Purpose: Although traditional and current treatment strategies may demonstrate success, persistence or recurrence of difficult-to-heal wounds remain significant problems. A novel product, Hyalomatrix$^{(R)}$ (Fidia Advanced Biopolymer, Abano Terme, Italy) is a bilayer of an benzyl esterified hyaluronan scaffold beneath a silicone membrane. The scaffold delivers hyaluronan to the wound, and the silicone membrane acts as a temporary epidermal barrier. We present the results obtained with Hyalomatrix$^{(R)}$ in the treatment of difficult-to-heal wounds. Methods: From November, 2008 to March, 2010, Hyalomatrix$^{(R)}$ has been used on total 10 patients with wounds that were expected difficult to heal with traditional and other current strategies. After average 37.4 days from development of wounds, Hyalomatrix$^{(R)}$ was applied after wound debridement. On the average, Hyalomatrix$^{(R)}$ application period was 17.6 days. After average 16.5 days from removal of Hyalomatrix$^{(R)}$, skin grafts was performed. Results: In all cases, regeneration of fibrous granulation tissues and edge re-epithelization were present after the application of the Hyalomatrix$^{(R)}$. And all of the previous inflammatory signs were reduced. After skin grafts, no adverse reactions were recorded in 9 cases. But in one case, postoperative wound infection occured due to a lack of efficient fibrous tissues. In this model, the Hyalomatrix$^{(R)}$ acts as a hyaluronan delivery system and a barrier from the external environments. In tissue repair processes, the hyaluronan performs to facilitate the entry of a large number of cells into the wounds, to orientate the deposition of extracellular matrix fibrous components and to change the microenvironment of difficult-to-heal wounds. Conclusion: Our study suggests that Hyalomatrix$^{(R)}$ could be a good and feasible approach for difficult-to-heal wounds. The Hyalomatrix$^{(R)}$ improves microenvironments of difficult-to-heal wounds, reduces infection rates and physical stimulus despite of aggravating factors.
Purpose: Aplasia Cutis Congenita (ACC) is a rare disease characterized by the focal defect of the skin at birth, frequently involving scalp, but it may affect any region of the body. There are no etiology known but some conditions such as intrauterine vascular ischemia, amniotic adherences and viral infections are associated. The ideal treatment for the ACC is not known. Superficial and relatively small sized defects (< $3{\times}5\;cm$) may heal spontaneously and large defects related with risks of infection and bleeding may require aggressive surgical treatment. Hyalomatrix$^{(R)}$ is a bilayer of an esterified hyaluronan scaffold beneath a silicone membrane. It has been used as a temporary dermal substitute to cover deep thickness skin defect and has physiological functions derive from the structural role in extracellular matrix and interaction with cell surface receptor. This material has been used for the wound bed pre-treatment for skin graft to follow and especially in uncooperative patient, like a newborn, this could be a efficient and aseptic way of promoting granulation without daily irritative wound care. For this reason, using Hyalomatrix$^{(R)}$ for the treatment of ACC was preferred in this paper. Methods: We report a case of a newborn with ACC of the vertex scalp and non-ossified partial skull defect. The large sized skin and skull defect ($6{\times}6\;cm$) was found with intact dura mater. No other complications such as bleeding or abnormal neurologic sign were accompanied. Escharectomy was performed and Hyalomatrix$^{(R)}$ was applied for the protection and the induction of acute wound healing for 3 months before the split-thickness skin graft. During the 3 months period, the dressing was renewed in aseptic technique for every 3 weeks. The skin graft was achieved on the healthy granulation bed. Results: The operative procedure was uneventful without necessity of blood transfusion. Postoperative physical examination revealed no additional abnormalities. Regular wound management was performed in out-patient clinic and the grafted skin was taken completely. No other problems developed during follow-up. Conclusion: Hyalomatrix$^{(R)}$ provides protective and favorable environment for wound healing. The combination of the use of Hyalomatrix$^{(R)}$ and the skin graft will be a good alternative for the ACC patients with relatively large defect on vertex.
교합평면은 시상면적인 치열궁 형태를 나타내는 것으로, 악구강계를 구성하는 요소의 하나인 교합을 형태적으로 구성한다. 본 증례에서는 생체의 정중 시상면에 대한 수평, 측방좌표를 교합기상에 재현하여 상하악 치아 및 결손부 치조제에 대한 좌우 동적인 위치관계 얻기 위해 Shilla system (Hamans, Tokyo, Japan)을 이용하여 교합평면을 재구성 하였다. 본 증례의 환자는 41세의 남자 환자로 상악 전치부 10본금속-도재 고정성국소의치 파절 및 탈락과 전반적인 치료를 주소로 내원하였다. 임상 검사, 방사선 검사, 모형분석을 통해 임플란트 식립을 동반한 완전구강회복술을 하기로 결정하였다. Gothic Arch device (Centrofix; AmannGirrbach GmbH, Germany)를 이용하여 악간관계를 채득하고 마운팅 후, Shilla system을 이용하여 기존 교합평면을 평가, 분석하여 진단 납형을 제작하였다. 이를 토대로 방사선 스텐트를 제작하여 임플란트 식립하였고, 진단 납형을 이용하여 임시수복물을 제작하였다. 임시 수복물의 형태 및 교합 양식을 재현하기 위해 맞춤 전방유도판을 제작하고, 교차 마운팅을 시행하였으며 최종 수복물은 지르코니아 코핑을 이용한 완전 도재관으로 제작하였다. 이상과 같이 전악수복환자에서 진단과 치료 과정에서 Shilla system을 이용하여 적절한 심미적, 기능적 수복 결과를 얻을 수 있었다.
완전 무치악 환자의 고정성 보철 수복 시 수술 전 적합한 치아 위치를 확인하고 잔존 치조골과 연조직의 평가를 통해 수술을 계획하여 정확한 임플란트를 식립하는 것이 필수적이다. 가이드 임플란트 수술과 CAD-CAM 기술은 보철 치료와 수술을 동시에 계획할 수 있고 치료에 대한 예지력을 확보할 수 있는 장점이 있다. 본 임상 증례에서는 이러한 디지털 기술을 활용하여 임시의치 제작, 고정성 수복 치료 전 심미성 평가 및 가이드 수술을 계획하였다. 수술 후에도 기존에 얻었던 정보들을 통해 고정성 임시 보철물을 제작하는데 유용하게 사용되었으며 최종적으로 임시 보철물을 반영하여 단일 구조 지르코니아 보철물을 제작하였고 기능적, 심미적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.
Cofferdam is a temporary levee or dam structure built by using sheet pile or earth materials to prevent water infiltration during construction work of bridge, dam, harbour dock, or hydraulic structures in the river. In this regard, it is required to secure cutoff ability for dry work and workability for rapid installation and removal of the temporary dam or levee structures. In this paper, case studies for design and construction of cofferdam were performed, and water diversion method was briefed with some examples of cofferdam type as well. For the case study details of design and construction were reviewed based on cofferdams under construction related to 16 submerged weirs of "The 4-river restoration project" and dam type cofferdam respectively. From the review, it was known that the method for changing the water flow is selected based on the data from geological and geo-hydraulic site investigation in order to mitigate environmental effects by making sure if the design cross-sectional area of flow and maximum working days are sufficiently guaranteed. Finally, the primary findings and main conclusion derived are summarized that determination of applicable type of cofferdam should be checked by case study and meet design requirements such as water inflow control, constructability.
PURPOSE. The purpose of this report was to evaluate the effect of the fabrication method and material type on the fracture strength of provisional crowns. MATERIALS AND METHODS. A master model with one crown (maxillary left second premolar) was manufactured from Cr-Co alloy. The master model was scanned, and the data set was transferred to a CAD/CAM unit (Yenamak D50, Yenadent Ltd, Istanbul, Turkey) for the Cercon Base group. For the other groups, temporary crowns were produced by direct fabrication methods (Imident, Temdent, Structur Premium, Takilon, Systemp c&b II, and Acrytemp). The specimens were subjected to water storage at $37^{\circ}C$ for 24 hours, and then they were thermocycled (TC, $5000{\times}$, $5-55^{\circ}C$) (n=10). The maximum force at fracture (Fmax) was measured in a universal test machine at 1 mm/min. Data was analyzed by non-parametric statistics (${\alpha}$=.05). RESULTS. Fmax values varied between 711.09-1392.1 N. In the PMMA groups, Takilon showed the lowest values (711.09 N), and Cercon Base showed the highest values (959.59 N). In the composite groups, Structur Premium showed the highest values (1392.1 N), and Acrytemp showed the lowest values (910.05 N). The composite groups showed significantly higher values than the PMMA groups (P=.01). CONCLUSION. Composite-based materials showed significantly higher fracture strengths than PMMA-based materials. The CADCAM technique offers more advantages than the direct technique.
심한 마모를 가진 환자의 치료에서 광범위한 보철 수복을 필요로 하는 경우가 있는데, 마모의 원인과 수직 고경의 변화 유무 등을 판단하여 현재 구강상태의 회복과 장기간의 안정성을 이루어야 한다. 수직 고경을 증가시킬 때는 임시 수복을 하고 장기간의 관찰을 통해 환자의 적응을 평가해야 한다. 본 증례의 환자54세 남자 환자로 전반적인 치아 마모와 심미적 불만족을 주소로 내원하였다. 환자의 구강 내 소견 상 교모와 부식이 동반되어 과도한 마모가 발생한 것으로 진단하였다. 수직 고경의 변화를 평가하기 위해 안정위시 악간공극, 발음, 안모, 심미, 임상적 치관 길이, 중절치 절단과 순측 전정과의 수직거리 등을 평가하였고, 교합기의 incisal pin기준으로 수직 고경을 2.5 mm 증가하기로 하였다. 임시 보철물을 이용하여 8주의 안정화 기간 이후 최종 보철물을 제작하였다. 6개월 간 경과 관찰 결과 기능적, 심미적으로 양호한 결과를 보였다.
구치부 지지 상실로 부적절한 교합을 보이는 환자를 전악 보철 수복하기 위해서는 적절한 교합수직고경의 증가와 중심위에서 안정된 교합을 나타내는 보철물의 제작이 필요하다. 본 증례는 대구치가 상실되고 치아 마모를 보이는 환자에서 임플란트 보철과 지르코니아 전부 전장관을 사용해 완전구강회복한 증례이다. 증가된 교합수직고경에의 적응 여부를 평가하기 위해 occlusal splint 및 임시 수복물이 사용됐으며, 임시 수복물의 전방 유도를 따르는 최종 보철물을 제작함으로써 안정된 교합 회복을 이룰 수 있었다.
상악 전치부 치료 시에는 다양한 구도 속에서 체계적인 진단을 통해 적절한 형태와 배열, 색조를 가지는 수복물을 제작하여 심미적으로 환자가 만족할 수 있는 결과를 이끌어내야 한다. 이를 위해 치아 구도, 치은구도, 안면구도, 치아-안면 구도의 4가지 구도 속에서 조화를 이루는 진단 및 치료계획이 필요하다. 심미적인 욕구가 강한 경우, 주관적인 환자의 심미안을 만족시키면서 기능적인 부분을 확인하기 위해, 적절한 임시 수복물을 제작하여 기능적, 심미적 측면의 평가가 필요하다. 최종 보철물 제작에 이중 스캔을 이용하여 임시 수복물의 정보를 반영할 수 있다. 본 증례는 상악 전치부가 비심미적인 환자에서, 체계적인 심미 분석 하에 임시 수복물을 제작하고 기능적, 심미적 측면을 평가 및 수정하여, 이중 스캔을 통해 임시수복물의 정보를 최종 수복물에 반영 후 최종 수복한 증례이다.
광범위한 치아 상실과 마모로 인한 교합면의 파괴로 수직고경이 감소된 경우 안모의 변화, 전방 유도의 상실로 인한 반대 교합을 보일 수 있으며, 또한 턱관절의 문제를 발생 시킬 수도 있다. 이러한 경우 교합 수직 고경의 증가를 통한 전악 수복을 고려할 수 있으며, 적절한 환자의 수직 고경 평가가 필요하다. 본 증례는 80세의 남자 환자로 전반적인 보철계획을 주소로 내원하였으며, 치아상실 및 마모에 따른 전치부의 반대 교합과 교합 수직 고경의 감소가 관찰되었다. 진단 모형과 방사선 사진, 임상적 검사를 이용하여 교합 관계를 분석하여 교합 수직 고경 증가를 결정하였다. 진단 wax up을 시행하였고 임시 보철물을 제작하였다. 3개월의 적응 기간 후 상악 고정성 보철물과 하악 임플란트 지지 피개의치로 최종 수복하였다. 장착 후 8개월경과 관찰 중이며 양호한 심미적 기능적 개선을 보였다.
