Stable polyion-complexed polymeric monolayers were prepared by spreading perfluoroalkyl monomaleate copolymers, $C_2F_8MA-VE_2$ and $C_2F_8MA-VE_3$, on a aqueous poly(allylamine) subphase. The monolayer properties have been studied by the surface pressure-area (${\pi}$-A) isotherms. The $C_2F_8MA-VE_3$ containing longer oligoethyleneglycol pendant showed more expanded monolayer phase than the $C_2F_8MA-VE_2$. The polyion-complexed monolayers were transferable on various substrates, and the resulting Langmuir-Blodgett (LB) films were characterized by FT-IR spectroscopy and scanning electron microscopy (SEM). Two-dimensional crosslinking to form a polymer network was achieved by amide formation through heat treatment under vacuum with concurrent removal of perfluoroalkyl tails. SEM observation of this film on a porous membrane filter showed that the four layer film was sufficiently stable to cover the filter pore size of 0.1 ${\mu}$m. The $C_2F_8MA-VE_3$ revealed better covering capability than the $C_2F_8MA-VE_2$. Immersion of this film in water or in benzene did not cause any change in its appearance and in Fl-IR spectra.
Chronic eosinophilic leukemia (CEL) is characterized by eosinophilia and organ damage. Imatinib is widely used for treating CEL, chronic myeloid leukemia (CML) and acute myeloid leukemia (AML). Unfortunately, the cancer cells gain resistance against the drug after prolonged molecular-targeted therapies. Imatinib-resistant EoL-1 (EoL-1-IR) cells were produced from chronic eosinophilic leukemia cells (EoL-1) after treatment with imatinib for a long duration. Two-dimensional electrophoresis (2-DE) analysis revealed numerous protein variations in the EoL-1 and EoL-1-IR sub-types. Compared to the EoL-1 cells, expression levels of TIP49, RBBP7, ${\alpha}$-enolase, adenosine deaminase, C protein, galactokinase, eukaryotic translation initiation factor, $IFN-{\gamma}$, and human protein homologous to DROER were increased, whereas core I protein, proteasome subunit p42, heterogeneous ribonuclear particle protein, chain B, and nucleoside diphosphate were decreased in the EoL-1-IR cells. Taken together, these results contribute to understanding the pathogenic mechanism of drug-resistant diseases.
Pathogenic Acanthamoeba spp. cause granulomatous amoebic encephalitis and keratitis. Acanthamoeba keratitis (AK) is a rare but serious ocular infection that can result in permanent visual impairment or blindness. However, pathogenic factors of AK remain unclear and treatment for AK is arduous. Expression levels of proteins secreted into extracellular space were compared between A. castellanii pathogenic (ACP) and non-pathogenic strains. Two-dimensional polyacrylamide gel electrophoresis revealed 123 differentially expressed proteins, including 34 increased proteins, 7 qualitative increased proteins, 65 decreased proteins, and 17 qualitative decreased proteins in ACP strain. Twenty protein spots with greater than 5-fold increase in ACP strain were analyzed by liquid chromatography triple quadrupole mass spectrometry. These proteins showed similarity each to inosine-uridine preferring nucleoside hydrolase, carboxylesterase, oxygen-dependent choline dehydrogenase, periplasmic-binding protein proteinases and hypothetical proteins. These proteins expressed higher in ACP may provide some information to understand pathogenicity of Acanthamoeba.
Titanium dioxide (TiO2) is one of the most intensively investigated materials in materials science. Mostly, TiO2 has been used in the form of nanoparticles, but recently new highly ordered TiO2 nanotubes (U-tube) has been introduced and applied to various applications due to their one-dimensional charge path way. In the present paper, we described the formation process and physical properties of U-tube then, gave examples of applications in sequence. Firstly, in photocatalysis, U-tube was used with Au/Pt co-catalysts and showed enhanced photogenerated H2 efficiency compared to bare TiO2. Secondly, photoelectrochemical performance of U-tube was evaluated with different heat-treatment temperatures. As a further application, two different types of electrical cell (Ti-TiO2-Pt and Ti-TiO2-PtNP) was configurated to observe memristive behavior of U-tube. Both cells behaved as switching electrodes and follow a memristive movement in the high and low resistance state extremely well with high reproducibility.

Objective : The latest version of the Leksell Gamma Knife IconTM allows for mask- and frame-based fixation. Although mask fixation provides fractionated treatment and immobilization using a noninvasive method, it is not free from collision. The authors investigated the collision problem with a modified mask fixation method. Methods : This study presents a case of two meningiomas in the frontal area, where a collision occurs in the occipital area. A modified mask fixation method was introduced to avoid the collision : first, the edges of the head cushion were cut off and polystyrene beads with a diameter of approximately 5 cm were removed. Next, the head cushion was sealed using a stapler. Finally, the head cushion was flattened in the adapter. We compared the shot coordinates, 3-dimensional (3D) error, clearance distance, and vertical depth of the head cushion between the initial and modified mask fixations. Results : When comparing the initial and modified mask fixations, the difference in the shot coordinates was +10.5 mm along the y-axis, the difference in the 3D error was approximately 18 mm, and the difference in clearance was -10.2 mm. The head cushion was approximately 8 mm deeper in the modified mask fixation. Conclusion : Based on these findings, we recommend a modified mask fixation method for gamma knife radiosurgery using ICON with a collision.
목적: 치료방법이 정립되지 않은 진행된 간세포암에서의 간문맥 혈전증에 대한 방사선치료의 효과와 간독성을 측정함으로써 향후 방사선치료의 효용성에 대하여 알아보고자 한다. 대상 및 방법: 2000년 5월부터 2005년 12월까지 서울아산병원에서 원발성 간세포암으로 진단받은 환자 중 간문맥 혈전증이 동반된 70명의 환자를 대상으로 후향적 분석을 하였다. 대상 환자들의 나이는 $24{\sim}74$세(중앙값 51세)였으며, 대부분이 Child-Pugh's class A, B였고 Eastern Cooperative Oncology Group (ECOG) 2 이하였다. 방사선치료는 3차원입체조형치료를 시행하였으며, 1회 선량은 $2{\sim}4$ Gy로 총방사선량은 $40{\sim}60$ Gy (중앙값 48 Gy)였으며, 생물학적 유효선량(biologic effective dose)은 $31.3{\sim}78.0\;Gy_{10}$ (중앙값 61.6 $Gy_{10}$)였다. 결과: 치료 반응평가는 치료 전과 치료 종료 후 최소 1개월 이상 경과 후 컴퓨터 단층촬영(CT)을 시행하여 평가하였으며, 대상 환자들의 추적관찰기간은 $2{\sim}24$개월(중앙값 9개월)이었다. 방사선치료 후 완전반응인 경우가 2명(2.9%), 부분반응 31명(44.3%), 무반응 35명(50.0%), 진행병변 2명(2.9%)이었다. 1년 무진행 생존율(progression-free survival)은 60%였으며, 중앙 무진행 생존기간은 17개월이었다. 전체 환자의 중앙 생존기간(overall survival)은 11개월이었으며, 치료반응 유무에 따라 부분 반응 이상을 보인 환자군의 중앙 생존기간은 15개월, 무반응이거나 진행병변을 보인 환자군은 8개월로 통계학적 차이를 보였다(p=0.032). 치료 중 일시적인 간기능 저하가 4명(5.7%), 방사선 간염은 1명(1.4%)에서 발생하였다. 결론: 진행된 간세포암에서의 간문맥 혈전에 대해 방사선치료를 시행한 경우 비교적 유용하며 안전한 치료법이며, 앞으로의 연구에서는 방사선치료가 생존율을 향상시킬 수 있는지 여부와 적절한 방사선량을 결정하기 위한 연구가 필요할 것으로 생각된다.
목 적: 4-dimensional computed tomography (4DCT) 영상과 on board imaging (OBI) 및 real time position management (RPM) 장치로 매 회 치료 시마다 얻은 호흡연동 직각 kilovolt (KV) 준비 영상(gated orthogonal kilovolt setup image)을 이용해 간암 환자를 치료하는 동안 발생하는 종양 위치의 불확실성을 평가하고자 했다. 대상 및 방법: 3차원입체조형치료가 예정된 20명의 간암 환자를 대상으로 RPM과 전산화단층촬영모의치료기를 이용해 치료계획용 4DCT를 시행했다. 표적 근처에 위치한 간동맥화학색전술 후 집적된 리피오돌(lipiodol) 혹은 횡격막을 종양의 위치 변이를 측정하는 표지자로 선택했다. 표지자의 위치 차이를 이용해 온라인 분할간 및 분할중 내부 장기 변이와 움직임 진폭을 측정했다. 측정된 자료의 정량적 평가를 위해 통계 분석을 실시했다. 결 과: 20명 환자로부터 측정된 표지자의 분할간변이의 중앙값은 X (transaxial), Y (superior-inferior), Z (anterior-posterior) 축에서 각각 0.00 cm (범위, -0.50~0.90 cm), 0.00 cm (범위, -2.4~1.60 cm), 0.00 cm (범위, -1.10~0.50 cm) 였다. 4명의 환자에서 X, Y, Z축 중 하나 이상에서 0.5 cm를 초과하는 변이가 관찰되었다. 4DCT와 호흡연동 직각 준비 영상으로부터 얻은 표적의 움직임 진폭의 차이는 X, Y, Z 축에서 각각 중앙값이 -0.05 cm (범위, -0.83~0.60 cm), -0.15 cm (범위, -2.58~1.18 cm), -0.02 cm (범위, -1.37~0.59 cm) 였다. 두 영상간 표적의 움직임 진폭 차이가 1 cm를 초과하는 환자가 Y축 방향으로 3명 관찰되었으며, 0.5 cm 초과 1 cm 미만의 차이를 보이는 환자도 Y축과 Z축 방향을 합쳐 5명 관찰되었다. 분할중 표지자 위치 변이의 중앙값은 X, Y, Z축에서 각각 0.00 cm (범위, -0.30~0.40 cm), -0.03 cm (범위, -1.14~0.50 cm), 0.05 cm (범위, -0.30~0.50 cm)였으며 2명의 환자에서 1 cm를 초과하는 변이가 Y축 방향으로 관찰되었다. 결 론: 4DCT와 호흡연동 직각 KV 준비 영상으로 얻은 표지자의 분할간, 분할중 및 움직임 진폭에서 큰 변이가 관찰되었다.

Purpose: We evaluated the efficacy and toxicity of repeated high dose 3-dimensional conformal radiation therapy (3D-CRT) for patients with unresectable hepatocellular carcinoma. Materials and Methods: Between 1998 and 2011, 45 patients received hepatic re-irradiation with high dose 3D-CRT in Samsung Medical Center. After excluding two ineligible patients, 43 patients were retrospectively reviewed. RT was delivered with palliative or salvage intent, and equivalent dose of 2 Gy fractions for ${\alpha}/{\beta}=10Gy$ ranged from $31.25Gy_{10}$ to $93.75Gy_{10}$ (median, $44Gy_{10}$). Tumor response and toxicity were evaluated based on the modified Response Evaluation Criteria in Solid Tumors criteria and the Common Terminology Criteria for Adverse Events (CTCAE) ver. 4.0. Results: The median follow-up duration was 11.2 months (range, 4.1 to 58.3 months). An objective tumor response rate was 62.8%. The tumor response rates were 81.0% and 45.5% in patients receiving ${\geq}45Gy_{10}$ and $<45Gy_{10}$, respectively (p = 0.016). The median overall survival (OS) of all patients was 11.2 months. The OS was significantly affected by the Child-Pugh class as 14.2 months vs. 6.1 months (Child-Pugh A vs. B, p < 0.001), and modified Union for International Cancer Control (UICC) T stage as 15.6 months vs. 8.3 months (T1-3 vs. T4, p = 0.004), respectively. Grade III toxicities were developed in two patients, both of whom received ${\geq}50Gy_{10}$. Conclusion: Hepatic re-irradiation may be an effective and tolerable treatment for patients who are not eligible for further local treatment modalities, especially in patients with Child-Pugh A and T1-3.
One of the methods to consider the effect of respiratory motion of a tumor target in radiotherapy is to establish a treatment plan with the internal target volume (ITV) created based on an accurate analysis of the target motion displacement. When this method is applied to intensity modulated radiotherapy (IMRT), it is expected to yield a different treatment dose distribution under the motion condition according to the IMRT method. In this study, we prepared ITV-based IMRT plans with conventional IMRT using fixed gantry angle beams, RapidArc using volumetric modulated arc therapy, and tomotherapy using helical therapy. Then, the variation in dose distribution caused by the target motion was analyzed by the dose measurement in the actual motion condition. A delivery quality assurance plan was prepared for the established IMRT plan and the dose distribution in the actual motion condition was measured and analyzed using a two-dimensional diode detector placed on a moving phantom capable of simulating breathing movements. The dose measurement was performed considering only a uniform target shape and motion in the superior-inferior (SI) direction. In this condition, it was confirmed that the error of the dose distribution due to the target motion is minimum in tomotherapy. This is thought to be due to the characteristic of tomotherapy that treats the target sequentially by dividing it into several slices. When the target shape is uniform and the main target motion direction is SI, it is considered that tomotherapy for the ITV-based IMRT method has a characteristic which can reduce the dose difference compared with the plan dose under the target motion condition.
진료 및 수술 집도 시 조명용 광원으로 할로겐램프와 플라즈마램프가 주로 사용되고 있다. 두 광원은 램프의 수명 및 과도한 발열로 인한 다양한 문제점을 야기할 뿐 아니라 냉각기로 인하여 부피 또한 매우 크다는 단점이 있다. 따라서 본 연구에서는 의료용 신조명 부품으로 주목받고 있는 고휘도 LED를 사용하여 진료실 및 수술실에서 환자의 환부에 대한 국소 무영 촬영이 가능하도록 링라이트를 설계 개발하였다. 개발에 적용한 LED는 환부 근접 무영 촬영시 피사체 고유의 Tone에 대한 섬세한 표현력과 강한 하이라이트, 간결한 쉐도우, 그리고 입체감을 부각시키기 위하여 다양한 색상 구현과 광량 조절이 가능하고 백색광을 얻을 수 있는 FULL COLOR용 고휘도 3색 LED를 사용하였다.
