뇌졸중 환자의 마비쪽에 따른 골반의 기울임과 보행의 상관관계 분석 (Correlation Analysis of Pelvic Tilt and Gait according to the Paralytic Side of Stroke Patients)

  • Objective: This study investigated the effect of pelvic tiltng according to the paralytic side on gait in stroke patients during a 10 m functional movement timed up and go (TUG) test. Method: In this study, gait parameters were measured using a gait analyzer for 20 stroke patients and their gait was analyzed during a 10 m TUG test. For statistical analysis, an independent sample t-test were performed for age, height, and weight among general characteristics of subjects and homogeneity was tested by performing a chi-square test for gender, paralysis side, period of onset, and K-MMSE score. In order to understand the relationship between each variable, Pearson correlation analysis was performed on the variables. Results: First, the right-hand paralyzed group showed correlations in cadence and gait velocity in the up and down tilt of the pelvis, and the left-hand paralyzed group showed correlations in cadence and step length in the anterior and posterior tilt of the pelvis. Second, the tilt of the pelvis was correlated with the Sit to stand, walk forward, walk backwards, turn around at the end point, sit on a chair and the total TUG time in the right hemiplegic group compared to the left hemiplegic group. Conclusion: In this study, a significant correlation was confirmed as a result of gait analysis of right-handed stroke patients divided into a right paraplegic group and a left paraplegic group. In the future, it is suggested that treatment for improving gait of stroke patients should be treated differently for the right and left paralyzed side.

우측 대동맥활이 동반된 좌측 반회하후두신경 1예 (A Case of Left Recurrent Inferior Laryngeal Nerve with Right Sided Aortic Arch)

  • A non-recurrent laryngeal nerve on the left side is a rare anomaly which is reported in 0.04% and it is associated with abnormal developments of the aortic arch during embryogenesis. Although the possibility is extremely low, it is important to consider the possible existence of a non-recurrent laryngeal nerve to prevent a nerve injury during thyroidectomy. We experienced a 42 year-old male with left thyroid papillary cancer who had right side aortic arch and aberrant left subclavian artery. Even though we found that this patient had a recurrent laryngeal nerve, we present this case of the right aortic arch with an aberrant left subclavian artery variation with a brief review of literature.

대동맥궁 증후군의 수술치료 -2례 보고- (Srugical treatment of aortic arch syndrome -Two cases report-)

  • Aortic arch syndrome is an unusual disease entity characterized by the narrowing or obliteration of major branches of the arch of the aorta regardless of etiology. We have experienced 2 cases. One of them was 22 years old office girl with 3 months history of headache, intermittent syncope and weakness and claudication on left arm especially during her physical exercise. On physical examination, pulseless on left antecubital and radial artery and blood pressure on left arm was inable to check and coldness with weakness were noted on the same side. Aortic angiography reealed 34% narrowing of left subclavian artery as that of right. But both common carotid artery and both axillary arterial patency were relatively good. Through right supraclavicular and left axillary incision, bypass graft with Gore-tex prosthesis (I.D. 6mm, Length 25 cm) was implanted from right subclavian artery on 2cm distal to origin of right common carotid arery to left axillary artery distal to axillary fossa. End to side anastomosis with preservation of left subclavian artery was done. Postoperative state was stable with blood pressure of 110/70 mmHg on left arm and palpable antecubital and radial pulsation. Another one was 41 year old male patient with 8 months history of pain and numbness on right upper arm and shoulder. On admission, right arm blood pressure was 110/80 mmHg, left arm was 160/110 mmHg, but other physical findings had no abnormalities. Angiography revealed segmental narrowing of right axillary artery on the beginning with 2 cm in length. Operative treatment with right wupraclavicular and right axillary incision, bypass graft with great saphenous vein (Length; 15 cm) from right subclavian artery between scalenus anticus and medius to axillary artery at distal end of axillary fossa was done. The authors report two cases of Aortic arch syndrome treated with bypass graft using Autograft or Gore-tex with good result.

시각적 등가성에 대한 한국인의 성향 연구 (A study of the Korean Inclination to visual equivocality)

  • 본 연구는 시각적 등가성에 대한 한국인의 성향분석을 목적으로 수행되었다. 자연현상에서 흔히 조우할 수 있는 시각적 동등성에 우리가 동일한 가치를 부여할 수 있는지 없는지에 대해 일부 심리학자들의 유사연구를 참고로 하였고, 설문조사를 통해서 한국인의 성향을 조사하였다. 그 결과는 다음과 같은 발견점을 가져다 주었다. - 일부 특수한 경우를 제외하고 시각적 등가성은 한국인에 있어서는 현실적으로 존재하기 어렵다. - 화면에서의 대각선은 오른쪽 아래에서 왼쪽 위방향보다 왼쪽 아래에서 오른쪽 위방향으로 보다 상승감을 느끼게 해준다는 지적은 사실로 드러났다. - 화면의 오른쪽이 왼쪽보다 무거워보인다는 지적은 우리 나라 사람들에게 있어서는 확인되지 않았거나 무시할만한 정도 이내이다. - 자연현상의 묘사나 이에 대한 관찰자의 시각물 수용에서, 사용자 인터페이스가 작용하는 것으로 드러났다. 이 경우 오른손 듣기가 대부분인 한국인에 있어서는 이러한 현상을 자연스러운 것으로 받아들이는 것이 좋다. - 한국인의 좌측 우위사상에서 오는 전래적인 시각적 표현은 지금도 우리 문화 속에 녹아 있다. 이상의 내용을 한 마디로 정리한다면, 한국인에 있어서 시각적 등가성은 대체적으로 실존하지 않는다는 점이며, 여러 가지 측면에서 오른쪽보다는 왼쪽에 경도되는 일반적인 성향을 한국인이 가지고 있다는 점이 발견되었다.

  • The arteries in and under the brain of the fowl were invesitigated. Arteries of the brain were injected with neoplen latex into the arteria carotis communis. Those were fixed in 10--- formalin. After fixation, those brain were carefully removed from the cranial cavity doing not wound the arteries of the brain. The results of observation on the arterial supply under the brain surface were essentially the same as those obtained by Hofmann Shiina and Mjyata, Kaku, and Kithoh. The nomenclature of the arteries used by other authers were written in the discussion. In this observation, several conclusions were obtained. 1. In the arteries under the brain of the fowl, their front anastomoses forming the circle of Willis under the brain surface of the mammalia were not seen in all brain. 2. It is very difficult to indicate so called A. cerebri anterior as only one artery ramus. 3. Artery distribution of the two-third in the fissura longitudinal cerebri surface originate from on1y one side in the left or right A. cerebri posterior, and origins of the right side were man number than left. 4. The origin of the A. basilaris originate from only one side R. posterior, therefore their origin were more left than right. 5. There is Rami arteries that branch from A. ethmoidalis to Chiasma opticum in all cases.

습관적(習慣的) 저작(咀嚼)과 저작근(咀嚼筋)의 동통유발(疼痛誘發)과의 관계에 대한 근전도학적(筋電圖學的) 연구(硏究) (AN INTEGRATED EMG STUDY OF RELATIONSHIPS BETWEEN PREFERRED CHEWING AND SIDE OF INITIAL MUSCLE PAINS)

  • The purpose of this study was to investigate electromyographically the relationship between preferred chewing side and side of initial muscle pains. In this study, 20 normal healthy subjects were selected , and each subject chewed randomly chewing gum for 20 minutes to establish preferred chewing side. To induce initial muscle pains, biting force of 10Kg on the gnathodynamometer was maintained by the subjects. And the Bioelectric processor EM2(Myo-Ironies Research, Inc. U.S.A.) with the surface electrodes was used to record the EMG activity during all experimental procedures. The results were as follows; 1. A majority of the present subjects (60%) had a preferred chewing side, but with few exceptions, subjects were unable to explain why a given side was preferred; explanations were only 'comfort' and 'habit' 2. The chewing, or working side was determined largely by the mean voltage of the surface electromyogram (EMG); in comparison with EMG from the non-wlring (contralateral) side, the working (ipsilateral) side showed a higher amplitude. 3. After the effort, the right masseter muscle is the most frequent site of pains, followed by the left masseter muscle, the anterior part of the right temporalis muscle and tile anterior part of the left temporalis muscle. 4. After the effort, mean voltages of masseter muscles were slightly increased, but mean voltages of temporalis anterior were slightly decreased at physiologic rest position. 5. No relationships could be established between preferred chewing side and side of initial muscle pains.

CO concentration distribution in a tunnel model closed at left end side using CFD

  • A primary air pollutant as an indicator of air quality released from incomplete combustion is Carbon monoxide. A study of the distributions of CO concentration with no heat source in a tunnel model closed at left end side is simulated with a commercial CFD code. The tunnel model is used to investigate the CO concentration distributions at three Reynolds numbers of 990, 1970, and 3290. which are computed by the inlet velocities of 0.3, 0.6 and 1.0 m/s. The CFD predictive approaches can be useful for a better design to analyze the distributions of CO concentrations. In the case of the tunnel model closed at left end side alone, the concentration changes of x/H=-5 and -2.5 have the similar laminar characteristics like the case of the tunnel model closed at both end sides expecially at low values of Reynolds number. Irregular average CO concentration variations at Re=1790 are considered that the transition from laminar to turbulent flow occurs even in three different tunnel models.

This underwent caldwell-Lue approach the Extracted Caine, premolars and molars and Curettage maxillary sinus walls and Closed primary sutured under Diagnosis of Odontogenic maxillary Sinusitis and Therapeutic principles. Patients had healed Completely one year after operation and no complaints and no Oro-Antral fistula.

Correction of Unilateral Nostril Hypoplasia with Z-Plasty in a Child

  • Unilateral nostril hypoplasia is an extremely rare congenital malformation of unknown etiology, and only a few cases have been reported in literature. Owing to variability and complexity of the deformity, surgical correction of unilateral nostril hypoplasia represents one of the most significant reconstructive challenges to reconstructive plastic surgeons. We report a 7-year-old Vietnamese child with nasal and periocular deformity resembling a craniofacial cleft. Grossly, the right nostril was patent but with alar rim deformity, and the left nostril was not readily identifiable. A dystopic medial canthus was present on the left side as well. Closer inspection and palpation of the left side of nose revealed a patency through the soft tissue and underlying bony structure, Thus, a new alar rim were reconstructed with an irregularly shaped Z-plasty to create patency on the involved side. Simulatneously, a second Z-plasty was performed to address the medial canthal deformity. Postoperative appearance and function was sastisfactory at one-year follow up visit. In the treatment of patients with nostril hypoplasia, a careful preoperative physical examination is a prerequisite, and Z-plasty can be a valuable option for surgical correction.

Impact of the Anterior-Posterior Slope Types of the Scapulae on the Pressure Distribution of the Plantar Surface of the Foot

  • Purpose : This study was conducted among 195 adults in their 20s. To analyze the impact of the slope types of the scapulae on the plantar surface of the foot, the average pressure (AP), the maximum pressure (MP), the average of local distribution values, and the average movement of the center of pressure (COP) of the different slope types of the scapulae were compared. Method : The anterior-posterior slopes of the scapulae were measured by comparing the slopes of the left and right sides of the scapulae based on the differences in the height and the slope of the coracoid process and the angulus inferior scapulae. Those whose left side of the scapulae had an anterior slope were categorized as type 1, and those whose right side of the scapulae had an anterior slope, as type 2. The average plantar pressure, the center of plantar pressure, the maximum plantar pressure, and local distribution values were analyzed using a plantar pressure analyzer of the FSA. Result : In terms of the AP of the left and right feet, there was no statistically significant difference both in types 1 and 2 on the left and right feet. The comparison results of the MP and the average of local distribution values of the two slope types of the scapulae showed that there was no statistically significant difference on the X-axis both in types 1 and 2 on the left and right feet, but that there was a large statistically significant difference on the Y-axis both in types 1 and 2. That is, the MP of the right foot of the left anterior slope type was located more on the hindfoot than that of the right anterior slope type, and the MP of the left foot of the left anterior slope type was located more on the hindfoot than that of right anterior slope type. Conclusion : This study can be used as fundamental data to predict differences in the location and size of the COP and changes in plantar pressure distribution depending on the slope types of the scapulae, and control the distribution for therapeutic purposes.
