Community Health Worker Hepatitis B Education for Cambodian American Men and Women

  • Background: Cambodian Americans have high rates of chronic hepatitis B virus (HBV) infection and liver cancer. There is very limited information about the utility of community health worker (CHW) approaches to cancer education for Asian American men. We have previously reported our positive findings from a trial of CHW education about HBV for Cambodian Americans who had never been tested for HBV. This report describes similarities and differences between the outcomes of our CHW HBV educational intervention among Cambodian American men and women. Methods: The study group for this analysis included 87 individuals (39 men and 48 women) who were randomized to the experimental (HBV education) arm of our trial, participated in the CHW educational intervention, and provided follow-up data six months post-intervention. We examined HBV testing rates at follow-up, changes in HBV-related knowledge between baseline and follow-up, and barriers to HBV testing (that were reported to CHWs) by gender. Results: At follow-up, 15% of men and 31% of women reported they had received a HBV test (p=0.09). HBV-related knowledge levels increased significantly among both men and women. With respect to HBV testing barriers, women were more likely than men to cite knowledge deficits, and men were more likely than women to cite logistic issues. Discussion: Our study findings indicate that CHW interventions can positively impact knowledge among Cambodian American men, as well as women. They also suggest CHW interventions may be less effective in promoting the use of preventive procedures by Cambodian American men than women. Future CHW research initiatives should consider contextual factors that may differ by gender and, therefore, potentially influence the relative effectiveness of CHW interventions for men versus women.

  • 동시에 여러 개의 가설검정 수행시 귀무가설이 참일 경우 귀무가설을 기각할 확률이 커지는 문제가 발생한다. 이러한 다중검정 문제 해결을 위해 여러 연구에서는 가설검정시 필요한 집단별 오류율(FWER; family-wise error rate), 위발견율 (FDR; false discovery rate) 또는 위비발견율 (FNR; false nondiscovery rate) 과 통계량을 고려하여 검정력을 높이고자 하였다. 본 연구에서는 T 통계량, 수정된 T 통계량, 그리고 LPE (local pooled error) 통계량 기반 P값을 이용한 Bonferroni (1960) 방법, Holm (1979) 방법, Benjamini와 Hochberg (1995) 방법과 Benjamini와 Yekutieli (2001) 방법 그리고 Z 통계량 기반 Sun과 Cai (2007) 방법을 고찰하고 모의실험을 통해 다중검정 능력을 비교하였다. 또한 실제 데이터로 애기장대 유전자 발현 데이터에 대해 여러 가지 다중검정법을 통해 유의한 유전자들을 선별하였다.

Fluid Infiltration Effect on Breakdown Pressure in Laboratory Hydraulic Fracturing Tests

  • Observations on the influence of the fluid infiltration on the breakdown pressure during laboratory hydraulic fracturing tests, along with an analysis of the applicability of the breakdown pressure prediction for cylindrical samples using Quasi-static and Linear Elastic Fracture Mechanics approaches were carried out. These approaches consider fluid infiltration through the so-called radius of fluid infiltration or crack radius, a parameter that is not a material property. Two sets of tests under pressurization rate controlled and injection rate controlled tests were used to evaluate the applicability of these methods. The difficulty of the estimation of the radius of fluid infiltration was solved by back calculating this parameter from an initial set of tests, and later, the obtained relationships were used to predict breakdown pressures for a second set of tests. The results showed better predictions for the injection rate than for the pressurization rate tests, with average errors of 3.4% and 18.6%, respectively. The larger error was attributed to differences in the testing conditions for the pressurization rate tests, which had different applied vertical pressures. On the other hand, for the tests carried out under constant injection rate, the Linear Elastic Fracture Mechanics solution reported lower errors compared to the Quasi-static solution, with values of 3% and 3.8%, respectively. Moreover, a sensitivity analysis illustrated the influence of the radius of fluid penetration or crack radius and the tensile strength on the breakdown pressure, suggesting a need for a careful estimation of these values. Then, the calculation of breakdown pressure considering fluid infiltration in cylindrical samples under triaxial conditions is possible, although larger data sets are desirable to validate and derive better relations.

돼지 로타바이러스(Gottfried 주)의 VP4 항원구조분석 (Analysis of antigenic sites on the VP4 of porcine rotavirus, Gottfried strain)

  • The neutralization epitopes of the outer capsid protein VP4 of a porcine rotavirus, Gottfried strain, were studied using neutralizing monocolonal antibodies(N-MAbs). Eight N-MAbs which are specific for the VP4 of Gottfried strain were used for analyzing the antigenic sites of VP4. Three different approaches were used for this analysis; i)testing the serological reactivity of each N-MAb against different G and P types of human and animal rotavirusese ii) analyzing N-MAb-resistant viral escape mutants and iii) performing nucleotide sequence analysis of the VP4 gene of each N-MAb-resistant viral escape mutant. From experimental results, at least four antigenic sites(I, II, III, and IV) were identified. Antigenic site I recognized by N-MAbs 24B9, 23G10, and 26A2 was separated from antigenic site II recognized by N-MAbs 30H5, 32B3, and 29B3. However, these antigenic sites were overlapped with antigenic site III recognized by N-MAb 21A1. The other antigenic site IV recognized by N-MAb 16D2 was separated from antigenic sites I, II, and III.

Breast Tumor Cell Nuclei Segmentation in Histopathology Images using EfficientUnet++ and Multi-organ Transfer Learning

  • In recent years, using Deep Learning methods to apply for medical and biomedical image analysis has seen many advancements. In clinical, using Deep Learning-based approaches for cancer image analysis is one of the key applications for cancer detection and treatment. However, the scarcity and shortage of labeling images make the task of cancer detection and analysis difficult to reach high accuracy. In 2015, the Unet model was introduced and gained much attention from researchers in the field. The success of Unet model is the ability to produce high accuracy with very few input images. Since the development of Unet, there are many variants and modifications of Unet related architecture. This paper proposes a new approach of using Unet++ with pretrained EfficientNet as backbone architecture for breast tumor cell nuclei segmentation and uses the multi-organ transfer learning approach to segment nuclei of breast tumor cells. We attempt to experiment and evaluate the performance of the network on the MonuSeg training dataset and Triple Negative Breast Cancer (TNBC) testing dataset, both are Hematoxylin and Eosin (H & E)-stained images. The results have shown that EfficientUnet++ architecture and the multi-organ transfer learning approach had outperformed other techniques and produced notable accuracy for breast tumor cell nuclei segmentation.

Analysis of forced convection in the HTTU experiment using numerical codes

  • The High Temperature Test Unit (HTTU) was an experimental set-up to conduct separate and integral effects tests of the Pebble Bed Modular Reactor (PBMR) core. The annular core consisted of a randomly packed bed of uniform spheres. Natural convection tests using both nitrogen and helium, and forced convection tests using nitrogen, were conducted. The maximum material temperature achieved during forced convection testing was 1200 ℃. This paper presents the numerical analysis of the flow and temperature distribution for a forced convection test using 3D CFD as well as a 1D systems-CFD computer code. Several modelling approaches are possible, ranging from a fully explicit to a semi-implicit method that relies on correlations of their associated phenomena. For the comparison between codes, the analysis was performed using a porous media approach, where the conduction and radiative heat transfer were lumped together as an effective thermal conductivity and the convective heat transfer was correlated between the solid and gas phases. The results from both codes were validated against the experimental measurements. Favourable results were obtained, in particular by the systems-CFD code with minimal computational and time requirements.

  • 화재시 당면할 수 있는 위험성의 여러 인자 가운데 연소 독성 영향은 피난 및 최종 생존에 중요한 영향을 미치고 있다. 화재 독성에 지금까지 대한 대부분의 연구는 치사 측면에 국한되어 왔다. 이에 본 연구에서는 대표적인 질식성 가스인 HCN을 대상으로 가스유해성시험방법(KS F 2271)을 통하여 실험동물(흰쥐)에 직접 독성가스를 흡입시킨 후 평균행동정지시간을 측정하고, 이들 실험동물군의 내부 장기 변화, 혈액학 분석 및 단세포전기영동분석을 통해 연소 독성가스가 생체에 미치는 영향을 정량적으로 분석하고자 하였으며, 기존 가스유해성시험에서 확인할 수 없었던 접근(병리검사, 혈액검사, 혈액생화학검사, 단세포전기영동분석)을 통해 연소 독성학 분야의 생물학적 위험도를 측정하고자 하였다.

  • 본 연구는 창업기업들이 초기단계 성공의 가장 핵심인 성공적인 비즈니스 모델의 수립과 실행을 사전적으로 가늠해 볼 수 있는 방법론을 제시하는데 그 목적이 있다. 특히, 창업기업들은 창업 성공확률을 제고하기 위해 사전적인 철저한 분석과 준비가 요구되며 동시에 직관적으로 수립된 비즈니스 모델들이 사업타당성을 갖는지 사전적으로 검증하는 노력이 요구된다. 본 논문은 창업기업들이 본격적인 사업을 시작하기 이전에 보다 타당하고 성공가능성이 높은 사업기반을 구축하는데 필요한 요소를 인식하고 이에 대한 효과적인 대처방안을 제시하였다. 따라서 본 논문은 크게 두 가지의 접근을 시도하였다. 첫째 본 논문은 이론적인 검토를 통해 창업기업의 성공에 있어 비즈니스 모델이 차지하는 중요성을 고찰하였다. 둘째 본 논문은 창업기업의 비즈니스 모델 타당성을 분석하기 위해 Bruce Merrifield 박사가 고안한 BMO 방법이 창업기업의 비즈니스 모델 타당성 분석에 유용한 모형을 제공하고 있음을 입증하였다. 본 논문은 이러한 분석을 통해 창업기업들이 성공적인 창업을 위해 사전적으로 타당한 비즈니스 모델 수립의 중요성을 인식하고 비즈니스 모델 타당성 분석에 있어 BMO 방법을 활용해야 하는 당위성을 제공하였다.

개인-조직가치 부합수준과 성과관계 검증 (Testing the Relationship between Person-Organizational Value Fit and Performance)

  • 조직연구에서 개인 직무, 개인-조직, 개인-환경 사이의 부합수준은 성과에 대한 주요한 영향요인으로 다루어져 왔다. 개인-조직이 추구하는 가치부합수준은 조직의 구성원으로 하여금 동기유발의 주요한 요인으로 조명되고 있다. 그러나 이런 부합수준 성과관련 연구에서 부합수준별 성과, 혹은 성과의 극대화률 설명하는 만족적 부합수준을 경정하기 위해 적용가능한 방법론의 개발관련 연구는 미미한 수준이다. 기존 연구에서 이런 부합수준-성과 간의 관계를 연구하는데 활용되는 지표들은 두 프로파일의 요인별 차이값, 차이제곱값, 차이 절대값, 측정 도구 사이의 상관관계를 나타내는 Q 값, 두 프로파일 사이의 순위상관을 나타내는 $Q_r$이다 그러나 이런 지표들은 두 프로파일의 부합수준 성과간의 관계를 연구하는데 많은 한계를 보여주고 있다. 특히, Edwards의 단일 설명변수에 대한 다항회귀분석과 반응표면분석을 활용한 방법론은 두 프로파일의 개별 설명변수 차이 값의 종속변수에 대한 영향만을 개별적으로 보여주고 설명변수를 모두 고려할 경우 차이 프로파일을 구성하는 개별요소별 성과를 극대화하는 값이나 수준이 어떠해야 하는지에 대해서는 해답을 주지 못하고 있다. 따라서 본 연구는 이러한 문제의 해결을 위해 다변량 이차다항회귀 모형을 적용하여 다중반응표면분석을 통해 개인-조직, 부합수준-성과 간의 관계를 검증하였다.

미국의 지역간 의료이용의 변이 연구: 비판적 검토와 함의 (Research on Geographic Variations in Health Services Utilization in the United States: A Critical Review and Implications)

  • This paper critically reviews three decades of research on geographic variations. in health services utilization in the United States, thereby drawing policy and research implications for Korea. The recent renewed interest in variations research in the United States, precipitated by studies on regional variations in Medicare expenditures, stems mainly from the policy implication that a substantial amount of Medicare expenditures could be saved without compromising quality and access. From the research perspective, this policy implication was made tenable by integrating micro- and macro-level analysis of variations in health services utilization. Still, theoretical limitations inherent in the research pose great challenges to developing effective strategies at the health system level. Variations research in the United States can serve as a case study as to how health services research has responded to efficiency and quality issues in an ever expanding health system Considering the current health policy and research environment in Korea, the following implications can be drawn. Variations research will help formulate a national policy agenda for health care quality and also advance the framework of approaches to health policy issues. For such purposes, both relevant descriptive and hypothesis-testing studies are needed. Further advancement in variations research will require interdisciplinary explorations and methodological sophistication. To the extent that Korean health policies will strive to achieve complex goals, variations research will increasingly prove to be useful.
