Development of a Transcutaneous Optical Information Transmission System for Total Artificial Heart Using Near Infrared Laser

  • In the total artificial heart(TAH), a transcutaneous information transmission system(TITS) is vely important to monitor the TAH status and detect the device failure, and repair the possible problems. First of all, the communication channel(skin) and method were simulated in terms of transmittance, scattering, reflection and absorption, then the system was designed with size reduction including low power consumption and reliability compared to the previous one. The informations are transmitted through the skin(approximately 1cm in depth) by frequency modulated near infrared(NIR) pulses using 780nm laser diodes as transmitters and photodiode as receiver with high speed and high spectral sensitivity. The logic high and low frequencies are 3MHz, 1MHz respectively. The system is a bidirectional data link for more than 38.4Kbps data rate, full-duplex with a bit error rate of less than $10^{-5}$.

  • DTET(Dual Transcutaneous Energy Transmission System) is the stable power transferring unit for TAH(Total Artificial Heart) which uses more power than any other artificial organ. It has better efficiency and safety than an ordinary single TET. By reducing the load, it can reduce the change of supplying current for each single TET and it causes the lower change of efficiency. The increment of magnetic flux in coils enables delivering power through thick skin. It can enable internal batteries remove when will be used in a failure of TET, for a lithium ion battery has heavier weight than TET. A DTET should be studied the effect between each coils that generate magnetic flux. It should be compared the advantage with the weakness when it is used.

  • An infrared remote control-type transcutaneous control device using a $\mu$-processor is design for the totally implantable middle ear system. An infrared light transmission model for the tissue of skin was introduced and then a radiant intensity and the required current of the infrared light emitting diode(IR LED) driving circuit at transmission part were calculated for the external control device. And the transmission part generates IR signal by the system's own data protocol which prevents interferences from other infrared remote controls of the household appliances. The control part of the implanted device was designed to analyze functions of the received infrared(IR) signal that indicate the power ON/OFF and volume UP/DOWN. After the system is implemented, a data transmission experiments using 4 mm thickness of porcine skin were carried out. From the experiment, it was verified that the infrared control signal was transmitted to receiving module of the implemented system without any error.

  • A dc-dc converter which transfers l2-48W of power(regulated output voltage 24V) across a large, variable air gap(1-2cm) has been designed. This converter employs double resonance to compensate the large leakage inductance of the primary and secondary of the transcutaneous transformer. To maximize the effect of resonance, a constant frequency, duty cycle control method is used. The duty cycle control presents advantages over a frequency control in terms of increase in dc voltage gain and reduction of the primary current. The analysis and design procedure for the proposed scheme is presented and the performance result is simulated and verified experimentally.

  • A transcutaneoos information transmission system using transmitted infrared light for an implantable total artificial heart(TAH) an developed and the performance of the system was tested. In an in vitro test, transmittance of the developed system was measured through various thicknesses of pig shins. In an in vivo test, a loopback test using a personal computer was performed to determine the error rate and an experiment using a mock system was taken. In this experiment, within acceptable lateral and vertical displacements, the error rate of at least 3.8E-6 was guaranteed. Monitoring signals from the internal controller(Intel 87C196) of the mock system were successfully transmitted to the external controller(IBM PC) and the operating commands from the external controller were successfully conducted by the mock system. Communication was done in half duplex mode according to RS-232 protocol at the speed of 4800 bps.

  • It is said that the desirable bio-signal measurement and stimulation system should be an implantable type if the several problems such as biocompatibility, electrical safety, and so on are overcome. In addition to the biocompatibility issue, a robust RF communication and a stable electrical power source for the implantable bio-signal measurement and stimulation system are very important matters. In this paper, a wireless telemetry system which adopts the FCC's approved MICS (medical implant communication service) protocol and a wireless power transmission has been proposed. The proposed system composed of a base station (BS) and an implantable medical device (IMD) has the advantages that the interference with other RF devices can be reduced by the use of the specially assigned MICS frequency band of 402.MHz to 405 MHz. Also, the proposed system includes various functions of a multi-channel bio-signal acquisition and an electric stimulation. Since the electrical power for the IMD can be provided by the inductive link between PCB patterned coils, the IMD needs no battery so that the IMD can be smaller size and much less dangerous than the active type IMD which includes the internal battery. Finally, the validity as a wireless telemetry system has been demonstrated through the experiments by using the implemented BS and IMD.

  • In systems in which inductive coupling between a pancake-shaped coil on the surface of the body and a similar coil within the body is utilized for the transfort of electromagnetic energy, the minimization of temperature rise in the tissue is intimately related to the achievement of minimum losses in the region of the implanted coil. The new class of amplifiers, named "class E", for inverter is defined and is illustrated by a detailed description and a set of desist equations for one simple member of the class. For TET circuit the authors measured 65 to 76 percent efficiency at 1985kHz at 30 to 50 W output from IRF250 MOSFET transistor.

Development of A Biotelemetry System for A Totally Implantable Artificial Heart (완전이식 인공심장용 무선정보 전송장치의 개발)

  • In this paper, a new biotelemetry system for a transcutaneous data communication between an implanted artificial heart with a control system and an external human-interfaced management system has been developed. A radio telemetry using radio frequency is a commonly used method in the conventional telemetry systems. But, it is not suitable for the medical applications because of not only an interference due to a radio broadcasting but also a harmfulness to the human body. In this paper, therefore, a new biotelemetry system applied to an artificial heart has been developed with the results of the recent research for an optical telemetry system based on the infrared light transmission with good skin permeability. The performance of the biotelemetry system developed has been assessed through mock circulatory experiments, and the clinical applicability has been also confirmed with the successful results in the animal experiments.
