  • It is very important to understand the chief complaint of a patient prior to evaluation of prognosis and establishment of treatment plan. The purpose of this study was to prepare the basic data available for dental practitioners in establishing the treatment plan of periodontal patients. The chief complaints were statistically analyzed according to age and sex from 5.843 periodontal cases(age range: $20{\sim}70,male$: 3,048, female: 2,795) by means of Chi-Square Test in Microstat program. The results were as follows : 1.The proportion of each chief complaint, except routine check was in the following order: pain(32.84%) ; gingival swelling(13.35%); tooth mobility(13.2S%); hypersensitivity(12.53%) ; gingival bleeding(8.45%) ; pus discharge(8.33%) ; food impaction(3.80%); gingival recession(3.59%); halitosis(2.70%) ; itching sensation(1.90%); spacing(O.48%) ; implant(O.39%) ; gingival discoloration(0.38%). 2.   • Dentin hypersensitivity medicaments such as Gluma, Scotchbond 2, All-Bond 2, which are resin adhesives, were used to compare the sealing effects of dentinal tubule under mechanical stress. Topical application of above medicaments on the dentin surfaces of extracted teeth followed by artificial tooth brushing for 6 weeks was performed for the comparison. The following conclusions on the degree of dentinal tubule exposure versus time by were reached by using polyvinyl siloxane impression material for taking the impression, epoxy resin for the duplication and SEM for observing the surface. 1. SEM was used to compare the accuracy of the duplicated surface, but no differences were found when teeth samples and the duplicated surfaces were observed. 2. After comparing the degree of dentinal tubules exposure with varnish applied contrast group, resin adhesive materials showed much less exposure as time went by. 3. The results indicated that AU-Bond 2 adhesive, under mechanical stress, showed lesser exposure of dentinal tubules comparing with Gluma and Scotchbond 2 adhesives After the results were put together, it was demonstrated that resin replica method is an useful way to evaluate the treatment effects of the dentinal tubule hypersensitivity medicaments. Also, it was noticed that under mechanical stress, All-Bond 2, classified as fourth generation, illustrated the best dentinal tubules sealing effects.

Single-session associative protocol for dentin hypersensitivity management: a 1-year randomized, blinded clinical study

  • Objectives: This study aimed to establish a single-session associative protocol for nonrestorative management of dentin hypersensitivity (DH). Materials and Methods: Twenty-four individuals with DH and a minimum sensitivity level of 4 on the visual analog scale (VAS) were selected. The study was conducted in a split-mouth design, with each participant (n = 20) having at least 1 affected tooth in all quadrants. The management protocols consisted of control group: universal adhesive, Neural Desensitizing Protocol group: 5% potassium nitrate, Mixed Desensitizing Protocol (PAM) group: 5% sodium fluoride and 5% potassium nitrate, Remineralizing Desensitizing Protocol (PDR) group: surface-partially reacted glass technology photopolymerizable varnish. Evaluations were performed immediately after application, at 1 week, 1 month, 2 months, and 12 months using the VAS sensitivity test. Results: The scores were subjected to statistical analysis using the Friedman test (p < 0.05), Durbin-Conover test (p < 0.05), and Wilcoxon test (p < 0.05). At the 12-month evaluation, all groups showed statistically significant differences compared to the initial assessment. For the evaluation after 12 months, there was a statistically significant difference between the PAM group, the control group, and the PDR group. Conclusions: It can be concluded that all groups were effective in controlling DH, but there were significant results in the control group and PDR group. The clinical relevance of this study is to demonstrate that the application of single-session desensitizing protocols can be effective in controlling DH for up to 12 months.

불소함유 접착 테이프의 상아질 지각과민증 치료효과 (THE THERAPEUTIC EFFECT OF FLUORIDE-CONTAINING ADHESIVE TAPE ON DENTIN HYPERSENSITIVITY)

  • 본 임상연구는 상아질지각과민증을 치료하기 위한 여러 가지 방법 중 효과적인 방법의 하나로 알려져 있는 불소(NaF)를 이용하여 불소테이프(SCMC-T-5)를 개발, 제조하고, 이를 이용하여 상아질 지각과민 치료 효과를 기존의 불소바니쉬(Cavity $Shield^{TM}$)와 비교, 평가하였다. 상아질 지각과민증이 있는 건강한 20대 남녀 22명(88개 치아)이 본 임상연구에 참가하였으며, 각각 11명씩 불소테이프를 부착시킨 실험군과 불소바니쉬를 도포한 대조군으로 분류하였다. 불소 제제는 제조사의 지시에 따라 적용되었으며, 불소 제제 도포 전, 도포 3일후, 1주일 후, 4주일 후 대상치아에 압축공기와 얼음막대를 이용하여 자극을 가한 뒤 visual analog scale(VAS)을 이용하여 통증 정도를 표시하도록 하였다. 실험군에서 VAS scores는 초기에 38.636(air), 62.273(ice), 3일 후 30.273(air), 49.545(ice), 1주일 후 28.182(air), 40.000(ice), 4주일 후 26.364(air), 37.727(ice)이었으며, 초기 VAS socres에 비해 모든 VAS scores는 통계학적으로 유의성 있게 감소되었다. 대조군에서 VAS scores는 초기에 42.274(air), 65.909(ice), 3일 후 34.091(air), 55.909(ice), 1주일 후 28.636(air), 40.909(ice), 4주일 후 27.727(air), 31.364(ice)이었으며, 3일 후에 시행된 압축공기에 의한 자극검사를 제외한 모든 VAS scores는 초기 VAS socres와 비교했을 때 통계학적으로 유의성 있게 감소되었다. 본 임상연구에 사용된 불소테이프와 불소바니쉬는 상아질 지각과민증을 효과적으로 감소시켰고, 두 약제간의 치료효과의 차이는 없었다.

지각과민처치제가 초음파 스케일링 처리한 비우식성 치경부 병소가 있는 치아의 지각과민증에 주는 영향 (Effect of Dentin Desensitizer on the Hypersensitivity of Teeth with Non-carious Cervical Lesions Subsequent to Ultrasonic Scaling)

  • The aim of this study was to evaluate the changes in dentinal permeability after application of dentin desensitizer on exposed dentin immediately after ultrasonic scaling to teeth with non-carious cervical lesions. Thirty caries-free extracted molars were fixed to slide glasses after horizontally being sectioned at 5 mm below the cemento- enamel junction (CEJ). The prepared specimen was connected to a fluid flow measuring device (nano-Flow), and a V-shaped cavity was formed at the CEJ to imitate the non-carious cervical lesion. After no fluid leakage was confirmed in the connected system with specimen, tooth surface was treated ultrasonic cleaning with piezoelectric ultrasonic scalers until dentinal tubules were exposed. And 6 different desensitizers were applied on exposed dentin. Real-time measurements of dentinal fluid flow were performed during ultrasonic scaling and application of dentin desensitizer. To evaluate the occlusion of exposed dentinal tubules, tooth surface was examined by SEM. Following results were observed. After ultrasonic scaling, more dentinal tubules were exposed on the tooth with non-carious cervical lesions compared to tooth without lesions. The rate of fluid flow measured with nano-Flow system had correlation with the degree of dentin occlusion observed with SEM after application of desensitizers on exposed dentin. Desensitizers with glutaraldehyde and HEMA did not decrease the rate of fluid flow and did not show dentin occlusion. Desensitizers with oxalate showed the limited effects on the rate of fluid flow and dentinal tubule occlusion. Desensitizer with resin monomer showed the significant effect on the rate of fluid flow and dentin occlusion.

민감성 치아에 대한 수산화인회석 함유 치약의 임상적 효과 (The clinical effects of a hydroxyapatite containing toothpaste for dentine hypersensitivity)

  • Purpose: The aim of this study was to compare the effectiveness of hydroxyapatite containing toothpaste with positive control toothpastes in reducing dentine hypersensitivity. Materials and methods: This clinical trial was a double-blind, randomized, parallel group comparison of two, namely hydroxyapatite containing toothpaste and strontium chloride containing toothpaste. A total of 55 subjects were included in this study. The subjects were given randomly assigned one of the two toothpastes after received tooth brushing instruction at baseline. Some clinical indices(PI, GI, PD), verbal rating score(VRS) for sensitivity to stimulus, the effect in relieving sensitivity and visual analogue scale(VAS) for sensitivity at baseline, week 2, week 4 and week 8 were assessed. All data were evaluated by intention-to-treat analysis. Results: Overall, PI and GI scores were significantly reduced compare baseline in all groups(p<0.05). In addition, there was significant difference in PI at 4 weeks and in GI at 4, 8 weeks between groups. The proportions of subjects relieved sensitivity were 70.4% in experimental group and 57.1% in control group at 8 weeks respectively. The VRS for sensitivity to three kinds of stimuli and VAS for sensitivity decreased according to time, there was no overall difference between two groups(p>0.05). Conclusion: This study demonstrated that the new hydroxyapatite containing toothpaste was similarly effective in reducing dentine hypersensitivity with pre-existing benchmark toothpaste.

  • Low-level laser therapy (LLLT) is the application of light to pathology to promote tissue regeneration, reduce inflammation, and relieve pain. LLLT has a photochemical effect whereby the light is absorbed and exerts a chemical change. The clinical applications of LLLT include improvement in wound and bone healing processes, control of pain and tooth hypersensitivity, modulation of periodontal inflammation, the prevention and treatment of cancer therapy-induced oral mucositis, management of burning mouth syndrome, and improvement in temporomandibular disorder symptoms. Further research is needed to better elucidate the cellular mechanisms of LLLT and provide a solid scientific basis for the clinical application of LLLT in dentistry.

치과용 지혈제가 상아질 투과도에 미치는 영향 (THE EFFECT OF HEMOSTATIC SOLUTIONS ON DENTIN PERMEABILITY)

  • Before impression making in the fixed restorations or other prosthesis, hemostatic solutions are used for hemostasis and moisture control. Hemostatic solutions effectively control bleeding but their major ingredients, acid removes smear layers which are formed in the tooth preparation, exposes the dentinal tubular orifices which are occluded by smear layers, makes dentinal tubular fluid displace more easily to the various external stimulus, and according to the hydrodynamic theory, consequently causes dentin hypersensitivity. To know the effect of hemostatic solutions on dentin permeability, coronal dentin discs, 1mm in thickness, were prepared from extracted third molars free from decay and wear, and a split chamber device was used. Hydraulic conductance values and SEMs, which were measured before and after treatment with $Astringedentr^{(R)},\;Altract^(R)\;and\;Epri-dent^{(R)}$, were compared and ana-lysed. The following conclusions were drawn: 1. Hydraulic conductance values which were measured after the treatment of hemostatic solutions were increased in all groups(p<0.05). 2. %change values of hydraulic conductance were compared but no significant difference was found among the three hemostatic solutions(p<0.05). 3. On SEM observations of all groups, after treatment smear layers were removed and dentinal tubular orifices were partially exposed. On the basis of these conclusions, the reckless use of hemostatic solutions should be restricted, and when in use, various methods should be considered to protect dentin.

비우식성 치경부병소를 가진 구치부 치아의 교합 및 치주상태에 관한 연구 (Occlusal and Periodontal Status of Teeth with Non-carious Cervical lesions)

  • A non-carious cervical lesion(NCCL) is the loss of tooth structure at the cementoenamel junction level that is unrelated to dental caries. This study was to evaluate the occlusal and periodontal status of teeth with non-carious cervical lesions. We evaluated 105 teeth with non-carious cervical lesions in 35 subjects aged 38-75 years and characterized them based on the shape and dimension, plaque retention, bleeding on probing(BOP), probing pocket depth(PPD), occlusal status, brushing type, hypersensitivity and wear facet. The results of this study were as follows 1. No significant association was observed between cervical lesions and occlusal contact in lateral excursions. 2. No significant difference occurred in plaque retention, PPD, BOP between teeth with and without cervical lesions. 3. Test teeth had a significantly higher percentage of hypersensitivity and occlusal wear facet than teeth without cervical lesions. 4. Wedge shaped lesions had a significantly higher percentage of plaque than saucer shaped lesions. 5. Teeth with plaque were found to have significantly deeper PPD than teeth without plaque retention in cervical regions. 6. Teeth with occlusal contacts were found to have significantly deeper PPD than teeth without occlusal contacts. 7. No significant association was observed between cervical lesions and PPD independent of plaque retention and occlusal contacts Although more knowledge is necessary, our results suggest that occlusal contact and bacterial plaque may influence on periodontal tissue, but NCCL is not directly associated with periodontal health

과산화수소가 배합된 제제의 치아미백효과에 관한 연구 (Tooth whitening effect of toothpaste containing hydrogen peroxide)

  • Objectives : The purpose of this study was to evaluate the tooth whitening effect of 0.74% and 2.80% hydrogen peroxide toothpastes and safety on tooth and gingival tissue. Methods : Toothpastes containing 0.74% and 2.80% hydrogen peroxide were evaluated. In in-vitro test, some additives (sodium metaphosphate, sodium pyrophosphate and titanium dioxide) were added to the toothpastes. Hydroxyapatite specimens (HAPs) were made and stained using modified Stookey's methods. HAPs were treated for 1 hour at shaking incubator and brushed for 1,000 times as 250 gF with each diluted toothpaste. Before and after color was measured by colorimeter. Using double blind method, 99 Korean with natural maxillary anterior teeth were selected and the initial brightness (baseline) was measured by SHADEEYE-EX. Based on this measurement they were crossly distributed into control group (0% hydrogen peroxide), test 1 (0.74% hydrogen peroxide) and test 2 (2.80% hydrogen peroxide). After 2 weeks, people of each group were provided toothpaste and told to use 3 times a day right after every meal for 3 minutes. The brightness of teeth was measured 3 times for every one month. Results : ${\Delta}L$ was statistically significant among three groups in shaking test. ${\Delta}L$ of two test groups was statistically significant compared with control group but not between each test group in brushing test. After using toothpaste for 3 months, test 1 group and test 2 group were 15.89% and 31.23% more whitened compared with control group respectively (p<0.05). Rate of more whitened person of each test group was 24.2% and 40.5% more than control group respectively (p<0.05). There was no difference in the hypersensitivity during 3 months using toothpastes and no side effect on teeth or gums. Conclusions : Toothpastes containing 0.74% and 2.80% hydrogen peroxide showed tooth whitening effect and both were safe enough to use for tooth whitening.
