Effect of Acute Unilateral Ureteral Obstruction on Handling of $Li^+$ by Contralateral Kidney in Rabbits (급성 일측 수뇨관 폐쇄후 상대신의 $Li^+$처리에 관한 연구)

  • Changes in handling of $Li^+$ by contralateral kidney during acute $Li^+$ loading were investigated immediately after unilateral ureteral obstruction. Carotid artery, jugular vein, renal vein and ureter of experimental animal were catheterized and renal venous flow was shunted to .external jugular vein. In experimental group right ureter was ligated. One to two hours after operation a single shot of LiCl solution (2 mEq/kg) was intravenously injected and then .arterial, renal venous blood and urine samples were taken sequentially for 1 to $1{\frac{1}{2}}$ hours. Urine volume, plasma and urinary concentrations of $Li^+$, $Na^+$ and $K^+$ were measured and urinary excretion of them were calculated. Results obtained were as follows: 1) In experimental group urine volume, urinary excretion of $Na^+$, and $K^+$ by contralateral kidney after unilateral ureteral obstruction were slightly larger than mean value of both kidney in control group. 2) During acute $Li^+$ loading contralateral kidney in experimental group showed limited $K^+$ excretion, but urinary flow and $Na^+$ excretion were comparable to mean value of both kidney in control group. 3) Urinary osmolar concentration in experimental group was much lower than that in control group, and it was maintained at low level even after Li loading. 4) In experimental group plasma$Li^+$ concentration decreased more slowly than in control group after a single shot of LiCl solution. 5) Urinary excretion of $Li^+$ in experimental group was markedly decreased, even lesseer than mean of both kidney in control group. 6) From the above results it was concluded that immediately after unilateral ureteral obstruction contralateral kidney showed normal water and $Na^+$ diuretic response to Li load but urinay $Li^+$ excretion was decreased and reclaimed $Li^+$ to systemic circulation.

Influence of Cardiac Contraction and its Phase Angle with Coronary Blood flow on Atherosclerosis of Coronary Artery (심장의 수축운동과 관상동맥 혈류와의 위상차가 관상동맥 혈관의 동맥경화 민감성에 미치는 영향)

  • Coronary arteries are subjected to very different flow conditions compared to other arteries in systemic blood circulation. We Performed a computational fluid dynamic research to investigate influence of such flow conditions in coronary arteries on development and progress of atherosclerosis in the same. The results showed big differences in the flow field of the coronary artery compared to the abdominal and femoral arteries. The coronary artery showed higher wall shear stresses due to the small vessel diameter. On the other hand, it showed only one vortex distal to the stenosis throat during a whole pulse cycle. However. several vortices were observed in the abdominal and femoral arteries in both proximal and distal sides of the stenosis throat The wall shear stresses and extent of recirculation area were increased with impedance phase angle increasing toward more negative values. Therefore, cardiac contraction and the negative impedance phase angle as large as -110。 may induce a flow field that accelerates atherosclerosis.

A Study of Influences Caused by Far Infrared and Anion on Human Body (원적외선과 음이온이 인체에 미치는 효과)

  • A survey was performed for 2 weeks in October 2004 in order to examine the effects and changes of far infrared and anion of office workers aged 40 and older before and after wearing the underwear. In the survey, questionnaires and photoplethysmography were used to measure the difference between the two tests conducted with one week interval. The findings could be summed up as follows: 1. There was no statistically significant difference in changes of vascular aging points and photoplethysmogram wave form before and after wearing the far infrared and anion underwear but the points showed the increasing tendency(p>0.05). 2. A statistically significant difference was confirmed since the total points of each item in the questionnaires increased after wearing the underwear(p<0.001). 3. In connection with the systemic symptoms, a significant difference was found with increased points in all items except for no. 6 after wearing the underwear(p<0.05). 4. With regard to the difference before and after wearing the underwear depending on the Kidney symptoms, there was a significant difference with increased points in all items except for item no. 1 after wearing the underwear(p<0.05). In conclusion, to put on the underwear with effect of far infrared and anion is believed to smooth metabolism and blood circulation, contributing by thus to promoting health and preventing disease. Further studies with wider sampling of subjects are requested to diversify and refine the findings from this sectional study.

Saturable Disposition of Taurine in the Cerebrospinal Fluid of the Rat

  • Taurine, a ${\beta}$-amino acid, plays an important role as a neuromodulator and is necessary for the normal development of the brain. Since de novo synthesis of taurine in the brain is minimal and in vivo studies suggest that taurine does not cross the blood-brain barrier, the blood-cerebrospinal fluid (CSF) barrier is likely to play a role in taurine transport between the central nervous system and the systemic circulation. Therefore, we examined in vivo elimination of taurine from the CSF in the rat to characterize in vivo kinetics of elimination for taurine from the CSF is consistent with the in vitro study. Using a stereotaxic device, cannulaes were placed into the lateral ventricle and the cisterna magna of the rat. Radio-labelled taurine and inulin (a marker of CSF flow) were injected into the lateral ventricle, and the concentrations of the labelled compounds in the CSF were monitored for up to 3 hrs in the cisterna magna. The apparent clearance of taurine from CSF was greater than the estimated CSF flow (p<0.005), indicating that there is a clearance process in addition to the CSF flow. Taurine distribution into the choroid plexus was at least 10 fold higher than that found in other brain areas (e.g., cerebellum, olfactory bulb and cortex). When unlabelled taurine was co-administered with radio-labelled taurine, the apparent clearance of the labeled taurine was reduced (p<0.01), suggesting a saturable disposition of taurine from CSF. Distribution of taurine into the choroid plexus, cerebellum, olfactory bulb and cortex was similarly diminished, indicating that the saturable uptake of taurine into these tissues is responsible for the non-linear disposition. A pharmacokinetic model involving first order elimination and saturable distribution described these data adequately. The Michaelis-Menten rate constant estimated from in vivo elimination study is similar to that obtained in the in vitro uptake experiment Collectively, our results demonstrate that taurine is transported in the choroid plexus via a taurine is cleared from the CSF via a saturable process. This process may be functionally relevant to taurine homeostasis in the brain.

  • Hwangjenegyeng somun. Treatise on the parting and Meeting of Yin-Yang is the theory of the parting and meeting about Yin-Yang. It is harmonized treatise Yin-Yang and systemic constant. Taechung and Gwangmyeng are the concretely applied form of the parting and meeting of Yin-Yang at human body. The parting and Meeting of Three-Yin- Three-Yang has six types symbols of circulated Yin-Yang. They have Six-meeting space structure as match like up-down, backside and front-side, left-side and right-side. These pairs express symbolized meaning of creation of Saeng-jang-shu-chang and also apply the same things. Somum Treatise on the Parting and Meeting of Yin-Yang explains acting of human body by the parting and meeting of Three-yin-three-yang as six structures of Opening-covering-axis of Three-yin-three-yang. This principal is expression of the parting and meeting of Yin-Yang and concretes the opening and closing of circulation process for symmetry by each acting of Three-yin-three-yang. As result. the match of six-meeting in space structure and the Opening-covering-axis of Three-yin-three-yang are the basic principal of oriental medicine about the treatise of Gyeng-Lak and the treatise of Six-Gyeng of Sanghanlon.

  • We came to the conclusion as follows from the research about pulse diagnostic technique (脈診) out of "Nan Jinghoeju jeonjung" (難經滙注箋正). 1. "Nan Jing" is practical medical book for clinician and holds different opinions about three portions and nine takings(三部九候脈法) from Huang Di Nei Jing's Taking the pulse of Bu Jung Chim Chon Kwan Chuk((浮中沈 寸關尺) is a quite creative means which has been a important role to future generation. 2. We pointed the differences between western medical science and traditional medical science. And can explain the pulse in western medicine divided from twelve channels of Oriental Medicine. We can explain special connection between heart and lungs using the theory of systemic & pulmonary circulation in western medicine. And this can be a basement of Lung controls every pulse theory in oriental medicine. 3. We have negative assertion about viewing the human body using theory of Yin and Yang-Five Elements. And have poor opinion of explaining about the matching internal organs to Chon Kwan Chuk using the theory of Yin and Yang-Five Elements. 4. We pointed out the mistake that Porak & Sang hwa act for heart. We took pulses considering heart is a real actual internal organ as others. 5. We denied the theory of Man's Chuk pulse is always weak(男子尺脈恒弱) & Woman's Chuk pulse is always strong(女子尺脈恒盛). The physical form of man and woman is not different from each other considering their inheritances from forefather & same shape of organs. So, we cannot insist on the theory.

Genetics of Pre-eclampsia

  • Pre-eclampsia is a major cause of maternal and perinatal mortality and morbidity worldwide, but remains unclear about the underlying disease mechanisms. Pre-eclampsia is currently believed to be a two-stage disease. The first stage involves shallow cytotrophoblast invasion of maternal spiral arteriole, resulting in placental insufficiency. The hypoxic placenta release soluble factors, cytokines, and trophoblastic debris into maternal circulation, which induce systemic endothelial damage and dysfunction. This cause the second stage of the disease: maternal syndrome. Epidemiological research has consistently demonstrated a familial predisposition to pre-eclampsia. Intensive research efforts have been made to discover susceptibility genes that will inform our understanding of the pathophysiology of preeclampsia and that may provide direction for therapeutic or preventative strategies. In this review, we summarize the current understanding of the role of genetic factors in the pathophysiology of pre-eclampsia and explain the molecular approach to search for genetic clues in pre-eclampsia.

  • Although the use of the antibiotics has minimized the dangers of the spread of odontogenic infection to adjacent important organs, sometimes the fatal cases, such as Ludwig's angina, mediastinitis, intracranial complications from the odotogenic infection, may be occurred. The odontogenic infections or oral and maxillofacial region may involve the intracranial area through systemic circulation or by direct spread into the intracranial cavity. Headache, malaise, loss of appetite, chills, fever, vomiting, apathy, and irritability are usually followed by more specific involvement of the nervous system. We experienced one patient who died of intracranial complication from odontogenic infection. So we report the case and the literatures about intracranial involvements following odontogenic infections and dental procedures.

  • Transdermal and topical drug delivery with minimal tissue damage has been an area of vigorous research for years. Our research team has initiated the development of an effective method for delivering drug particles across the skin (transdermal) for systemic circulation, and to localized (topical) areas. The device consists of a laser ablation based micro-particle acceleration system that can be integrated with endoscopic surgical techniques. We have successfully delivered 3μm size cobalt particles into gelatin models that represent soft tissue with remarkable penetration depth.

Pulmonary artery rupture due to bacterial endocarditis complicated by patent ductus arteriosus. (동맥관개존증에 합병한 심내막염에 의한 폐동맥파열 실험 1례)

  • Recently, we met a 12 year old female patient who suffered from bacterial endocarditis and pericarditis which were complicated by patent ductus arteriosus. She was admitted to our hospital because of dyspnea, fever, headache, and generalized ache for 10 days. The initial diagnosis was bacterial endocarditis and pericarditis complicated by patent ductus arteriosus and congestive heart failure. At first, we tried to treat the patient medically with digitalis, diuretics, and massive antibiotics. On echocardiography large amount of pericardial fluid was accumulated mainly right anterior aspect and also noted a large vegetation at pulmonary valve area. With vigorous medical treatment including repeated pericardiocentesis, the patient showed no improvement. So we decided to perform pericardiectomy for elimination of the most probable septic focus. On operation, we encountered an unpredicted event, the pericardium was thickened, distended, and its surface showed pulsating which meant connecting to systemic circulation. We decided to close the operative wound and reoperate her under cardiopulmonary bypass later. On the next day, we operated her under cardiopulmonary bypass later. On the next day we operated her under cardiopulmonary bypass. The operative findings were ruptured main pulmonary artery about 1.5cm in diameter on its ventral portion, the blood from the ruptured main pulmonary artery was filled up the localized pericardial sac due to previous pericarditis. Through the ruptured main pulmonary artery, we also found 0.5cm diametered patent ductus arteriosus. With the aid of partial cardiopulmonary bypass and inserting 24F ballooned Foley catheter at aorta, pericardiectomy was performed first. After completion of the pericardiectomy, total cardiopulmonary bypass was established. With minimum pump flow [0.3L/min/m2] the PDA was closed with two Teflon-felted 4-0 Prolene interrupted sutures. The ruptured main pulmonary artery was also closed using thickened pericardium with three Teflon-felted 4-0 Prolene interrupted sutures. The operation was successful and postoperative course was uneventful. She was discharged on the 16th POD. We report this case as a very rare secondary complication of bacterial endocarditis complicated by patent ductus arteriosus.
