This study was carried out to characterized the effect of Youndamsagan-tang on a Case of Septic Knee. After we gave Youndamsagan-tang to a patient of septic knee, we examined not only symptom of patient but also the thickness and circumference of knee, the change of WBC, ESR, body temperature, VAS. As admission time passes, the pain, flare, swelling and movement of left-side knee joint were improved. The thickness and circumference of knee joint were decreased, and the count of WBC, ESR were decreased. And patient's pain by VAS was subsided. From the above result, administration of Youndamsagan-tang reduces the progressing of pain, flare, swelling and movement in knee joint.
Leukemia in pregnancy was first reported by Virchow in 1845, and acute Leukemia that occurs with pregnancy is extremely rare. About 350 pregnancies with leukemia have been reported in literature. The incident of acute leukemia during pregnancy has been reported in one case per 100,000 pregnancies case. A 40-year-old patient with 30 weeks of pregnancy, (by promyelocyte which is contained granules and auer rods in the bone marrow and biopsy) was diagnosed with acute promyelocyte leukemia WITH t (15;17) (q22;q12); PML-RARA. (M3) in peripheral blood and bone marrow examination, and gave a birth to the fetus normally, January 24, 2013, after receiving the complete remission decision from the bone marrow, complete blood cell count, PML-RARA PCR test, showed normal findings until March 2013. The treatment of acute leukemia during pregnancy should be considered as treatment of a pregnant mother and the impact on the fetus. Decisions about when and how birth takes place is difficult and has to consider both mother and fetus. It is preferable to start immediate treatment without delay so that the treatment time to achieve complete remission or full recovery of the pregnant mother is longer.
Although clonorchiasis is one of the most common parasitic infections in Korea, it is unusual that the disease presents peripheral eosinophilia and pulmonary infiltrations.(eosinophilic pneumonia) A case of clonorchiasis manifested as diffuse nodular pulmonary infiltrations was presented. The patient had a mild to moderate pain on the right upper quadrant of the abdomen, cough, dyspnea, and unknown cause of marked eosinophilia (up to 71.4% of total white blood cell count). The causal organism, clonorchis sinensis was found by the identification of parasite ova in the stool We confirmed eosinophilic pneumonia with bronchoalveolar larvage analysis and transbronchial lung biopsy. With corticosteroid and praziquantel treatment, clinical symtoms and pulmonary infiltrations on the chest roentgenograms had rapidly improved. We report a case of eosinophilic pneumonia related to clonorchiasis and review the pertinent literature.

Paralytic ileus is a metabolic state in which the intestines fail to transmit peristalsis due to failure of the neuromuscular mechanism in the small intestines and colon. It is a major cause of morbidity in hospitalized patients especially during late presentations and points of mismanagement. The causes include infections, electrolyte imbalance (hypokalemia, hyponatremia), surgeries and medications. When the exact cause of the disease condition is identified and corrected, paralytic ileus is usually resolved. This case report is that of a 16 year old female who was admitted and managed as a case of paralytic ileus. The patient presented with symptoms of fever, abdominal pain, abdominal distension, vomiting and inability to pass stool or flatus. There was associated body weakness, reduced urine output and weight loss. She was properly examined clinically and sent for various investigations. Investigations such plain abdominal X-Ray, serum electrolyte estimation, chest X-Ray and full blood count were carried out. The results of the investigations done were in keeping with the diagnosis of paralytic ileus, electrolyte imbalance and ongoing sepsis. She was subsequently managed through nil per oral, adequate fluid rehydration, antibiotics and correction of electrolyte imbalance. Following stable clinical state and investigation results, she was discharged and advised on follow-up.

Objective: In this case report, we address the case of a 47-year-old man with anemia due to chronic kidney disease. Methods: A patient was treated with Korean medicine including an herbal medication, Shipjeondaebo-tang. We evaluated the improvement of symptoms by biochemical examination of blood, complete blood cell count and total score of Fatigue Severity Scale (FSS). Results: After 15 days of Korean medicine treatment, there was an increase in hemoglobin states, even after intervention ended, for over 30 days with improvement of the patient's fatigue. Conclusions: This study suggested that Shipjeondaebo-tang might be effective in patient's renal anemia and fatigue recovery.

Cryptococcus neoformans infection usually occurs in patients with advanced human immunodeficiency virus (HIV) infection or with a CD4 T lymphocyte count of <100 cells/µL. Pulmonary and central nervous system infections are the most frequently encountered forms of cryptococcosis; however, colonic cryptococcosis is uncommon. We describe the case of a 41-year-old antiretroviral-naïve man with HIV infection diagnosed eight years prior and intermittent diarrhea for 4 months who presented to the emergency department with a 1-day history of low-grade fever and confusion. Brain magnetic resonance imaging and cerebrospinal fluid analysis revealed normal results; however, he was diagnosed with Pneumocystis jirovecii pneumonia based on chest computed tomography and bronchoalveolar lavage analysis. Trimethoprim-sulfamethoxazole administration was initiated followed by antiretroviral treatment. Although his condition gradually improved, he developed fever and abdominal discomfort, and the diarrhea worsened. Endoscopy revealed a small ulcer in the distal transverse colon. Histopathological examination of a colon tissue sample revealed cryptococcal infection. He improved substantially during liposomal amphotericin B and fluconazole treatment. We encountered a rare case of colonic cryptococcosis that caused chronic diarrhea in a patient with advanced HIV infection. Colonic cryptococcosis should be considered when patients with acquired immune deficiency syndrome present with gastrointestinal symptoms.
Objectives: It is certain that Radioactive iodine thyroid uptake(RAIU) rate should be measured with the standard counts considering the thyroid gland depth in enlarged thyroid patients for the variation from geometric factors. The purpose of this paper is to consider the effects of geometric factors according to detector to source distance and the effective thyroid depth on RAIU rate with experiment test. Materials and Methods: I-131 370 kBq ($10{\mu}Ci$) point source was measured by Captus-3000 thyroid uptake system (Capintec, NJ, USA) with a change Detector-Source Distance from 20 cm to 30 cm at an interval of 1 cm. And we changed the Neck phantom surface-Source Depth in the phantom with 1 cm, 2 cm, 5 cm using the neck phantom in order to reproduce the effective thyroid depth. Results: Every experimental group follows power curve as inverse square curve ($$R2{\geq_-}0.915$$). The average count rates in the case not using a phantom and the every case applied the effective thyroid depth using a phantom was not identical each other. There was significant fluctuations upon the effective thyroid depths applied the effective thyroid depth above 1 cm in $364.4 keV{\pm}10%$ energy ROI (p<0.01). There was not significant difference between the count rates of 1 cm and 2 cm in $364.4keV{\pm}20%$ and $637.1keV{\pm}6.2%$ (p=0.354, p=0.397). In assumed RAIU rate from regression equation, $364.4keV{\pm}20%$ was lower difference than $364.4keV{\pm}10%$ as 6.42% and 5.09% per 1 cm. Every change of count rate upon depth appears decreased line on Linear Regression, but the case of $284.3keV{\pm}10%$ increased only. And also, The graphs of coefficient of variation upon depth increased as straight line on every experimental group. Conclusion: The result appears that application of $364.4keV{\pm}20%$ energy ROI is more suitable for reducing error from the effective thyroid depth. And also, we can estimate the error of 20 cm should be highly reduced than 30 cm for Inverse Square Law. Therefore, If there is not information of the thyroid depth, it is considered that the error from thyroid depth can reduce through set up energy ROIs for $364.4keV{\pm}20%$, and increase Detector-Source Distances.
Some of historical textiles were analyzed to identify the imprinted textures on the metal remains of Haengyeup(a horse strap pendant) and Doja (a knife), which were excavated in the ruins of Hwangsung-dong, Kyungju city in the fifth century and textiles imprinted on the human bones that were excavated in the Pungnae 4th Area, Namyangju county in the sixteenth century. The results analyzed arc as follows ; It was confirmed that the imprinted textures of Haengyeup and Doja are white ramie since the structural characteristics of cellulosic bast fibers, partially projected long oval cross sections with large lumens and the imprinted textures of human bones are silk fabrics since the triangular cross sections of fibroin is showed. All of the textiles were designed in plain weaves. In the case of weaving yarns, the warp threads were thicker than the weft threads, that is, the imprinted textures of Haengyeup were measured by 1.35 mm for warp and 1.21 mm for weft, and the Doja by 1.16mm for warp and 1.11 mm for weft. In connection with the direction of the yarn twist, the Z-twist were observed in the imprinted textures of human bones, whereas it presented the S-twist in the case of Haengyeup and Doja. The warp yarns were mostly harder twisted than the weft yarns on the amounts of twist, that is, it was observed that the case of Haengyeup were amounted to 0.33 twists per centimeter for warp yarns and 0.25 twists per centimeter for weft yarns. Also it was showed 0.32 twists per centimeter for warp yarns and 0.26 twists per centimeter in the case of the textures of human bones. On the fabric counts, it was showed that the finer the yarns the higher the densities since it were $4.3{\times}5.1$ threads per sqaure centimeters and $7.6{\times}7.1$ threads per sqaure centimeters each in the case of Haengyeup and Doja, whereas it was $18.2{\times}33.7$ threads per sqaure centimeters in the case of the textures of human bones.
A Maltese (case 1) and a Labrador Retriever (case 2) presented with urinary incontinence. General conditions were good and screening tests, including a complete blood count, serum chemistry and radiography, were performed. Excretory urography was conducted, and fluoroscopy was performed for case 1 and computed tomography was performed for case 2. The dogs were diagnosed as right extramural ectopic ureter in case 1 and bilateral intramural ectopic ureter in case 2. We performed surgical corrections, including a neoureterocystostomy for extramural ectopic ureter and neoureterostomy for intramural ectopic ureter. After surgery, the dogs were catheterized with an indwelling catheter for 3 days. Urinary incontinence improved completely and the clinical outcomes were good. The choice of adequate surgical procedure is important for correcting ectopic ureters in canines.

Background : Despite the clinical clues of bronchial asthma, some chronic coughers fail to be diagnosed due to negative test results. This study was aimed at evaluating the diagnostic performance of routine objective tests and identifying a cost-effective approach for asthmatics with a chronic cough. Methods : Patients with a chronic cough of more than 3 weeks duration, and showing normal chest radiograph and spirometry were enrolled. On the first visit, objective tests, composed of serum total IgE, peripheral blood eosinophil count, spontaneous sputum eosinophil count, methacholine bronchial provocation test (MBPT) and paranasal sinus radiograph, were performed, with the simultaneous administration of oral prednisolone (0.5mg/kg) for one week. The final diagnoses were made on the basis of the test results, and the patients grouped according to their steroid responsiveness. The role of the etiologic diagnosis tests was evaluated, and the medical costs of the final management plan simulated with respect to three assumed models. Results : Sixty chronic coughers were finally analyzed. The final diagnoses were as follows: bronchial asthma 21.7%, eosinophilic bronchitis 6.7%, paranasal sinusitis 18.3%, presumptive allergy 8.3% and non-diagnostic case 45.0%. Ninety percent were steroid responder. With the bronchial asthma cases, the positive rate of MBPT was 38.5%, with sputum eosinophil count in 84.6%, serum total IgE in 38.5%, and a peripheral blood eosinophil count rate of 30.8%. When the test results and steroid responsiveness data were applied to the 3 models, the chest radiograph, spirometry, sputum eosinophil count and paranasal sinus radiograph test results, and simultaneous short term steroid treatment seemed to have acceptable diagnostic performances, which could be used as a further guide to cost-effective planning. Conclusion : Objective tests, composed of chest radiograph, spirometry, paranasal sinus radiograph and sputum eosinophil count, with simultaneous short term steroid treatment, are suggested as cost-effective approaches for asthmatics with a chronic cough.
