Complication After Gastrectomy for Gastric Cancer According to Hospital Volume: Based on Korean Gastric Cancer Association-Led Nationwide Survey Data

  • Purpose: This study aimed to analyze the incidence and risk factors of complications following gastric cancer surgery in Korea and to compare the correlation between hospital complications based on the annual number of gastrectomies performed. Materials and Methods: A retrospective analysis was conducted using data from 12,244 patients from 64 Korean institutions. Complications were classified using the Clavien-Dindo classification (CDC). Univariate and multivariate analyses were performed to identify the risk factors for severe complications. Results: Postoperative complications occurred in 14% of the patients, severe complications (CDC IIIa or higher) in 4.9%, and postoperative death in 0.2%. The study found that age, stage, American Society of Anesthesiologists (ASA) score, Eastern Cooperative Oncology Group (ECOG) score, hospital stay, approach methods, and extent of gastric resection showed statistically significant differences depending on hospital volumes (P<0.05). In the univariate analysis, patient age, comorbidity, ASA score, ECOG score, approach methods, extent of gastric resection, tumor-node-metastasis (TNM) stage, and hospital volume were significant risk factors for severe complications. However, only age, sex, ASA score, ECOG score, extent of gastric resection, and TNM stage were statistically significant in the multivariate analysis (P<0.05). Hospital volume was not a significant risk factor in the multivariate analysis (P=0.152). Conclusions: Hospital volume was not a significant risk factor for complications after gastric cancer surgery. The differences in the frequencies of complications based on hospital volumes may be attributed to larger hospitals treating patients with younger age, lower ASA scores, better general conditions, and earlier TNM stages.

위선암에서 p53과 bcl-2의 발현이 예후와 생존율에 미치는 영향 (Expression of p53 and bcl-2 in Gastric Adenocarcinoma Affects the Prognosis and Survival Rate)

  • 목적: 세포자멸사의 중요인자인 p53과 bcl-2의 개별발현 및 동시 발현이 갖는 위선암의 예후 인자로서의 역할과 생존율에 미치는 영향을 알아보고자 하였다. 대상 및 방법: 1999년 12월에서 2007년 7월까지 위선암으로 분당제생병원 외과에서 근치적 위절제술을 시행 받은 238명의 환자들을 대상으로 자료의 후향적 분석을 시행하였다. p53과 bcl-2의 발현은 DAKO사의 Envision kit로 면역조직화학 염색을 하여 발현군을 양성으로 정의하였다. 결과: 전체 환자 중 p53은 149예(62.5%)에서 발현되었으며, 다른 임상병리학적 예후 인자와의 상관관계를 조사한 결과 세포분화도(P=0.028), TNM 병기(P<0.001)에서 유의성을 나타냈다. bcl-2은 29예(12.2%)가 발현되었으며, TNM 병기(P=0.005)에서 유의성을 나타냈다. 단변량 생존율 분석을 통해 p53과 bcl-2는 생존율 감소에 영향이 있으며, 다변량 생존율 분석을 통해 p53은 독립적 예후 인자로서의 확인되었다. 또한 두 단백의 동시 발현군도 TNM 병기(P=0.002)와 의미 있는 상관관계를 보였으며, 개별 발현 때보다 동시 발현 시 유의한 생존율 감소를 보였다(P<0.001). 결론: p53과 bcl-2의 개별 발현은 나쁜 예후를 나타내며, 이들의 동시 발현은 더욱 나쁜 예후를 나타냈다. 그러나 bcl-2는 다변량 분석에서 독립 예후인자로서 의미는 부족하여 더 많은 분석을 통해 명확히 할 필요가 있겠다.

Expression of C4.4A is a Potential Independent Prognostic Factor for Patients with Gastric Cancer

  • C4.4A, a metastasis-associated gene, encodes a glycolipid-anchored membrane protein which is overexpressed in several human malignancies. However, there are few data available on C4.4A expression and its relationship with progression in gastric cancer. Our study was designed to explore the expression of C4.4A in gastric cancer and to correlate it with clinical outcome. C4.4A expression was studied by quantitative real-time RT-PCR and immunohistochemistry for assessment of correlations with clinicopathological factors. C4.4A mRNA expression was significantly up-regulated in gastric cancer as compared with noncancerous tissue (p<0.05)., being observed in 107 (88.4%) of the 121 gastric cancer cases by immunohistochemistry. We found that the expression of C4.4A mRNA was correlated with size of the tumor, depth of invasion, lymph node metastasis, distant metastasis and TNM stage. Moreover, patients with overexpression of C4.4A has a significantly worse survival (p<0.05). Further multivariable analysis indicated that the expression of C4.4A was an independent prognostic indicator for gastric cancer (p<0.05). In conclusion, overexpression of C4.4A correlates with metastatic potential of gastric cancer and C4.4A could be a novel independent prognostic marker for predicting outcome.

Low Expression of the FoxO4 Gene may Contribute to the Phenomenon of EMT in Non-small Cell Lung Cancer

  • Because of its importance in tumor invasion and metastasis, the epithelial-mesenchymal transition (EMT) has become a research focus in the field of cancer. Recently, evidence has been presented that FoxO4 might be involved in EMT. Our study aimed to detect the expression of FoxO4, E-cadherin and vimentin in non-small cell lung cancers (NSCLCs). We also investigated clinical features and their correlations with the markers. In our study, FoxO4, E-cadherin and vimentin were assessed by immunohistochemistry in a tissue microarray (TMA) containing 150 cases of NSCLC. In addition, the expression level of FoxO4 protein was determined by Western blotting. The percentages of FoxO4, E-cadherin and vimentin positive expression in NSCLCs were 42.7%, 38.7% and 55.3%, respectively. Immunoreactivity of FoxO4 was low in NSCLC when compared with paired normal lung tissues. There were significant correlations between FoxO4 and TNM stage (P<0.001), histological differentiation (P=0.004) and lymph node metastasis (P<0.001), but no significant links with age (P=0.323), gender (P=0.410), tumor size (P=0.084), smoking status (P=0.721) and histological type (P=0.281). Our study showed that low expression of FoxO4 correlated with decreased expression of E-cadherin and elevated expression of vimentin. Cox regression analysis indicated FoxO4 to be an independent prognostic factor in NSCLC (P=0.046). These data suggested that FoxO4 might inhibit the process of EMT in NSCLC, and might therefore be a target for therapy.

pT1N3 위암 (pT1N3 Gastric Cancer)

  • 목적: 위내시경의 보급으로 조기위암의 빈도가 증가하고 축소치료가 활성화되고 있다. 이러한 치료는 장점도 있으나 림프절 곽청의 측면에서 제한점이 있다. 림프절 전이를 동반하는 조기위암의 대부분은 UICC-TNM 분류의 N1 또는 N2이지만 드물게 N3의 보고도 있다. pT1N3 위암은 증례보고는 있지만 review논문은 없어서 특성을 이해하거나 치료방침을 세우기가 어렵고 UICC-TNM에서 4기로 예후가 나쁜 위암으로 분류되어 있으나 객관적인 증거는 없다. 저자들은 국내 6개 대학에서 9예의 pT1N3 위암 치험예를 모아서 문헌 고찰과 함께 분석하였다. 대상 및 방법: 분당차병원,한양의대,충북의대,고려의대 구로병원, 아주의대, 가톨릭의대 성가병원의 6개 병원에서 기간은 서로 다르지만 의무기록 분석이 가능하였던 기간동안 수술을 받았던 2,772예의 조기위암 중에서 UICC-TNM의 pT1N3 위암 9예를 분석하였다. 비교 대조군은 분당 차병원에서 수술을 받은 210예의 조기위암 환자이다. 결과: 9예는 전체 조기 위암 2,772예의 0.32%이며 남자는 3명, 여자는 6명이었고 평균연령은 57세였다. 점막암이 2예, 점막하층암이 7예이고 전이된 림프절의 수는 18에서 52개로 평균 27개였다. 5예는 표층 팽창형으로 대조군에 비해서 많았고 종양의 크기도 N3군이 N0, N1, N2군보다 컸다. 7예에서 림프관 침윤이 있었으며 대조군에 비하여 높았다. 1예는 대동맥주위 림프절 전이가 있어서 비치유 절제가 되었고 2예에서 조기재발이 있는 등 예후가 불량하였다. 결론: pT1N3 위암의 위험 인자로는 여성, 점막하층암, 종양의 크기, 림프관 침윤을 들 수 있다. pT1N3 위암의 존재는 조기위암의 축소 치료 시 주의를 요하며 예후가 불량하기 때문에 적극적인 항암 보조 요법이 필요할 것으로 생각된다.

Role of Laparoscopic Gastrectomy in Very Elderly Patients with Gastric Cancer Who Have Outlived the Average Lifespan

  • Purpose: This study aimed to investigate the outcomes of laparoscopic gastrectomy in very elderly patients with gastric cancer, who have outlived the average lifespan of the Korean population (men: ${\geq}77years$, women: ${\geq}84years$). Materials and Methods: Between 2004 and 2015, 836 patients with gastric cancer underwent a laparoscopic gastrectomy. They were divided into the elderly group (EldG) and non-elderly group (nEldG). Propensity score matching for covariates of sex, tumor depth, node status, and extent of resection was performed. Clinicopathologic characteristics, and surgical and survival outcomes were compared between the 2 groups. Results: The EldG had a higher American Society of Anesthesiologists (ASA) score and a higher number of comorbidities. There was no significant difference in the post-operative complications, except for pulmonary complications, which were more frequent in the EldG (5/56, 8.9%) than in the nEldG (0/56, 0%). The EldG had a shorter overall survival (OS), but cancer-specific survival was similar for both groups. Among deceased patients, 2 (25%) and 8 patients (50%) died within a year of surgery in the nEldG and EldG, respectively. Univariate and multivariate risk factor analyses for OS showed that age, ASA score, tumor, node, metastasis (TNM) stage, and occurrence of complications were significantly related to deterioration in OS. Conclusions: Laparoscopic gastrectomy can be safely performed in very elderly patients with gastric cancer who have outlived the average lifespan of the Korean population. However, impact of laparoscopic gastrectomy on improving survival is not clear, and careful patient selection is recommended.

절제 가능한 위암에서 종양표지자의 발현과 임상적 의의 (Expression of Tumor Markers and its Clinical Impacts in Resectable Gastric Cancer)

  • 목적: 위암의 혈청 종양표지자인 CEA, CA 19.9, CA 72-4가 위암의 수술 전 평가 및 수술 후 재발 감시에 있어서 유용성이 있는지의 여부를 알아보기 위하여 본 연구를 시행하였다. 대상 및 방법: 1995년부터 2000년까지 위암으로 근치적 위절제술을 시행 받은 환자 중 수술 전, 수술 후 2주 그리고 6개월 간격의 추적 관찰 기간 동안 혈청 CEA, CA 19-9, CA 72-4 검사가 시행되었던 환자 255명을 대상으로 후향적 연구를 시행하였다. 종양표지자의 정상 참고치는 CEA의 경우 5 ng/ml, CA 19-9의 경우 36 U/ml, CA 72-4의 경우 4 U/ml로 하였다. 병기는 UICC TNM 병기분류법 제 5판에 준하여 분류하였다. 결과: 각 종양표지자의 수술 전 양성률은 CEA $10.5\%$, CA 19-9 $9.7\%$, CA 72-4 $12.4\%$였고, 세 종양표지자 모두 근치수술 후 혈청치가 감소하였다. CEA가 종양크기와 통계적인 연관성이 없는 것을 제외하고, 세 종양표지자의 수술 전 혈청치는 종양침윤깊이, 종양크기, 림프절 전이, 병기 그리고 재발과 의의있는 연관성이 있었다. 재발 환자에 있어서 종양표지자의 민감도는 CEA $43.3\%$, CA 19-9 $41.8\%$, CA 72-4 $50.0\%$였고, 특이도는 CEA $85.1\%$,CA 19-9 $96.8\%$, CA 72-4 $87.8\%$였다. 결론: CEA,CA19-9, CA 72-4의 수술 전 혈청치는 낮은 양성률 때문에 위암의 초기 진단에 유용성이 낮다. 그렇지만 수술 전 혈청치의 양성률이 종양침윤깊이, 림프절 전이, 종양크기, 병기, 재발과 연관성이 있고, 또한 수술 후 추적 관찰 중에 측정한 세 종양표지자의 혈청치도 비록 민감도는 낮지만 재발과 통계적으로 의의있는 연관성이 있다는 점을 고려해야한다.

Comparison of Urologist Satisfaction for Different Types of Prostate MRI Reports: A Large Sample Investigation

  • Objective: To evaluate urologist satisfaction on structured prostate MRI reports, including report with tumor-node-metastasis (TNM) staging (report B) and with Prostate Imaging Reporting and Data System (PI-RADS) score with/without TNM staging (report C, report with PI-RADS score only [report C-a] and report with PI-RADS score and TNM staging [C-b]) compared with conventional free-text report (report A). Materials and Methods: This was a prospective comparative study. Altogether, 3015 prostate MRI reports including reports A, B, C-a, and C-b were rated by 13 urologists using a 5-point Likert Scale. A questionnaire was used to assess urologist satisfaction based on the following parameters: correctness, practicality, and urologist subjectivity. Kruskal-Wallis H-test followed by Nemenyi test was used to compare urologists' satisfaction parameters for each report type. The rate of urologist-radiologist recalls for each report type was calculated. Results: Reports B and C including its subtypes had higher ratings of satisfaction than report A for overall satisfaction degree, and parameters of correctness, practicality, and subjectivity (p < 0.05). There was a significant difference between report B and C (p < 0.05) in practicality score, but no statistical difference was found in overall satisfaction degree, and correctness and subjectivity scores (p > 0.05). Compared with report C-b (p > 0.05), report B and C-a (p < 0.05) showed a significant difference in overall satisfaction degree and parameters of practicality and subjectivity. In terms of correctness score, neither report C-a nor C-b had a significant difference with report B (p > 0.05). No statistical difference was found between report C-a and C-b in overall satisfaction degree and all three parameters (p > 0.05). The rate of urologist-radiologist recalls for reports A, B, C-a and C-b were 29.1%, 10.8%, 18.1% and 11.2%, respectively. Conclusion: Structured reports, either using TNM or PI-RADS are highly preferred over conventional free-text reports and lead to fewer report-related post-hoc inquiries from urologists.

Retrospective Analysis of Neoadjuvant Chemotherapy for Breast Cancer in Turkish Patients

  • Background: Neoadjuvant systemic chemotherapy is the accepted approach for women with locally advanced breast cancer. Anthracycline- and taxane-based regimens have been extensively studied in clinical trials and consequently are widely used. In this study aimed to research the complete response (pCR) rates in different regimens for neoadjuvant setting and determine associated clinical and biological factors. Methods: This study included 63 patients diagnosed with breast carcinoma among 95 patients that had been treated with neoadjuvant chemotherapy between 2007 and 2010. TNM staging system was used for staging. The histologic response to neoadjuvant chemotherapy was characterized as a pCR when there was no evidence of residual invasive tumor in the breast or axillary lymph nodes. Biologic subclassification using estrogen receptor (ER), progesterone receptor (PR), HER2 were performed. Luminal A was defined as ER+, PR+, HER2-; Luminal B tumor was defined as ER+, PR-, HER2-; ER+, PR-, HER2+; ER-, PR+, HER2-; ER+, PR+, HER2+; HER2 like tumor ER-, PR+, HER2+; and triple negative tumor ER, PR, HER2 negative. Results: Patients median age was 54.14 (min-max: 30-75). Thirty-two patients (50.8%) were premenapousal and 31 (49.2%) were postmenapousal. Staging was performed postoperatively based on the pathology report and appropriated imaging modalities The TNM (tumor, lymph node, metastasis) system was used for clinical and pathological staging. Fifty-seven (90.5%) were invasive ductal carcinomas, 6 (9.5%) were other subtypes. Thirty nine (61.9%) were grade II and 24 (38.1%) were grade III. Seven (11.1%) patients were stage II and 56 (88.9) patients were stage III. The patients were classified for ER, PR receptor and HER2 positivity. Seventeen patients had complete response to chemotherapy. Forty patients (63.5%) were treated with dose dense regimen (cyclophosphamide 600 mg/m2 and doxorubicine 60 mg/m every two weeks than paclitaxel 175 mg/m2 every two weeks with filgrastim support) 40 patients (48%) were treated anthracycline and taxane containing regimens. Thirteen patients (76%) from 17 patients with pCR were treated with the dose dense regimen but without statistical significance (p=0.06). pCR was higher in HER2(-), ER(-), grade III, premenopausal patients. Conclusion: pCR rate was higher in the group that treated with dose dense regimen, which should thus be the selected regimen in neoadjuvant setting. Some other factors can predict pCR in Turkish patients, like grade, menopausal status, triple negativity, percentage of ER positivity, and HER2 expression.

Clinicopathological Profile of Breast Cancer Patients at a Tertiary Care Hospital in Marathwada Region of Westen India

  • Background: Breast cancer is the most prevalent malignancy among women with wide differences in clinical profile from region to region. The present study aimed to describe the profile of breast cancer patients attending a tertiary care hospital in Marathwada region of Western India. Materials and Methods: In this descriptive retrospective study, we reviewed records of pathologically diagnosed patients of breast cancer managed at our center from years 2009 to 2015. Data with respect to demographic status, detailed past, medical, familial and personal history, findings of clinical examination and histological features were obtained. Patients were staged according to the Tumor Node Metastasis (TNM) system. Results: Among 260 cases, mean age of presentation was 52.6, with average age of menarche of 11.3 and menopause of 52.6 years. The majority of patients were from urban regions and were postmenopausal (64.3%). Main clinical features presentation were breast lumps. Most patients were in stage II and had infiltrating duct carcinomas. Conclusions: Most common risk factors for breast cancer observed are increasing age, low parity and obesity. Breast cancer was more prevalent among postmenopausal women presenting in stage II with infiltating duct carcinoma in our region of India.
