Failure Analysis of SCM435 Bolt for Fixing Automotive Air Brake Spring (자동차 에어 브레이크 스프링 고정용 SCM435 볼트의 파손 해석)

  • This study analyzed the causes of failure of SCM435 bolts that fix the springs of automobile air brakes that have been fractured during use. The cause of failure was analyzed using SEM, EDS, metallogical microscope and Vickers hardness tester. In the fracture, the ratchet mark began at the outer boundary of crack origin, and the grains at crack origin were found to have clear intergranular corrosion. One SCM435 bolt was subjected to a stress of 398 MPa, it's a stress of about 80% of the fatigue limit. As a result of such a large applied stress, cracks occurred at the corrosion origin and were fractured. In order to prevent the SCM435 bolt from fracture, it is necessary to use the correct composition, the accuracy of heat treatment, preventing damage by external impact, preventing corrosion of the damage part by moisture, and introduction a compressive residual stress by peening.

The surgical treatment of chronic constrictive pericarditis: a report of 70 cases (만성 교약성 심낭염의 외과적 치료70례 수술 보고)

  • Between 1958 and 1982, 70 patients have undergone pericardiectomy for constrictive pericarditis at the Thoracic Department of Seoul National University Hosp. 58 males and 12 females, with an average age of 27 years [ranging 3 to 60 years], of which 55% were between 10 and 30 years old, were treated. Eight patients died, of whom 4 were in the immediate postoperative period, less that 24 hours after operation. The cause of death was myocardial failure in 3 patients and hypotension during operation in one patient. The remaining four deaths occurred between the fifth and eighteenth postoperative day, and the causes of death varied: bilateral phrenic nerves injury, congestive heart failure, dissemination of tuberculosis, and cardiac arrest. Two patients suffered from congestive heart failure pre-and postoperatively due to the associated valvular heart disease. There were 8 wound infections on which resulted in perichondritis of costal cartilages requiring segmental resection 2 months later. There was one postoperative bleeding requiring immediate reopening for bleeding control. Tuberculosis was confirmed as the cause of constrictive carditis in 27 patients [39%]. Acute pyogenic pericarditis was precursor in 8 patients [11%]. In 2 patients [2.9%], the constrictive pericarditis developed following OHS. Both suffered from congestive heart failure postoperatively due to the residual valvular heart disease. In the others, the cause of the constrictive pericarditis was considered idiopathic or non-specific inflammation.

  • An endodontically treated tooth is likely to be brittle than a vital tooth. Internal structure of the tooth has been weakened due to a significant removal of dentin by coronal access, canal preparation. There are many controversies concerning with various methods of reinforcing an intact anterior tooth that has endodontic treatment. In this experiment, 128 extracted maxillary anterior teeth were endodontically treated, and prepared with 4 methods of restorations; Composite resin filling with zinc phosphate cement, composite resin filling without zinc phosphate cement, composite resin filling with post, and metal crown with post. An Instron testing machine was used to measure the fracture loads of the specimens. The means of the failure loads for the 4 groups were compared by F-test statistically and the failure modes were observed. The results were as follows; 1. There were no statistically significant difference between the failure loads of the four methods of restoration. 2. Teeth without post were fractured in a horizontal or oblique plane through upper or middle third of the root. 3. In the posted teeth, fractures were occurred around the post. 4. In the metal crowned teeth with post, the fracture were occurred around the post or coronal area.

A Study for Bond Strengths of Acrylic and Silicone Based Soft Lining Materials (애크릴릭 및 실리콘 계열 연성 의치상 이장재의 결합력에 관한 연구)

  • One of the methods to improve the softness and comfortness of denture base is the use of soft denture liners. In this study, specimens were made by 2 kinds of acrylic based soft lining materials and 2 kinds of silicone based soft lining materials, and bonded to acrylic resin(Lucitone $199^{(R)}$). Then they were tested the differences of tensile bond strengths according to the materials, thickness, surface treatment and failure mode. 1. Tensile bond strength according to soft lining materials was increased in order of Coe-$soft^{(R)}$, $Mollosil^{(R)}$, $Trusoft^{(R)}$, Ufi-Gel $C^{(R)}$. The differences between groups were statistically significant at level of 0.05. 2. Tensile bond strength according to thickness of soft lining materials was increased in order of 3mm, 2mm, 1mm. The differences between groups were not statistically significant. 3. Tensile bond strength of treated surface showed higher bond strength than nontreated surface. The difference between groups was not statistically significant. 4. The failure mode of Coe-$soft^{(R)}$, $Trusoft^{(R)}$, $Mollosil^{(R)}$ were mainly cohesive failure, and that of Ufi-Gel $C^{(R)}$ were mainly adhesive failure.

  • Purpose: To report the effect of Korean medical treatments on 1 patient with secondary amenorrhea after ovarian surgery for bilateral endometriomas diagnosed as premature ovarian failure. Methods: The patient who had secondary amenorrhea was treated with Korean medical treatments such as herbal medication, herbal acupuncture and moxibustion therapy, and so forth. Results: After the Korean medical treatments, the patient could have her periods again. And high FSH level was decreased to normal. Conclusions: The case report shows that Korean medical treatment can be an effective option for treating secondary amenorrhea after ovarian surgery for bilateral endometriomas diagnosed as premature ovarian failure.

FAILURE OF ODONTOGENESIS AFTER CHEMO-RADIATION THERAPY FOR RHABDOMYOSARCOMA (횡문근육종의 항암제-방사선치료 후 치아발육장애)

  • This report details a case of 8-year-old girl showing failure of odontogenesis after chemo-radiation therapy for the rhabdomyosarcoma at the age of 4. The observed results were as follows : 1. Past history revealed that she had received for a total radiation dose of 4430cGy, 29 fractions in 6 weeks and chemotherapy with vincristine, actinomycin D and cytoxan, followed as maintenance phase for 2 years. 2. The patient was symptom -free and appointed for the treatment of multiple dental caries. 3. Oral examination showed hypoplastic enamel on whole erupted permanent teeth and showed retarded eruption. 4. Conventional radiograms showed failure of root development including abrupt cessation of root formation and root agenesis, and microdontia, missing teeth, irregular enamel, dislocation of the impacted teeth. Additional finding showed good healing bone pattern on the left mandibular ramus and angle area. 5. Cephalometric analysis revealed failure of bite raising due to incomplete eruption of all the first molars and made it possible to suspect entrapped mandibular growth and then Class II tendency growth. 6. There was correlation between the time of chemo-radiation therapy and the damage of the teeth.

miR-23a Regulates Cardiomyocyte Apoptosis by Targeting Manganese Superoxide Dismutase

  • Cardiomyocyte apoptosis is initiated by various cellular insults and accumulated cardiomyocyte apoptosis leads to the pathogenesis of heart failure. Excessive reactive oxygen species (ROS) provoke apoptotic cascades. Manganese superoxide dismutase (MnSOD) is an important antioxidant enzyme that converts cellular ROS into harmless products. In this study, we demonstrate that MnSOD is down-regulated upon hydrogen peroxide treatment or ischemia/reperfusion (I/R) injury. Enhanced expression of MnSOD attenuates cardiomyocyte apoptosis and myocardial infarction induced by I/R injury. Further, we show that miR-23a directly regulates the expression of MnSOD. miR-23a regulates cardiomyocyte apoptosis by suppressing the expression of MnSOD. Our study reveals a novel model regulating cardiomyocyte apoptosis which is composed of miR-23a and MnSOD. Our study provides a new method to tackling apoptosis related cardiac diseases.

The Effect of Plantaginis Semen Herbal Acupuncture on Rat by Glycerol-Induced Acute Renal Failure (차전자약침(車前子藥鍼)이 Glycerol로 유발(誘發)된 급성신불전(急性腎不全) 백서(白鼠)에 미치는 영향(影響))

  • This study was undertaken to determine if Plantaginis Semen Herbal Acupuncture(PSA) has a protective effect against glycerol-induced acute renal failure in rats. Rats were dehydrated for 24hr and then injected with 8 ml/kg of $50\%$ glycerol, one-half of dose in each hindlimb muscle. In experiments for PSA effect, rats received 0.1 ml of PSA extraction in both sides of corresponding Shenso($BL_{23}$) of human body for 3 days after injection of glycerol. The experimental group were di vided into the Normal group, the Control group, the PSA group. Glycerol injection decreased glomerular filtration rate and increased urine volume, serum creatinine, BUN level and fractional excretion of glucose, $Na^+$, $K^+$ and $CI^-$. These result show that glycerol injection result in acute renal failure. PSA significantly increased glomerular filtration rate and significantly decreased serum creatinine, BUN level and fractional excretion of glucose, $Na^+$ and $CI^-$ as compared Control group. This suggests that PSA could be used in prevention and treatment of acute renalfailure. However, the precise mechanisms of PSA protection remain to be determined.

Alpers-Huttenlocher Syndrome First Presented with Hepatic Failure: Can Liver Transplantation Be Considered as Treatment Option?

  • Mitochondria play essential role in eukaryotic cells including in the oxidative phosphorylation and generation of adenosine triphosphate via the electron-transport chain. Therefore, defects in mitochondrial DNA (mtDNA) can result in mitochondrial dysfunction which leads to various mitochondrial disorders that may present with various neurologic and non-neurologic manifestations. Mutations in the nuclear gene polymerase gamma (POLG) are associated with mtDNA depletions, and Alpers-Huttenlocher syndrome is one of the most severe manifestations of POLG mutation characterized by the clinical triad of intractable seizures, psychomotor regression, and liver failure. The hepatic manifestation usually occurs late in the disease's course, but in some references, hepatitis was reportedly the first manifestation. Liver transplantation was considered contraindicated in Alpers-Huttenlocher syndrome due to its poor prognosis. We acknowledged a patient with the first manifestation of the disease being hepatic failure who eventually underwent liver transplantation, and whose neurological outcome improved after cocktail therapy.

Sudden Death and Ventricular Arrhythmias in Heart Failure With Preserved Ejection Fraction

  • Heart failure with preserved ejection fraction (HFpEF) accounts for approximately half of all heart failure (HF) cases. The prevalence of HFpEF is increasing due to an aging population with hypertension, diabetes mellitus, and obesity. HFpEF remains a challenging clinical entity due to a lack of effective treatment options. Traditional HF medications have not been shown to reduce mortality of patients with HFpEF, and an implantable cardioverter-defibrillator is not indicated due to normal ejection fraction. Sudden death is the most common mode of death in patients with HFpEF; however, the underlying mechanisms of sudden death are not fully elucidated. Although ventricular arrhythmias are responsible for the majority of sudden deaths in general, their contribution to sudden deaths in HFpEF patients is likely less significant. The mechanisms of ventricular arrhythmias in HFpEF are 1) reduced conduction velocity due to ventricular hypertrophy, 2) delayed repolarization due to potassium current down-regulation, 3) calcium leakage due to altered excitation-contraction coupling, and 4) increased ventricular fibrosis caused by systemic inflammation. Hypertension and subsequent ventricular hypertrophy reduce the conduction velocity in HFpEF hearts via heterogeneous distribution of connexin 43. Delayed repolarization caused by potassium current down-regulation in HFpEF hearts provides a window for early afterdepolarization to trigger ventricular arrhythmias. Altered excitation-contraction coupling in HFpEF can cause calcium to leak and trigger delayed afterdepolarization. Increased systemic inflammation and subsequent ventricular fibrosis provide substrates for re-entry. Further research is warranted to investigate the detailed mechanisms of ventricular arrhythmias in HFpEF.
