Temporomandibular Disorder Caused by Nasopharyngeal Cancer (비인두암종에 의한 측두하악장애)

  • Orofacial pain and limited range of mouth opening as symptoms of temporomandibular disorder are mainly triggered by the structural and/or functional changes of temporomandibular joint and related structure itself. But careful diagnostic evaluation should be needed because they may be occurred by another pathologic conditions such as neoplasm in head and neck region. If there would be atypical pain characteristics or clinical features, systemic comorbid symptoms, or poor response to treatment, advanced imaging modalities such as CT or MRI will be mandatory for differential diagnosis. We experienced the case which was diagnosed as nasopharyngeal cancer mimicking temporomandibular disorder, and reviewed clinical considerations for proper differential diagnosis.

Comparative Clinical Study between the Acupuncture Treatment and the Chuna Treatment on Temporomandibular Disorder (측두하악 증후군에 대한 침치료 군과 추나치료 군의 임상적 고찰)

  • Objectives : The purpose of study was to compare and analyze the effects of the distance Acupuncture and the Chuna on Temporomandibular Disorder. Methods : Clinical studies were done on 31 patients who were treated with Temporomandibular Disorder at Dept. of Acupuncture & Moxibusition of Hospital of Ha-na Oriental Medicine from May 23 2005 to Apr 30 2006. Subjects were divided Into two group, Distance acupuncture group and Chuna group. In Distance acupuncture group, we treated patients with Wijungyug(胃正格) or Damjungyug(膽正格). In Chuna group, we treated patients with Distraction & Translation technique. For evaluating Function of Temporomandibular joint of Patients, we use the Modified Craniomandibular Index(mCMI) before treatment and after 6th treatment. Results : In Mandibular Movement(MM) of patients, the patients of Acupuncture Group were more effective than Chuna group. In TMJ noise(TN) of patients, the patients of Chuna Group were more effective than Acupuncture group. Conclusions : The Distance acupuncture may be better effect on Mandibular Movement and The chuna maybe better effect on TMJ noise.

Effects of Masseter and Cervical Muscle Activity in Temporomandibular Joint Disorder (저작근 및 경부근 긴장도가 측두하악장애에 미치는 영향)

  • Objectives : The purpose of this study was to investigate the relationship between masseter and cervical muscle activity and temporomandibular disorder in female office workers. Methods : Experimental group of 24 healthy subjects complained of temporomandibular joint related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Control group of 20 healthy subjects had no complaints of minimal discomfort on the day of test, and had no discomfort in the past 7 days. If they had reported discomfort in the past 12 months, it was of a short duration(<3 months) and resolved at least 3 months prior to participation. Outcomes were assessed by meridian-electromyography(MEMG), whole spin x-ray, mandibular function impairment questionnaire(MFIQ), neck disability index(NDI), visual analog scale(VAS), Beck depression inventory(BDI), stress reaction inventory(SRI) and Holmes & Rahe social readjustment rating scale(SRRS). Results : The contraction power of masseter muscle, upper trapezius, sternocleido-mastoid muscle and erector spinae by MEMG was significantly higher in the experimental group. The muscle fatigue of masseter muscle and sternodeido-mastoid muscle by MEMG was significantly higher in the experimental group. SRI was significantly higher in experimental group. There was no significant difference between two groups in the Jackson's angle, Cobb's method and cranio-cervical posture. Conclusions : The results suggest that temporomandibular disorder related mental stress but physical stress does not change cervical structure significantly.

Influence of Temporo-mandibular Joint Training Using Physical Therapy on the Vowel Acoustic Characteristics (TM Joint의 물리치료를 통한 훈련이 모음의 음향학적 특성에 미치는 영향)

  • This study was to examine the change of vowel acoustic characteristics of the temporomandibular joint disorder patients by maintaining normal vocalization pattern of the temporomandibular joint through increasing the range of motion, that was, the oral cavity sonorant cavity of the temporomandibular joint, related to vowel articulation through temporomandibular training using the physical therapy. The subjects of this study were 3 male adults in 20-30s that were diagnosed with temporomandibular joint disorder. As a result of conducting temporomandibular training program using the physical therapy, the $1^{st}$ Formant Frequency(F1), $2^{nd}$ Formant Frequency(F2), and Fundamental Frequency(F0) of the temporomandibular joint disorder patients were increased compared to before and this showed the change of the $1^{st}$ Formant Frequency(F1) related to the open mouth grade of a vowel, as well as the $2^{nd}$ Formant Frequency(F2), and Fundamental Frequency(F0) related to the front-back of a vowel which shows the relationship between the temporomandibular joint, vowels and voice calculation.

A STUDY OF MASSETERIC SILENT PERIOD ON THE NORMAL SUBJECTS AND TEMPOROMANDIBULAR DISORDER PATIENTS (측두하악장애 환자와 정상인의 교근 휴지기에 관한 연구)

  • This study was conducted for the assessment of the usefulness of masseteric silent period on electromyogram as a diagnostic method for temporomandibular disorder. Of students and dentists in Dental College of Yonsei University and patients, 36 experimental subjects with symptoms such as clicking bound, pain in the temporomandibular joint area and limitation of jaw movement, and 33 control subjects without such symptoms were selected for this study. On each subject electromyographic masseter muscle silent period followed by mention tap was recorded with surface electrodes and analysed with computerized system. The following results were obtained: 1. The mean silent period was $36.97{\pm}9.23$ msec in experimental group, and $25.62{\pm}5.24$ msec in control group respectively. 2. There were no statistically significant differences in silent period between male and female in either experimental and control group. 3. Silent periods in experimental group were more prolonged than those of control group. (P < 0.01) Taken all together, electromyographic masseter muscle silent period may be useful for diagnosis and evaluation of temporomandibular disorder.

The Clinical Study for the Effects of Chuna Treatment on Patients with Temporomandibular Disorder (한방병원에 내원한 악관절 환자에 대한 추나치료의 임상적 고찰)

  • Objectives : The propose of this study is to find out the effects of Chuna treatment on temporomandibular disorder. Methods : The 52 patients were divided into 2 groups: a group was treated except Chuna treatment and B group was treated with Chuna treatment. Both groups were treated with acupuncture treatment. Patients were evaluated by visual analogue scale(VAS) and facial pain score scale(FPSC). Results : 1. Both groups were significantly decreased in VAS and FPSC after 4 weeks of treatment. 2. B group compared with A group was not significantly decreased in VAS and FPSC after 4 weeks of treatment. Conclusions : We found out that Chuna treatment and acupuncture is considered to be effective and useful on temporomandibular disorder.

The Effect of Ultrasound and Myofascial Release on a Functional Recovery of Neck in Patients with Temporomandibular Disorder (측두하악관절장애(TMD)환자에게 초음파와 근막이완술이 측두하악관절 및 경부의 기능적 회복에 미치는 영향)

  • Purpose:The purpose of this study is to examine the effectiveness of ultrasound and myofascial release in patients who have opening mouth limitation and temporomandibular disorder. Methods:Twenty students were randomly assigned to a myofascial release group(n=10) or an ultrasound group(n=7). Each group was treated for 15 minutes per a every treatment session for four weeks. Cervial range of motion(CROM) was used to measure cervical R.O.M., and a mm(millimeter) graduated ruler was used to measure opening mouth limitation. Results:Results of the follows : 1. The neck extension of range of motion of both groups was significantly increased in length of time (p<.05). 2. The neck flexion of range of motion of both groups was significantly increased (p<.05). 3. The quantity of opening mouth of temporomandibular joint of both groups was significantly increased (p<.05). Conclusion:These data suggest that myofascial release and ultrasound are beneficial to increasing a functional recovery of neck(flexion and extension) and opening mouth.

Bibliometric Analysis of Research Trends of Acupuncture on Temporomandibular Disorders Treatment over the Past 20 Years (최근 20년간 턱관절 장애의 침 치료 연구에 대한 계량서지학적 분석)

  • Objectives By bibliographically analyzing acupuncture treatment studies for temporomandibular disorders over the past 20 years, we found an overview of global trends and a new perspective on future research directions. Methods We searched on the Web of Science webpage through the formula (TS=[temporomandibular] OR TS=[craniomandibular] OR TS=[jaw]) AND (TS=[*acupuncture] OR TS=[dry needl*] OR TS=[warm needl*] OR TS=[thread embed*]) AND (PY=[2003-2022]). And it was analyzed by year, research field, academic journal, country, research institute, author, and keyword. Results 194 papers were searched, and 92 papers were finally selected. The number of papers published over the past 20 years has been on the rise. Research has been the most active in the field of Dentistry Oral Surgery Medicine. Brazil published the most papers. And by institution, Universidade de Sao Paulo published the most papers. Among the authors, Fernández-de-las-Peñas has published the most papers. In the analysis by keyword, the top five keywords were temporomandibular joint disorder, acupuncture, myofascial pain, pain and management. Conclusions This study will provide useful guidelines for setting the direction of research by referring to the research status and keyword analysis when conducting research on the acupuncture on temporomandibular disorder in the future.

Clinical Comparison Study on 40 Cases of Temporomandibular Disorder Patients with Idiopathic Scoliosis Treated by Chuna & General Oriental Method (추나치료를 병행한 척추측만증을 동반한 악관절장애환자 40례에 대한 임상적 비교 고찰)

  • Objectives : This study was to evaluate the effect of Chuna on Temporomandibular Disorder Patients with idiopathic Scoliosis, utilizing Cobb's angle to determine the degree of curvature progression in the thoracic and lumbar regions. Methods: From October 1st 2004 to June 31st 2005, the clinical comparison study was carried out 40 cases of Temporomandibular Disorder Patients with idiopathic Scoliosis treated by both Chuna and General oriental method(Experimental Group) & only General oriental method(Control Group), who had been treated at Dept. of Acupuncture and Moxibustion Daejon Oriental Medical Hospital. Results : The results obtained as follows; 1. The facial pain of Experimental Group and Control Group was improved significantly. but the difference of two groups is not significant.(p<0.05) 2. The function of Temporomandibular joint of Experimental Group and Control Group was improved significantly. Experimental Group is more effective than Control Group significantly.(p<0.05) 3. The limitation of activity of Temporomandibular joint of Experimental Group and Control Group was improved significantly. Experimental Group is more effective than Control Group significantly. (p<0.05) Conclusion : This result sugests that the Chuna be effective treatment modality on Temporomandibular Disorder Patients with idiopathic Scoliosis. So further research is needed continuously.

A STUDY ON THE SIZE OF CONDYLE AND THE POSTERIOR SLOPE OF THE ARTICULAR EMINENCE IN PATIENTS WITH TEMPOROMANDIBULAR JOINT DISORDERS. (측두하악장애 환자에서 과두크기와 관절융기의 후방경사에 관한 연구)

  • The purpose of this study was to investigate the interrelationship between temporomandibular joint disorders, and the condyle size and angle of the posterior slope of the articular eminence. The subjects used in this study were 100 patients with temporomandibular joint disorders and 100 volunteers with normal temporomandibular joints. All the patients and the volunteers were subjected to take panoramic and temporomandibular radiographs for the morphologic evaluation. The films were traced, measured, and analyzed. The data were processed with SPSS/PC+ package for statistical analysis. The obtained results were as follows; 1. The posterior slope of the articular eminence in the group of temporomandibular joint disorders was larger than that in the normal group (p<0.05). The mean articular eminence angle was $31.6^{\circ}\;{\pm}\;6.3$ in the group of temporomandibular joint disorder, and $29.9^{\circ}\;{\pm}\;8.4$ in the normal group. 2. There were no statistically significant differences in the discrepancies of the left and right articular eminence angular measurements between the group (p>0.05). 3. There were no statistically significant differences in the condylar ratio between the groups (p>0.05). 4. There were no statistically significant differences in the discrepancies of the left and right ramus length measurements between the groups (p>0.05). 5. The relative size of condyle to fossa in the group of temporomandibular disorders was smaller than that in the normal group (p<0.05).
