가상현실 프로그램과 현실화된 과제 지향 프로그램이 노인의 균형에 미치는 효과 비교 (Comparison of the Effects of Virtual Reality and Task-Oriented Programs of Balance for the Elderly)

  • 목적 : 가상현실 프로그램과 현실화된 과제 지향 프로그램이 노인의 균형에 어떠한 영향을 미치는지 그 효과를 비교해 보고자 한다. 연구방법 : 12명의 건강한 노인을 대상으로 가상현실 프로그램과 현실화된 과제 지향 프로그램을 각각 6주간 주 2회씩 실시하였다. 균형 능력 변화를 위해 버그 균형 척도(Berg Balance Scale: BBS), 이동 능력검사(Timed Up and Go: TUG), 한 발 서기 검사(One-Legged Stance Test: OLST), 5회 반복 앉았다 일어서기 검사(5-Repetition Sit-To-Stand Test: 5R-STS Test)를 사용하였다. 결과 : 분석결과 프로그램 전 후에서는 통계적으로 유의한 결과를 얻지 못하였다. 하지만 세 집단 간 비교에서는 버그 균형 척도, 한 발 서기 검사 중 눈 뜨고 좌, 우에서 통계적으로 유의한 결과를 얻었으며 사후 검정결과 눈 뜨고 검사 - 좌에서 가상현실 프로그램과 현실화된 과제 지향 프로그램 간에 유의한 결과를 얻었다. 결론 : 현실화된 과제 지향 프로그램은 노인의 균형 능력 향상에 좀 더 긍정적이었으며 임상 및 지역사회에서 좀 더 쉽게 수행할 수 있는 프로그램으로 유용하게 사용될 수 있을 것이라 생각된다.

Contraction Ratio Variation of the Lateral Abdominal Muscles in Elderly Gait

  • Purpose: This study aimed to describe and identify the relationship between gait and contracted ratios of the transverse abdominal (TrA), internal oblique (IO), and external oblique (EO) muscles. Methods: This study was conducted on 50 elderly people. The contracted ratios of the lateral abdominal muscles (LAM) were measured using the abdominal drawing-in maneuver (ADIM) and ultrasonographic imaging. Gait was measured using the timed up and go test and the 10 m walk test. Results: The contracted ratios of the TrA and IO muscles significantly increased after ADIM. Those of the TrA muscles showed a significant correlation with gait in the limited community ambulatory group. The contracted ratios of the IO and EO muscles showed a significant correlation with gait in the community ambulatory group. Conclusion: Our findings suggest a specific training on the relationship between gait speed and the activation of the LAM in elderly people.

Balance Exercise Program Using Training Mats Improves the Postural Balance of Elderly Individuals: A Randomized Controlled Trial

  • Purpose: The purpose of this study was to investigate the effect of balance training using a training mat on the postural balance of the elderly individuals. Methods: Thirty-five participants were selected from a falling prevention class and were randomly allocated to two groups; 17 in an exercise group (EG, $72.7{\pm}5.1$ years) and 18 in the control group ($74.9{\pm}4.0$ years). The EG underwent balance training using training mats for 60 minutes a day, 2 days a week, for 4 weeks. Postural balance parameter (timed up and go test, functional reach test, and one leg standing) were measured pre- and post- training. Results: The EG showed significant improvements in all variables that were analyzed. Conclusion: This study confirmed that balance training using a training mat effectively improves the postural balance in elderly people at risk for falling.

Effects of Functional Electrical Stimulation (FES) on the Temporal-spatial Gait Parameters and Activities of Daily Living in Hemiplegic Stroke Patients

  • PURPOSE: This study examined the effects of functional electrical stimulation (FES) on temporal-spatial gait and the activities of daily living in hemiplegic stroke patients. METHODS: The subjects were 29 hemiplegic stroke patients (57.7 ± 10.3). The patients walked at a self-controlled speed in four states: (1) walking without FES (non-FES), (2) walking with FES on the gluteus medius in the stance phase (GM), (3) walking with FES on the common peroneal nerve and tibialis anterior in the swing phase (PT), (4) walking with both GM and PT. A GAITRite system, Timed-Functional Movements battery, and Timed UP and Go test were used to measure the variables. RESULTS: Significant improvements were observed in all variables of the GM+PT, GM, and PT states compared to the non-FES state (p < .05). There were significant improvements in the GM+PT state compared to GM and PT states (p < .05). Moreover, significant improvements were noted in the single support time on the affected side, backward walking 10ft, and side stepping 10ft on the affected side of the GM state compared to the PT state (p < .05). There were significant improvements in the stride length on the affected side and side stepping 10ft on the unaffected side of the PT state compared to the GM state (p < .05). CONCLUSION: FES is effective in improving the temporal-spatial gait and activities of daily living in hemiplegic stroke patients.

천장관절에 적용한 움직임을 동반한 관절가동술이 뇌졸중 환자의 보행에 미치는 효과 (The Effect of Mobilization With Movements Applied Sacro-Iliac Joint on Gait of Stroke Patient)

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

자세조절 증진이 편마비 환자의 보행과 균형에 미치는 영향 -증례 보고- (Effects on Walking & Balance upon Improvement of Postural Control Therapy for Hemiplegic Patient)

  • Purpose : The neurologically impaired patients have sensory, motor, cognition, perception problems which cause reduction of body schema, balance control and postural control provoke disfunction. The purpose of this case report was to evaluate effects on gait&balance upon improvement of postural control for left hemiplegia. Methods : This study has performed single subject design from March to April 2013 for 6 weeks. The subject of this study was a 67years old female patient with left hemiplegia. Timed Up and Go (TUG) test and Functional Reach (FR) test were used as evaluation tool. The subject was treated 5 times a week for 30 minutes each. Results : The result is that the walking velocity was decreased, the ability of gait was improved than before the training. The length of FR was increased from 8.33cm to 22.67cm. Conclusion : According to the results, the treatment improves subject's ability of walking in this study.

Physiological Approach on the Physical Fitness and Postural Balance Effects of a Whole-Body Vertical Vibration Intervention in Young Women

  • With the advent of westernized diet and a lack of exercise, young female college students are paying more attention to their bodyweight and health. Whole-body vibration has been demonstrated to be a suitable training method for improving knee extension maximal strength in young female athletes, as well as the gait performance in elderly women. This study aimed to evaluate the effects of a vertical vibration intervention on the physical fitness and postural balance in young females. Fifty-four young women were recruited; all subjects were randomly assigned to the intervention group and control group. The intervention group underwent vertical vibration with a platform for 12 weeks. The results showed that body mass index and body fat percentage had decreased (P<0.05). In addition, their muscle endurance as indicated by a sit-up test and their flexibility as indicated by a sit-and-reach test were both increased. With regard to postural balance, their 30-second sit-to-stand and timed up and go test results were improved. At the same time, their mean single-leg stance with eyes closed time increased (P<0.05). However, there were no significant differences, meanwhile, for the control group. Overall, the results showed that the whole body vibration (WBV) intervention had some beneficial effects on physical fitness and postural balance in young women.

뇌졸중 환자의 양하지 교차밀기 근력 검사와 균형의 상관 관계 연구 (Correlation between Bilateral Reciprocal Leg Press Test and The Balance in Chronic Stroke Patient)

  • Purpose: This study was to find a correlation between the bilateral reciprocal leg press test and a the balance in chronic stroke patients. Methods: Eighteen patients performed an isokinetic leg press test consisting of a bilateral reciprocal and unilateral mode. Following the isokinetic leg press testing, subjects performed the balance test: Berg Balance Scale (BBS), Timed Up & Go (TUG) test, and stability limit. Pearson product moment correlation coefficients were used to determine the correlation between the mean score of the isokinetic leg press test, balance test in both affected and non-affected side. Results: This study indicated a significant correlation between the bilateral reciprocal leg press test and stability limit. There were significant correlation between non-affected side bilateral leg press(NBL) and BBS (r=0.501), affected side bilateral leg press (ABL) and non-affected side stability limit(NS) (r=0.614), ABL and total stability limit (TS) (r=0.493), NBL and affected side stability limit(AS) (r=0.480), NBL and NS (r=0.560), NBL and TS (r=0.563), among the patients. Conclusion: Measurement of the lower extremity strength using the bilateral reciprocal leg press test can be used as an evaluating tool of the balance test.

체성감각계 지원이 여성노인의 보행 및 평형감각 유지에 미치는 효과 (Effect of somatosensory input on the gait ability and equilibrium sensory of elderly women)

  • 평형능력은 보행에 영향을 미치고, 보행속도는 노인의 건강한 척도를 말해주는 기준이 된다. 이 연구의 목적은 체성감각계 지원이 여성 노인의 보행 및 평형감각에 미치는 영향을 보고자 하였다. 평소 지팡이를 사용하지 않고, 규칙적으로 운동을 하지 않으며, 이비인후과적으로 이상이 없는 노인으로 평균 연령이 75세 여성 노인 61명을 실험군 31명과 통제군 30명으로 분류하였다. 종속변인으로는 2.44m왕복걷기, 10m 일반걷기 및 빠르게 걷기, 6분 걷기를 실시하였다. 평형감각 검사로는 뉴로컴사의 EquiTest를 이용하여 분석하였다. 연구 결과 2.44m 왕복걷기와 10m 일반걷기에서는 유의한 차이가 없었으나 10m 빠르게 걷기, 6분걷기에서는 유의한 효과를 보이는 것으로 나타났다. 평형감각에서는 조건 2, 3, 4, 6에서 유의한 효과를 보이는 것으로 나타났다. 결론적으로 체성감각계 지원이 노인 여성들의 보행 및 평형유지 능력에 유의한 효과를 보이는 것으로 나타나 지팡이의 적극적 사용이 권장된다 할 수 있겠다.

선 자세 몸통 안정화 운동이 길리안 바레 환자의 균형과 보행에 미치는 효과: 증례보고 (Effects of standing position of the trunk stabilization exercise on balance and gait of Guillain-Barre patients, case report)

  • Background: In the present study to investigate the immediate effects of standing position of the trunk stabilization exercise on Guillain-Barre syndrome patient's balance and gait abilities were examined. Design: Case report and conducted as a single-blind. Methods: Standing position of trunk stabilization exercise was conducted for five Guillain-Barre syndrome patient's who met the selection criteria were recruited. The subjects were conducted conservative exercise and trunk stabilization exercise. Conservative exercise was implemented for thirty min, five times for 8 weeks, and trunk stabilization exercise was implemented for 15 min. The participants' balance was measured via force plate (COP), functional reach test (FRT), timed up and go test (TUG), gait was measured using the 10 m walk test, and 6 minute walk test. Results: After training, the change values of the balance and gait ability in the subjects were significantly greater than pre-test. participants showed significant improvements in COP, FRT, TUG, 10 m walk test and 6 minute walk test pre and post the training (p<0.05). Conclusions: standing position of the trunk stabilization exercise was effective on Guillain-Barre syndrome patient's balance (COP, FRT, TUG) and gait (10 m walk test and 6 minute walk test) abilities were examined.
