비글견에서 실험적으로 유발한 흉수의 정량 평가: 방사선, 컴퓨터단층촬영 및 초음파 검사 비교 (Quantification of Experimentally Induced-Pleural Effusion in Beagle Dogs: Radiography versus CT and Ultrasonography)

  • 흉수량은 흉수천자의 실시 여부 및 진단과 치료에 이용되는 방법의 결정에 영향을 미치므로 흉수량 측정은 중요하다. 본 실험의 목적은 흉수를 정량화하기 위해 흉부 방사선 및 초음파와 컴퓨터단층촬영을 시행하여 그 결과를 비교하고 정확도를 측정하는데 있다. 임상적으로 건강한 6 마리의 비글견을 이용하여 생리식염수를 최종 용량이 60 ml/kg 될 때까지 체중당 10ml씩 흉강에 주입하면서 흉부 방사선 검사, 초음파 검사, CT 검사를 차례로 실시하였다. 흉부 방사선 사진에서 흉수의 유무 평가 및 흉수 정량화를 실시하였으며 초음파 영상에서 흉수량은 폐의 표면과 흉강벽 사이의 최대 수직 거리를 측정하여 평가하였다. CT 영상에서 흉수량은 soft tissue window에서 평가하였으며 최대 흉수량을 나타내는 영상에서 최소 수직거리를 측정하였다. 흉수량과 흉부 방사선 사진 및 초음파 영상에서의 측정치와 통계학적으로 유의적인 결과를 얻지 못했다. 그러나 CT 검사에서의 측정값과 흉수량과의 유의적인 관계를 나타내었다. 특히 흉수의 정량화에서 좌우측 측정치의 평균값이 좌우측 각각의 값보다 더 높은 정확도를 나타내었다. 흉수를 확인하기 위해서는 흉부 방사선 검사 초음파 검사 및 CT 검사가 모두 가능하나 흉수를 정량화하기 위해서는 흉부 방사선 및 초음파 검사보다 CT 검사가 유용함을 확인하였다.

  • A 15-year-old, castrated male shih-tzu dog was referred to Veterinary Medical Teaching Hospital of Gyeongsang National University due to dyspnea, anorexia, depression of 1 week's duration. On thoracic radiography, triangular shaped soft tissue opacity mass presented in the left cranial lung lobe region. On computed tomography, there was a $3.8{\times}2.5$ cm mass in the left thoracic cavity, which was lobulated and adhered to ventral pleural surface. Histopathologically, the neoplastic cell population consisted of a lot of lymphocytes and a few of Hassal's corpuscles. The immunohistochemistry, lymphocytes were diffusely positive for CD3 and focal positive $CD79{\alpha}$. Based on gross, histologic, and immunohistochemical findings, the neoplasm was diagnosed a lymphocyte predominant thymoma.

Recurrent Pericardial Effusion with Feline Infectious Peritonitis in a Cat

  • A five-month-old, male Domestic Korean shorthair was referred to our hospital with a history of lethargy, anorexia, and globoid cardiac silhouette on thoracic radiography. Physical examination showed dehydration and anemia was revealed on blood analysis. On thoracic radiography and echocardiography, the patient showed pericardial effusion and ultrasound-guided pericardiocentesis was performed. A Rivalta test of the pericardial effusion showed a positive result. As the patient had recurrent pericardial effusion, pericardiectomy was performed. He was tentatively diagnosed with wet form feline infectious peritonitis (FIP) and treated with Polyprenyl immunostimulant (PI). Neurological signs were eventually seen and he was euthanized. Histopathologic changes with markedly expanded neutrophils, lymphocytes, plasma cells, and macrophages with fibrous connective tissue and collagenous fibers were detected. Immunohistochemistry for FIP antigen was performed and results showed FIPV-positive multifocal aggregates of cells. Pericardial effusion is an atypical condition in cats with FIP, but can be presented. This case report describes FIP with pericardial effusion in a cat, in which definitive diagnosis of FIP was done using biopsy via pericardiectomy.

Free Hand Pedicle Screw Placement in the Thoracic Spine without Any Radiographic Guidance : Technical Note, a Cadaveric Study

  • Thoracic pedicle screw fixation techniques are still controversial for thoracic deformities because of possible complications including neurologic deficit. Methods to aid the surgeon in appropriate screw placement have included the use of intraoperative fluoroscopy and/or radiography as well as image-guided techniques. We describe our technique for free hand pedicle screw placement in the thoracic spine without any radiographic guidance and present the results of pedicle screw placement analyzed by computed tomographic scan in two human cadavers. This free hand technique of thoracic pedicle screw placement performed in a step-wise, consistent, and compulsive manner is an accurate, reliable, and safe method of insertion to treat a variety of spinal disorders, including spinal deformity.

A Case of Mediastinal Cavernous Hemangioma

  • We report a case of cavernous hemangioma of the anterior mediastinum that was incidentally detected by chest radiography taken at a routine health check-up. A mass lesion was seen in the anterior mediastinum on computed tomography and magnetic resonance imaging. Direct surgical removal was performed for diagnosis and treatment through right thoracotomy. Histopathology confirmed the mass as a cavernous haemangioma. Post-operative course was uneventful.

원발성 개 늑연골육종 1예 (Primary Rib Chondrosarcoma in a Dog)

  • Primary rib tumors occur in dogs at a much lower frequency than appendicular bone neoplasms. A 3-year-old 3.5kg female maltese was referred to the Veterinary Medical Teaching Hospital because of a 2-month history of severe coughing and a 1-month history of dyspnea, gaging, and anorexia. The plain thoracic radiography revealed a huge well-defined mass in the left thoracic cavity, and a diagnosis of primary rib chondrosarcoma was established by histopathologic evaluation of the surgical biopsy specimen. At operation, a 8-x12-x4.5-cm relatively large firm mass was found filling the lumen of the thoracic, which was diffusely attached to the left 2-9th rib. This mass oppressed the heart, left lung and diaphragm.

포메라니안견에서 발생한 횡격막허니아의 임상학적, 방사선학적, 초음파학적 및 수술적 소견 (Clinical, Radiographic, Echocardiographic, Intraoperative Findings of Diaphragmatic Hernia in a Pomeranian Dog)

  • A 8 years old male Pomeranian weighing 4 kg was referred because of coughing of 4 months' duration. Heart sounds and cardiac apex beat were showed more intense on the right side. On radiographic views, loss of normal line of the diaphragm, gas-containing intestines and stomach in thoracic cavity, and right displacement of heart were observed. Ultrasonography revealed that liver located adjacent to the heart. Although the dog died due to severe respiratory disorder in surgical procedure, in thoracic and abdominal surgery, a large defect was found in the left and right ventral muscular portion and left central tendon of the diaphragm, extending from the esophageal hiatus to rib. Left and right cranial lobe of liver, small intestines, stomach and spleen were herniated in the thoracic cavity. Because of the size and chronicity of the defect in the diaphragm, closure was impossible with an abdominal muscle graft.

Cervical lung lobe herniation in a Pekingese dog

  • An 8-year-old intact male Pekingese (weighing 13 kg) was presented for evaluation of chronic coughing. Thoracic radiography found a redundant tissue swelling (protruded on expiration and collapsed on inspiration) on the ventral neck just cranial to the thoracic inlet. Fluoroscopy also identified that the cranial portion of the right/left cranial lung lobe was seen to protrude through the thoracic inlet into the ventral neck on expiration. On the echocardiogram, there were no abnormal jets in all cardiac valves and no dilation in all cardiac chambers. Based on diagnostic imaging studies, the case was diagnosed as cervical lung lobe herniation.

Calcified Dirofilaria immitis Mimicking Thoracic Foreign Body in a Dog

  • A 13-year-old neutered male Pomeranian, weighting 3 kg, presented with respiratory distress and depression. Radiographic examination revealed calcified ring-like opacities in the main pulmonary artery, mimicking thoracic foreign bodies. Additionally, right heart and main pulmonary artery enlargement and notable lung infiltrations were also observed. Echocardiography showed coil shaped structures in the main pulmonary artery with increased echogenicity compared to other nearby heartworms, which is consistent with calcified Dirofilaria immitis (heartworms). The dog was diagnosed with caval syndrome, which is the advanced and severe manifestation of heartworm infection. This report presents a rare case of calcified heartworm infection observed during a radiological examination, which resemble foreign bodies. Therefore, chronic heartworm disease should be considered as a differential diagnosis when radiopaque ring-like opacities are observed in the pulmonary artery on thoracic radiographs.

Clinical Implementation of Deep Learning in Thoracic Radiology: Potential Applications and Challenges

  • Chest X-ray radiography and computed tomography, the two mainstay modalities in thoracic radiology, are under active investigation with deep learning technology, which has shown promising performance in various tasks, including detection, classification, segmentation, and image synthesis, outperforming conventional methods and suggesting its potential for clinical implementation. However, the implementation of deep learning in daily clinical practice is in its infancy and facing several challenges, such as its limited ability to explain the output results, uncertain benefits regarding patient outcomes, and incomplete integration in daily workflow. In this review article, we will introduce the potential clinical applications of deep learning technology in thoracic radiology and discuss several challenges for its implementation in daily clinical practice.
