Early and Mid-term Changes of the Distal Aorta after Total Arch Replacement for Acute Type A Aortic Dissection

  • Background: Total arch replacement (TAR) is being more widely performed due to recent advances in operative techniques and cerebral protective strategies. In this study, the authors reviewed the relationship between TAR and early- and mid-term changes of the false lumen after TAR in acute type A aortic dissection. Materials and Methods: Twenty-six patients (aged, $54.7{\pm}13.3$ years) who underwent TAR for acute type A dissection between June 2004 and February 2012 were reviewed. The relationship between the percentage change in the aortic diameter and the false lumen patency status was assessed by examining the early and late postoperative computed tomography imaging studies. Results: There were two in-hospital mortalities, one late death, and three follow-up loses. The mean follow-up duration for the final 21 patients studied was $54{\pm}19.0$ months (range, 20 to 82 months). The incidence of false lumen thrombosis within 2 weeks of surgery in the proximal, middle, and distal thoracic aorta, and the suprarenal and infrarenal abdominal aorta were 67%, 38%, 38%, 48%, and 33%, respectively, and 57%, 67%, 52%, 33%, and 33% for those examined at a mean of $49{\pm}18$ months after surgery, respectively. The false lumen regressed in 11 patients (42.3%). The aortic diameters were larger in the patients with a patent false lumen than those with a thrombosed false lumen at all levels of the descending aorta (p <0.05). Conclusion: TAR and a more complete primary tear-resection can be accomplished with a relatively low-risk of morbidity and mortality. Enlargement of the distal aorta significantly correlated with the false lumen patency status.

Imaging Diagnosis of Dilated Cardiomyopathy in a Maltese Dog

  • A 6-year-old, spayed female, Maltese dog with tachypnea and dry cough was presented to Gyeongsang National University Veterinary Medical Teaching hospital. On physical examination, its respiration rate was 132 per minute. Decreased partial pressure of oxygen, partial pressure of carbon dioxide, and hyperlactatemia were found on arterial blood gas analysis. Its diastolic blood pressure was 80 mmHg. Auscultation revealed arrhythmia. Electrocardiogram revealed P pulmonale, P mitrale, and ventricular premature complexes. Thoracic radiographs revealed mild enlargement of both atrium and moderate enlargement of the left ventricular. There was also a moderate alveolar pattern in the right and caudal part of the left cranial lung lobe. Two-dimensional echocardiography showed enlargement of generalized four chambers without remarkable findings of valvular degeneration. M-mode echocardiography showed decreased left ventricular fractional shortening and enlarged left ventricular internal diameter at both end-systolic and end-diastolic. Color-flow Doppler imaging revealed eccentric turbulent flow starting below the left ventricular outflow tract and extending into the left atrium during systole. Spectral Doppler recordings revealed a high velocity flow through the mitral, tricuspid, aorta, and pulmonic regurgitation. Restrictive transmitral flow revealed high E-wave velocity, short E-wave deceleration time, and reduced A-wave velocity. There was also low ejection velocity thorough left ventricular out tract flow. Based on echocardiographic examination, dilated cardiomyopathy was the tentative diagnosis. The dog was medicated with inotropes, angiotensin converting enzyme inhibitor, and diuretics. At the 10-day following-up, the dog died suddenly. This report describes echocardiographic diagnosis and prognosis of dilated cardiomyopathy rarely reported in small breed dogs.

  • 대혈관전위 환자에서 동맥치환술 시행 시 관상동맥의 위치는 수술의 성적 및 환자의 예후에 매우 중요한 요소이다. 본 증례는 과거 대혈관전위로 동맥치환술을 시행 받았던 환자에서 발생한 관상동맥 압박에 대한 보고이다. 14세 남자 환자로 운동 시 협심증을 주소로 내원하였고, 영상학적 검사 결과 좌관상동맥 기시부가 대동맥과 폐동맥 사이에 위치하여 눌리고 있는 양상이어서 수술적 치료를 결정하였다. 수술은 좌관상동맥 기시부를 대동맥에서 버튼 절제한 후 좌외측으로 재이식하여 폐동맥에 눌리지 않도록 하였다. 수술 후 관상동맥의 대혈관에 의한 압박 소견은 사라졌으며 환자는 현재까지 합병증이나 증상 재발 없이 추적 관찰 중이다.

The Change of Adjacent Segment and Sagittal Balance after Thoracolumbar Spine Surgery

  • Objective : To characterize perioperative biomechanical changes after thoracic spine surgery. Methods : Fifty-eight patients underwent spinal instrumented fusions and simple laminectomies on the thoracolumbar spine from April 2003 to October 2008. Patients were allocated to three groups; namely, the laminectomy without fusion group (group I, n = 17), the thoracolumbar fusion group (group II, n = 27), and the thoracic spine fusion group (group III, n = 14). Sagittal (ADS) and coronal (ADC) angles for adjacent segments were measured from two disc spaces above lesions at the upper margins, to two disc spaces below lesions at the lower margins. Sagittal (TLS) and coronal (TLC) angles of the thoracolumbar junction were measured from the lower margin of the 11th thoracic vertebra body to the upper margin of the 2nd lumbar vertebra body on plane radiographs. Adjacent segment disc heights and disc signal changes were determined using simple spinal examinations and by magnetic resonance imaging. Clinical outcome indices were determined using a visual analog scale. Results : The three groups demonstrated statistically significant differences in terms of angle changes by ANOVA (p<0.05). All angles in group I showed significantly smaller angles changes than in groups II and III by Turkey's multiple comparison analysis. Coronal Cobb's angles of the thoracolumbar spine (TLC) were not significantly different in the three groups. Conclusion : Postoperative sagittal balance is expected to change in the adjacent and thoracolumbar areas after thoracic spine fusion. However, its prevalence seems to be higher when the thoracolumbar spine is included in instrumented fusion.

In Vivo Spinal Distribution of Cy5.5 Fluorescent Dye after Injection via the Lateral Ventricle and Cisterna Magna in Rat Model

  • Objective : The purpose of this study was to find an optimal delivery route for clinical trials of intrathecal cell therapy for spinal cord injury in preclinical stage. Methods : We compared in vivo distribution of Cy5.5 fluorescent dye in the spinal cord region at various time points utilizing in vivo optical imaging techniques, which was injected into the lateral ventricle (LV) or cisterna magna (CM) of rats. Results : Although CM locates nearer to the spinal cord than the LV, significantly higher signal of Cy5.5 was detected in the thoracic and lumbar spinal cord region at all time points tested when Cy5.5 was injected into the LV. In the LV injection Cy5.5 signal in the thoracic and lumbar spinal cord was observed within 12 hours after injection, which was maintained until 72 hours after injection. In contrast, Cy5.5 signal was concentrated at the injection site in the CM injection at all time points. Conclusion : These data suggested that the LV might be suitable for preclinical injection route of therapeutics targeting the spinal cord to test their treatment efficacy and biosafety for spinal cord diseases in small animal models.

심장 이식된 백서에서의 거부반응 진단의 핵의학적 방법 (Nuclear Medicine Methods of Rejection Diagnosis in Transplanted Rat Model)

  • 배경: 장기 심장 이식 후에 나타나는 거부반응의 정도와 상태에 대한 정확한 진단은 내심근 조직검사 (endomyocardial biopsy)로 이루어지고 있으나, 이 방법은 환자에 침습적이고 보다 자주 시행하여 감시하기에는 부적합하여, 덜 침습적이고 계속 지속적으로 감시할 수 있는 방법을 찾고자 하였다. 대상 및 방법: 250~300 gm 정도의 백서에서 이소성의 심장이식(heterotopic heart transplantation)을 Ono-Lindsey Method로 시행한 후 1개월 후에 $^{99m}$ Tc-pyrophosphate (PYP) 1.5 mCi를 꼬리 정맥을 통해 주입한 후 스캔(scan)을 시행하고 심장과 척추의 섭취비를 구하여 이식된 심장의 거부반응에 민감하게 반응할 수 있을 지에 대해 조직 검사와 비교관찰 하였다. 걸과: 20마리의 이식된 심장에 $^{99m}$ Tc-PYP스캔에서의 심장/척추 섭취비와 조직 검사 결과를 분석하여 심장/척추 섭취비가 0.09 이상을 거부반응이 있는 것으로 판정하였을 때 20마리에서 진양성 3예, 진음성 12예, 가양성 3예, 가음성 2예로 예민도 (Sensitivity) 60%,특이도(Specificity) 80%의 결과를 얻었다. 걸론: 이식된 장기의 거부반응이 장기이식의 예후에 미치는 중요한 원인이 되므로, 이에 대한 조기 진단할 수 있는 방법으로 $^{99m}$ Tc-PYP스캔이 유용하였으며, 비침습적이고 간단한 검사방법이므로 거부반응 여부를 감시하는 데 도움이 될 것으로 판단된다.

  • 대동맥 동맥류/대동맥 박리증, 두눈먼거리증, 목젖갈림증/입천장갈림증, 그리고 동맥혈관계의 이상과다 뒤틀림 등을 독특한 표현형으로 하는 Loeys-Dietz 증후군은 새로이 기술된 공격적 성향의 결체 조직 질환으로, transforming growth factor-$\beta$ receptor type 1 또는 type 2를 encoding 하는 유전자 돌연 변이가 발병 원인이다. Loeys-Dietz 증후군은 Marfan 증후군, Ehlers-Danlos 증후군 4형 등의 표현형과 일부 비슷한 형태를 공유한다. 그러나, Loeys-Dietz 증후군은 다른 질환보다 더 심한 병태생리적 특성을 가지고 있기 때문에, 임상의들은 이들 결체 조직 질환들을 감별하여야 한다. 강한 의심, 조기 진단, 예방적 수술, 그리고 지속적 영상 검사가 적절한 Loeys-Dietz 증후군 치료를 위해 실행되어야 한다. 저자들은 대동맥 파열, 목젖갈림증, 그리고 두눈먼거리증의 3징후를 가진 Loeys-Dietz 증후군의 환자를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

양전자단층촬영/전산화단층촬영(integrated PET/CT)을 이용한 비소세포폐암의 림프절 병기판정 (Accuracy of Nodal Staging with Integrated PET/CT Scanning in Non-small Cell Lung Cancer)

  • 배경: 최근 양전자단층촬영/전산화단층촬영(PET/CT) 검사가 점차 보편화 되어가고 있다. 림프절 병기 진단에 있어서 CT와 PET/CT의 유용성 및 실효성에 대해 알아보고자 하였다. 대상 및 방법: 2006년 1월부터 2009년 8월까지 본원에서 악성종양으로 폐절제술을 받은 110명의 환자 중, 술 전 본원에서 조영증강 흉부전산화단층촬영 (CT)과 전신 양전자단층촬영/전산화단층촬영 결합영상(PET/CT fusion imaging, integrated PET/CT) 검사를 모두 시행 받고, 술 후 원발성 비소세포폐암을 진단 받은 48명의 환자들을 대상으로 하였다. 림프절을 superior mediastinal nodes, aortic nodes, inferior mediastinal nodes, 그리고 Nl nodes의 4군으로 분류하여 CT와 PET/CT의 진단력에 대한 통계분석을 실시하였다. 결과: 4개의 군 가운데, inferior mediastinal nodes를 제외한 나머지 군들은 CT보다 PET/CT에서 민감도가 향상되는 소견을 보였다. 하지만 두 검사의 진단력 차이를 각각의 군에 대해 McNemar's test를 사용하여 분석한 결과 통계적으로 유의한 차이는 보이지 않았다(p-values; superior mediastinal nodes=0.109, aortic nodes=1.000, inferior mediastinal nodes=0.625, Nl nodes=0.424). 결론: 본 연구에서 종격동 림프절에 대한 PET/CT의 진단력은 조영증강 CT와 비슷한 정도로 나타났다. 현재 PET/CT는 CT와 상호보완적인 수단으로써 사용되어야 할 것으로 사료되며, 앞으로 촬영 기술 및 판독 수준이 더욱 발전하고 결과들이 많이 축적되었을 때에는 보다 좋은 성적을 보일 것으로 생각된다.

  • 신경매독은 스피로헤타(spirochete)인 Treponema pallidum에 의한 뇌와 척수의 드문 감염증이다. 저자들은 53세 남자환자에서 시신경, 삼차신경, 안면신경, 전정와우신경, 중뇌동맥, 후뇌동맥, 척수와 척수막을 침범한 복합적인 수막혈관성, 수막척수염 형태의 신경매독을 경험하여 이를 보고하고자 한다. 본 증례를 통해 뇌수막염과 더불어 뇌신경염, 뇌혈관염이 있고 수막척수염을 동반하는 복합적인 중추신경계 이상 소견이 있어 임상적인 진단이 어려운 경우 신경매독을 감별진단으로 고려하여 영상 소견을 바탕으로 한 빠른 진단이 필요함을 강조하고자 한다.

체열촬영으로 관찰한 전기수지자극의 효과 (Alteration in Infrared Thermal Imaging by Use of Acupuncture-like Electric Stimulation on Finger Control Gate)

  • Acupuncture-like transcutaneous electrical nerve stimulation(ALTENS) on acupuncture site(dorsal and ventral side of finger) were compared with a placebo site(forearm) by infrared thermal imaging. Six disease-free volunteers underwent, on different days, an ALTENS treatment and a placebo treatment in a cross-over sequences of stimulation control and inhibition control in excess of 50 treatments. ALTENS treatments were given at 30Hz at an intensity just below pain threshold delivered to acupuncture points on fingers. Placebo stimulations were administered in similar manner. After every thirty minutes of ALTENS and placebo treatment with stimulation, inhibition control sequence and vice versa, we examined whole body infrared thermal imaging and checked changed skin temperature on frontal, anterior chest, upper and lower abdomen, dorsal and ventral aspect of hand, thoracic and lumbar area, anterior and posterior aspect of lower leg. There were significant skin temperature elevations with ALTENS treatment, especially finger control gate corresponding organ area. Placebo treatment revealed no skin temperature change. We concluded that ALTENS on finger control gate influence physiologic state as opposed to conventional electric stimulation.
