CALCIFYING ODONTOGENIC CYST: A CASE REPORT (치성석회화 낭종 : 증례 보고 및 문헌 고찰)

  • The calcifying odontogenic cyst(COC) is considered to occupy a position between a cyst and an odontogenic tumor-having charateristics of both. Gorlin and col. described the COC for first time as an own pathological entity in 1962. Clinically, the COC represents 1% of the odontogenic lesion. It is possible to be found from the first decade to the eight decade but is more frequent during the second decade. It affects in same proportion the maxilla and jaw, being the most in tooth-bearing area of the jaw. This case of COC associated with an unerupted tooth which appeared in the right mandible of 22-year-old woman, was reported. This case report is to present a review of the literature relates to this case of COC and its treatment, discuss clinical, radiographic, histological and therapeutic aspects.

Intramedullary Spinal Cord Metastasis : A Report of Two Cases and a Review of the Literature (척수내 전이암 - 2례 보고 및 문헌고찰 -)

  • Intramedullary spinal cord metastases (ISCM) account for only $3.4\%$ of symptomatic metastases to the spinal cord. The survival of patients with ISCM is characteristically short, often no longer than 2 months, due to a rapid neurologic deterioration and the presence of widespread metastases, Including metastases to the brain. We report two cases of ISCM arising from primary sphenoid sinus carcinoma and primary lung cancer along with a review of the literature. The case of ISCM from the primary sphenoid sinus is the third case of secondary syringomyelia due to ISCM In the world literature, and ISCM from the primary lung cancer is the first case reported in Korea. One case showed a slow progression of symptoms and a longer survival (26 months after the radiotherapy to the spine), and the other showed a rapid deterioration of symptoms with a shorter survival. More effective palliation can be achieved if the disease is diagnosed at an early stage when the neurologic deficits are still reversible.

  • Objective & Methods : The case report is to observe 27 case of venesection with negative pressure therapy(buhang) operated at CV17 (Danjuong) on sudden palpitation. We carried out to analyze the distribution and Martin's evaluation and weighted scale by sex & age, patient conditional grade, accompanied symptoms, duration, number of treatments as well. Results : 1. Forties is significantly distributed on sudden palpitation. 2. Depression instance out of many accompanied symptoms is significantly distributed. 3. Mean of operant frequency is significantly resulted on 10 or 11 times. 4. The improvement on the weighted scale is over one grade. 5. Younger is high on the assessment of treatment on Martin's evaluation and weighted scale by age, while older is low. 6. Lesser one year and more three years is some significantly resulted on the assessment of treatment on Martin's evaluation and weighted scale by duration. 7. Pleurodynia, insomnia, distress in the stomach are effective more 90%, and depression, asthma are more 80% on the assessment of treatment on Martin's evaluation by accompanied symptoms, while asthma and pleurodynia are very significantly resulted on the assessment of treatment on weighted scale. 8. The assessment of treatment on Martin's evaluation and weighted scale have no connection with operant frequency. Conclusion : These results suggest that venesection with negative pressure therapy(buhang) operated at CV17 (Danjuong) has a therapeutic effect on on sudden palpitation.

Case of Adductor Spasmodic Dysphonia Patient Complaining of Voice Tremor and Hoarseness Treated with Combined Korean Medical Therapies (음성 떨림과 애성을 호소하는 내전형 연축성 발성장애 환자에 대한 복합 한의치험 1례)

  • Background: Adductor spasmodic dysphonia (ASD) is caused by the involuntary contraction of laryngeal muscles due to dystonia localized to the larynx. In the case of ASD, conventional treatment is mainly performed with a botulinum toxin injection. However, the botulinum toxin injection has a short-lasting effect and requires repeated injections. Alternatives are needed due to concerns over adverse effects, such as general weakness and airway aspiration caused by the botulinum toxin injection. Case report: A 46-year-old female patient with ASD complained of voice tremor and hoarseness. The combined Korean medical treatments-Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and transcutaneous electrical nerve stimulation (TENS)-were administered on the first day the patient was hospitalized. The Voice Handicap Index (VHI) was evaluated during the treatment. The VHI taken on the second day totaled 92 points. On the ninth day, 81 points were recorded. Total score gradually improved, and on the 16th day, 62 points were recorded. Combined Korean medical treatment lasted 19 days. Conclusion: The present case report suggests that a combined Korean medical treatment approach with Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and TENS might be effective for symptoms such as voice tremors and hoarseness. Combined Korean medical treatment can be a therapeutic option for patients with ASD.

Improvement in Balance and Walking in a Patient with Guillain-Barre Syndrome Treated with Acupuncture and Electroacupuncture: A Case Report (침 치료 및 전침 치료로 호전된 길랑바레증후군 환자의 균형 및 보행 개선 : 증례보고)

  • Introduction: Gullain-Barre syndrome (GBS) is a rapid, symmetric muscle weakness that often follows respiratory or gastrointestinal infections. Standard treatments include plasmapheresis and intravenous immunoglobulin, but outcomes can vary among patients. The integration of Korean medicine, notably unexplored in quantitative assessments of balance and walking, provides a novel approach to treatment. Case presentation: A 53-year-old male diagnosed with GBS presented to the hospital 8 days post-onset. He received treatments consisting of acupuncture, electroacupuncture, and rehabilitation over a period of approximately 4 weeks. Upon admission, the patient exhibited symptoms of both limb weakness and paresthesia, requiring the use of a wheelchair. Post-treatment, muscle strength and balance were significantly improved, leading to the restoration of independent ambulation, including running. Conclusions: This case illustrates the potential of integrated Korean and conventional medical treatments to accelerate recovery in GBS patients, overcoming typical prognosis timelines. The improvements in gait and balance were substantiated by quantitative assessments, suggesting a promising area for further research into the effects of combined therapeutic approaches to GBS recovery.

  • Herein, we report a case of multiple systemic arteries to pulmonary artery fistulas without any underlying causes, presenting recurrent hemoptysis. Transcatheter embolization was successfully performed several times on multiple systemic feeding arteries. Multiple systemic arteries to pulmonary fistulas can be a source of uncontrolled bleeding, and embolization may be a reasonable therapeutic option to control the bleeding.

  • The primary treatment of the nasopharyngeal carcinoma is at this time, mainly based on radiotherapy. In most studies for patient treated with radiotherapy, overall 5-year survival rate is in the range of 35 to 55%. Obviously, these therapeutic results may be influenced by various factors such as clinical stages, histopathological types and radiation techniques, etc. Though the radiotherapy had a good therapeutic result, there is a limitation to apply the radiotherapy only in cases of the advanced nasopharyngeal carcinoma. Anatomical complexity of the skull base and difficulties in complete surgical intervention were the trouble of the surgery but in the case of radiation failure, there is no adequate choice of other curative modalities. So it is appropriate to investigate whether surgical resection may improve the outcome of treatment of nasopharyngeal carcinoma. The purpose of this papaer is to report our surgical experience of the nasopharyngeal carcinoma, then to take into consideration of the new model of treatment strategy of nasopharyngeal carcinoma.

Pituitary Macroincidentaloma - Report of 3 Cases - (Pituitary Macroincidentaloma - 증례보고 -)

  • With improvements in diagnostic imaging techniques for the brain, pituitary tumors without neurological signs or symptoms have occasionally been found. To evaluate therapeutic strategy for incidentally found pituitary tumors ("pituitary incidentaloma"), we analyzed the result of magnetic resonance imaging findings and of ophthalmological and endocrinological studies in 3 cases with follow up. Incidentally found functioning tumors were excluded. All of 3 cases is greater than 10mm in tumor size("pituitary macroincidentaloma"). The follow-up period was 49 months, 16 months and 6 months(mean, 25.3 months) in each case. There was no evidence of tumor enlargement, endocrinological problems and visual field defect during follow-up period. Patients with pituitary incidentalomas usually follow a benign course and neurosurgical intervention is not initially required in the management even those greater than 10mm in diameter. Observation over time may be good approach to the patient with a pituitary macroincidentaloma to avoid the unnecessary risk for surgery in a patients with a stable mass.

  • Objectives : The purpose of this case report is to know the effect of Korean medical treatments on occupational contact dermatitis. Methods : We reported 2 cases who visited Allergy and Derma-Cosmetic Clinic of Kyung Hee University Hospital at Gangdong. They were diagnosed with occupational contact dermatitis and treated by Korean medical treatment. Results : One patient recovered after 13 days of admission and total 9 months of outpatient period. Another patient recovered after 9 days of admission and total 7 months of outpatients period through Korean Medical Treatment. Conclusions : We may expect that Korean medical treatment had therapeutic effects on occupational contact dermatitis, but the additional study will be conducted for revealing the exact therapeutic effects.
