환자이탈군 특성요인과 이탈환자 예측모형에 관한 연구 (A Study on the Characteristics of Prematurely Discharged Patients and the Model for Predicting Premature Discharge)

  • Background : We developed a model for predicting premature discharge and identifying related factors. Methods : Prediction model was developed by data mining techniques. Basic data were collected from the total discharge data base of a university hospital in Chungnam Province during the period from July 1, 1999 to June 30, 2000. Results : 1. Among 22,873 patients, the number of patients discharged with usual discharge orders were 21,695 or 94.8%. The number of the prematurely discharged patients were 1,178 or 5.2%. 2. The primary reason for unusual discharge was transfer to other hospital. Move to a local hospital closer to their home and burdensome medical expenses were main reasons. 3. Predictability of each model was tested using the top 10 percent of patients with the highest probabilities of premature discharge. The neural network model was chosen as the most appropriate model for predicting prematurely discharged patients. 4. Ten percent of the total number of patients had been selected randomly to test the effectiveness of the neural network model. We have chosen the threshold of the neural network model as 0.7. The number of patients who were expected to discharge prematurely was 312. Among them, 241 had been discharged prematurely (77.2%). Conclusion : Of the several data mining techniques used, the neural network model was the most effective, It can be used to identify and manage the patients who are expected to discharge prematurely.

  • This paper on the status of radiotherapy machine and related facilities, clinical activities and radiation safety management is based on the statistical data which collected through the questionaire sent to a total number of 37 hospitals holding the Department of Therapeutic Radiology in Korea. It is true that the quality of instruments installed in the hospitals equal to that of the instrument in the industrialized conuntries' hospitals. But the clinical specialists and physicists who can utilize such instruments fall short of the required number, which might be a main factor in hindering the development of therapeutic radiology of Korea. According to the nation-wide cancer statistics, we can estimate the number of annual cancer patients as 45,000 to 50,000. As a result, probably around 25,000 should receive radiation therapy. It is expected that in the future the number of cancer patients to whom radiation therapy should be applied will become twice as much as that of the cancer patients in 1990. Given such a condition, the problem facing the Korean Society of Therapeutic Radiology now is to increase the number of medical doctors and physicists.

베이비붐세대가 65세 노인인구로 전환 시의 만성질환 환자수와 진료비 예측 (Estimate over the Number of Chronic Disease Patients and Medical Care Expenditure at the Time of Transition of Baby Boomer into 65 Years Old Aging Population)

  • Purpose: The purpose of study is to estimate the number of chronic disease patients and medical care expenditure at the time baby-boomers belong to 65 years old aging population, and compare with current 65 year-old aging population. Methods: Analysis method used an estimating formula devised by the researcher and estimated the number of chronic disease patients and medical care expenditure of each generation. Results: When comparing the estimated number of chronic diseases patients of each generation, 40.6% of the first generation, 76.4% of the second generation, 95.2% of third generation are expected to get chronic disease. When comparing each generation's total medical care expenditure, based on the estimated number of chronic diseases patients of each generation, the second generation( 1,206,251,224 thousand won) showed higher than other generation. This study compared the number of chronic disease patients and medical care expenditure between the second generation of the elderly and current elder generation. As a result, the second generation patients was higher than the fourth generation in high blood pressure, diabetes, psychological and behavioral disorder, and neurological diseases whereas the fourth generation is only high the number of patients in heart disease. As for total medical care expenditure, the second generation paid more in high blood pressure, psychological and behavioral disorder while the fourth generation in neurological disease and heart disease. Conclusion: It is desired that considering the number of chronic disease patients and medical care expenditure of baby boomers accounting for 14.6% of total population, in-depth follow-up study is carried out that inquires into what are issues with a current chronic disease management project, what business is needed in order to manage these issues, and how to fund to cover increasing medical care expenditure.

치매 환자의 구강관리와 치과치료 시 고려사항 (Oral Care and Considerations for Dental Treatment in Dementia Patients)

  • The number of patients with dementia in Korea is estimated to be 576,000 in 2013 and 648,000 in 2015, and 840,000 in 2020 and 2,710,000 in 2050. Therefore, it can be expected that the number of patients with dementia will increase among the elderly patients who are looking for dentistry. Dementia and oral hygiene are closely related, so the future role of the dentist is very important. In order to treat effectively the patients with cognitive disorders, we should consider effective communication methods with them. Xerostomia is another important factor affecting oral health of dementia patients. In establishing the dental treatment plan, the most appropriate treatment method should be selected considering the characteristics of individual dementia patients and all possible efforts should be made for successful treatment results.

  • According to the National Health Insurance Corporation in 2008, there were 17,764,428 physical therapy patients, exceeding 31 percent for the population covered by health insurance. This means that three out of 10 Koreans received physical therapy. And now, 10 years later, due to the aging population and the increase in the sports population, the number of patients with physical therapy is expected to be much more than a decade ago. Among them, many physical therapy patients were orthopedic and neurologic disorder. However, in the medical field applied to physical therapy, it is widely applied across all medical fields, including orthopedics, neurosurgery, pediatrics, gynecology, thoracic surgery and dentistry. It is believed that various cases of patients receiving physical therapy will be secured. as mentioned earlier, there will be a large number of patients with physical therapy treatments, making big data analytics easier. based on this, physical therapy applications are thought to be helpful in the analogy of disease and the development of effective physical therapy and will ultimately promote the development of physical therapy.

Learning fiberoptic intubation for awake nasotracheal intubation

  • Background: Fiberoptic nasotracheal intubation (FNI) is performed if it is difficult to open the mouth or if intubation using laryngoscope is expected to be difficult. However, training is necessary because intubation performed by inexperienced operators leads to complications. Methods: Every resident performed intubation in 40 patients. Success of FNI was evaluated as the time of FNI. First intubation time was restricted to 2 min 30 s. If the second attempt was unsuccessful, it was considered a failed case, and a specialist performed nasotracheal intubation. If the general method of intubation was expected to be difficult, awake intubation was performed. The degree of nasal bleeding during intubation was also evaluated. Results: The mean age of the operators (11 men, 7 women) was 27.8 years. FNI was performed in a total of 716 patients. The success rate was 88.3% for the first attempt and 94.6% for the second attempt. The failure rate of intubation in anesthetized patients was 4.9%, and 13.6% in awake patients. When intubation was performed in anesthetized patients, the failure rate from the first to fifth trial was 9.6%, which decreased to 0.7% when the number of trials increased to > 30 times. In terms of awake intubation, there was no failed attempt when the resident had performed the FNI > 30 times. The number of FNIs performed and nasal bleeding were important factors influencing the failure rate. Conclusion: The success rate of FNI increased as the number of FNI performed by residents increased despite the nasal bleeding.

민간의료기관을 이용하는 결핵환자의 의료이용 분석 (Medical Care Utilization of Tuberculosis Patients in Private Sector)

  • In this study we analyzed the insurance claims data to investigate the medical care utilization pattern of tuberculosis patients in private sector. We selected the claims of principal or secondary diagnosis with tuberculosis from claims database of National federation of Medical Insurance, from December 1995 to November 1996. Both spell-based analysis and person-based analysis were carried out. In spell-based analysis, type and location of treatment facilities, distribution of diagnoses, number of outpatient/inpatient treatments were analyzed. Additionally in person-based analysis, number of tuberculosis patients, demographic characteristics, number of treatments per person, frequency and pattern of change in source of care were analyzed. The results were as follows 1. The number of treatments with tuberculosis was 863,641 from 1 December 1995 to 30 November 1996. The number of patients was 313.964. 2. Most of tuberculosis patients in private sector were treated in general hospital (45.8%) and clinics(42.2%) 3. About 77.7% of tuberculosis patients who were treated more than two times did not change the source of care. 18,9% of tuberculosis patients changed source of care only once. Even when we limited tuberculosis patient to those who were treated more than five times and whose treatment period were longer than six months, 94.7% of patients did not change source of care at all, or changed treatment facility only once. 4. The probability of change in source of rare was higher in pulmonary tuberculosis, in twenties, and in rural area respectively than other tuberculosis. In conclusion, healer shopping of tuberculosis patients was not serious as expected. However special attention is needed to pulmonary tuberculosis in twenties and rural area.

임상 전문간호사 운영 효과 분석 (A Study on Operation-Effectiveness Analysis of Clinical Nurse Specialists : Focused on Patient Satisfaction and Expected Profit)

  • Purpose: The aim of this study was to evaluate patient satisfaction and indirect marketing effects (recommendation, hospital re-visit intention) and additionally to calculate the expected incomes by clinical nurse specialists (CNS). Methods: Sixty-six patients who had lower varicose vein removal surgery performed at two general hospitals from August 1 to November 30, 2008, were recruited. The effects were evaluated by degree of patient satisfaction, the number of personal recommendations given, rate of hospital re-visit intention, and indirect marketing effects. Indirect marketing effects were calculated as the total cost of surgery multiplied by the score of other variables. Results: The patient satisfaction score was 0.82 for CNS and 0.43 for general nurses (GN). The number of personal recommendations given was 2.5 for CNS and 1.1 for GN. The rate of hospital re-visit intention was 0.31 for CNS and 0.21 fot GN. The ratio of expected incomes between CNS and GN is 2.07 to 1. Conclusion: We found that when a CNS serves a patient, satisfaction, number of personal recommendations, hospital re-visit intention, and indirect marketing effects were higher than those of GN. These results show that CNS have dominant roles and functions and that the effects of CNS employment can be measured using economics parameters.

Palliative and end-of-life care for heart failure patients in an aging society

  • The populations of Asian countries are expected to age rapidly in the near future, with a dramatic increase in the number of heart failure (HF) patients also anticipated. The need for palliative and end-of-life care for elderly patients with advanced HF is currently recognized in aging societies. However, palliative care and active treatment for HF are not mutually exclusive, and palliative care should be provided to reduce suffering occurring at any stage of symptomatic HF after the point of diagnosis. HF patients are at high risk of sudden cardiac death from the early stages of the disease onwards. The decision of whether to perform cardiopulmonary resuscitation in the event of an emergency is challenging, especially in elderly HF patients, because of the difficulty in accurately predicting the prognosis of the condition. Furthermore, advanced HF patients are often fitted with a device, and device deactivation at the end of life is a complicated process. Treatment strategies should thus be discussed by multi-disciplinary teams, including palliative experts, and should consider patient directives to address the problems discussed above. Open communication with the HF patient regarding the expected prognosis, course, and treatment options will serve to support the patient and aid in future planning.

  • Esophageal cancer (EC) incidence rates in Kazakhstan were assessed by component analysis based on primary registered cases in 2001-2010. It was found that despite an apparent general decrease in the number of EC patients in Kazakhstan, a potential increase should be evaluated, due to changes in aging as well as the increase in population. Some problems of EC patients' registration were broached with an emphasis on the importance of the expected absolute number and reasons for undercounting in the country. Based on these, ways of improving the recording and registration of such patients in the country were suggested.
