Neuromodulation of the median nerve in carpal tunnel syndrome, a single-blind, randomized controlled study

  • Background: This study aimed to evaluate the efficacy of pulsed radiofrequency applied using transcutaneous electrodes in carpal tunnel syndrome (CTS). Methods: After randomization, the patients received two cycles of noninvasive pulsed radiofrequency (NiPRF), once weekly, or splinting (the control group) for three months. Clinical evaluations were recorded at baseline and weeks 4 and 8. The Boston Carpal Tunnel Syndrome Questionnaire (BCTQ) was used to determine the functional status and symptom severity. Results: Sixty-two patients were followed up for three months. There was no difference between the groups in the BCTQ scores before and after treatment. The NiPRF group found a significant difference between the BCTQ measurements at all time intervals (paired sample t-test; P < 0.001). In the splint group, there was a significant difference only between the basal-1st month and basal-3rd month (paired samples t-test; P < 0.001). The main effect of the time variable was statistically significant (ANOVA; P < 0.001), but the group variable was not. There was no correlation between the BCTQ results measured at any time and the electroneuromyelogragphy findings in either group. Conclusions: NiPRF effectively improves symptoms and functionality in patients with CTS for up to 3 months. Thus, NiPRF can be considered an easy, safe, and useful alternative treatment modality for CTS.

Effectiveness of Botulinum Toxin A in Treatment of Frey's Syndrome (Frey 증후군의 치료에 있어서 보툴리눔 독소 A형의 유용성)

  • Purpose: Several therapeutic approaches have been introduced and tried to treat Frey syndrome following parotidectomy. However they were not proved as an effective treatment. A new therapeutic modality using botulinum toxin injections was presented previously by several study groups. But, the duration of the demonstrated positive effect was essentially unknown so far. The purpose of this clinical investigation is to demonstrate the effectiveness of Botulinum toxin type A(BTXA) in patients with Frey syndrome. Methods: For this study, 12 patients were treated, They were assessed with the Minor's iodine-starch test and interviewed before and after treatment. Botulinum toxin is injected with $1.0cm^2$ apart into the skin where symptoms of Frey's syndrome has manifestated. The patients were classified according to the concentration of botulinum toxin and dosage of botulinum toxin. Results: The outcome measures were the time of reappearance of gustatory sweating(subjective study), and the results of an Minor's iodine-starch test(objective study) of 3 weeks, 3 months, 6 months, 9 months, 12 months after treatment. This treatment was effective in all groups of patients. The concentrations and the amount of dosages did not affect the treatment. But high concentration produced faster effectiveness in subjective. Conclusion: Botulinum toxin type A(BTXA) for Frey syndrome is easy, convenient and effective withouts severe complication. And the patients has no difficulty and limitation in their life.

Follow Up Study of Carpal Tunnel Syndrome Patients Underwent Partial Release of Transverse Carpal Ligament Using Inching Test (인칭검사를 이용하여 부분 절개를 시행한 수근관 증후군 환자의 추적관찰)

  • Purpose: Complete release of the transverse carpal ligament (TCL) is accepted as the standard treatment for carpal tunnel syndrome (CTS). However, loss of grip and pinch power are reported in some patients after complete release of the TCL. This study was designed to evaluate the effectiveness of complete versus partial carpal tunnel release by using the inching technique. Methods: Nineteen patients (a total of 27 hands) who each had a confirmed diagnosis of CTS were selected from September 2002 to February 2003. The cases were divided into three groups(mild, moderate and severe) based on preoperative electrodiagnostic studies. The patients with partial carpal tunnel syndrome were classified into the mild or moderate groups, while patients with complete carpal tunnel syndrome were classified into the moderate or severe groups. Patient oriented data (functional and symptomatic) were collected and electrophysiologic studies were undertaken preoperatively and postoperatively(on the 2nd week, 1st month, 3rd month and 6th month after surgery).Results: In this study, the mild and moderate groups showed both good functional and symptomatic results and improvements in electrophysiologic studies.Conclusion: Carpal tunnel syndrome patients classified into mild or moderate groups based on nerve conduction studies, and whose precise compression sites were pinpointed using the inching technique, can be treated by partial carpal tunnel release.

The Effects of Joint Mobolization on Neck Myofacial Pain Syndrome (경부근막동통증후군 환자의 가동범위에 대한 연구)

  • Purpose : To identify the effects of Hold-Relax of proprioceptive neuromuscular facilitation (PNF) on the functional improvement of patients with neck myofascial pain syndrome. Methode : The present research investigated 60 patients with neck myofascial syndrome, dividing them into a group doing Hold-Relax of proprioceptive neuromuscular facilitation and a group doing Stretching, This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared two groups to find difference in the degree of recovery from myofascial pain syndrome. Results : 1. For Hold-Relax of proprioceptive neuromuscular facilitation group, the pain rating score (PRS) were significantly decreased for six weeks treatment. 2. For Stretching group, the pain rating score (PRS) did not indicate significant decrease for six weeks treatment. 3. For Hold-Relax of proprioceptive neuromuscular facilitation group, range of motion of flexion, left rotation, right rotation indicated significant difference after pre test and after two week but no significant difference after four week. 4. For Stretching group, range of motion of flexion, left rotation, and right rotation except extension indicated significant difference after pre test and after two week but no significant difference after four week. Conclusion : The Hold-Relax of proprioceptive neuromuscular facilitation is considered to be effective in increasing joint range of motion of patient and decreasing pain. Accordingly physical therapists has to learn a wide range of patterns and to develop various treatment techniques.

Effectiveness of lifestyle intervention on the management of metabolic syndrome (생활습관 중재프로그램이 성인의 건강생활습관실천과 대사증후군 개선에 미치는 효과 분석)

  • Objectives: The purpose of the this study was to evaluate lifestyle intervention program for the treatment of the metabolic syndrome. Methods: Subjects of this study were 700 adults with metabolic syndrome who took health examinations in health promotion centers of Korea Association of Health Promotion between May 1 and June 30, 2006. Subjects were randomly assigned to an experimental group and a comparison group. Participants in the experimental group received intensive 3-month lifestyle modification intervention and participants in the comparison group received minimal information on lifestyle modification. Pre test and Post test were carried out to evaluate the effectiveness of the intervention program. Results: After the intensive intervention on lifestyle modification(healthy diet, physical activity, moderate drinking, stress management, and smoking cessation), the levels of blood pressure, waist circumference, fasting blood glucose and triglycerides were significantly reduced for those in experimental group(P<0.001). For those in comparison group, the levels of blood pressure(P<0.001), waist circumference(P<0.001), and triglycerides(P<0.01) were significantly reduced after the intervention. No change in the levels of high-density lipoprotein cholesterol were observed in both groups. After 3-month intervention, the prevalence of metabolic syndrome was reduced to 35.7% in experimental group and 48.5% in comparison group. Conclusion: This study has demonstrated the efficacy of therapeutic lifestyle intervention for the management of metabolic syndrome.

Comparison the Prevalence of Metabolic Syndrome and Obesity Index between Patients with Silent Lacunar Infarction and Normal Controls in a University Hospital (일개 대학병원에 내원한 무증상성 열공성 뇌경색 환자와 정상인에서 비만 지수와 대사증후군 유병률 비교)

  • This clinical study was done to examine comparison the prevalence of metabolic syndrome and obesity index between patients with silent lacunar infarction and normal controls. We compared the components of metabolic syndrome and obesity index between patients silent lacunar infarction(n=62) and normal controls(n=72). The analysis of the data was done by Frequency Analysis, two sample t-test, Chi-square Test. Hypertension and diabetes mellitus was significantly higher in patients of silent lacunar infarctions. Body mass index (BMI), waist-hip ratio (WHR), body fat percentage, obesity ratio, metabolic syndrome were higher significantly in patients of silent lacunar infarctions than normal controls. Although statistically insignificant, waist circumference showed relatively higher in patients of silent lacunar infarction. This result emphasizes the importance of managing that hypertension, diabetes, body mass index (BMI), waist-hip ratio (WHR), body fat percentage, obesity ratio, metabolic syndrome in silent lacunar infarction.

Effects of Pulsed Electromagnetic Therapy with Acupuncture Therapy for Sacroiliac Joint Syndrome (천장관절 증후군에 박동성 전기자장 치료와 병행한 침치료의 효과 연구)

  • Objectives : Although the controversy surrounding the biomechanics of the sacroiliac joint remains unresolved at this time, the clinical importance of this joint in the cause of back pain has been established since 1930's. Recently, there has been renewed interest in the sacroiliac joint. This study was performed to evaluate the effects of pulsed electromagnetic therapy(PEMT) with acupuncture therapy for patients, who were suffering from sacroiliac joint syndrome, and to conduct more researches in the usage of acupuncture therapy for treating sacroiliac joint syndrome. Methods : 25 patients, who were diagnosed as sacroiliac joint syndrome were selected. They were treated twice a week during 3 weeks. They were measured after all the treatment and firs week and fourth week after termination of treatment by using visual analogue scale(VAS) and Roland Morris disability index(RMDI). The VAS and RMDI patterns were analyzed by using 'pared T-test' and 'Kruskal-Wallis' test. Results : 1. Each times of PEMT with acupuncture therapy, there were statistical significance in improvement of VAS(p<0.05) and each times of therapy except 1st one, there were statistical significance in improvement of RMDI(p<0.05). 2. After 4th therapy, there were most significant improvement of VAS with RMDI(p<0.001), when we compared the change in VAS and RMDI before and after the each therapy. 3. There was no statistical significance in VAS and RMDI by onset, sex and age. Conclusions : The results indicate that pulsed electromagnetic therapy and acupuncture therapy had good effect on sacroiliac joint syndrome.

The Educational Needs of Mothers of Nephrotic Syndrome Patients and the Degree of the Importance of Educational Activity Perceived by Nurses (신증후군 환아 어머니의 교육요구와 간호사가 인지한 교육활동의 중요도에 관한 탐색적 연구)

  • This is descripitive study conducted to identify educational needs of mothers of nephrotic syndrome patients and the degree of the importance of educational activity perceived by nurses who look after nephrotic syndrome patients. The study subjects were composed of 60 mothers & 63 nurses of nephrotic syndrome patients whose children were hospitalized in 2 Pediatric wards of University Hospital in Seoul and 1 in Pusan from Mar. in 1997 to July 1997. A questionaire for this study was item Kikert type 5 point scale, developed on the basis of previous literature and researcher's clinical experience and the reliability of the used instruments was α=.97. The data analysis was done by SAS. t-test, and ANOVA were done to determine the effect of general characteristics of subjects on their educational needs. T-test was done to measure relations between the degree of educational needs and importance of educational activity. The results were as follows. 1. The Educational needs of mothers ranked as the highest in home care. 2. In character of mothers, the degree of education, economic state, the number of child and occupation was significant in statistically. 3. The importance of educational activity of nurses ranked the highest in diagnosis and treatment of the disease. 4. The educational needs of nephrotic syn. children' mother and importance of nurses were different in diagnosis and treatment of the disease and home care. On the basis of these results, we suggest as follows. 1. It is proposed that nurses use these results of the study actively for the educational program for Nephrotic Syndrome patients and their mothers. 2. It is suggested to make a comparative study of the degree of nurses' understanding of the importance on educational items with the instruments of this study.

  • The purpose of this study was to confirm the relationship between the subjective symptoms of smartphone addiction and visual display terminal syndrome in nursing college students and learning flow in the COVID-19 pandemic. For the collection of materials, the materials of the final 134 people were analyzed by collecting the students at the nursing colleges located in S city and M city for convenience. The collected materials were subjected to descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficients using the SPSS / WIN 26 program. As a result of this study, the learning flow of nursing college students was negative correlated to the tendency of smartphone addiction and the subjective symptoms of visual display terminal syndrome. Therefore, to improve the learning flow of nursing college students, it is necessary to reduce the symptoms of smartphone addictive use and visual display terminal syndrome. The need for intervention and development of various effective programs for smart phone addiction management and display terminal syndrome management was suggested.

Investigation of Factors Related to Physical Health in the Old People : Focusing on sarcopenia and locomotive syndrome (노인의 신체건강 관련 요인 조사 : 근감소증과 운동기능저하증후군을 중심으로)

  • Purpose : This study was conducted to investigate and analyze the physical health of older Koreans with sarcopenia and locomotive syndrome and identify the related factors. Methods : In this study, the sarcopenia and locomotive syndrome evaluations were applied to 210 elderly people, and the sarcopenia group was 36, the locomotive syndrome group was 164, and the normal group was 10. After group selection, a physical health status survey was conducted. The physical health status was assessed via body composition analysis, physical characteristics survey including measurement of waist and calf circumference, investigation of diseases currently being diagnosed by a doctor, and frailty measurements. The collected data were statistically analyzed using one-way ANOVA, the Kruskal-Wallis test, and the Chi-square test. Results : There were significant differences between groups in all elements of physical characteristics including body mass index, waist circumference, and calf circumference. Among them, a consistent result was found that the normal group had the largest amount of muscle mass and the sarcopenia group had the least amount of muscle mass in the factors related to muscle mass. However, the factors relating to fat mass and obesity also showed significant differences between the groups, but the results were not consistent. Considering the group differences in current diseases, a significant difference was only detected for osteoporosis among 12 diseases. Moreover, those in the sarcopenia group had the highest rate of osteoporosis. And there was no significant difference between the groups in the total score of the frailty measurement, but there was a significant difference between the groups in the frailty measurement levels. Conclusion : This study on physical health status confirmed that muscle mass-related factors, osteoporosis, and frailty levels were significantly related to sarcopenia and locomotive syndrome.
