Longitudinal Relationships between Cigarette Smoking and Increases Risk for Incident Metabolic Syndrome: 16-year Follow-up of the Korean Genome and Epidemiology Study (KOGES)

  • This study aimed to determine whether smoking affects the metabolic syndrome and its components through long-term follow-up. Of the 10,030 cohort subjects in the community-based Korean Genome and Epidemiology Study (KoGES) from 2001 to 2018, 2,848 people with metabolic syndrome and 4,854 people with insufficient data for analysis were excluded for this study. The study population comprised 2,328 individuals (1,123 men, 1,205 women) who were eligible for inclusion. The mean age of the participants was 49.2±7.5 years, and 21.9% were current smoker. In log rank test, current smoker had a significantly higher cumulative incidence of metabolic syndrome compared with non smoker (P<0.001). In the Cox proportional hazards model adjusted for key variables, metabolic syndrome (hazard ratio [HR] 1.57, P<0.001), high fasting glucose (HR 1.40, P<0.01), hypertriglyceridemia (HR 1.60, P<0.001), low HDL-cholesterol (HR, 1.30, P<0.01), and abdominal obesity (HR 1.32, P<0.01) in current smoker compared with non smoker were statistically significant, respectively, but not hypertension (HR 1.00, P>0.05). After adjustment for confounders, the time (P-time<0.001) and group (P-group<0.001) effects on metabolic syndrome score change were statistically significant. Furthermore, the interaction analysis of time and smoking group on the change in metabolic syndrome score was statistically significant (P-interaction<0.001). In long-term follow-up, smoking worsens metabolic syndrome.

대학생을 위한 대사증후군 예방교육프로그램 개발 및 효과-어플리케이션(Application) 매체 적용 (Development and Effect of a Metabolic Syndrome Prevention Program for University Students using Mobile Application)

  • 목적 본 연구는 대학생을 위한 대사증후군 예방교육 프로그램을 개발하고 그 효과를 평가하기 위하여 시행되었다. 방법 비동등성대조군 사전사후설계인 유사 실험연구와 질적 자료분석인 내용분석방법을 접목한 연구 방법간 트라이앵귤레이션을 적용하였다. 서울시 노원구 소재 S대학교 재학생 49명을 대상으로 하였으며, 자료 분석방법은 SPSS WIN 18.0 프로그램을 사용하여 서술통계 $x^2$-test, t-test, Fisher's exact test를 구하였다. 결과 본 대사증후군 예방교육 프로그램의 주 메뉴는 기본교육, 영양교육, 운동교육, 식사일기, 운동체크, 팁의 총 6개로 구성되어있고, 실험군은 대조군에 비해 대사증후군 예방에 대한 인식정도가 유의하게 높았으며(F=7.919, p=.007), 대상자들은 비만, 고혈압, 당뇨 등이 대사증후군과 관련이 있으며 예방을 위한 실천방안으로 식습관 개선과 운동의 중요성에 대한 인식을 가지고 있었다. 결론 스마트폰에 친숙한 대학생에게 어플리케이션을 교육매체로 한 대사증후군 예방교육 프로그램은 매우 유의한 효과가 있음이 확인되어 그 개발의 필요성과 효용성이 입증되었다.

Intra-Rater Test-Retest Reliability of Ely's Test Using Smartphone in Patients With Lumbar Extension-Rotation Syndrome

  • Ely's test is commonly used to assess rectus femoris muscle flexibility. however, a reliability limit of this test was demonstrated by a previous study. In this study, we present an alternative method using an application for a digital horizontal level in a smartphone (DHLS) to complement the reliability limit of Ely's test. The aim of this study was to examine the reliability of Ely's test using DHLS on the pelvis, compared to using visual observation (VO) of pelvic and hip motions. Nineteen patients with lumbar extension-rotation syndrome were recruited for this study. An examiner examined the rectus femoris flexibility (both pass/fail and goniometer scoring) through Ely's test using both DHLS and VO. A retest session was completed two hours later for within-day reliability and seven days later for between-day intra-rater test-retest reliability. Results showed higher Kappa values for pass/fail scoring and higher intraclass correlation coefficient values for goniometer scoring in Ely's test using DHLS, compared to using VO. Measurement error and Bland and Altman plots further demonstrated the degree of intra-rater variance during Ely's test using DHLS in a clinical setting, compared to using VO. These results demonstrated that Ely's test using DHLS showed acceptable reliability compared to using VO. Ely's test using DHLS could be widely used for measuring the rectus femoris muscle flexibility in patients with lumbar extension-rotation syndrome, although the inter-rater reliability needs to be established first.

대사증후군 노인의 운동지속 영향요인 (Factors affecting on maintenance of exercise among elderly with metabolic syndrome)

  • 본 연구의 목적은 대사증후군 노인의 운동유지 여부에 따라 운동유지에 대한 영향요인을 확인하기 위해 시행된 횡단적 조사연구이다. 본 연구의 대상자는 총 136명이며, 자료의 수집은 대사증후군 노인의 운동유지 영향요인을 알아보기 위해 운동유지, 운동기대감, 운동 관련 환경, 운동 사회적지지, 운동자기효능감에 대한 도구로 수집되었다. 수집된 자료는 SPSS Win 20.0 program을 사용하여 requency, X2, t-test, Logistic regression으로 분석하였다. 연구 결과 연령, 운동기대감, 운동자기효능감이 이들의 운동유지 영향요인임이 밝혀졌다. 이러한 결과는 대사증후군 노인의 운동유지를 위해서는 내적 동기에 해당하는 자기효능감을 함양시키고 운동기대감을 증진시키는 것이 중요하다고 할 수 있다. 또한 대사증후군 노인을 위한 건강증진프로그램을 계획할 때 운동에 대한 결과를 알려주고 이에 대한 이점을 부각시켜 스스로 운동을 잘 해낼 수 있도록 계획해야 하는 데에 본 연구결과가 기초자료로 활용할 수 있음이 기대된다.

Frey씨 증후군의 진단에 있어서 Thermography의 유용성 (The Effectiveness of Thermography in Diagnosis of Frey's Syndrome Following Parotidectomy)

  • Objectives and Backgrounds : After parotidectomy, some of patients complain gustatory sweating, facial flushing and discomfort in the same area. A series of these symptoms are supposed to be caused by the aberrant regeneration of the secretory parasympathetic fibers to sweat glands and blood vessels of the skin following parotidectomy. In this study, we want to compare the efficacy of thermography to the Minor's starch-iodine test for determining the presence of Frey's syndrome. Materials and Methods : 48 patients who underwent total or superficial parotidectomy from March 2002 to December 2004 were selected for this study. A subjective clinical questionnaire and the objective Minor's starchiodine test were performed to evaluate the incidence of this syndrome. Total 21 patients were confirmed as positive Frey's syndrome and infrared thermography was performed for them. Result : Frey's syndrome occurred in 21 patients(43.8%). The average temperature of parotidectomy site and normal opposite area were $27.65^{\circ}C\;and\;26.41^{\circ}C$ respectively. Thermography showed temperature difference in 20 patient(95.2%) and the difference of temperature was statistically significant above $1.0^{\circ}C$(p<0.001). The severity of symptoms were related with the difference of temperature(p<0.05). Conclusion : Thermography is useful, non-invasive, simple and quantifying method to diagnose Frey's syndrome. Additionally, this geographic diagnosis is available to show the accurate area for botulinum toxin injection.

발 반사요법이 여대생의 월경 불편감에 미치는 효과 (The Effect of Foot Reflexology on Premenstrual Syndrome and Dysmenorrhea in Female College Students)

  • The purpose of this study was to identify the effect of foot reflexology on premenstrual syndrome and dysmenorrhea in female college students. The research design of this study was a quasi-experimental design. Of the forty female college students, twenty were assigned to the experimental group and, twenty to the control group. The data were obtained over 2 months(November 26, 2001 to January 31, 2002) from a nursing of C college located in S city. The instrument used to assess premenstrual syndrome and dysmenorrhea was Keele's VAS(Visual Analogue Scale) and opening records. Subjects in the experimental group received foot reflexology for 6 times with 1 hours during 60 days, and subjects assigned to the control group did not receive foot reflexology. Data were analysed with percentage, mean, standard deviation, $x^2$-test, unpaired t-test, and repeated measure ANOVA, using SAS Program. The results of the study are as follow, 1.The symptoms which the group of experimental and the group of control discomforts the most are sensitiveness(35%), abdominal pain (30%),lower abdominal pain (30%) and lumbago (20%). The method of relieve premenstrual syndrome and dysmenorrhea by which the subjects employ the most to solve their premenstrual syndrome and dysmenorrhea is the getting along by enduring(67.5%) and bed rest (32.5%). 2.The mean score of the premenstrual syndromes and dysmenorrhea before foot reflexology was 8.35, it was 4.16 at the first menstruation after foot reflexology and 3.25 at the second menstruation for the experimental group. 3.The relieved symptoms after foot reflexology was fatigue(50%), insomnia(40%), abdominal pain(35%), lower abdominal pain (30%) and constipation(30%). Foot reflexology was effective in improve the symptoms of the female college students who have the premenstrual syndrome and dysmenorrhea.

코로나19 팬데믹 전후 청소년 대사증후군 유병률과 위험요인의 변화: 2018년과 2020년도 국민건강영양조사를 이용한 2차 자료분석 (Changes in the Prevalence of Metabolic Syndrome and its Risk factors in Korean Adolescents before and during the COVID-19 Pandemic: Secondary Data Analysis of the 2018 and 2020 Korea National Health and Nutrition Examination Surveys)

  • Purpose: The purpose of the study was to investigate the changes in the prevalence of metabolic syndrome and its risk factors in Korean adolescents before and during the COVID-19 pandemic. Methods: The study employed a descriptive cross-sectional design. Data were obtained from the 2018 and 2020 Korea National Health and Nutrition Examination Surveys. Out of a total of 15,351 participants in the surveys, 971 adolescents aged 12 to 18 years were included. The data were analyzed using the rao-scott 𝝌2 test and t-test. Results: There was no significant difference in the prevalence of adolescent metabolic syndrome before and after the outbreak of COVID-19. As for general characteristics, changes in high blood pressure were confirmed from waist circumference, diastolic blood pressure, and metabolic syndrome risk factors. When analyzing the general characteristics according to gender, there were significant differences in waist circumference and diastolic blood pressure in boys. Concerning the risk factors for metabolic syndrome, there was a significant difference in high blood pressure in both boys and girls and in abdominal obesity in girls. Conclusion: Hypertension and abdominal obesity, which are risk factors for metabolic syndrome, should be managed by including them in the school health area. Communities and schools should take an active role in preparing the youth for healthy adulthood.

Predictive Value of Sensory Nerve Conduction in Carpal Tunnel Syndrome

  • Objective : Electrodiagnostic test has shown diagnostic sensitivity and specificity in carpal tunnel syndrome[CTS]. This study was to evaluate the correlation between clinical outcome of endoscopic carpal tunnel ligament release[ECTR] and the predictive value of sensory nerve conduction. Methods : From January 1998 to December 2004, 87 patients [44 right hand, 37 left hand, 6 bilateral hands] with CTS who underwent ECTR were followed up in our hospital for an average of 24 months. We retrospectively analyzed the results with previous medical records. All patients underwent electrodiagnostic test and ECTR. The patients were divided into three groups according to the electrodiagnostic test results. Group [A] was normal sensory nerve response, Group [B] was slowing sensory response and Group [C] was no sensory response. Improvement of the symptom after ECTR was assessed using a visual analogue scale[VAS] score. Results : Differences between the three groups on the correlation of severity of sensory potential and duration of preoperative symptoms were significant. The mean value of improved VAS scores for the three groups were $6.0{\pm}0.96$ in the Group A, $6.11{\pm}0.48$ in the Group B and $6.14{\pm}0.53$ in the Group C. There was no statistically significant difference between the severity of sensory nerve response and improvement in VAS score after ECTR. Complications included a wound infection, a case of skin necrosis, and two patients with persistent symptoms without any improvement. Conclusion : Although electrodiagnostic test has been known to be useful, sensory nerve response is considered not to be a good prognostic value for carpal tunnel syndrome after ECTR.

만성 요통환자와 정상인의 균형반응 비교 (Comparison of the Balance Relations Between Healthy Subjects and Patients With Chronic Low Back Pain)

  • The purpose of this study was to identify the differences of the static and the dynamic balance reactions in the flexion syndrome (FS) and the extension syndrome (ES) group of the patients with chronic lowback pain (LBP) and healthy subjects. Twenty subjects were included in each group. By using EquiTest 5.02, the static balance was measured by the equilibrium score and the strategy score of sensory organization test (SOT), while the dynamic balance was measured by the latency of motor control test (MCT) and the sway energy of adaptation test (ADT). Oswestry Disability Index (OSI) was used to measure level of the disability in patients with chronic LBP. The equilibrium scores, the strategies of SOT, and thelatencies of MCT of three groups were compared with one way ANOVA, while the sway energy of ADT was compared with repeated measures one way ANOVA. The results of this study showed that the equilibrium scores and the strategy scores of SOT were significantly lower in patients with chronic LBP than in healthy subjects. The equilibrium scores and the strategy scores of SOT were significantly differences between the FS and ES groups in condition 5 (support surface was sway-referenced and visual information waseliminated by eye closure), and 6 (support surface was sway-referenced and visual information was altered by sway-referencing). The FS group showed delayed average reaction time at large posterior translation, however, the ES group showed delayed average reaction time at large anterior translation, Even though the sway energy of the patients with chronic LBP were greater than that of healthy subjects during the toe down (plantar flexion rotation), the values between the FS and ES groups didn't show any significant difference. The disability level showed highly correlation with the equilibrium score of the condition 5. As the results, the FS and ES groups divided by the their symptoms and signs in patients with chronic LBP showed different balance reaction. Therefore, more accurate evaluation and balance treatments are needed to focus on their symptoms and signs in patients with chronic LBP.

The Change in Exercise Capacity, Cardiac Structure and Function in Pre-Metabolic Syndrome Adults

  • This study divided a group of healthy adults aged 20 or older who had a health examination at J General Hospital in Gyeonggi Province into three groups according to the degrees of metabolic syndrome risk factors. They include the normal group (n=58), the pre-metabolic syndrome group (n=112) and the metabolic syndrome group (n=32). They were compared in exercise capacity and cardiac structure and function and impacts of exercise capacity on the cardiac diastolic function. All the groups took echocardiography to have their cardiac structures and functions examined and an exercise stress test to have their exercise capacity measured. The research findings were as follows: There were differences in exercise capacity, cardiac structure, and diastolic heart function among three groups. Between exercise capacity and diastolic heart function was found to be related. It turned out exercise capacity affected the cardiac diastolic functions. In conclusion, there were significant differences in exercise capacity between the normal group and the metabolic syndrome group and in the cardiac structure and function among the normal, metabolic syndrome, and pre-metabolic syndrome group. In addition, METs (metabolic equivalents) and heart rate recovery of exercise capacity turned out to affect cardiac diastolic functions.
