Trends in Reports on Climate Change in 2009-2011 in the Korean Press Based on Daily Newspapers' Ownership Structure

  • Objectives: The mass media play a crucial role in risk communication regarding climate change. The aim of this study was to investigate the trend in journalistic reports on climate change in the daily newspapers of Korea. Methods: We selected 9 daily newspapers in Korea, which according to the ABC Association, represented 77% of newspaper circulation, out of a total of 44 Korean daily newspapers. The collected articles were from 2009 to 2011. All of the articles were sorted into the following 8 categories: greenhouse gas, climate change conventions, sea level rise, Intergovernmental Panel on Climate Change synthesis reports, expected damage and effect, use of fossil fuels, global warming, and mitigation or adaptation. A chi-squared test was done on the articles, which were counted and classified into cause, effect, and measurement of climate change according to the newspaper's majority or minority ownership structure. Results: From the 9 selected newspapers, the number of articles on climate change by month was greatest in December 2009. Generally, the articles vague about climate change (lack of precise data, negative or skeptical tone, and improper use of terminology) were much more common than the articles presenting accurate knowledge. A statistical difference was found based on ownership structure: the majority-owned newspapers addressed the cause of climate change, while the minority-owned newspapers referred more to climate change measurement. Conclusions: Our investigation revealed that generally Korean daily newspapers did not deliver accurate information about climate change. The coverage of the newspapers showed significant differences according to the ownership structure.

A case study on a metastatic adrenocortical carcinoma patient treated by Wheel Balanced Cancer Therapy with improvement of quality of life and related symptoms (수레바퀴 암치료법을 시행한 전이성 부신피질암 환자의 증상 개선 및 삶의 질 향상에 대한 증례보고)

  • Objective: The purpose of this study is to report a metastatic adrenocortical carcinoma patient treated with wheel balanced cancer therapy (WBCT) regimen. Methods: A 52-year-old female patient diagnosed and incised with metastatic adrenocortical carcinoma visited the East West Cancer Center (EWCC) on Sep. 1st, 2018 for WBCT. The patient was treated with WBCT for an approximately 2 months, from Sep. 1st to Nov. 9th. Computed tomography (CT) was used to follow-up the tumor site. Laboratory analysis and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety of WBCT. Results: The surgical beds after surgery (Lt. Laparoscopic adrenalectomy) were maintained without recurrence at follow up chest CT, and related symptoms and quality of life (QOL) were improved during the WBCT. Conclusion: This case study suggests that WBCT may help to improve QOL of adrenocortical carcinoma patient.

  • Cardiac magnetic resonance (CMR) imaging is widely used in many areas of cardiovascular disease assessment. This is a practical, standard CMR protocol for beginners that is designed to be easy to follow and implement. This protocol guideline is based on previously reported CMR guidelines and includes sequence terminology used by vendors, essential MR physics, imaging planes, field strength considerations, MRI-conditional devices, drugs for stress tests, various CMR modules, and disease/symptom-based protocols based on a survey of cardiologists and various appropriate-use criteria. It will be of considerable help in planning and implementing tests. In addressing CMR usage and creating this protocol guideline, we particularly tried to include useful tips to overcome various practical issues and improve CMR imaging. We hope that this document will continue to standardize and simplify a patient-based approach to clinical CMR and contribute to the promotion of public health.

A Case of Patient with Recurring Non-Small Cell Lung Carcinoma Treated with Samchilchoongcho-Jung in Conjunction with Afatinib (재발성 비소세포성 폐암환자의 Afatinib과 삼칠충초정 병용투여 경과관찰 1례)

  • Objective: The purpose of this study is to report the case of a patient with recurring non-small cell lung carcinoma (NSCLC) taking Samchilchoongcho-Jung with Afatinib. Methods: An NSCLC patient diagnosed with multiple bone and pulmonary metastasis was taking Afatinib (20 mg/day) and suffering from stomatitis caused by the Afatinib. The patient was treated with Samchilchoongcho-Jung (1,500 mg/ day) for 3 months. The tumor size was measured with computed tomography, and laboratory findings, including tumor markers (CEA, Cyfra 21-1), were also followed up. Stomatitis was measured by a numeric rating scale, and adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 4.0. Results: After combined treatment, stable disease was shown on computed tomography. The tumor marker levels of CEA and Cyfra 21-1 were decreased, and the stomatitis significantly improved. NCI-CTCAE 4.0 showed no adverse events. Conclusion: This case study suggests that Samchilchoongcho-Jung may have a synergic effect, in conjunction with Afatinib, on the treatment of patients with recurring NSCLC.

Inclusion of the Traditional Korean Medical Terms into the UMLS (한의학 용어의 UMLS 등재 - KIOM 용어정제연구 중 경혈명(經穴名)을 중심으로-)

  • 본 연구의 목적은 미국국립의학도서관(NLM)의 Unified Medical Language System(UMLS)에 한의학의 용어를 등재하는 것이다. 1차 등재 대상으로 한국한의학연구원(KIOM)에서 진행중인 한의학 용어 정제 연구 결과물 중 임(任) 독맥(督脈)과 12정경(正經)에 속하는 360개의 경혈명(經穴名)의 Concept Unique Identify(CUI)를 선정하였다. UMLS Knowledge Source Server (UMLSKS)의 메타시소러스 검색을 통해 UMLS 내 경혈(經穴)용어에 대한 terminology정보를 수집, 분석하였다. 이를 바탕으로 용어간 개념 비교를 통해 UMLS의 경혈(經穴)용어와 KIOM의 경혈(經穴)용어를 매핑하였다. 마지막으로 Rich Release Format(RRF)로 데이터를 저장하고, Unicode Transformation Format-8 (UTF-8)로 인코딩하여 NLM으로 송부하였다. NLM에서 용어 등재에 대한 적합성 여부를 판별한 후 2010AB버젼에 "TKMT2010"이라는 소스명으로 한글로 된 한의학 용어가 최초로 UMLS에 등재되었다. 향후 UMLS와 같은 국제표준의학용어와 연계를 통해 한의학 용어가 다양한 의학용어체계와 상호호완성을 확보하고 표준화, 세계화할 수 있도록 지속적인 연구가 필요하다.

In vivo dosimetry and acute toxicity in breast cancer patients undergoing intraoperative radiotherapy as boost

  • Purpose: To report the results of a correlation analysis of skin dose assessed by in vivo dosimetry and the incidence of acute toxicity. This is a phase 2 trial evaluating the feasibility of intraoperative radiotherapy (IORT) as a boost for breast cancer patients. Materials and Methods: Eligible patients were treated with IORT of 20 Gy followed by whole breast irradiation (WBI) of 46 Gy. A total of 55 patients with a minimum follow-up of 1 month after WBI were evaluated. Optically stimulated luminescence dosimeter (OSLD) detected radiation dose delivered to the skin during IORT. Acute toxicity was recorded according to the Common Terminology Criteria for Adverse Events v4.0. Clinical parameters were correlated with seroma formation and maximum skin dose. Results: Median follow-up after IORT was 25.9 weeks (range, 12.7 to 50.3 weeks). Prior to WBI, only one patient developed acute toxicity. Following WBI, 30 patients experienced grade 1 skin toxicity and three patients had grade 2 skin toxicity. Skin dose during IORT exceeded 5 Gy in two patients: with grade 2 complications around the surgical scar in one patient who received 8.42 Gy. Breast volume on preoperative images (p = 0.001), ratio of applicator diameter and breast volume (p = 0.002), and distance between skin and tumor (p = 0.003) showed significant correlations with maximum skin dose. Conclusions: IORT as a boost was well-tolerated among Korean women without severe acute complication. In vivo dosimetry with OSLD can help ensure safe delivery of IORT as a boost.

A Study on Written Year and Contents of 『Naeuiweonshikryef』 (내의원 편 『내의원(內醫院) 식례(式例)』의 저술 시기와 내용 연구)

  • Naeuiweon (內醫院) is the royal medical office of Joseon Dynasty. "Naeuiweonshikrye (內醫院式例)" contains various regulations of Naeuiweon in early 19th century. Therefore, attention has been utilized by several researchers. However, these studies show partial side of this document. The purpose of this study is to introduce and analyze contents of "Naeuiweonshikrye". Additionally, as through the body of the written time of the contents was analyzed this document. The authors of this study found that. First, "Naeuiweonshikrye" is estimated in the volume whether modified or supplemented prior "Naeuiweonji (內醫院志)". Second, the written time of this document is about Sunjo'10(1810). Third, "Naeuiweonshikrye" is the primary document that provides a wealth of information about the actual operational and regulatory Naeuiweon (內醫院) in the early 19th century. There is no other material information has been recorded only in the literature. For example, there are several building names in the Naeuiweon. Finally, this document informs the concept of pharmaceutical terminology used in Joseon Dynasty.

Medicine Ontology Building based on Semantic Relation and Its Application (의미관계 정보를 이용한 약품 온톨로지의 구축과 활용)

  • An ontology consists of a set and definition of concepts that represents the characteristics of a given domain and relationship between the elements. To reduce time-consuming and cost in building ontology, this paper proposes a semiautomatic method to build a domain ontology using the results of text analysis. To do this, we Propose a terminology processing method and use the extracted concepts and semantic relations between them to build ontology. An experiment domain is selected by the pharmacy field and the built ontology is applied to document retrieval. In order to represent usefulness for retrieving a document using the hierarchical relations in ontology, we compared a typical keyword based retrieval method with an ontology based retrieval method, which uses related information in an ontology for a related feedback. As a result, the latter shows the improvement of precision and recall by $4.97\%$ and $0.78\%$ respectively.

A Study on the Narratives of the Chapters Shanggutianzhenlun and Siqidiaoshendalun (「상고천진론(上古天眞論)」과 「사기조신대론(四氣調神大論)」의 서사(敍事)에 대한 연구(硏究))

  • Objectives : Recently in the field of medical education, the study of narrative has been gaining much attention. This study examines the Huangdineijing in terms of narrative studies. Methods : The chapter Shanggutianzhenlun, which is narratively sound in its composition, and the chapter Siqidiaoshendalun, whose narrative structure is simple but composed in a way that is difficult to understand were studied. Results : The narrative structure of Shanggutianzhenlun is interrogatory with a main character, making it appropriate for narrative study. However, the Siqidiaoshendalun arranges its content rather simply. Conclusions : The Shanggutianzhenlun needs to be converted into appropriate narrative terminology, while the Siqidiaoshendalun needs narrative supplementation using methods such as visual thinking and mind mapping.

The Problems of Cooperative Medical System of Oriental and Western Medicine and Their Solutions (한.양방 협진의 실태 및 문제점과 나아가야 할 방향)

  • The Korean system of health and medical care has been organized with both Oriental and Western medical sciences. To get complete clinical treatment results is not possible with only one-sided medical care, therefore we need to formulate an interdisciplinary plan for better health care, that is to say our ultimate purpose is the cooperative medical care for the promotion of social welfare and health. Hereupon, I made a searching inquiry into the present condition of cooperative medical care and its problems and also took a consideration into the medical state of other countries like China. Japan and North Korea where the Oriental medical care is used. The results of this investigation are as follows. There are some problems in both Oriental and Western(general) medical care, such as a lack of mutual confidence, a severance of interdisciplinary study, a shortage of professional human resources and so on. There also used to be problems of the system such as, the responsibility of medical care, the double charge for medical treatment, the governmental passive participation and policy, the private-oriented study system and so on. The solutions of these problems are that the mutual understanding and coexistence between both Oriental and Western medical sciences should be preceded and the interdisciplinary study, identified terminology and cooperative medical specialists would be necessary. Furthermore, the government has to seek some policies and legislation for the cooperative medical system and needs to support the public research institutes and centers of the cooperative medical care. After all, we have to train the cooperative medical specialists for the mutual aid of both Oriental and Western medical sciences and the government also has to support it with some policies and legislation for the better medical care system.
