Surgical Treatment of Killian-Jamieson Diverticulum

  • Killian-Jamieson diverticulum is a rare diverticular disease. This disease differs from Zenker's diverticulum in its location and mechanism. Various treatment modality have been attempted, but traditional surgical treatment has been recommended for a symptomatic Killian-Jamieson diverticulum due to the concern of possible nerve injury. We performed surgical treatment by cervical incision. We report here on a case of Killian-Jamieson diverticulum and we briefly review the relevant literature.

  • Objectives : The purpose of this study is to investigate oriental medical treatment of pediatric bronchial asthma. Method : Chinese medical journals which is about pediatric bronchial asthma were analyzed for a reference. Result : Oriental medical treatment can be classified into three ways; internal treatment, external treatment, and combination treatment which means combining with Chinese and Western ways of treatments. Conclusion : Oriental medical treatment of pediatric bronchial asthma is more effective therapy than Western's.

한약 복용이 간기능에 미치는 영향 : 국내 문헌에 대한 체계적 고찰 (Korean Herbal Medicine on Liver Function : A Systematic Review in Korean Literature)

  • Background : The safety of Korean herbal Medicine (KHM: prescribed herbal medicine by doctors of traditional Korean medicine) is an important issue in Korea. Although both fields. western medicine and traditional Korean medicine. have been studied on the safety of herbs and KHM, their results were not concordance with each other. Objectives : This study aims to review the influence of KHM on liver function in Korean literature systematically. Additionally, we tried to estimate the change of data of liver function test (LFT) and the incidence of liver injury (LI) after the use of KHM. Methods : Systematic literature searches were performed on 4 major databases of Korea from their inception to May 2008. Screening and selection of the studies and the extraction of data were performed independently by two authors. There were no restrictions on the types of publication including grey literatures. Results : Forty studies were included. Only sixteen studies were performed prospectively and fifteen studies collected data from outpatients. Only 8 studies reported the occurrence of LI after the use of KHM. Nineteen cases of LI showed no or mild symptoms and the elevation of LFT was not high. All of LI patients used conventional western drug and KHM concomitantly. and causality of LI was not assessed properly. The incidence of LI related to the use of KHM was estimated as 0.59%-0.76% from all data of these studies. The conflicting results were shown on the change of alanine aminotransferase (ALT) and total bilirubin (TB) after the use of KHM. Conclusions : KHM might be a minor cause of LI in Korea. However the results are not strongly supported as enough to make the safety issue clear because of the limitations of original studies. More rigorous studies are required for answering the safety issue of KHM with the cooperative investigation of both fields of Korean traditional medicine and western medicine.

조선 왕실의료문헌과 문집에 나타난 우각구법(牛角灸法) 연구 (Study on Horn-shaped Moxa Treatment in the Annals and Anthologies of Joseon-Korea)

  • Objectives: Moxa treatment has outstanding effects that are frequently facilitated in a clinical setting, and it is also known to have positive effects for immunity and relief of pain through various studies. Currently, moxa treatment has become diversified in many ways. One of the moxa treatment techniques that was frequently used during the Joseon Dynasty was Woogakgubeop (牛角灸法, horn-shaped moxa treatment). This articles aims to disclose the fact that the Woogakgubeop is the technique applicable to the origin of the Large moxa treatments of today. Also, the writings of Woogakgubeop recorded in the royal medical documentations, such as the Chronicles of the Royal Families of the Joseon Dynasty, the Seungjeonwonilgi (The Daily Records of Royal Secretariat) and others, are analyzed and the characteristics of the Woogakgubeop are cleared through it. Methods: From the royal medical documents, the Chronicles of the Royal Families of the Joseon Dynasty and the Seungjeonwonilgi, articles related Woogakgubeop were extracted and analyzed. Records of each document were facilitated for the DB search. In addition to these two documents, the records of written literature were surveyed as well. The written literature are appropriate to express the experiences of intellectuals at the time. Results and Conclusions: 1. Considering that Woogakgubeop is a single therapy, there exists relatively many records in addition to the specialized medical documents, and through them, it is known that Woogakgubeop was a widely known therapy in the middle to later parts of Joseon in general. 2. Woogakgubeop is a method that addresses problems contained within the existing moxa treatment techniques. By enlarging the size of the mugwort wick, it strengthens the heat energy of the moxa treatment to reduce the frequency of applying moxa treatment. When undertaking the method to empty the wick inside the mugwort, the pain of a fever patient will be reduced and burn damage to the skin is minimized. 3. Wind-cold type weakness and other general moxa treatment techniques have been used for various symptoms detailed, including chest pain, tinnitus, carbuncle, cellulites, cold sense of leg, colic, diarrhea and other illnesses. In addition, it may generate strong fever, and was used to implement the Yeonjebeop (煉臍法). 4. Woogakgubeop is applicable to the origin of Large moxa treatment of today and it provides important bibliographic base thereof.

한열 변증 설문지 개발에 관한 연구 (Study on Development of Cold-Heat Pattern Questionnaire)

  • Acupuncture and herbal treatment are based on diagnosis of cold and heat pattern in Traditional Korean Medicine. This diagnosis is accomplished through pulse, tongue and question examination, which are not objective. Quantification and objectification of this diagnosis process are required for efficacious treatment and traditional medicine development. In this study, we developed the cold-heat pattern questionnaire for this purpose. Seventy nine patients who visited oriental medical hospital were included in this study. The cold-heat pattern questionnaire was composed of many questions about patient's physical condition, which were derived from The Traditional Oriental Medical Literature with Delphi Technique. Patients filled out the cold-heat pattern questionnaire by themselves. Diagnosis of cold and heat pattern are conducted separately by oriental medical doctors with more than 5 years' clinical experience. Various physical condition factors were derived for the cold-heat pattern questionnaire. (Preference temperature, Body temperature, Pain type, Face color, Urine, Stool and secretion features) Each cold and heat symptoms group acquired internal consistency. (Cronbach's ${\alpha}$ : Cold - 0.605, Heat - 0.722) There were significant associations between doctor's diagnosis and cold symptoms in 'Aversion to cold', 'Desire for heat', 'Pale face', 'Loose stools'. (p-value < 0.05) There were significant associations between doctor's diagnosis and heat symptoms in 'Desire for cold', 'Body feverishness', 'Thirst'. (p-value < 0.05) The internal consistency results suggest that the cold-heat pattern questionnaire assured reliability. Besides, these results showed that cold-heat symptoms are apt to appear together with, and this can be indirect evidence that diagnosis of cold-heat pattern is valuable for comprehension about disease pattern. Moreover, respective symptoms of cold-heat pattern showed different significance with doctor's diagnosis. Consequently these significant symptoms can be more considered for comprehension of cold-heat pattern.

한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구 (A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

대퇴골두 무혈성 괴사의 한의학적 치료에 대한 국내·외 연구동향 분석 (Analysis of Domestic and Foreign Research Trends on the Korean Medical Treatment of Avascular Necrosis of the Femoral Head)

  • Objectives This paper is an analysis of domestic and foreign trends for the Korean medical treatment of avascular necrosis of the femoral head. Methods Domestic and foreign databases were searched and analyzed for Korean medical treatment interventions, research types, and treatment effects for avascular necrosis of the femoral head. Results There were 10 case reports in Korea, 2 case reports and 3 randomized controlled studies abroad. A total of 12 treatment interventions were used, and the types include acupuncture, herbal medicine, phamacopunture, electroacupuncture, cupping, physical therapy, chuna, acupotomy, moxibustion, warm acupuncture, laser acupuncture, and motion style treatment. As for the evaluation index, visual analogue scale, range of motion and harris hip score were the most common with 7 cases each. Conclusions Through this study, it was possible to understand the trends of domestic and foreign research on the treatment of avascular necrosis of the femoral head with Korean medicine. However, due to the small number of literature and no randomized control studies in Korea, treatment evidence is insufficient. It is considered that further research is needed in the future.

  • Traditional Chinese Medicine is based on Five Phase theory that link Liver, Heart, Spleen, Lung, Kidney to Wood, Fire, Earth, Metal, Water respectively. Recently, some argue that this correspondence scheme had been established by political or socio-humane issues in early Eastern Han dynasty. But according to our analysis on Changgongyeoljeon(in Sagi), it has been revealed that this scheme had their own history from early Western Han dynasty. So we may conclude that people with medical profession had preserved the current correspondence scheme in Five Phase theory regardless trends of literature and politics in Western and Eastern Han dynasty.

  • Objective: The aim of this study was to provide a systematic review of the Korean literature on the influence of herbal medicine on liver function. Additionally, we tried to estimate the change in the data on liver function tests (LFTs) and the incidence of liver injury (LI) after taking herbal medicine. Methods: Systematic literature searches were performed on 6 major Korean databases (Research Information Sharing Service (RISS), Korea Institute of Science and Technology information (KISTI), National Assembly Electronic Library, Korean Medical database (KMBASE), Korean Traditional Portal, and Dongui University Electronic Library) from January 2010 to June 2020. No restrictions were placed on the types of publications, which included grey literature. Results: The data indicate that herbal medicine might be a minor cause of LI in Korea. However the results are not supported strongly enough to verify the safety issues because of the limitations of the original studies. Conclusions: Additional well-planned studies are still needed that can overcome the heterogeneity and the considerable methodological flaws in the analyzed studies.
