Houttuynia cordata belongs to the Saururacease family and its leaves, stems, and roots have been used as oriental medicines to treat pneumonia, acute or chronic bronchitis, enteritis, and abscesses and to remove extravasated blood. Recently, the antioxidant, anti-inflammation, antibacterial, and anti-proliferation activities and protection abilities of H. cordata against liver and neuron cell damage have been reported. In this study, ethanol extract and its solvent fractions (fractions of hexane, ethyl acetate, butanol, and water residue) were prepared, and their antithrombosis, antidiabetes, antioxidant, and hemolysis activities were evaluated. The ethyl-acetate fraction of H. cordata (EF-HC) showed the highest polyphenol and flavonoids contents among the fractions and exhibited strong antithrombosis and antioxidant activities. The EF-HC at 5 mg/ml showed 2.09-folds of thrombin time, 2.19-folds of prothrombin time, and 1.69-folds of activated partial thromboplastin time compared to the their solvent control and 30.9, 19.9, and 49.6 ㎍/ml of RC50 against DPPH, ABTS, and nitrite radicals, respectively. Furthermore, the EF-HC did not show any hemolytic activity up to 1 mg/ml, whereas the hexane fraction of H. cordata showed 55% hemolysis at 1 mg/ml. This is the first report of the antithrombosis activity of H. cordata. Our results suggest that quercitirin, hyperoside, orientin, and isoquercitrin in EF-HC are related to its antithrombosis and antioxidant activities and that the EF-HC could be developed as a promising antithrombosis agent.

As the viscosity of the blood increases, the blood becomes more sticky and difficult to flow, so the possibility of thrombosis increases and the probability of ischemic cerebral infarction increases. The importance of measuring blood viscosity has recently been emphasized for the prevention of circulatory system diseases, and the need for a viscometer capable of easily and accurately measuring blood viscosity has emerged. In this study, the measured values of a viscosity standard solution and an artificial blood by a parallel-plate viscometer ARS-Medi were compared with the those by Ares-G2 of TA instrument, which is internationally recognized for its accuracy and reliability. The viscosity of N44 standard solution, which is a Newtonian solution, was almost perfectly matched between the two instruments at all shear rates. In the case of an artificial blood, which is a non-Newtonian solution, the measured values between the two instruments showed a difference of about 10% at the lowest shear rate 1 rad/s; however, at a clinically significant shear rate of 10 rad/s or higher, the measured values between them were consistent within the error range. We expect that ARS-Medi, a newly developed parallel-plate viscometer for blood, using disposable plates, will be very useful in clinical practice as it improves the convenience and hygiene of blood viscosity measurement.

Hot pepper (Capsicum annuum L.) is a valuable plant that is widely used worldwide for food and medicinal purposes. This study compared ethanol extracts of five native Korean varieties (Yuwol-cho, Subi-cho, Sumihyang, Gounbit, and Chilseong-cho) and five improved varieties (Dabok, Cheongyang, Chungseong, Olbokhap, and Shin-honggildong) of peppers cultured in Korea. The extracts were analyzed for color difference, polyphenol content, and their antioxidant, antidiabetic, and antithrombotic activities. The extracts of the improved varieties exhibited higher levels of redness and lower levels of yellowness compared to the native varieties. Polyphenol and flavonoid content analysis revealed significantly higher levels in the Yuwol-cho and Sumihyang varieties, which also exhibited scavenging activities in 1,1-diphenyl-2-picryl hydrazyl anion-, 2,2-azobis(3-ethylbenzothiazoline-6-sulfonate) cation-radical scavenging, and reducing power assay. The Chilseong-cho, Yuwol-cho, and Dabok varieties showed significantly higher nitrite scavenging activity. Antidiabetic activity based on α-glucosidase inhibition was observed in the Subi-cho, Sumihyang, and Gounbit extracts. Evaluation of antithrombotic activity showed that the Yuwol-cho extract prolonged thrombin time by 1.61 times compared to the solvent control at a concentration of 5 mg/ml, while the Dabok extract prolonged prothrombin time and activated partial thromboplastin time by 1.33 times and 2.21 times, respectively. All pepper extracts showed no erythrocyte hemolysis activity up to a concentration of 5 mg/ml. Our results suggest that native Korean peppers have the potential to serve as valuable sources of antioxidant, antidiabetic, and antithrombotic agents. This research also indicates the possibility of replacing improved pepper varieties, which incur significant seed usage fees, with native Korean pepper varieties.
Purpose To evaluate the safety and efficacy of the percutaneous manual aspiration thrombectomy technique to treat thrombotic occlusion of native arteriovenous fistulas. Materials and Methods A retrospective review of 20 patients who underwent percutaneous manual aspiration thrombectomy for native thrombotic arteriovenous fistula occlusion from March 2012 to December 2017 was performed. We evaluated technical and clinical success rates and complications. The primary and secondary patency rates were calculated using the Kaplan-Meier analysis. Results Percutaneous manual aspiration thrombectomy was performed in 20 patients (n = 20) with concomitant balloon angioplasty. The overall technical and clinical success rates were both 85% (n = 17). The native arteriovenous fistulas, based on their site, were the left radiocephalic (n = 13), left brachiocephalic (n = 4), and right radiocephalic (n = 3) fistulas. An underlying stenosis was detected in the juxta-anastomotic venous site (n = 16), outflow draining vein (n = 12), and central vein (n = 4). The primary and secondary patency rates at 1, 3, 6, and 12 months were 100%, 70.6%, 70.6%, and 56.5% and 100%, 94.1%, 94.1%, and 86.9%, respectively. There were no complications associated with procedure. Conclusion Percutaneous manual aspiration thrombectomy is a safe and effective method to treat thrombotic native arteriovenous fistula occlusion.

Objective: Percutaneous portal vein (PV) stent placement can be an effective treatment for symptoms associated with portal hypertension. This study aimed to evaluate the effect of PV stenting on the overall survival (OS) in patients with malignant PV stenosis. Materials and Methods: Two groups of patients with malignant PV stenosis were compared in this retrospective study involving two institutions. A total of 197 patients who underwent PV stenting between November 2016 and August 2019 were established as the stent group, whereas 29 patients with PV stenosis who were treated conservatively between July 2013 and October 2016 constituted the no-stent group. OS was compared between the two groups before and after propensity score matching (PSM). Risk factors associated with OS were evaluated using the Cox proportional hazards model. Procedure-associated adverse events were also evaluated. Results: The stent group finally included 100 patients (median age, 65 [interquartile range, 58-71] years; 64 male). The no-stent group included 22 patients (69 [61-75] years, 13 male). Stent placement was successful in 95% of attempted cases, and the 1- and 2-year stent occlusion-free survival rate was 56% (95% confidence interval, 45%-69%) and 44% (32%-60%), respectively. The median stent occlusion-free survival time was 176 (interquartile range, 70-440) days. OS was significantly longer in the stent group than in the no-stent group (median 294 vs. 87 days, p < 0.001 before PSM, p = 0.011 after PSM). The 1- and 3-year OS rates before PSM were 40% and 11%, respectively, in the stent group. The 1-year OS rate after PSM was 32% and 5% in the stent and no-stent groups, respectively. Anemia requiring transfusion (n = 2) and acute thrombosis necessitating re-stenting (n = 1) occurred in three patients in the stent group within 1 week. Conclusion: Percutaneous placement of a PV stent may be effective in improving OS in patients with malignant PV stenosis.
The regulation of platelet aggregation is crucial for maintaining normal hemostasis, but abnormal or excessive platelet aggregation can contribute to cardiovascular disorders such as stroke, atherosclerosis, and thrombosis. Therefore, identifying substances that can control or suppress platelet aggregation is a promising approach for the prevention and treatment of these conditions. Artemisinin, a compound derived from Artemisia or Scopolia plants, has shown potential in various areas such as anticancer and Alzheimer's disease research. However, the specific role and mechanisms by which artemisinin influences platelet activation and thrombus formation are not yet fully understood. This study investigated the effects of artemisinin on platelet activation and thrombus formation. As a result, cAMP production were increased significantly by artemisinin, as well as phosphorylated VASP and IP3R which are substrates to cAMP-dependent kinase by artemisinin in a significant manner. The Ca2+ normally mobilized from the dense tubular system was inhibited due to IP3R phosphorylation from artemisinin, and phosphorylated VASP by artemisinin aided in inhibiting platelet activity via αIIb/β3 platelet membrane inactivation and inhibiting fibrinogen binding. Finally, artemisinin inhibited thrombin-induced thrombus formation. Therefore, we suggest that artemisinin has importance with cardiovascular diseases stemming from the abnormal platelets activation and thrombus formation by acting as an effective prophylactic and therapeutic agent.
Cardiovascular disease (CVD) is increasingly increasing as the main cause of death worldwide, and activation of platelet in vascular damage is one of the important causes of CVD. In recent, there is a growing interest in anti-thrombotic materials through platelet suppression, and efforts are being made to reduce side effects by using natural bioactive compounds. Known as one of the Flavonoids, hydroxygenkwanin (HGK) is a purified substance in Daphne Genkwa, which is known to have antibacterial, anti-inflammatory and anti-cancer effects, and has been reported to serve as an inhibitor of tissue factor that prevents thrombosis, but its anti-platelet effects and the action mechanisms is not known. In this study, we confirmed that the effects of HGK on the collagen-induced human platelets activation. HGK suppressed phosphorylation of PI3K/AKT and mitogen-activated protein kinases during platelet signaling, and reduced granule secretion in platelets such as ATP and serotonin. In addition, HGK inhibited the phosphorylation of cPLA2 and strongly undermined the production of TXA2, which is a powerful aggregation amplifier. As a result, the platelet aggregation derived by Collagen, a cohesive induced substance, was strongly suppressed by HGK to an IC50 of 86.36 µM. Therefore, HGK might be worth the antithrombotic substance that inhibits the activation and aggregation of human platelets that occur through blood vessel damage.

Purpose The present study was to evaluate the outcomes of Denali filter retrieval. Materials and Methods We retrospectively reviewed 143 patients who received Denali filter insertion from September 2015 to April 2020. Ninety-seven patients who required removal of the filters were include in this study. Filters were retrieved with either standard or advanced techniques. Venography before and after retrieval was obtained to evaluate technical success, complications and duration of filter insertion. Results All 97 filters were retrieved successfully without complications. Ninety-two (94.8%) were retrieved with standard technique and 5 filters (5.2%) required the advanced technique. There were two cases with a filter angle greater than 15 degrees. Inferior vena cava penetration was shown in 17 patients (17.5%) on venography but was not associated with contrast media extravasation after filter removal. Conclusion The Denali filter showed a high rate of successful retrieval without complications. This study adds value to previous studies and trials showing that the Denali filter is a reliable and safe filter that can potentially improve retrieval rates, with increasing use of this device.

Purpose To assess the safety and feasibility of intentionally positioning the catheter tip in the right atrium (RA) without an abutment during implantation of a totally implantable venous access port (TIVAP). Materials and Methods We enrolled 330 patients who had undergone TIVAP implantation between January and December 2016 and postoperative chest CT. The TIVAP was placed using the single-incision technique to access the axillary vein directly from the incision line. To position the catheter tip in the RA without abutment, blood return was checked before cutting. Catheter length and complications were evaluated by retrospectively reviewing medical images and records. Results All patients achieved successful catheter tip positioning without abutment or dysfunction. The median tip position was 15.3 mm distal to the cavoatrial junction (CAJ) on fluoroscopy and 6 mm distal to the CAJ on CT. Catheter tips migrated a median of 10.4 mm cephalically on CT compared to fluoroscopy. Thromboses were detected in the RA and superior vena cava in one patient each. Conclusion Intentional catheter tip positioning in the RA without abutment is a safe and feasible technique with a low incidence of thrombosis and no observed dysfunction.

Splenic artery steal syndrome (SASS) is a cause of graft hypoperfusion leading to the development of biliary tract complications, graft failure, and in some cases to retransplantation. Its management is still controversial since there is no universal consensus about its prophylaxis and consequently treatment. We present a case of SASS that occurred 48 hours after orthotopic liver transplantation (OLTx) in a 56-year-old male patient with alcoholic cirrhosis and severe portal hypertension, and who was successfully treated by splenic artery embolization. A literature search was performed using the PubMed database, and a total of 22 studies including 4,789 patients who underwent OLTx were relevant to this review. A prophylactic treatment was performed in 260 cases (6.2%) through splenic artery ligation in 98 patients (37.7%) and splenic artery banding in 102 (39.2%). In the patients who did not receive prophylaxis, SASS occurred after OLTx in 266 (5.5%) and was mainly treated by splenic artery embolization (78.9%). Splenic artery ligation and splenectomies were performed, respectively, in 6 and 20 patients (2.3% and 7.5%). The higher rate of complications registered was represented by biliary tract complications (9.7% in patients who received prophylaxis and 11.6% in patients who developed SASS), portal vein thrombosis (respectively, 7.3% and 6.9%), splenectomy (4.8% and 20.9%), and death from sepsis (4.8% and 30.2%). Whenever possible, prevention is the best way to approach SASS, considering all the potential damage arising from an arterial graft hypoperfusion. Where clinical conditions do not permit prophylaxis, an accurate risk assessment and postoperative monitoring are mandatory.
