Simultaneous Occurrence of Medullary and Papillary Thyroid Carcinoma with Lymph Node Metastasis: A Case Report (림프절 전이를 동반한 갑상선에 동시 발생한 수질암과 유두상 암종 1례)

  • Simultaneous occurrence of medullary and papillary thyroid carcinomas in the same gland is very rare. In fact. there are only 18 cases of simultaneous occurrence of medullary and papillary thyroid carcinomas in the literature. We report a case of simultaneous medullary and papillary carcinoma of thyroid gland. A 67-year-old woman was diagnosed with medullary carcinoma of right lobe of thyroid gland and papillary carcinoma of left lobe of thyroid gland by fine needle aspiration cytology. Total thyroidectomy, anterior neck dissection, bilateral modified radical neck dissection and tracheotomy was undertaken. The tumor metastasized to regional lymph node and extrathyroidal muscle invasion of left papillary carcinoma was also revealed by pathological report. This report describes a case of thyroid carcinoma that demonstrated both medullary carcinoma and papillary components in the thyroid with lymph node metastasis.

  • Malignant lymphoma is the tumor of immune system which is known as the most common nonepitheloid tumor of head and neck region. It is mainly found in the cervical, axillary and inguinal lymph node, but gastrointestinal tract, palatine tonsil, nasopharynx, oropharynx, nasal cavity and salivary glands may be involved. Primary lymphoma of the thyroid gland is an uncommon condition, comprising approximately 2% of all malignant lymphoma and 5% of all thyroid malignant neoplasms. Recently, we experienced a case of malignant lymphoma of the thyroid gland, which was histopathologically proven. So we report our findings in this patient with review of literatures.

Determination of Scattered Radiation to the Thyroid Gland in Dental Cone Beam Computed Tomography

  • Background: Cone beam computed tomography (CBCT) is a specialized medical equipment and plays a significant role in the diagnosis of oral and maxillofacial diseases and abnormalities; however, it is attributed to risk of exposure of ionizing radiation. The aim of the study was to estimate and determine the amount of scattered radiation dose to the thyroid gland in dental CBCT during maxilla and mandible scan. Materials and Methods: The average scattered radiation dose for i-CAT 17-19 Platinum CBCT (Imaging Sciences International) was measured using a Multi-O-Meter (Unfors Instruments), placed at the patient's neck on the skin surface of the thyroid cartilage, with an exposure parameter of 120 kVp and 37.07 mAs. The surface entrance dose was noted using the Multi-O-Meter, which was placed at the time of the scan at the level of the thyroid gland on the anterior surface of the neck. Results and Discussion: The surface entrance dose to the thyroid from both jaws scans was 191.491±78.486 µGy for 0.25 mm voxel and 26.9 seconds, and 153.670±74.041 µGy from the mandible scan, whereas from the maxilla scan the surface entrance dose was 5.259±10.691 µGy. Conclusion: The surface entrance doses to the thyroid gland from imaging of both the jaws, and also from imaging of the maxilla and mandible alone were within the threshold limit. The surface entrance dose and effective dose in CBCT were dependent on the exposure parameters (kVp and mAs), scan length, and field of view. To further reduce the radiation dose, care should be taken in selecting an appropriate protocol as well as the provision of providing shielding to the thyroid gland.

  • Primary malignant lymphoma of the thyroid gland is uncommon malignancies. Its fine needle aspiration cytology (FNAC) findings are rarely described in the literature. This article highlights the FNAC diagnosis of primary malignant lymphoma of the thyroid gland. A 70-year-old female presented with a rapidly enlarging thyroid mass of five months' duration. FNAC smears showed low cellularity consisting of predominantly atypical enlarged lymphoid cells admixed with a few small lymphocytes, plasma cells, and oncocytic cells. Some disrupted lymphoid cells were also present. The tumor cells infiltrated into the thyroid follicular epithelium forming lymphoepithelial lesion, The cytologic appearance showed a diffuse mixture of cell types with only a few small, mature lymphocytes and many enlaraed lymphoid cells. The enlarged lymphoid cells were atypical and pleomorphic with nuclear clefting and irregularities. Grossly, the left lobe of the thyroid was nearly replaced by a diffuse firm to soft solid mass with smooth tan fish-flesh homogeneous cut surface. Histological diagnosis was diffuse large B-cell lymphoma with areas of marginal zone B-cell lymphoma of MALT type.

Immunohistochemical Expression of Galectin-3, Cytokeratin 19 and HBME-1 in Papillary Microcarcinoma of the Thyroid Gland (갑상선 미세 유두암종에서 Galectin-3, Cytokeratin 19와 HBME-1의 발현)

  • With the wide use of ultrasonography and fine needle aspiration of the thyroid gland, the incidence of papillary microcarcinoma of the thyroid gland is rapidly increasing nowadays. To improve the diagnostic accuracy of histopathologic findings of papillary thyroid carcinoma, various molecular markers have been used recently. We analysed the expression of galectin-3, cytokeratin 19 and HBME-1, using immunohistochemical technique in 37 cases of papillary microcarcinoma of the thyroid gland to evaluate the diagnostic value of these molecular markers. Immunohistochemically, galectin-3 expression was found in 37 cases of papillary microcarcinoma. Its localization was mostly cytoplasmic. Cytokeratin 19 expression was found in 36 cases. It was mostly localized to the cytoplasm and membrane. HBME-1 expression was found in all cases. Its localization was plasma membrane. The expression of these three molecular markers was negative in the adjacent normal thyroid tissue and accompanying benign lesions, although there are scattered foci of incomplete positive staining in cases of Hashimoto's thyroiditis. Our findings suggest that the immunohistochemical staining using antibodies for galectin-3, cytokeratin 19 and HBME-1 is an useful adjunctive method for the histopathological diagnosis of a papillary microcarcinoma of the thyroid gland.

Ultrasonographic Features and the Diagnostic Role of Core Needle Biopsy at Metastatic Breast Cancer in the Thyroid gland: A Case Report (갑상선에 생긴 전이성 유방암의 초음파 소견 및 중심부 바늘 생검의 진단적 가치: 증례 보고)

  • Metastases to the thyroid gland have rarely been reported in clinical settings, and the thyroid gland is an uncommon site for breast carcinoma metastasis. We report a case of a 64-year-old breast cancer patient diagnosed with metastatic breast carcinoma in the thyroid gland after performing ultrasonography (US)-guided core needle biopsy (CNB) and subsequent total thyroidectomy. On US, the thyroid lesion appeared to be mildly enlarged with multiple internal hypoechoic lines and a few microcalcifications without mass formation. Under US-guidance, CNB was performed by targeting the area with microcalcifications and subsequently diagnosed as metastatic breast carcinoma. Total thyroidectomy revealed that the patient had metastatic invasive ductal carcinoma of the breast with lymphatic spread involving both lobes and the isthmus of the thyroid gland. Although the thyroid gland is an uncommon metastatic site, the unusual features of thyroid metastasis can be observed on US; thus, US-guided CNB effectively aids the diagnosis of thyroid metastasis.

  • Upper gastrointestinal series is an examination that uses X-rays. It is important to defend against exposure to radiation during upper gastrointestinal examination because the organs, such as thyroid gland, lens, breasts, and gonads, with relatively high biological sensitivity to radiation are distributed on the examination area. We have made a whole body phantom that can change the depth of organs. radiation dose of eye, thyroid gland, breast and gonads were measured by the same procedure as the actual upper gastrointestinal examination. When performed only fluoroscopy the mean dose reduction of lens, thyroid gland, breast and gonads was 62.2%. The mean dose reduction of lens, thyroid gland, breast and gonads was 59.0% when both fluoroscopy and spot shoot were performed. Therefore, when performed upper gastrointestinal examination it was confirmed that shielding of the lens, thyroid gland, breast and gonads was effective in decreasing the exposure dose. The manufactured human phantom can be used in measuring radiation dose for deep organ because it can adjust the height corresponding to the organs located in the human body.

Fine Needle Aspiration Cytology of Metastatic Adenocarcinoma of the Thyroid Gland from the Colon - Report of a Case - (갑상선으로 전이된 대장선암종의 세침흡인 세포학적 소견 - 1예 보고 -)

  • Most common metastatic tumors of the thyroid gland are squamous cell carcinoma of head and neck region, adenocarcinoma of breast and lung, malignant melanoma, and renal cell carcinoma of kidney in order of frequency. Metastasis from gastrointestinal tract to the thyroid gland rarely occurs. We experienced a case of fine needle aspiraton cytology of metastatic adenocarcinoma of both thyroid glands from the colon. Cytologic smears showed picket-fence like arrangement of nuclei of carcinoma cells and syncytial type of tissue fragments with acinar pattern on necrotic or mucoid background.

A Case of Primary Squamous Cell Carcinoma of The Thyroid Gland (갑상선에서 발생한 원발성 편평상피세포암 1예)

  • Primary squamous cell carcinoma of thyroid is a very rare malignant tumor with poor prognosis. It is usually diagnosed as an advanced disease infiltrating adjacent organs, and characterized by aggressive clinical course with an average postoperative survival time of less than 1 year. Recently, we had a 79- year-old woman with a painful neck mass who was diagnosed as primary squamous cell carcinoma of thyroid gland. She underwent total thyroidectomy and selective neck dissection(level III, VI) with no further postoperative managements such as radiation therapy or chemotherapy; she died of poor general condition and pneumonia resulting from rapid progression of the lesion on the 38th day after surgery. We report this case with a review of relevant literatures.

Metastatic Melanoma Mimicking a Papillary Carcinoma of the Thyroid in Fine Needle Aspiration Cytology -A Case Report- (갑상샘 유두암을 닮은 전이성 흑색종의 세침 흡인 세포 소견 -1예 보고-)

  • Metastasis to the thyroid gland is very rare in clinical practice. We recently encountered a 65-year-old woman who presented with a large thyroid nodule that mimicked the cytologic features of a papillary thyroid carcinoma on fine needle aspiration biopsy (FNAB). Although initially diagnosed as a papillary thyroid carcinoma, a thorough clinical work-up revealed multiple lesions in the bones, liver, and nasal cavity, which were confirmed as metastases of a malignant melanoma. Despite a thorough physical examination, however, the primary skin lesion could not be identified. Although FNAB shows a high degree of accuracy in diagnosing primary thyroid tumors, it is less accurate in diagnosing metastases to the thyroid gland. A thorough clinical history with appropriate immunohistochemical staining assays is necessary for the accurate diagnosis of metastatic malignant melanoma.
