Result of Joint Preserving Surgery Using Axial Shortening Metatarsal Osteotomy for the Treatment of Severe Hallux Valgus and Claw Toes Deformity in Advanced Rheumatoid Arthritis (류마티스 관절염에 의한 고도의 무지 외반증과 소족지 갈퀴족 변형에 대한 중족골 종축 감압 절골술을 이용한 관절 보존술의 결과)

  • Purpose: The purpose of this study is to evaluate the effect of axial shortening metatarsal osteotomy on the treatment of advanced rheumatoid arthritis patients with severe hallux valgus and claw toe deformity of lesser toes which is used for preserving the metatarsophalangeal joint. Materials and Methods: From January 2005 to June 2009, 18 cases of axial shortening metatarsal osteotomy in advanced rheumatoid arthritis were reviewed ; all of them followed up for more than 2 years after surgical procedures and the mean follow up period was 3.4 years. We performed axial shortening Scarf osteotomy and Akin osteotomy for hallux valgus and Weil osteotomy with soft tissue release for claw toe of lesser toes, respectively. We measured preoperative and postoperative hallux valgus angle, each metatarsal shortening length and the range of motion of the metatarsophalangeal joints through radiographic and clinical examination and compared them each other. Clinical results were evaluated by American Orthopedic Foot and Ankle Society (AOFAS) score and subjective satisfaction of the patients. Results: The hallux valgus angle was reduced from the preoperative mean value of 44.8 degree to 9.0 degree postoperatively and the range of motion of the metatarsophalangeal joint of great toe and lesser toes was increased from the mean of 21.7 degree and 11.0 degree preoperatively to 38.0 degree and 32.5 degree, respectively at postoperation. Also, the mean AOFAS score was improved from 26.5 points to 67.4 points. Conclusion: Axial shortening osteotomy is a useful method to correct the deformity and preserve the metatarsophalangeal joint for severe hallux valgus and claw toe deformity in advanced rheumatoid arthritis.

Yeoheon Jang Hyeongwang's perspective on schooling and the implication of moral education (여헌(旅軒) 장현광(張顯光)의 학교교육론(學校敎育論)과 그 도덕교육적(道德敎育的) 함의)

  • Jang Hyeongwang is one of the leading scholars since the era of Toe'gye and Yulgok in Korean Confucianism. Thus, this paper will summarize his schooling, i.e., education in local school(Hyanggyo), specific activities, and the meaning of moral education. Although Toe'gye and Yulgok recognized the problems of local school and their indictment revealed in 16th century, they sought different solutions to overcome the issues. Toe'gye paid attention to establishment of local academy(Seowon) and strengthening its stature in order to renew Confucian education while Yulgok kept an eye on reformation of local school as government educational institution for revitalizing schooling, fostering the talented and establishing moral discipline. In this context, I will summarize Yeoheon's perspective on schooling in terms of the difference with Toe'gye and Yulgok's ideas. Especially, I will analyze his problematique paying attention to restoration of moral order in local communities devastated since Japanese Invasion of Korea in 1592, and how it makes connection with local education and its implication. Furthermore, I will systematically examine the educational regulation of Indong local school(Indong Hyanggyo) he proposed and what its contents and real body he intended. In addition to these, I will suggest how much Yeohon's ideas on local education are meaningful in moral education and what kind of insightful things they would give us in current educational environments.

Assessment of The Biomass Potential Recovered from Oil Palm Plantation and Crude Palm Oil Production in Indonesia (인도네시아 오일 팜 바이오매스 잠재량 평가)

  • In this study, the potential of biomass, which is generated from oil palm cultivation and crude palm oil (CPO) production of Indonesia was assessed in the aspect of energy content. The types of oil palm biomass were classified on the basis of the cultivation stage and the CPO production stage. In the cultivation stage, biomass is considered to be produced from its' root, trunk and frond. Other possible biomass resources such as empty fruit bunch (EFB), palm kernel shell (PKS) and fiber were included in the CPO production stage. As results, total biomass from damaged plantation area of Indonesia was estimated to be annually from 3 million to 16 million tons in 2011. From CPO mills, approximately 49 million tons/yr of biomass residues were estimated to be annually occurred. Their total energy content from each biomass source in cultivation stage was analyzed to be from 593,000 to 3,197,000 TOEs in terms of gross calorific value. In the case of CPO mills, around 22.7 million TOEs was estimated to be potential energy producible by biomass based on gross calorific value of dry basis. If moisture content considered, net calorific value was analyzed to be decreased to 16.3 million TOEs. Based on the results, the total energy contents of all oil palm biomass were estimated to be up to 25,919,000 TOE in terms of gross calorific value. A Study on Left and Right Shape and Deformation of Feet of Korean Adults (한국 성인 발 형태의 좌우 및 변형 연구)

  • An analysis of anthropometric data of feet of Korean adults was made in this study. Anthropometric data were measured with the direct measurement method and indirect measurement method. Martin scales and a tapeline were used for direct measurement. Digital images taken with a digital camera as an indirect measurement method were analyzed using AutoCAD program. Total 114 adults aged from 20's to 40's participated in this study. Especially right and left feet were measured. Right and left side were compared and differences according to age were compared too. Feet shape, arch height, and deformation of toe were analyzed. The comparison between right and left foot showed that many subjects had different anthropometric data in their feet. Feet deformation including arch height and claw toe shape showed very serious status. Close and continuous attention to feet deformation is strongly needed.

  • Global e-Trade system which electronically processes all trade transactions of trading companies is an essential platform where export and import firms enhance international competitiveness. This study is to develop research models suitable for e-Trade and empirically analyze them in order to examine and verify factors affecting e-Trade system of trading companies on the basis of previously-verified studies on global e-Trade such as TAM, TOE, and Task-Technology Fit Model(TTF). Among many factors, this paper comprehensively analyzes an acceptance factor, which is one of the factors affecting the implementation and performance of e-Trade, from the technological, organizational, and environmental context. The finding from this paper will be applied to e-Trade projects carried out by the government in the future by analyzing the correlation between acceptance and performance.

  • Antiphospholipid syndrome (APS) is an acquired systemic autoimmune disorder characterized by a combination of clinical criteria, including vascular thrombosis or pregnancy morbidity and elevated antiphospholipid antibody titers. It is one of the causes of deep vein thrombosis and pulmonary embolism that can be critical due to the mortality risk. Overall recurrence of thromboembolism is very low with adequate anticoagulation prophylaxis. The most effective treatment to prevent recurrent thrombosis is long-term anticoagulation. We report on a 17-year-old male with APS, who manifested blue toe syndrome, deep vein thrombosis, pulmonary thromboembolism, and cerebral infarction despite adequate long-term anticoagulation therapy.
