Prediction of Cement Volume for Vertebroplasty Based on Imaging and Biomechanical Results

  • Control of bone cement volume (PMMA) may be critical for preventing complications in vertebroplasty, the percutaneous injection of PMMA into vertebra. The purpose of this study was to predict the optimal volume of PMMA injection based on CT images. For this, correlation between PMMA volume and textural features of CT images was examined before and after surgery to evaluate the appropriate PMMA amount. The gray level run length analysis was used to determine the textural features of the trabecular bone. Extimation of PMMA volume was done using 3D visualization with semi-automatic segmentation on postoperative CT images. Then, finite element (FE) models were constructed based on the CT image data of patients and PMMA volume. Appropriate material properties for the trabecular bone were assigned by converting BMD to elastic modulus. Structural reinforcement due to the changes in PMMA volume and BMD was assessed in terms of axial displacement of the superior endplate. A strong correlation was found between the injected PMMA volume and the area of the intertrabecular space and that of trabecular bone calculated from the CT images (r=0.90 and -0.90, respectively). FE results suggested that vertebroplasty could effectively reinforce the osteoporotic vertebra regardless of BMD or PMMA volume. Effectiveness of additional PMMA injection tended to decrease. For patients with BMD well lower than 50mg/ml, injection of up to 30% volume of the vertebral body is recommended. However, less than 30% is recommended otherwise to avoid any complications from excessive PMMA because the strength has already reached the normal level.

Anatomical variations of trabecular bone structure in intraoral radiographs using fractal and particles count analyses

  • Purpose : This study was performed to evaluate possible variations in maxillary and mandibular bone texture of normal population using the fractal analysis, particles count, and area fraction in intraoral radiographs. Materials and Methods : Periapical radiographs of patients who had full mouth intraoral radiographs were collected. Regions of interest ($100{\times}100$ pixels) were located between the teeth of the maxillary anterior, premolar, and molar area, as well as the mandibular anterior, premolar, and molar areas. The fractal dimension (FD) was calculated by using the box counting method. The particle count (PC) and area fraction (AF) analyses were also performed. Results : There was no significant difference in the FD values among the different groups of age, gender, upper, and lower jaws. The mean FD value was $1.49{\pm}0.01$. The mean PC ranged from 44 to 54, and the mean AF ranged from 10.92 to 11.85. The values of FD, PC, and AF were significantly correlated with each other except for the upper molar area. Conclusion : According to the results, patients with normal trabecular pattern showed a FD of approximately 1.5. Based on these results, further investigation would be recommended if the FD value of patient significantly differenct from this number, since the alteration of this value indicates microstructural modification of trabecular pattern of the jaws. Additionally, with periapical radiographs, simple and cost-effective, PC and AF could be used to assess the deviation from the normal.

대퇴부 방사선영상에서 대퇴골 근위부의 형태학적 측정과 골소주의 특성을 이용한 골다공증 예측에 관한 연구 (A Study of Osteoporosis Prediction using Morphological Measuring of Proximal Femoral Part and Trabecular Characteristics Based on Femoral Radiographic Image)

  • This study was designed to examine the morphological measurement and characteristics of trabecullae based on femoral radiographic image for prediction of osteoporosis. Study subjects were 34 females (average age of 62.1 years) and 6 males (average age of 60.1 years), they were categorized into normal group and osteoporosis group in accordance with the T-score value. Measurement of the bone density of femoral bone was measured with DEXA(Dual Energy X-ray absorptiometry). ROI(Region of interests) was selected on femoral neck and trochanter. Characteristics of trabecullae was analyzed by using the skeletonization analysis of trabecular image. Morphological measurement was analyzed through femoral radiographic image in order to examine the correlation with osteoporosis. The result demonstrated statistically significant correlation between neck cortical thickness, shaft width, shaft cortical thickness, periphery, mean gray level and trabeculae area with BMD average (T-score) of femoral part. The results show that morphological measurement and characteristics of trabecullae based on femoral radiographic images for osteoporosis prediction could be effective.

이소플라본 조성물이 골다공증에 미치는 효과 (Effects of the Dietary Mixture of Isoflavone on Osteoporosis)

  • 동물실험을 통하여 이소플라본의 골다공증 억제 효과를 연구하였다. 난소절제된 흰쥐에 기본사료, 칼슘, 비타민D$_3$, 비타민K$_1$를 함유하는 조성물과 이소플라본을 추가한 조성물을 7개월간 투여한 후 기본사료만 투여한 대조군과 비교하였다. 골 흡수 지표로 deoxypyridinoline을 분석한 결과 칼슘 비타민 조성물 투여군의 경우 대조군과 비교하여 9.0%, 이소플라본 추가 조성물 투여군의 경우 29.5% 감소하였다. 경골의 trabecular 골면적을 측정한 결과 칼슘 비타민 조성물 투여군의 경우 대조군과 비교하여 9.0%, 이소플라본 추가 조성물 투여군의 경우 15.8% 증가하여 이소플라본의 골다공증 억제 효과를 확인할 수 있었다.

The effects of voltage of x-ray tube on fractal dimension and anisotropy of diagnostic image

  • Purpose: The purpose of this study was to evaluate the effect of the kV on fractal dimension of trabecular bone in digital radiographs. Materials and Methods: 16 bone cores were obtained from patients who had taken partial resection of tibia due to accidents. Each bone core along with an aluminum step wedge was radiographed with an occlusal film at 0.08 sec and with the constant film-focus distance (32 cm). All radiographs were acquired at 60, 75, and 90 kV. A rectangular ROI was drawn at medial part, distal part, and the bone defect area of each bone core image according to each kV. The directional fractal dimension was measured using Fourier Transform spectrum, and the anisotropy was obtained using directional fractal dimension. The values were compared by the repeated measures ANOVA. Results : The fractal dimensions increased along with kV increase (p < 0.05). The anisotropy measurements did not show statistically significant difference according to kV change. The fractal dimensions of the bone defect areas of the bone cores have low values contrast to the non-defect areas of the bone cores. The anisotropy measurements of the bone defect areas were lower than those of the non-defect areas of the bone cores, but not statistically significant. Conclusion: Fractal analysis can notice a difference of a change of voltage of x-ray tube and bone defect or not. And anisotropy of a trabecular bone is coherent even with change of the voltage of x-ray tube or defecting off a part of bone.

Herbal Extract Prevents Bone Loss in Ovariectomized Rats

  • This research aims to test a new drug candidate based on a traditional medicinal herb, F1, an herbal extract obtained from Astragalus membranaceus and its main ingredient, 1-monolinolein that may have fewer side effects and less uterine hypertrophy. In vitro experiments, human osteoblast-like cell lines, MG-63 and Saos-2, were analyzed by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide (MTT) and an alkaline phosphatase (ALP) assays. Mouse osteoclasts were induced through a calcium-deficient diet and inhibition effects were measured. In vivo experiments were done using ovariectomized (OVX) rats for 9 weeks. At necropsy, uterus weights were measured, trabecular bone area (TBA) of tibia and lumbar vertebra were measured bone histomorphology. In results, cell proliferation and ALP activity in Saos-2 by ether F1 or 1-monolinolein did not increased significantly compared to the control. The F1 inhibited osteoclast development ($IC_{25}=3.37{\times}10^{-5}$mg/mL) less than 17$\beta$-estradiol. The OVX rats administered F1 (2 mg/kg/day and 10 mg/kg/day) showed an increase in TBA of the tibia significantly (136.3${\pm}4.2% and 138.5{\pm}$10.3% of control). In conclusions, the herbal extract, F1 inhibited tibia and lumbar bone loss and did not cause uterine hypertrophy. However, 1-monolinolein, the main ingredient of the herbal extract, did not inhibit bone loss.

Consideration of Lateral Cortical Bone Thickness and IAN Canal Location During Mandibular Ramus Bone Grafting for Implant Placement

  • Purpose: This study aimed at examining the thickness of lateral cortical bone in the mandibular posterior body and the location of the inferior alveolar nerve canal as well as investigating the clinically viable bone grafting site(s) and proper thickness of the bone grafts. Subjects and Methods: The study enrolled a total of 49 patients who visited the Department of Oral and Maxillofacial Surgery at Kyung Hee University Dental Hospital to have their lower third molar extracted and received cone beam computed tomography (CBCT) examinations. Their CBCT data were used for the study. The thickness of lateral cortical bone and the location of inferior alveolar nerve canal were each measured from the buccal midpoint of the patients' lower first molar to the mandibular ramus area in the occlusal plane of the molar area. Results: Except in the external oblique ridge and alveolar ridge, all measured areas exhibited the greatest cortical bone thickness near the lower second molar area and the smallest cortical bone thickness in the retromolar area. The inferior alveolar nerve canal was found to be located in the innermost site near the lower second molar area compared to other areas. In addition, the greatest thickness of the trabecular bone was found between the inferior alveolar nerve canal and the lateral cortical bone. Conclusions: In actual clinical settings involving bone harvesting in the posterior mandibular body, clinicians are advised to avoid locating the osteotomy line in the retromolar area to help protect the inferior alveolar nerve canal from damage. Harvesting the bone near the lower second molar area is judged to be the proper way of securing cortical bone with the greatest thickness.

대퇴 근위부의 골소주 패턴에 대한 복합적인 분석을 통한 골다공증 예측 연구 (Prediction of Osteoporosis using Compositive Analysis of Trabecular Patterns on Proximal Femur)

  • 본 연구에서는 각기 다른 측정 부위에서 도출된 골밀도와 골소주 패턴 간의 상관관계를 복합적으로 분석하여 측정부위에 따른 평가변수의 골다공증 예측 가능성을 평가하고자 하였다. 실험대상은 40대 이상의 폐경기 이후 여성 40명을 선정하였고, T-Score 분류법에 따라 각각 20명의 정상군 및 골다공증군으로 분류하였다. 실험대상의 골밀도는 이중 에너지 X선 흡수 계측법을 통해 대퇴경부(Femoral Neck), 전자부(Trochanter) 및 워드 삼각부(Ward's Triangle)에서 측정하였다. 또한 각 측정부위에 $50{\times}50$ 픽셀 크기의 관심영역을 지정하고, 기존 연구에서 제안된 영상처리기법을 적용하여 각 영역의 골소주 패턴을 추출하였다. 골다공증을 예측하기 위한 평가변수는 구조적 변수(평균 회색조, 골면적, 골둘레, 골두께, 말단거리), 골격화 변수(골개수, 골길이) 및 프랙탈 차원(fractal dimension)으로 구분되는 총 8가지를 선정하였다. 실험결과, 골면적, 골둘레, 골두께, 말단거리, 골개수, 골길이 및 프랙탈 차원이 골밀도를 유의하게 반영하였고(p<0.003), 평가변수의 분석을 통한 골다공증의 높은 예측 가능성을 확인하였다.

Diabetes disrupts osteometric and trabecular morphometric parameters in the Zucker Diabetic Sprague-Dawley rat femur

  • Type 2 diabetes mellitus is increasingly becoming more prevalent worldwide together with hospital care costs from secondary complications such as bone fractures. Femoral fracture risk is higher in diabetes. Therefore, this study aimed to assess the osteometric and microarchitecture of the femur of Zucker Diabetic Sprague-Dawley (ZDSD) femur. Ten-week-old male rats (n=38) consisting of 16 control Sprague-Dawley (SD) and 22 ZDSD rats were used. The rats were terminated at 20 weeks and others at 28 weeks of age to assess age, diabetes duration effects and its severity. Bilateral femora were taken for osteometry, bone mass measurements and micro-focus X-ray computed tomography scanning to assess the trabecular number (TbN), thickness (TbTh), spaces (TbSp), bone tissue volume to total volume (BV/TV) and volume (BV). Diabetic rats had shorter (except for 20-weeks-old), lighter, narrower, and less robust bones than SD controls that wered more robust. Although cortical area was similar in all diabatic and control rats, medullary canal area was the largest in ZDSD rats. This means that the diabetic rats bones were short, light and hollow. Diabetic rats aged 20 weeks had reduced BV, BV/TV, TbN with more spacing (TbSp). In contrast, the 28 weeks old diabetic rats only showed reduced BV and TbN. Discriminant function analysis revealed, for the first time, that osteometric parameters and TbTh, TbN, and TbSp were affected by diabetes. This knowledge is valuable in the management of diabetic complications.

Correlation between Bone Mineral Density Measured by Dual-Energy X-Ray Absorptiometry and Hounsfield Units Measured by Diagnostic CT in Lumbar Spine

  • Objective : Use of quantitative computed tomography (CT) to evaluate bone mineral density was suggested in the 1970s. Despite its reliability and accuracy, technical shortcomings restricted its usage, and dual-energy X-ray absorptiometry (DXA) became the gold standard evaluation method. Advances in CT technology have reduced its previous limitations, and CT evaluation of bone quality may now be applicable in clinical practice. The aim of this study was to determine if the Hounsfield unit (HU) values obtained from CT correlate with patient age and bone mineral density. Methods : A total of 128 female patients who underwent lumbar CT for back pain were enrolled in the study. Their mean age was 66.4 years. Among them, 70 patients also underwent DXA. The patients were stratified by decade of life, forming five age groups. Lumbar vertebrae L1-4 were analyzed. The HU value of each vertebra was determined by averaging three measurements of the vertebra's trabecular portion, as shown in consecutive axial CT images. The HU values were compared between age groups, and correlations of HU value with bone mineral density and T-scores were determined. Results : The HU values consistently decreased with increasing age with significant differences between age groups (p<0.001). There were significant positive correlations (p<0.001) of HU value with bone mineral density and T-score. Conclusion : The trabecular area HU value consistently decreases with age. Based on the strong positive correlation between HU value and bone mineral density, CT-based HU values might be useful in detecting bone mineral diseases, such as osteoporosis.
