  • The incidence of verotoxin-producing Escherichia coli(VTEC) was surveyed in domestic foods including hamburger, raw meats and vegetables from 1997 to 1999. The molecular biological characteristics of the isolates were analyzed. Three VTEC strain were isolated from 1,700 samples. Serotypes of those isolates were 0157 : H7, 026 H4, and 056 : Hl2, respectively. Serotype O26 : H4 produced VT I and VT II, and 055 Hl2 isolate produced VT I, however the 60 MDa plasmid DNA and eae gene were not found from both strains. One 0157 : H7 isolate produced VT II and harbour 60 MDa plasmid DNA, however eae gene was not found in the strain. Although they produced VT, it seemed that the virulence of two strains were relatively weak because of the lack of the eae gene. In addition, the serotype O157 : H7 isolate resistant to ampicillin and streptomycin, while isolates of serotype O26 : H4 and O55 : Hl2 were multi-resistant to antibiotics including ampicillin, carbenicillin , cephalothin, trimethoprim/sulfamethoxazole and tetracycline. Supernatants of cultures of all three isolates were showed cytotoxic effect to vero and HeLa cell

  • The incidence of Yersinia enterocolitica in raw meat was determined over 10 month period. Y. enterocolitica was isolated from 8.5% of beef, 17.0% of pork and 25.6% of chicken. Overall prevalence of Y. enterocolitica in raw meat was 17.5%. Seasonal difference was observed in isolation rate in which pork and chicken samples collected in the second half of the year twice was more than that of the first half of the year. Serotypes of Y. enterocolitica isolates were O:5 (17.3%), O:8 (8.6%), O:3 (6.2%), O:1,2 (1.2%), and others. The antibiotics susceptibility tests showed Y. enterocolitica isolates were resistant to carbenicillin, ampicillin, erythromycin, penicillin and bacitracin. In contrast it showed sensitivity to polymyxin B, kanamycin, ciprofloxacin, gentamicin, trimethoprim/sulfamethoxazole, nalidixic acid, and tetracycline. PCR with specific primers derived from ail gene of Y. enterocolitica was applied to detect and confirm pathogenic Y. enterocolitica. About 10% of the isolated Y. enterocolitica proved to be pathogenic and most of them were found in pork. However, proper cooking and meat process can kill and remove all Y. enterocolitica in meat, because the organism is very sensitive to heat.

  • This study was conducted to evaluate the removal efficiencies of organic matter and pharmaceuticals and to identify the removal mechanism of pharmaceuticals using sand obtained from Hwangryong River in Jangsung. Batch and column studies were used to simulate managed aquifer recharge (MAR) systems. All experiments were performed using field effluent containing pharmaceuticals from Damyang Wastewater Treatment Plant as an influent. Based on the removal results of organic matter and pharmaceuticals from the batch and column experiments, soil organic matter (SOM) and microbial activity were found to effectively remove target contaminants. The removal of organic matter was found to increase under biotic conditions. Neutral and cation pharmaceuticals (iopromide, estrone, and trimethoprim) exhibited removal efficiencies higher than 70% from natural sand and baked sand media in batch and column studies. Carbamazepine persisted in the sand batch and column studies. Anion pharmaceuticals (ketoprofen, ibuprofen, and diclofenac) can be removed under conditions featuring high SOM and adenosine triphosphate (ATP) concentrations in the sand surface. Based on the experimental Batch and column results, biodegradation and sorption were found to be important mechanisms for the removal of pharmaceuticals within the simulated MAR systems.

  • Recently, antimicrobial resistance of pathogenic bacteria has been increasing due to excessive use of antimicrobial agents in both humans and livestock. PCR amplification and nucleotide sequence analyses were conducted to investigate16S ribosomal RNA methyltransferase (RMTase) genes and integrons in P. mirabilis strains isolated from clinical specimens and chickens in an area of the Chungcheong providence. In addition, clonality analysis of P. mirabilis strains was performed using a repetitive extragenic palindromic sequence-based PCR (REP-PCR) method. Of the total 38 P. mirabilis isolates, 7 (18.4%) strains were isolated from clinical specimens contained in the RMTase genes and showed resistance to amikacin, tobramycin, and gentamicin. A total of 23 (60.5%) isolates carried class 1 integrons, but no isolates in our study harbored class 2 and class 3 integrons. Class 1 integrons detected in our study harbored genes encoding resistance to aminoglycosides (aadA2, aadA5, aadA7, and aacCA5), ${\beta}$-lactams ($bla_{PSE}$), erythromycin (ereA), lincosamides (linF), and trimethoprim (dfrA12, dfrA17, and dfrA32). We confirmed that the RMTase genes had spread among only the P. mirabilis isolates from clinical specimens, but class 1 integrons had widely disseminated among P. mirabilis isolates from clinical specimens and chickens. In addition, identical REP-PCR banding patterns were evidenced in only P. mirabilis isolates from chickens. Our results suggest the horizontal spreading of P. mirabilis isolates in the chicken farm. To prevent further spreading of antimicrobial resistant genes among P. mirabilis isolates, monitoring and clinical policing will be required.

  • This study evaluated the abundance of fecal coliforms in oysters Crassostrea gigas, sea squirts Halocynthia roretzi and sea cucumbers Apostichopus japonicus in fisheries along the coast of Korea in 2014, and investigated the prevalence of antimicrobial resistance in Escherichia coli isolated from these fishery products. The ranges of fecal coliforms found in oysters, sea squirts and sea cucumbers were <18-20, <18-330 and <18-3,300 MPN (most probable number)/100 g, respectively. Sea squirts contained the greatest range of E. coli (<20-140 MPN/100 g), followed by sea cucumbers (<20-130 MPN/100 g) and oysters (<20-20 MPN/100 g). A total of 26 strains of E. coli were isolated from 34 sea squirt, 25 sea cucumber and 13 oyster samples. Strains thus isolated were tested for their susceptibility to 22 antimicrobial agents used in Korea for medical or veterinary therapy. E. coli isolates showed the greatest resistance to ampicillin (84.6%), followed by trimethoprim (34.6%), nalidixic acid (34.6%), tetracycline (30.8%), pipemidic acid (26.9%), streptomycin (23.1%), chloramphenicol (23.1%), trimethoprim/sulfamethoxazole (23.1%), and gentamicin (15.4%). Resistance to at least one antimicrobial agent was present in 88.5% of E. coli isolates. Of the 26 isolated, six strains (23.1%) were resistant to multiple antimicrobial agents.

  • This study was suggested as fundamental data to control medical materials remained in Nakdong range gauge. The level of Iopromide detected in Nakdong mainstream was $0.0015{\sim}0.37{\mu}g/L$, Mefenamic acid $0.0087{\sim}0.056{\mu}g/L$, Diclofenac $N.D.{\sim}0.01{\mu}g/L$, Atenolol $N.D.{\sim}0.024{\mu}g/L$, Propranolol $N.D.{\sim}0.0038{\mu}g/L$, Lincomycin $0.0005{\sim}0.038{\mu}g/L$, and Trimethoprim $N.D.{\sim}0.0083{\mu}g/L$. At sewage disposal plant in the region, most of them were detected high levels of density. Especially, the level of Iopromide was found the highest up to $5.38{\mu}g/L$. At livestock wasted water disposal plant, the level of lincomycin was detected the highest figure of $477{\mu}g/L$. As a result, medical materials from Nakdong River mainstream got increasing the concentration due to inflow from sewage disposal plant in Gumi and River Geumho in Daegu, which affects residential and industrial areas significantly. Therefore, to control medical materials remained in Nakdong River efficiently, Geumho River and sewage disposal plants shall be continuously monitored and managed, which is recommendable.

  • The urinary tract infection (UTI) is one of the most important infections in hospital. The overuse and misuse of antimicrobial agents and the resulting emergence of resistant microorganisms have made choices regarding antimicrobial therapy more difficult. This study examined the changes in the antibiotic susceptibility to the causative organisms of urinary tract infections to provide useful information on the choice of adequate drugs in the treatment of urinary tract infections. The medical records of 2,707 patients with more than $10^5/ml$ microorganism in urine culture between January 2010 and December 2015 were reviewed retrospectively. The most common pathogenic organism was E. coli (28.1%). In the case of E.coli, there were no differences in frequency from 2010 to 2015 in men, but since 2014, the frequency decreased gradually since 2014 in women. For E. coli, the resistance rates to antibiotics were 72.2% in ampicillin, 44.9% in trimethoprim/sulfamethoxazole (TMP/SMX), and 41.3% in ciprofloxacin, but the 2nd, 3rd, and 4th cephalosporin (5%) had low antibiotic resistance rates. The pathogens of urinary tract infection are becoming diverse and their frequencies are also changing over time. These results suggest that the recommended drugs for UTI should be selected more carefully for in-patients and out-patients.

  • Purpose: Staphylococcus aureus is a major cause of skin and soft tissue infections (SSTIs). This study aimed to determine the temporal trends in antibiotic susceptibility of S. aureus in SSTI patients aged <19 years. Methods: This retrospective observational study was conducted in pediatric patients with SSTI caused by community-associated S. aureus. Microbiologic and demographic data were collected, and the trends of antibiotic susceptibility results were evaluated. Results: From January 2010 to December 2018, a total of 807 S. aureus isolates were included. An overall increase in susceptibility of isolates to oxacillin was noted (P<0.001), with 75.0% of isolates being oxacillin-susceptible in 2018. S. aureus remained highly susceptible to trimethoprim/sulfamethoxazole and tetracycline, with 97.6% and 95.2% isolate susceptibility in 2018, respectively. Isolates from younger children aged 1 to 5 years had a significantly lower rate of susceptibility to oxacillin than older children aged 6 to 18 years (53.4% vs. 75.0%, P<0.001). Conclusions: The proportion of methicillin-resistant S. aureus isolates appears to decrease in pediatric patients with community-associated SSTI caused by S. aureus. Clinicians should be aware of regional susceptibility patterns when choosing empirical regimens.

  • Vibrio parahaemolyticus is one of the major agents responsible for food poisoning during summer in Korea, which is transmitted via seawater or seafoods. Recently, distribution of the bacteria in the marine environment has been increased due to global warming. Great concern also has been raised regarding public hygiene as well as marine culture by the emergence of pathogens with antibiotic resistance. Therefore, distribution of V. parahaemolyticus and antibiotic resistance of the isolates were monitored in 7 coastal areas of Kyonggi Province and Incheon by sampling seawater, fishes and clams monthly. V. parahaemolyticus was detected from 47.7% of 966 samples (seawater 61.9%, seafoods 41.8%) analyzed using $CHROMagar^{TM}$ and TCBS agar plates as well as multiplex PCR. Among 13 antibiotics tested, resistance to vancomycin and ampicillin was observed in 97.3% and 87.3% of the isolates, respectively, and the ratios of them resistant to cephalothin (48.8%) and rifampin (46.1%) were also high. The isolates were most highly sensitive to chloramphenicol (91.7%) and trimethoprim-sulfamethoxazole (91.8%). The ratio of sensitivity for other antibiotics was also high in the descending order of gentamycin (82.3%), tobramycin (74.8%), nalidixic acid (71.6%), tetracyclin (69.4%), cefotaxime (63.0%). About 69% of the isolates showed multiple drug resistance toward 3 antibiotics including vancomycin and ampicillin. Two of them exhibited resistance for 11 antibiotics used in this study. Plasmid profile analysis of the isolates with antibiotic resistance revealed that 55.1% of them retained plasmids of 24 different types. However, no clear inter-relationship between the resistance and the plasmid profile has been observed.
