지역사회 통합돌봄 정책에서 방문운동지도 서비스가 노인의 우울증과 통증 그리고 균형에 미치는 영향: 사례 연구 (The Impact of Community-based Integrated Care Service Policy's Home-visiting Exercise Guidance Services on Depression, Pain, and Balance among the Elderly: A case study)

  • Purpose : The home-visiting exercise guidance service is a component of the community-based integrated care policy in District J of Busan Metropolitan City. This study aims to investigate the effects of the service provided by physical therapists on depression, pain, and balance among the elderly. Methods : Three elderly individuals were selected as study participants. Depression was assessed using the beck depression inventory (BDI) and the short geriatric depression scale-Korea (SGDS-K). Pain was evaluated using the visual analog scale (VAS). Balance was assessed using the berg balance scale (BBS), functional reach test (FRT), and timed up and go test (TUGT). The evaluation data were compared by averaging the values from the first and eighth sessions. Additionally, the overall satisfaction with the home-visiting exercise guidance service was surveyed. Results : In the depression assessment, the beck depression inventory score decreased (by 16.67±5.69 points), and the short geriatric depression scale score decreased (by 4.67±2.52 points). The visual analog scale score for pain decreased (by 2.67±0.58 points). In the balance assessment, the berg balance scale score increased (by 7.67±2.52 points), the functional reach test distance extended (by 4.67±0.58 cm), and the timed up and go test time decreased (by 5.33±0.58 s). Overall satisfaction with the home-visiting exercise guidance service was high (4.26±0.14 points). Conclusion : The home-visiting exercise guidance service received high satisfaction ratings from the participants. It was effective in reducing depression and pain while improving balance, potentially reducing fall risk factors among the elderly. These findings suggest that the home-visiting exercise guidance service, delivered by physical therapists, could significantly enhance the quality of life for elderly individuals by addressing issues related to depression, pain, and fall prevention within the community. Consequently, it is recommended that this service be continued and expanded as part of government policy.

Discriminative validity of the timed up and go test for community ambulation in persons with chronic stroke

  • Objective: The timed up and go (TUG) test is method used to determine the functional mobility of persons with stroke. Its reliability, validity, reaction rate, fall prediction, and psychological characteristics concerning ambulation ability have been validated. However, the relationship between TUG performance and community ambulation ability is unclear. The purpose of this study was to investigate whether the TUG performance time could indicate community ambulation levels (CAL) differentially in persons with chronic stroke. Design: Cross-sectional study. Methods: Eighty-seven stroke patients had participated in this study. Based on the self-reporting survey results on the difficulties experienced when walking outdoors, the subjects were divided into the independent community ambulation (ICA) group (n=35) and the dependent community ambulation group (n=52). Based on the area under the curve (AUC), the discrimination validity of the TUG performance time was calculated for classifying CAL. The Binomial Logistic Regression Model was utilized to produce the likelihood ratio of selected TUG cut-off values for the distinguishing of community ambulation ability. Results: The selected TUG cut-off values and the area under the curve were <14.87 seconds (AUC=0.871, 95% confidence interval=0.797-0.945), representing a mid-level accuracy. Concerning the likelihood ratio of the selected TUG cut-off value, it was found that the group with TUG performance times shorter than 14.87 seconds showed a 2.889 times higher probability of ICA than those with a TUG score of 14.87 seconds or longer (p<0.05). Conclusions: The TUG can be viewed as an assessment tool that is capable of classifying CAL.

  • Purpose : The purpose of this study was to investigate the effect of pilates mat exercise (PME) and instability support surface exercise (ISSE) for TUG (Time up & go) and static balance in the elderly person aged 65 or more. Method : 30 subjects in Y-equestrian were randomly divided two group, pilates mat exercise (PME) group and instability support surface exercise (ISSE) group. Each group carried out 40 minutes exercise two times a week for 12 weeks. TUG (Time up & go) and Biorescue were measured for static balance. Result : The results were as follows, the TUG between pilates mat exercise (PME) group and instability support surface exercise (ISSE) group in post-test, were significantly different in measures (p<.05). And there were significant in two group after exercise (p<.05). The static balance between pilates mat exercise (PME) group and instability support surface exercise (ISSE) group in post-test, were significantly different in measures (p<.05). And there were significant in two group after exercise (p<.05). Conclusion : These finding revealed that pilates mat exercise was effective on TUG and static balance in the elderly person aged 65 or more so that these exercise can be new alterative exercise for obesity management in the elderly person.

Physical Functions, Health-Related Outcomes, Nutritional Status, and Blood Markers in Community-Dwelling Cancer Survivors Aged 75 Years and Older

  • Background: A cancer survivor is defined as anyone who has been diagnosed with cancer, from the time of diagnosis through the rest of their life. The purpose of this study was to examine whether physical functions, health-related outcomes, nutritional status and blood markers in community-dwelling cancer survivors aged 75 years and older are different from those who do not have cancer Materials and Methods: Two hundred seventy-five participants were asked by physicians, nurses, and physical therapists, questions regarding cancer history in a face-to-face interview. Data were collected for demographic information, physical functions, such as handgrip strength, knee extension power, abdominal muscle strength, static standing balance, walking speed and the timed-up-and-go test, health-related outcomes, nutritional status, and blood markers. The measured parameters of survivor diagnosed with cancer were compared with those without a history of cancer. Results: Thirty-seven older adults were previously diagnosed with cancer. Female cancer survivors had lower knee extension power (p<0.05), abdominal muscle strength (p<0.05), walking speed (p<0.05), timed-up-and-go test score (p<0.05), and time to spend on walking per day (p<0.05) than older women without a history of cancer. In men, none of the measured parameters were significantly different between cancer survivors and older men with no history of cancer. Conclusions: The present study shows that partial physical function of women cancer survivors aged 75 years and older differs from that in women with no history of cancer.

버그 균형 척도 평가 빈도수가 뇌졸중 환자의 동적 균형 및 보행 속도에 미치는 영향 (The Effect of Berg Balance Scale Evaluating Frequency for Dynamic Balance and Walking Speed of Patients With Stroke)

  • Background : The purpose of this study is to determine the effect Berg Balance Scale(BBS) evaluating frequency on the walking speed and dynamic balance control in patient with stroke. Method : sixteen patient with stroke were randomly allocated to an experimental and control group of eight patients each. For the experimental group, we performed both general physical therapy and BBS and general physical therapy only for the control group. The general physical therapy programs for the 2 group were conducted for 1 hour 1 a day, 5 times a week for 4 weeks, and BBS for the experimental group was conducted for 1 time a week. Result : A comparison of the Berg Balance Scale(BBS). Timed Up Go test(TUG) and 10 meter Walking Test(10mWT) score obtained before and after the 4-week treatment revealed statistical significant different(p<.05) for the experimental group. BBS evaluated weekly and the first day and the last day evaluated in both groups after 4 weeks of BBS assessment improved significantly were (p<.05) especially in the assessment group on a weekly basis more improvement was. BBS weekly assessment group and the first day and on the last day, a group evaluation after 4 weeks in both the change of the TUG, 10mWT was significantly improved in the evaluation group(p<.05). Conclusion : 1 time a week of the BBS assessment of with stroke patients BBS, TUG, 10mWT that can help to improve. especially on a weekly basis, more has been improved.

동작관찰훈련프로그램이 편마비환자의 보행 및 균형에 미치는 영향 (Effects of Action-observational training Program on Gait and Balance of Patients with Hemiplegia)

  • 본 연구는 만성뇌졸중 편마비 환자에게 적용한 동작관찰 훈련이 보행과 균형에 미치는 영향을 알아보고자 하였다. 연구에 동의한 만성 뇌졸중 편마비 환자 4명중 동작관찰훈련을 시행한 실험군 2명 일반과제지향훈련을 시행한 대조군 2명에게 총 4주간 하루 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 보행은 10m검사를 시행하였고 균형은 버그균형척도로 검사하고 훈련 후 다시 재측정하였다. 이에 동작 관찰 훈련군이 일반과제지향훈련군에 비해 보행과 균형에서 유의미한 차이를 보였다. 이에 동작관찰훈련이 만성뇌졸중 편마비환자의 보행과 균형에 효과적인 프로그램임을 알 수 있다.

겐트리에 대한 구동 시간의 비교 (A Comparison of the Moving Time about Gantry)

  • SMT장비는 전자 부품을 흡착하여 PCB상에 정확히 실장 하는 장비이다. 이를 위해서는 중간 위치에 설치된 카메라 앞에서 정지하여 비전검사를 한 후 실장 위치로 이동한다. 본 논문에서는 카메라 앞에서 멈추어 비전 검사를 한 후 실장하는 방법(stop-motion)과 카메라 앞에서 움직이면서 비전 검사를 한 후 실장하는 방법(fly-motion)을 비교하였다. 그 결과 fly-motion의 시간 효율이 stop-motion보다 9% 증가한 것을 보여주었다.

ANALYSIS ON GENERALIZED IMPACT ANGLE CONTROL GUIDANCE LAW

  • In this paper, a generalized guidance law with an arbitrary pair of guidance coefficients for impact angle control is proposed. Under the assumptions of a stationary target and a lag-free missile with constant speed, necessary conditions for the guidance coefficients to satisfy the required terminal constraints are obtained by deriving an explicit closed-form solution. Moreover, optimality of the generalized impact-angle control guidance law is discussed. By solving an inverse optimal control problem for the guidance law, it is found that the generalized guidance law can minimize a certain quadratic performance index. Finally, analytic solutions of the generalized guidance law for a first-order lag system are investigated. By solving a third-order linear time-varying ordinary differential equation, the blowing-up phenomenon of the guidance loop as the missile approaches the target is mathematically proved. Moreover, it is found that terminal misses due to the system lag are expressed in terms of the guidance coefficients, homing geometry, and the ratio of time-to-go to system time constant.

Does the Wealthier Elderly Show Better Standing Balance? Socioeconomical Factors and Standing Balance of the Elderly Living in Rural and Urban Areas in South Korea

  • Background: South Korea is one of fastest aging countries in the world. Poor balance and falls of the elderly are main health issues. Objects: The goal of this study was to understand the association between the socioeconomical factors and the standing balance of elderly living in the rural and urban area. Methods: One hundred sixty-six elderly participants who were older than 65 and were able to walk without an assistive device were recruited in the city of Gwangju and in the rural area of Jeonnam, South Korea. All participants performed the static and dynamic standing balance tests. Static standing balance was measured with chronometer in seconds while standing on one leg. Dynamic balance was tested with the timed up and go test (TUG), measured in seconds while getting up from a chair and walking 3 meters and back to sit. The static and dynamic standing balance was analyzed using analysis of variance and the Fisher's Least Significant Difference post hoc test. Results: Male participants from both areas had no difference in one leg standing and TUG. The female elderly living in rural area took shorter in TUG than females living in urban area. Age decreased the one leg standing time in both areas while did not affect the TUG significantly. As the monthly income increased, both of one leg standing and TUG increased in urban area, while the medium monthly income showed best performance (it was not statistically significant) in both of one leg standing and TUG in rural area. Conclusion: Socioeconomical factors affects differently the standing balance of the elderly living in rural and urban South Korea. Female living alone in urban area with low monthly income demonstrated worst standing balance in this study.

A Study On Wartime Sealift Operation Using Simulation

  • The ROK TRANSCOM is trying to establish a concrete wartime sealift operation plan. But there are many problems to be solved for setting up the plan. The most serious problem is to procure war materiel to be shipped in U.S.A and next one is to determine the number of sealift assets and to allocate them. The process of sealift operation can be described as follows. Before the order of vessel mobilization, all vessels appointed for activation would be scattered in worldwide in the state of Beaded or unloaded. After the order of vessel mobilization, vessels would go to SPGE(seaport of embarkation) to load war materiel. Some loaded ships should offload the commercial cargo to the near port as soon as they are activated, before they would go to SPOE. All vessels would load procured materiel in SPOE and then go to SPOD(seaport of debarkation). They would offload war materiel in SPOD and then go back to SPOE to load war materiel again. We will simulate this process using ARENA(1), evaluate the sealift capability of ROK and find omit problems of the sealift plan. This model ultimately evaluates the sealift capability and provides planners with critical information far establishing and correcting the plan. This study can also provide military planners with a flexible and accessible decision support tool to provide advance planning under a variety of conditions on the sealift capability. The military planner is expected to make use of this model as a standard for establishing effective and concrete sealift operation in the near future. We can conclude how procurement capability significantly affects the result of sealift operation through this model. We could decide the appropriate levee of sealift asset such as the number of vessels and the number of available berth. So we could allocate effectively the resources for completing the sealift operation within the TGT(Target) time.
