Outcomes of Triple-Negative Versus Non-Triple-Negative Breast Cancers Managed with Breast-Conserving Therapy

  • Background: Triple negative breast cancer is associated with aggressive behavior and high risk of local and regional failure. Aggressive surgical intervention is considered suitable. This makes role of breast conserving therapy (BCT) debatable in these patients. The objective of this study was to compare outcome of BCT for triple negative versus non-triple negative breast cancer. Materials and Methods: Medical records of patients who underwent breast conserving therapy from 1999 to 2009 at Shaukat Khanum Cancer Hospital and had complete receptor status information were extracted. Patients were divided into triple negative breast cancer (TNBC) and non-TNBC. Patient characteristics, medical treatment modalities and adverse events were compared. Expected five year locoregional recurrence free, disease free and overall survival was calculated. The Cox proportional hazard model was used to identify independent predictors of outcome. Results: A total of 194 patients with TNBC and 443 with non-TNBC were compared. Significant difference was present for age at presentation (p<0.0001), family history (p=0.005), grade (p<0.0001) and use of hormonal therapy (p<0.0001). The number of locoregional failures, distant failures and mortalities were not significantly different. No significant difference was present in 5 year locoregional recurrence free (96% vs 92%, p=0.3), disease free (75% vs 74%, p=0.7) and overall survival (78% vs 83%, p=0.2). On multivariate analysis, tumor size, nodal involvement and hormonal treatment were independent predictors of negative events. Conclusions: Breast conserving therapy has comparable outcomes for triple negative and non-triple negative breast cancers.

Replica technique과 Triple-scan protocol을 이용한 지르코니아 전부도재관의 변연 및 내면 적합도에 관한 비교 연구 (Marginal and internal fit of all ceramic crown using the replica technique and the triple-scan protocol)

  • 목적: 본 연구는 보철물의 적합도를 계측하는 두 가지의 측정방법을 평가하기 위해 지르코니아로 전부도재관을 제작하고 replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 연구하고자 하였다. 재료 및 방법: 타이타늄의 지대치 모형을 제작하여 20개의 지르코니아 전부도재관을 제작하였다. 제작된 지르코니아 전부도재관 10개는 replica technique을 이용해 복제하고 복제된 시편을 협설, 근원심으로 절단하여 변연 및 내면 적합도를 측정하였고, 다른 10개의 전부도재관은 triple-scan protocol을 이용하여 협설과 근원심으로 단면을 형성하고 변연 및 내면 적합도를 계측하여 통계 분석하였다 (${\alpha}=.05$). 결과: 변연과 내면 적합도는 replica technique $49.86{\pm}29.69{\mu}m$, triple-scan protocol $75.35{\pm}64.73{\mu}m$로 유의한 차이가 있었고 (P < .001), 내면 적합도만을 측정한 결과 각각 $58.38{\pm}31.77{\mu}m$, $64.00{\pm}46.43{\mu}m$로 유의한 차이가 없었다 (P > .343). 결론: Replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 측정한 결과 triple-scan protocol에서의 변연 적합도가 크게 측정되었고, 내면 적합도만 비교해 본 결과 통계적인 유의한 차이가 없었다.

Site-response effects on RC buildings isolated by triple concave friction pendulum bearings

  • The main object of this study is to evaluate the seismic response effects on a reinforced concrete building isolated by triple concave friction pendulum (TCFP) bearings. The site-response effects arise from the difference in the local soil conditions at the support points of the buildings. The local soil conditions are, therefore, considered as soft, medium and firm; separately. The results on the responses of the isolated building are compared with those of the non-isolated. The building model used in the time history analysis, which is a two-dimensional and eight-storey reinforced concrete building with and without the seismic isolation bearings and/or the local soil conditions, is composed of two-dimensional moment resisting frames for superstructure and of plane elements featuring plane-stress for substructure. The TCFP bearings for isolating the building are modelled as of a series arrangement of the three single concave friction pendulum (SCFP) bearings. In order to investigate the efficiency of both the seismic isolation bearings and the site-response effects on the buildings, the time history analyses are elaborately conducted. It is noted that the site-response effects are important for the isolated building constructed on soft, medium or firm type local foundation soil. The results of the analysis demonstrate that the site-response has significant effects on the response values of the structure-seismic isolation-foundation soil system.

주 40시간 근무제의 도입이 근로시간, 임금 및 고용에 미치는 영향 (The Impacts of the 40 Hour Work Week Standard on Actual Working Hours, Wages and Employment)

  • 본 연구는 2004년부터 점진적으로 실시된 주40시간 근무제, 이른바 '주5일 근무제'의 도입이 근로시간, 임금 및 고용에 미친 효과를 "고용형태별근로실태조사" 원자료를 이용하여 추정하였다. 법정 근로시간 단축이 연도별 산업별 및 사업체 규모별로 단계적으로 적용되었던 점에 착안하여 삼중차감 모형을 사용하였다. 희귀분석 결과, 주 40시간 근무제의 도입으로 실제 근로 시간은 약 43분 단축되었고, 시간당 임금은 약 6.6% 상승한 것으로 나타났으나 신규 고용은 약 2.3%포인트 하락시킨 것으로 나타났다.

고추 건조기(乾燥機)의 개발(開發)에 관한 연구(硏究) -시뮬레이션 및 최적화- (Development of Red Pepper Dryer -Simulation and Optimization-)

  • Simulation model was developed to analyze drying process for tray type red pepper dryer and validated by experiments. This model could predict satisfactorily temperatures and moisture contents of red pepper and temperatures of drying air during drying. Optimize algorithm was developed to search control valiables (drying air temperature, air recycle ratio and air flow rate) of red pepper dryer based on a criterion of minimizing energy consumption under the constraint conditions that statisfied carotenoid retension of at least 210mg per 100g dry matter, the moisture content of bottom layer of 15% (d.b) and drying time of less than 35 hours. Step changes in drying air temperature and air recycle ratio were considered in the optimization. In single step in control variables, the difference of the moisture content between top layer and bottom layer was great and more fan power was required. As the drying trays were exchanged when the moisture content of bottom layer reached to 100% (d.b), fifty percent of energy was saved and the difference of moisture content was little. In double step changes in control variables, optimal conditions were found by changing the step when the moisture content of bottom layer reached to 100% (d.b) (about 19.8 hours from starting drying). Optimum air flow rate was $18.1cmm/m^2$. Optimum drying air temperature and air recycle ratio in the first step was $55.8^{\circ}C$ and 0.80, and in the second step $65.6^{\circ}C$ and 0.88, respectively. In triple step changes in control variables, the optimal conditions were found by changing the steps when the moisture content of bottom layer reached to 250% (d.b) and 150% (d.b). Optimal air temperatures were $66.2^{\circ}C$, $58.4^{\circ}C$ and $66.9^{\circ}C$, and optimal air recycle ratios were 0.778, 0.785, 0.862 at each step, respectively. Optimal air flow rate was $18.9cmm/m^2$. The best operating mode was triple step mode considering energy consumption, drying time, fan power, and quality of dried red pepper. When the triple step mode was used to dry the red pepper, the energy consumption was about 16.5%~57.2% less than that of the single step mode and the drying time was 6.6 hours shorter than that of the double step mode.

단일구조 지르코니아 크라운의 소결 후 추가 소성 과정이 변연 및 내면 적합도에 미치는 영향 (Effect of additional firing process after sintering of monolithic zirconia crown on marginal and internal fitness)

  • 목적: 본 연구는 단일구조 지르코니아 크라운의 소결 후 소성 과정이 변연 및 내면 적합도에 미치는 영향에 대하여 3차원으로 평가해 보고자 하였다. 재료 및 방법: 타이타늄 지대치 모형을 제작하여 10개의 단일구조 지르코니아 크라운을 제작하였다. 제작된 단일구조 지르코니아 크라운을 소결 한 상태를 대조군으로, 소결 후 광택을 위해 추가적인 소성단계를 거친 후를 실험군으로 설정하였다. 각 군에서 triple-scan protocol을 이용하여 협설과 근원심으로 단면을 형성하고 변연 및 내면 적합도를 계측하여 통계 분석하였으며, 삼차원 표면 비교를 시행하였다 (${\alpha}=.05$). 결과: 변연과 내면 적합도를 분석한 결과 근심 축벽에서 대조군($32.0{\pm}24.3{\mu}m$)과 실험군 간($17.0{\pm}10.8{\mu}m$)의 통계적 유의한 차이가 있었고 (P < .020), 원심 축벽에서 대조군($60.2{\pm}24.3{\mu}m$)과 실험군($71.8{\pm}21.5{\mu}m$)간의 통계적으로 유의한 차이가 있었다 (P < .01). 나머지 측점지점에서는 통계적으로 유의한 차이가 없었다. 결론: 단일구조 지르코니아 크라운에서의 소결 후 추가적인 소성은 내면의 변형에 큰 영향을 미치지 않았고 임상적으로 허용 가능한 범위에 있었다.

소수성 미니채널 내 움직이는 접촉선을 가진 액체슬러그의 압력 강하에 대한 수치해석 (Numerical study on pressure drop with moving contact lines of dry slug flow in a hydrophobic minichannel)

  • In this study, a single-phase analysis of droplet slug with different contact angles was performed based on the visualization of experimental results. Droplet slug - flowing between gases in a hydrophobic mini channel - moves with a triple contact line without a gas liquid film on the wall. The results show that the rotational flow inside the droplet occurred; this was compared and verified with the results of two-phase analysis. The pressure field shows pressure rise at the front and rear ends. The effective length - the section that satisfies the laminar flow condition - became shorter as the droplet velocity increased. The Choi's correlation for the effective length agrees with this analysis results with a slight difference. This difference is judged as the difference in the contact angle of the slug model.

절삭 및 적층 가공법으로 제작된 3본 고정성 국소의치의 변연 및 내면 적합도에 관한 연구 (Marginal and internal discrepancy of 3-unit fixed dental prostheses fabricated by subtractive and additive manufacturing)

  • 목적: 본 연구의 목적은 절삭 가공법 및 적층 가공법에 의해 제작된 3본 고정성 국소의치의 변연 및 내면 적합도를 비교하는 것이다. 재료 및 방법: 상악 좌측 제2소구치가 상실된 3본 고정성 국소의치 지대치를 제작하고(주모형), 구강 스캐너를 이용해 주모형 스캔 데이터를 얻었다. 3본 고정성 보철물은 다음과 같은 3가지 방법으로 제작하였다: Milled 3-unit fixed dental prostheses (FDP) (MIL군), digital light processing (DLP) 3D printed 3-unit FDP (D3P군), stereolithography apparatus (SLA) 3D printed 3-unit FDP (S3P군). 보철물의 변연 및 내면 적합도와 내면 정밀도 평가하기 위해 각각 triple-scan protocol과 콤비네이션 수식을 사용하여 스캔 데이터를 중첩하였다. 3차원 분석 프로그램(Geomagic control X)을 사용하여 root mean square (RMS) 값과 color difference map 통해 정량 및 정성 분석 시행하였다. Kruskal-Wallis test (α = .05)와 Mann-Whitney U test 및 Bonferroni correction (α = .05/3 = .017)을 이용하여 통계 분석하였다. 결과: S3P군의 변연 적합도는 MIL군과 D3P군에 비해 우수하였으며, MIL군과 D3P군은 비슷한 수준을 보였다. D3P군과 S3P군은 MIL군에 비해 상대적으로 우수한 내면 적합도를 보였으며, D3P군과 S3P군 사이에는 유의한 차이가 없었다. 한편, MIL군, S3P군, 그리고 D3P군 순으로 우수한 정밀도를 보였다. 결론: 본 연구의 한계 내에서 적층 가공법으로 제작된 3본 고정성 보철물은 절삭 가공법으로 제작된 3본 고정성 보철물에 비해 우수한 변연 및 내면 적합도를 보인 반면 정밀도는 떨어지는 것으로 나타났다.

효과적인 산전관리를 위한 고객관계관리(CRM)의 도입 (The Application of Customer Relationship Management for the Effective Prenatal Care)

  • The prenatal care is the preventive medical service to help the pregnant mother deliver the healthy baby. It's regular examines give some chances to check-up the healthy conditions. This thesis concentrates on the CRM system to support an effective prenatal care system and prove the effectiveness of it. As CRM is the adapted management related to the customer's own information, it is important to develop the CRM model classified by the patients characteristics. A general hospital in Busan operated the CRM system to carry out the effective prenatal care and there is an analysis to ensure the effectiveness of CRM system for the pregnant women in our maternity ward. The results can be summarized as follows: 1) According to the comparisons with the CRM system, we can conclude the system is desirable. (1) Maternal Age : In the age distribution, the prenatal visit frequency, triple marker freqency, oral GTT and targeted ultrasonography in the experimental group in 30 to 34 years old is higher on the whole. For over 35 years old group, the higher frequency comes out in the oral GTT and targeted ultrasonography and for 25 to 29 years old group the different figure shows just in the targeted ultrasonography. (2) Area of residence: There is a clear difference in all the items in Busan and near area but no sign of difference in prenatal visits and oral GTT in other residencial area. Especially in the targeted ultrasonography the higher figure shows in the experimental group located in the both areas. The targeted ultrasonography is known as the specific examination which should be examined by the specialists, on the contrary the other examinations can be operated in the small clinic. So the public information and seminars related with ultrasonography increases the check-up frequency. The clinic requests some ultrasonographical examinations to the specialists in general hospital. (3) Parity: The clear difference shows that the CRM system causes the prenatal visit frequency to become higher in experimental group. The figure is 9.7 times and 8.6 times each. This is opposite that the past study said multiparity reduced the average prenatal visits. But the result of CRM is considered as the method to help the multiparity understand the importance of the prenatal care. (4) Obstetrical history: In the experimental group of the spontaneous delivery group, the figure is higher in the prenatal visit frequency, triple marker, oral GTT and targeted ultrasonography but the Caesarean section delivery group has higher figure in targeted ultrasonography. (5) In the first check-up, the rate of targeted ultrasonography in under 16 week pregnancy, in the 16 week pregnancy to 32 week pregnancy and the over 32 week pregnancy in the experimental group is upper than the compared one. For the oral GTT, there is a difference in under 16 week pregnancy but no difference in prenatal visits and triple marker. 2) The analysis of characteristics of prenatal care through the decision tree resulted in the fact that the most important variable is the residential area. After the delivery frequency is following, the obstetrical history and maternal age are in order. It is the same result in the triple marker and oral GTT. Consequently it is the same order of important variables in CRM system. The effectiveness of CRM system is proved in this study. The CRM system is a marketing method to control and lead the customers through the segmentation of customer data. It increases the new customer aquisition, maintenance of loyal customers, augmentation of customers value, activation of potential customers and creation of life time customers. So eventually it can enlarge the customers value. The medical institution should make efforts to establish the data base enforced by the customer's information on the underlying ordinary data system to carry out the CRM system effectively. In addition, it should develop the a variety of marketing strategy in order to set up one to one marketing satisfying the needs of individual patients.

하이브리드 레벨 셋을 이용한 이미지 분할 (Image segmentation Using Hybrid Level Set)

  • 기존의 레벨셋을 이용한 이미지 분할 방법은 화소값의 기울기를 이용하기 때문에 지역적 형태에 좌우되는 문제점을 지니고 있다. 본 논문에서는 평활한 구동력을 위하여 레벨 셋 함수와 새로운 보상 평활화 함수를 결합시키는 하이브리드 방법을 이용한 방법이 소개된다. 대부분의 경우에 3 교점을 가지고 있지 않다는 가정하에 보상함수를 얻는 방법을 대안으로 고려하였다. 보상함수의 주요 역할은 원보상 함수와 평균 보상함수의 차가 새로운 레벨셋 함수의 합리적인 구동력으로 소개될 수 있다. 본 논문에서 제안한 하이브리드 방법은 기존 레벨셋을 이용한 방법의 단점을 최소화시키는 방법이다.
