  • Purpose: Although serosal invasion is a critical predisposing factor for peritoneal dissemination in advanced gastric cancer, the accuracy of preoperative assessment using routine imaging studies is unsatisfactory. This study was conducted to identify high-risk group for serosal invasion using preoperative factors in patients with advanced gastric cancer. Materials and Methods: We retrospectively analyzed clinicopathological features of 3,529 advanced gastric cancer patients with Borrmann type I/II/III who underwent gastrectomy at Korea Cancer Center Hospital between 1991 and 2005. We stratified patients into low-(${\leq}40%$), intermediate-(40~70%), and high-risk (>70%) groups, according to the probability of serosal invasion. Results: Borrmann type, size, longitudinal and circumferential location, and histology of tumors were independent risk factors for serosal invasion. Most tumors of whole stomach location or encircling type had serosal invasion, so they belonged to high-risk group. Patients were subdivided into 12 subgroups in combination of Borrmann type, size, and histology. A subgroup with Borrmann type II, large size (${\geq}7$ cm), and undifferentiated histology and 2 subgroups with Borrmann type III, large size, and regardless of histology belonged to high-risk group and corresponded to 25% of eligible patients. Conclusions: This study have documented high-risk group for serosal invasion using preoperative predictors. And risk stratification for serosal invasion through the combination with imaging studies may collaboratively improve the accuracy of preoperative assessment, reduce the number of eligible patients for further staging laparoscopy, and optimize therapeutic strategy for each individual patient prior to surgery.

  • This study has been carried out to investigate the effect of the beverage contained Radix puerariae (15%) and Bambusae caules in Liquamen Phyllostachyos (1%) on the alcohol administered mouse. The control group was treated with alcohol (40% alcohol, 8.0 mL/kg of body weight), experimental group 1 (Exp.1) was administered with beverage (8.0 mL/kg of body weight) before alcohol treatment, and experimental group 2 (Exp.2) was administered with beverage after alcohol treatment. Blood alcohol concentrations were decreased by 11% (in Exp. 1) and 17% (in Exp. 2) compared with control group. The concentrations of serum aspartate aminotransferase (AST) were slightly increased in Exp. 1 and Exp.2, but alanine aminotransferase (ALT) levels were not changed compared with control group. Mn-Superoxide dismutase activity was increased by about 70% in the experimental groups in comparison to control group. The Exp. 1 and Exp. 2 contained more periodic-acid Schiff (PAS)-positive materials than the control group. Electron microscopical observations showed that, cell organelles of hepatocytes were more disoriented in the control group than in Exp. 1 and Exp. 2. These results suggest that the beverage contained Radix puerariae and Bambusae caules in Liquamen Fhyllostachyos is a safe and effective therapeutic agent for alcohol abuse.

  • Background: Reactive oxygen species play critical roles in homeostasis and cell signaling. Dexmedetomidine, a specific agonist of the ${\alpha}2$-adrenoceptor, has been commonly used for sedation, and it has been reported to have a protective effect against oxidative stress. In this study, we investigated whether dexmedetomidine has a protective effect against $H_2O_2$-induced oxidative stress and the mechanism of $H_2O_2$-induced cell death in normal human fetal osteoblast (hFOB) cells. Methods: Cells were divided into three groups: control group-cells were incubated in normoxia without dexmedetomidine, hydrogen peroxide ($H_2O_2$) group-cells were exposed to $H_2O_2$ ($200{\mu}M$) for 2 h, and Dex/$H_2O_2$ group-cells were pretreated with dexmedetomidine ($5{\mu}M$) for 2 h then exposed to $H_2O_2$ ($200{\mu}M$) for 2 h. Cell viability and apoptosis were evaluated. Osteoblast maturation was determined by assaying bone nodular mineralization. Expression levels of bone-related proteins were determined by western blot. Results: Cell viability was significantly decreased in the $H_2O_2$ group compared with the control group, and this effect was improved by dexmedetomidine. The Hoechst 33342 and Annexin-V FITC/PI staining revealed that dexmedetomidine effectively decreased $H_2O_2$-induced hFOB cell apoptosis. Dexmedetomidine enhanced the mineralization of hFOB cells when compared to the $H_2O_2$ group. In western blot analysis, bone-related protein was increased in the Dex/$H_2O_2$ group. Conclusions: We demonstrated the potential therapeutic value of dexmedetomidine in $H_2O_2$-induced oxidative stress by inhibiting apoptosis and enhancing osteoblast activity. Additionally, the current investigation could be evidence to support the antioxidant potential of dexmedetomidine in vitro.

  • Efforts have been made to explore the possibility of using garlic as an antimicrobial therapeutic agent since garlic extract and its individual sulfur compounds show antimicrobial activities against all kinds of microorganisms including bacteria, molds, yeasts and protozoa. Staphylococcus aureus has been the most studied bacteria along with many other Gram positive and negative pathogenic bacteria, including species of the genera Clostridium, Mycobacterium, Escherichia, Klebsiella, Bacillus, Salmonella and Shigella. Candida albicans has been the most studied among the eukaryotic microorganisms. A pathogenic protozoa, Giardia intestinalis, was also tested. All the microorganisms tested was inhibited by garlic extract or its sulfur components. Garlic has been known to be growth inhibitory only when fresh garlic is crushed, since allicin-generating reaction is enzyme-catalyzed. Allicin is known to be growth inhibitory through a non-specific reaction with sulfhydryl groups of enzyme proteins that are crucial to the metabolism of microorganisms. Another plausible hypothesis is that allicin inhibits specific enzymes in certain biological processes, e.g. acetyl CoA synthetase in fatty acid synthesis in microorganisms. Allicin transforms into other compounds like ajoene and various sulfides which are also inhibitory to microorganisms, but not as potent as their mother compound. It is reported recently that garlic heated at cooking temperatures is growth inhibitory especially against yeasts, and that the growth inhibitory compound is allyl alcohol thermally generated from alliin in garlic.

  • Objective: The purpose of this study was to investigate the effects of telerehabilitation on stroke patients through remotely operated intervention and monitoring. Methods: Literature from 2000 to April 2018 was collected through PubMed, Embase, Cochrane, and RISS. We used telerehabilitation, telemedicine, and stroke as the search terms in regard to foreign literature, and the terms telerehabilitation, stroke, and CVA in regard to Korean literature. A total of 406 foreign and 15 Korean published studies were found. As a result, a total of seven documents was selected for the analysis. Results: As a result of the analysis, all the interventions applied through telerehabilitation were provided remotely, and significant effects were reported between pre-post assessments. In addition, the significant effects of telerehabilitation were reported through analyzing pre-post(n=7) and between groups(n=4) assessments. The monitoring used could be categorized according to purpose, that is, for checking (n=3) and as an intervention (n=4). Conclusions: This study confirmed, within limits, that the application of telerehabilitation could be a potential alternative for stroke patients with limited rehabilitation services. In order to apply telerehabilitation in Korea, it is necessary to study the cost effectiveness, according to the current domestic situation, and confirm the most effective monitoring method based on the intervention.

  • Objectives : Benign prostatic hyperplasia (BPH) is one of the most common diseases among elderly men. Though medicines such as 5${\alpha}$-reductase inhibitor (finasteride) have recently been developed for treating BPH, their adverse effects and low efficacy should not be overlooked. Curcuma longa has a long history of use in traditional medicines of Asian countries. Many reports conclude the component curcumin in Curcuma lonfa, has the potential to treat various diseases including prostate cancer. In this study, we investigated the therapeutic effects and action mechanism of Curcuma longa with a BPH rat model. Methods : Sprague-Dawley rats were used with subcutaneous injection of testosterone after castration, which were histologically similar to human BPH. A total of 30 rats were equally divided into five groups: Group 1 served as control (sham-operated group): Group 2 was the model group: Group 3 and Group 4 animals were administered Curcuma longa at dose levels of 0.5g/kg and 1.0g/kg: Group 5 served as a positive control group and was treated with finasteride at a dose of 1 mg/kg. The drugs were administered orally once a day for 30 days consecutively. After 31 days, the prostates were removed, and analyzed for their prostatic weight and histological examination. Results : The oral Curcuma longa ingestion group showed statistically significant decreases in their prostatic weights compared with the BPH-induced group and the oral finasteride ingestion group (p<0.05). Curcuma longa is also very safe in liver and kidney up to a dose of lg/kg. Injected testosterone histologically led to prostatic hyperplasia in rats, but oral Curcuma longa ingestion decreased this change. Conclusions : These results suggest that Curcuma longa has a definite inhibitory effect on BPH and might be an alternative medicine for treatment and prevention of human BPH.

  • In this study, we investigated the effects of total ginseng saponin (TGS) on the cutaneous wound healing process using histological analysis. A total of 24 ICR mice, 5-weeks-old, were used for all in vivo experiments. Mice were divided into control and TGS-treated groups and four equidistant 1-cm full-thickness dorsal incisional wounds were created. The wounds were extracted at days 1, 3, 5, and 7 post-injury for histomorphometrical analysis including wound area and contracture measurements, keratinocyte migration rate, and calculation of infiltrating inflammatory cells. The results showed that the wound area was smaller and keratinocyte migration rate was higher in the TGS-treated group than that of the control group from days 3 to 7. Inflammatory cells in the TGS-treated group at days 1 and 3 were reduced compared to the control group. Wound contraction in the TGS-treated group was greater than in the control group on days 3 to 5, and collagen deposition in the TGS-treated group was higher than in the control group during wound healing. The results indicate a beneficial effect of TGS when used to treat skin wounds.

  • Tofisopam is a new type of tranquilizer valuable for the relief of anxiety and tension in a wide range of emotional disorders. Tofisopam has the therapeutic characteristics of a minor tranquilzer and a mild stimulatory effect. The purpose of the present study was to evaluate the bioequivalence of two tofisopam tablets, $Grandaxin^{TM}$ (Hwan In Pharmaceutical Co., Ltd.) and $Tofim^{TM}$ (Kyung Dong Pharmaceutical Co., Ltd.), according to the guidelines of Korea Food and Drug Administration (KFDA). Eighteen normal male volunteers, $23.11\;{\pm}\;2.83$ years in age and $65.43\;{\pm}\;7.64\;kg$ in body weight, were divided into two groups and a randomized $2{\times}2$ cross-over study was employed. After one tablet containing 50 mg of tofisopam was orally administered, blood was taken at predetermined time intervals and the concentrations of tofisopam in serum were determined using HPLC method with UV detector. The pharmacokinetic parameters such as $AUC_t$, $C_{max}\;and\;T_{max}$ were calculated and ANOVA test was utilized for the statistical analysis of the parameters. The results showed that the differences in $AUC_t$, C_{max}\;and\;T_{max}$ between two tablets based on the $Grandaxin^{TM}$ were -5.59%, 2.22% and -13.18%, respectively. Minimum detectable differences $({\Delta})$ at ${\alpha}=0.10$ and $1-{\beta}=0.8$ were less than 20% (e.g., 14.95% and 19.34% for $AUC_t\;and\;C_{max}$, respectively). The powers $(1-{\beta})$ at ${\alpha}=0.10$, ${\Delta}=0.2$ for $AUC_t$ and $C_{max}$ were 95.21% and 81.93%, respectively. The 90% confidence intervals were within {\pm}20%$ (e.g., $-15.64{\sim}4.45$ and $-10.77{\sim}15.21$ for $AUC_t\;and\;C_{max}$, respectively). Two parameters met the criteria of KFDA for bioequivalence, indicating that $Tofim^{TM}$ tablet is bioequivalent to $Grandaxin^{TM}$ tablet.
