Can different agents reduce the damage caused by bleaching gel to pulp tissue? A systematic review of basic research

  • Objectives: This study aimed to investigate the effectiveness of different topical/systemic agents in reducing the damage caused by bleaching gel to pulp tissue or cells. Materials and Methods: Electronic searches were performed in July 2023. In vivo and in vitro studies evaluating the effects of different topical or systemic agents on pulp inflammation or cytotoxicity after exposure to bleaching agents were included. The risk of bias was assessed. Results: Out of 1,112 articles, 27 were included. Nine animal studies evaluated remineralizing/anti-inflammatories agents in rat molars subjected to bleaching with 35%-38% hydrogen peroxide (HP). Five of these studies demonstrated a significant reduction in inflammation caused by HP when combined with bioglass or MI Paste Plus (GC America), or following KF-desensitizing or Otosporin treatment (n = 3). However, orally administered drugs did not reduce pulp inflammation (n = 4). Cytotoxicity (n = 17) was primarily assessed using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay on human dental pulp cells and mouse dental papilla Cell-23 cells. Certain substances, including sodium ascorbate, butein, manganese chloride, and peroxidase, were found to reduce cytotoxicity, particularly when applied prior to bleaching. The risk of bias was high in animal studies and low in laboratory studies. Conclusions: Few in vivo studies have evaluated agents to reduce the damage caused by bleaching gel to pulp tissue. Within the limitations of these studies, it was found that topical agents were effective in reducing pulp inflammation in animals and cytotoxicity. Further analyses with human pulp are required to substantiate these findings.

Fluoroscopy-guided intra-articular steroid injection for sternoclavicular joint arthritis secondary to limited cutaneous systemic sclerosis: a case report

  • We report a case of fluoroscopy-guided intraarticular steroid injection for sternoclavicular joint (SCJ) arthritis caused by limited cutaneous systemic sclerosis (SSc). A 50-year-old woman diagnosed with limited cutaneous SSc presented with swelling and pain in the right SCJ. MRI revealed signs of inflammation consistent with right-sided sternoclavicular joint arthritis. After the failure of oral medications, we performed fluoroscopy-guided injection in this region. She reported complete resolution of her symptoms at 4 and 12-week follow-ups. This outcome suggests that a fluoroscopy-guided SCJ injection might be a safe and successful treatment option for sternoclavicular joint arthritis.

  • Polyarteritis nodosa (PAN) is a systemic vasculitis involving small- and medium-sized arteries, which presents with necrotizing inflammation. PAN occurs as a systemic disease or as a limited form confined to a single organ. Few cases have been reported with single organ involvement, and even fewer have been reported with skeletal muscle involvement. Herein, we report the ultrasonography and magnetic resonance imaging findings in a rare case of PAN with limited muscle involvement in a 66-year-old man.

Dynamic Patterns of Systemic Innate Immunity and Inflammatory Associated Factors in Experimental Caprine Coccidiosis

  • The present study was designed to assess the dynamic patterns of pro-inflammatory cytokines, including $IFN-{\gamma}$, $TNF-{\alpha}$, IL-4, IL-6, acute phase protein (${\alpha}1$-acid-glycoprotein, AGP), and an inflammation associated factor (adenosine deaminase; ADA) following experimental caprine coccidiosis. Ten kids aging from 2 to 4 months were infected orally with $5{\times}10^4$ sporulated oocysts and 10 animals served as controls. Blood samples were collected in both groups before infection and at days 3, 7, 14, 21, 28, and 35 post-infection (PI), and the levels of above-mentioned factors were measured. $IFN-{\gamma}$, $TNF-{\alpha}$, IL-4, IL-6, AGP, and ADA activities were significantly higher in infected animals from day 7 PI (P<0.05). In conclusion, the circulatory levels of most systemic inflammatory markers, including pro-inflammatory cytokines ($IFN-{\gamma}$, $TNF-{\alpha}$, IL-4, IL-6), AGP, and ADA increased significantly starting from day 3 to day 7 PI in caprine coccidiosis.

  • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that may affect many organ systems including the nervous system. The immune response in patients with SLE can cause inflammation and other damage that can cause significant injury to the arteries and tissues. A 48-year-old woman was admitted to the hospital because of transient monocular blindness. Magnetic resonance imaging and conventional angiography showed severe stenosis of the distal intracranial internal carotid artery. The patient was diagnosed as having SLE but the antiphospholipid antibodies were negative. Amaurosis fugax has not been previously reported as an initial manifestation of SLE in Korea. We report a patient with a retinal transient ischemic attack as the first manifestation of SLE.

Long-Term Outcome of Chronic Obstructive Pulmonary Disease: A Review

  • Chronic obstructive pulmonary disease (COPD) is a chronic airway inflammation characterized by fixed airflow limitation and chronic respiratory symptoms, such as cough, sputum, and dyspnea. COPD is a progressive disease characterized by a decline in lung function. During the natural course of the disease, acute deterioration of symptoms leading to hospital visits can occur and influence further disease progression and subsequent exacerbation. Moreover, COPD is not only restricted to pulmonary manifestations but can present with other systemic diseases as comorbidities or systemic manifestations, including lung cancer, cardiovascular disease, pulmonary hypertension, sarcopenia, and metabolic abnormalities. These pulmonary and extrapulmonary conditions lead to the aggravation of dyspnea, physical inactivity, decreased exercise capacity, functional decline, reduced quality of life, and increased mortality. In addition, pneumonia, which is attributed to both COPD itself and an adverse effect of treatment (especially the use of inhaled and/or systemic steroids), can occur and lead to further deterioration in the prognosis of COPD. This review summarizes the long-term outcomes of patients with COPD. In addition, recent studies on the prediction of adverse outcomes are summarized in the last part of the review.

외상에 의해 발생한 체동맥-폐혈관루 - 1예 보고 - (Traumatic Systemic Artery to Pulmonary Vessel Fistula - A case report -)

  • 체동맥-폐혈관루(systemic artery to pulmonary vessel fistula, SAPVF)는 비정상적으로 형성된 체동맥과 폐혈관 사이의 연결이다. 이는 대부분 선천적으로 발생하지만, 드물게는 폐나 흉막의 종양, 염증성 질환 및 외상 등으로 인해 발생할 수 있다. 내원 16년 전 교통사고로 인한 횡경막 파열로 수술한 과거력이 있는 38세 남자 환자가 객혈을 주소로 내원하였고, 흉부 컴퓨터 단층 촬영과 혈관 조영술로 늑간동맥 및 내흉 동맥과 폐혈관이 연결된 SAPVF를 진단하였다. 저자들은 외상에 의한 SAPVF 1예를 경험하였고, 혈관 색전술을 시행하여 좋은 결과를 얻었기에 국내 문헌상으로는 최초로 이를 보고하는 바이다.

호도약침이 알레르기에 미치는 영향 (Effect of Juglandis Semen Aqua-acupuncture and Acupuncture on the Allergic Response)

  • Experimental studies were done to research the dinical effects of Juglandis Semen aqua-acupuncture and acupuncture(BL 13) on the anti-allergic response. anaphylaxis provoked by the compound 48/80, delayed type hypersensitivity response to picryl chloride and SASC and inflammation response to egg albumin. The following results have been obtained: Juglandis Semen aqua-acupuncture and acupuncture(BL13) group were increased the survival rate in compound48/80 induced systemic anaphylactic reaction. Picryl chloride induced contact dermatitis and delayed type hypersensitivity in SRBC challenged mouse were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture (BL13) group. Inflammation response - WBC, CRP and Nitric Oxide in egg albumin induced allergic rat were significantly decreased in Juglandis Semen aqua-acupuncture and acupuncture(BL13) group. According to the above results, Semen aqua-acupuncture and acupuncture(BL13) both depress the allergy reaction.

Role of inflammasomes in inflammatory autoimmune rheumatic diseases

  • Inflammasomes are intracellular multiprotein complexes that coordinate anti-pathogenic host defense during inflammatory responses in myeloid cells, especially macrophages. Inflammasome activation leads to activation of caspase-1, resulting in the induction of pyroptosis and the secretion of pro-inflammatory cytokines including interleukin $(IL)-1{\beta}$ and IL-18. Although the inflammatory response is an innate host defense mechanism, chronic inflammation is the main cause of rheumatic diseases, such as rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), ankylosing spondylitis (AS), and $Sj{\ddot{o}}gren^{\prime}s$ syndrome (SS). Since rheumatic diseases are inflammatory/autoimmune disorders, it is reasonable to hypothesize that inflammasomes activated during the inflammatory response play a pivotal role in development and progression of these diseases. Indeed, previous studies have provided important observations that inflammasomes are actively involved in the pathogenesis of inflammatory/autoimmune rheumatic diseases. In this review, we summarize the current knowledge on several types of inflammasomes during macrophage-mediated inflammatory responses and discuss recent research regarding the role of inflammasomes in the pathogenesis of inflammatory/autoimmune rheumatic diseases. This avenue of research could provide new insights for the development of promising therapeutics to treat inflammatory/autoimmune rheumatic diseases.

전치부 Pontic ridge lap 제작에 대한 연구 (A Study on the Making of Anterior Pontic Ridge Lap)

  • 본 연구는 보철치료의 최종 목적인 결손치아 및 그 주위 조직의 정상적 기능의 회복과 유지를 위한 전치부 교의치의 반 자정(ridge lap)형 가공치의 체계적인 제작 방법을 연구하였다. 가공치가 갖추어야 할 설계 기준를 제도화 하였고, 치과의사의 소견으로 염증, 치주질환, 음식물 잔류 및 자정작용의 효과가 기존의 제작 방법보다 더 우수한 결과를 얻었다. 본 논문에서 제시하는 가공치 제작방법은 임상에 적용해도 무방하다고 판단된다.
