

  • 본 연구는 당뇨병 근로자의 우울 정도 및 영향요인을 파악하기 위해 국민건강영양조사 자료를 분석하였으며, 우울 정도는 PHQ-9를 활용하였고, 만 30세 이상의 직업이 있는 당뇨병 환자 685명을 대상으로 하였다. 자료분석은 복합표본 선형 회귀분석을 이용하였다. 연구결과, 여성, 40~49세, 65세 이상. 편부모와 미혼자녀로 구성된 세대, 비정규직 근로자가 우울이 높았으며, 가구소득이 높고, 교육수준이 고등학교 졸업 이상의 근로자는 우울이 낮게 나타났다(p>.05). 또한, 약물치료를 하는 경우, 스트레스가 많은 경우, 1주일간 걷기 일수가 2~3일/주, 주관적 건강상태가 보통이거나 나쁜 경우, 우울증으로 의사진단을 받은 경우 우울이 높게 나타났다(p>.05). 본 연구결과를 바탕으로 당뇨병 근로자의 우울 관리 개발 프로그램 연구와, 정부 및 지역사회에서 당뇨병 근로자의 우울에 대한 체계적인 사정 및 진단 프로그램과 직장내에서 당뇨병 근로자를 위한 건강증진행동을 위한 지원체계가 마련되어야 할 것이다.

Successful sulfonylurea treatment in a patient with permanent neonatal diabetes mellitus with a novel KCNJ11 mutation

  • Permanent neonatal diabetes mellitus refers to diabetes that occurs before the age of 6 months and persists through life. It is a rare disorder affecting one in 0.2-0.5 million live births. Mutations in the gene KCNJ11, encoding the subunit Kir6.2, and ABCC8, encoding SUR1 of the ATP-sensitive potassium ($K_{ATP}$) channel, are the most common causes of permanent neonatal diabetes mellitus. Sulfonylureas close the $K_{ATP}$ channel and increase insulin secretion. KCNJ11 and ABCC8 mutations have important therapeutic implications because sulfonylurea therapy can be effective in treating patients with mutations in the potassium channel subunits. The mutation type, the presence of neurological features, and the duration of diabetes are known to be the major factors affecting the treatment outcome after switching to sulfonylurea therapy. More than 30 mutations in the KCNJ11 gene have been identified. Here, we present our experience with a patient carrying a novel p.H186D heterozygous mutation in the KCNJ11 gene who was successfully treated with oral sulfonylurea.

Fall prevention strategies in community-dwelling older adults aged 65 or over with type 2 diabetes mellitus: a systematic review and meta-analysis

  • Objective: Independent walking is the most essential prerequisite to maintain quality of life in older persons. The purpose of this review was to investigate the effect of fall prevention strategies on fall risk for type 2 diabetes mellitus (T2DM) within community-dwelling older adults aged 65 and over. Design: A systematic review and meta-analysis. Methods: PubMed and three other databases were searched up to October 31st, 2018 and randomized controlled trials (RCTs) evaluating fall prevention strategies for fall risk in persons who were 65 years of age or above with T2DM were included. The review extracted the following information from each study selected: first author's surname, published year, country, study population, type of intervention, intensity of intervention, comparison, measurement variables, additional therapy, summary of results, and mean and standard deviation from selected studies. Results: This review selected fourteen RCTs with 460 older adults with diabetes mellitus. Of the 14 studies, the types of intervention used to improve the risk of falls were strengthening (5), aerobic exercises (2), multimodal exercises (4), one virtual reality exercise (1), whole body vibration with balance exercise (1), and Tai Chi exercise (1). Seven RCTs were eligible for the meta-analysis. Therapeutic interventions were more effective than the control group for the Timed Up-and-Go test (-1.11; 95% CI, -1.82 to -0.41) and the 6-minute Walk Test (-1.89; 95% CI, -8.33 to 4.54). Conclusions: The results of the review suggest that interventions to prevent fall risk in older adults with T2DM should focus on strengthening, balance, aerobic, and multimodal exercises.
