A Case-Report of a Taeeumin Patient with Diabetes Mellitus Treated by Mangeummunmu-tang gagambang (당뇨병으로 진단받은 태음인 만금문무탕가감방(萬金文武湯加減方) 치험 1례)

  • We report 1 case about Mangeummunmu-tang gagambang that was used for a Taeeumin patient with diabetes mellitus. We treated a Taeeumin patient who was diagnosed with diabetes at another hospital two months ago. We considered his Sasang constitution as Taeeumin and prescribed Mangeummunmu-tang gagambang. The improvement of his diabetes was evaluated by improvement of his symptoms, results of blood sugar test, bone muscle mass, body fat, body fat rate of inbody test and the results of Blood test & urine test. The evaluation index of the patient's disease has all improved since treatment after Mangeummunmu-tang gagambang medication.

A Case Report of the Agitation(Byun-jo) Caused by Cerebral Infarction Treated with Hwangryunhaedok-tang Herbal-Acupuncture Therapy (뇌경색 후 발생한 번조증에 대한 황련해독탕 약침의 병행 치험 1례)

  • Object : The purpose of this study is to evaluate the effect of Hwangryunhaedok-tang herbal-acupuncture therapy for Agitation(Byun-jo) caused by cerebral infarction. Methods : One patient was admitted who was suffering from insomnia, chest discomfort, anxiety, palpitation of Agitation(Byun-jo), after cerebral infarction. In the point of Differentiation of Syndromes(辨證), we diagnosed this patient as Excessive heat-fire syndrome of stroke(中風火熱證) and treated with Hwangryunhaedok-tang herbal-acupuncture therapy at CV17, BL15, GB20. Results : After Hwangryunhaedok-tang herbal-acupuncture therapy, symtoms of Agitation(Byun-jo) were improved remarkably, and the score of State-Trait Anxiety Inventory(STAI), Pittsburgh Sleep Quality Index(PSQI) were decreased. Conclusions : Hwangryunhaedok-tang herbal-acupuncture therapy has meaningful effect on the symptoms of Agitation(Byun-jo) caused by cerebral infarction and more researches should be followed.

A Case Report of an Elderly Patient with Psoriasis Treated with Jeongri-tang Gagam-bang: Focus on Digestive System Treatment

  • Objectives: This study aimed to report the efficacy of Korean medicine with Jeongri-tang Gagam-bang for treating psoriasis. Methods: A patient with psoriasis was treated with Jeongri-tang Gagam-bang (正理湯加減方). Moreover, acupuncture was performed on CV12 (中脘) and both sides of ST25 (天樞), GV20 (百會), LI4 (合谷), and LR3 (太衝) for 15 minutes. Psoriasis severity was assessed using the Psoriasis Area and Severity Index (PASI). Pruritis and dyspepsia intensities were ranked on a Visual Analog Scale (VAS) of 0–10. Results: After treatment, the PASI changed from 21.6 to 1.6. Patient's pruritus had almost disappeared and dyspepsia had improved. Conclusions: This study shows that Jeongri-tang Gagam-bang (正理湯加減方) and acupuncture on CV12 (中脘), ST25 (天樞), GV20 (百會), LI4 (合谷), and LR3 (太衝) might be helpful to treat psoriasis symptoms by improving digestive function.

Nutritional Status of Chronic Obstructive Pulmonary Disease Patients according to the Severity of Disease (만성 폐쇄성 폐질환 환자에서 병기에 따른 영양상태 평가)

  • The purpose of the study was to investigate nutritional status of chronic obstructive pulmonary disease (COPD) patients and to find out the differences according to the stages of disease. From March to October, 2006, 41 stable male patients of mild to severe COPD patients were recruited from Seoul National University hospital. The patients' of body weight and fat free mass were assessed by bioelectrical impedance analysis. The nutritional status of the patients was also assessed by 3-day recall, index of nutritional quality (INQ), dietary diversity score (DDS), dietary variety score (DVS), food group index pattern and dietary quality index (DQI). The total of 41 patients were classified into three groups, stage I, stage II and stage III groups according to the classification of Global Initiative for Chronic Obstructive Lung Disease (GOLD) standard. The mean age of the patients in each stage were 67.2-66.9 years showing no significant difference. The ratio of $FEV_1$/FVC were $57.5{\pm}7.3$, $46.9{\pm}7.6$ and $38.2{\pm}6.8%$, respectively showing significant differences according to the stages of disease. The fat free mass of the stage II ($48.2{\pm}4.7kg$) and III ($47.3{\pm}4.5kg$) was significantly lower than that of stage I ($53.1{\pm}6.9kg$) patients. There were significant correlation of fat free mass with $FEV_{1}$, and BMI (body mass index) with $FEV_{1}$/FVC ratio (p < 0.05). COPD patients showed the diet-related clinical symptoms of anorexia, dyspnea, dyspepsia, and chewing difficulty. Daily intakes of calorie, K, vitamin $B_2$ and folate of the patients were very low ($83.8{\pm}20.7%$, $58.9{\pm}14.4%$, $70.7{\pm}19.6%$ and $74.4{\pm}10.2%$, respectively) however, they did not significantly different according to the stages of disease. Daily intake of calcium was significantly lower in the stage III patients (p < 0.05). The mean scores of dietary variety score was significantly lower in the stage III patients (p < 0.001). Dietary quality index of the patients were not different among the stages of disease and the scores indicated poor quality of diet. As a summary, we found that body fat free mass, regularity of exercise, frequency of having snacks and dietary variety score were significantly associated with the severity of chronic obstructive pulmonary disease.

  • Objectives : This study aimed to investigate the risk factors of depression for patients with tuberculosis(TB). Methods : A total of 57 patients with TB were recruited. All participants completed the Becks Depression Inventory-II for evaluating depressive symptoms. The risk factor for depression was analyzed by binary logistic regression analysis. Nomogram was performed for probability of depression. Results : Low body mass index(BMI, OR 0.801, 95% CI 0.65, 0.98), interruption of treatment for TB(OR 5.908, 95% CI 1.19, 29.41), past history of depression(OR 24.653, 95% CI 1.99, 308.44) were associated with increased risk for depression. The calibration curve for predicting probability of survival showed a good agreement between the nomogram and actual observation(Original C-index=0.789, bias corrected C-index=0.754). Conclusions : The result of the present study indicate that low BMI, interruption of treatment for TB, and past history of depression were risk factors for depression in patients with TB. The psychiatric intervention may be needed to prevent depression if the patients with TB have risk factor during treatment for TB.

Analysis according to Age The first stage of thirties, forties, seventies is heavier than forties, fifties in improvement and Index of improvement of symptom 2. Analysis according to Family History We have the better result in first stage and improvement, index improvement when no family history. 4. Analysis according to Past History We have no special relation in past history like hypertension, DM, heart problem 5. Analysis devided two group, above group and under group on the basis of the average in first stage of all patient. We have the better result when the first stage is light, that the first score of barthel index and CNS is high. 6. Analysis of the effect of treatment about Korean medical treatment, Western medical treatment, cooperative treatment. In this study, the highest group of rate of treatment at four contrast groups (Korean medicine, Constitution medicine, Western medicine, cooperative treatment according to dyagnosis and range of treatment was the patient group of doing dyagnosis and method of treatment based on constitution medicine theory. This is that of doing demostation, A-Tx, po-herb-medication according to dyagnosis and treat method of constitution of Lee Je-ma In case of left, the case of dyagnosis any disease according to doctor view but, normal in TCDwas 22-beginning of attack, 20- two weeks later, 11 case-four weeks later in case of right, 15-beginning of attack, 12-two weeks later, 9 case four weeks later. So left vessel compares to right vessel is more interference, in fact more than a 1/2 of the patients of MCA disease can't do dyagnosis. In rate of imparement, the state of pacient improved but there isn't the improved case of result in TCD. 7. In TCD dyagnosis, between the case of inconsus the doctor view specially MCA in brain blood vessel is in large numbers and in total 86's patient, impossible case of dyagnosis according to interferiance of temporal is 21 case. 7. Result study about application of Kreaan medical treatment 1) The impossible patient of observation MCA blood vescular for interference temporal bone happened in large numbers. 2) There is the case having difference result to CT,MRI, MRA result. 3) Because individual difference is large, excluding to ananalogy of symptom. This is normal numerical value that has possibility of being checked as abnormal numerical value 4) there are a lot of cases that the speed of normal part is as similarly measured as that of abnormal part. It means that we cannot judge the disease by this measure 5) It is rare that this measure represent degree of improvement in patient's condition of disease. When we observe patient's condition become better, but we have no case that the result of TCD test better. 6) The result could be appear differently by the technique of the tester or by the experience of the tester 7) In the TCD test, abnormal symptoms is checked at 0 week, but at 2th week, normal symptoms is checked, again at 4th week abnormal is checked. According to the above result, CVA diagnosis is difficult only with TCD, as it appear in diagnosis error check which is suggested in the problem connected to project, for the aged persons who have the worst hardening of part of the cranium (1998. 5. 26 77 of 83 patients is 50s) there is a lot of cases that the measurement is impossible by TCD and the correction of measurement numerical value is decreased, as the age of cerebral infarction is high, TCD is inappropriate to diagnosis equipment through this study.

Correlations between Neurologic and Psychiatric Symptoms in Acute Stroke Patients (급성기 뇌졸중 환자에서 신경 및 정신 증상간 상호관계)

  • Objectives : This study aimed to investigate the correlations between neurological and psychiatric symptoms at two weeks after stroke. Methods : For 412 stroke patients, stroke severity was evaluated by the National Institutes of Health Stroke Scale(NIHSS), disability by the Barthel Index(BI) and modified Rankin Scale(mRS), cognitive function by the Korean Mini-Mental State Examination(K-MMSE), and muscle power by grip strength. Psychiatric symptoms were assessed by Symptom check list-90-Revision(SCL-90-R), consisted of nine symptom domains : Somatization, Obsessive-compulsive, Interpersonal sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid ideation, Psychoticism, and Additional items. The correlations between the neurological and psychiatric symptoms were investigated at the time of admission and before discharge(i.e. before and after treatment). Results : At the time of admission, NIHSS score was associated with scores on Phobic anxiety and Additional items ; and scores on BI and mRS were associated with Depression, Phobic anxiety and Additional items. At the time of discharge, NIHSS score was associated with scores on Somatization, Depression, Phobic anxiety, and Additional items ; scores on BI and mRS were associated with scores on Depression, Phobic anxiety and Additional items ; MMSE score was associated with Obsessive-compulsive, Depression, Phobic anxiety, and Additional items ; and grip strength was associated with Somatization, Depression, Anxiety and Additional items. Conclusions : More severe neurological symptoms were associated with higher psychiatric morbidity particularly in depression, phobic anxiety, sleep and appetite disturbance at acute stage of stroke. More intensive psychiatric care and intervention are needed for the high risk group.

A Clinical Study on the Effect of Korean Medical Treatments for Patients with Lumbar Disc Herniation (요추 추간판탈출증 환자에 대한 한약, 봉독약침, 침, 추나요법의 유효성 평가)

  • Objectives : To investigate therapeutic outcomes of back pain modalities in patients with disc hemiation; Korean medical treatments(Herbal medicine, chuna therapy, acupuncture, bee-venom acupuncture.). Methods : Operation(OP) recommended 20 patients with lumbar disc hemiation were treated with chuna therapy, acupuncture, bee-venom acupuncture one times a week and took herbal medicine after a meal two times daily. The patients' symptoms were assessed 1 week, 2 week, 4 week, 6 week, 8 week by Visual analogue scale(VAS), Oswestry disability index(ODI), 36-Item short-form health survey(SF-36). Results : 1. VAS(LBP and Sciatica) score was significantly improved after 8 weeks (P<0.05). 2. ODI, SF-36 score was significantly improved after 8 weeks (P<0.05). 3 There were significant changes in physical functioning(PF), bodily pain(BP), social functioning(SF), role emotinal(RE), mental health(MH) score of SF-36 after 8 weeks (P<0.05). Conclusions : This study suggests that Korean medical treatment(Herbal medicine, chuna therapy, acupuncture, bee-venom acupuncture) can be applicable to improve symptoms in patients with lumbar disc herniation.

The Effects of Aroma Therapy on Allergic Symptoms, Eosinophil, and the Quality of Life related to Rhinitis in University Students with Allergic Rhinitis (아로마 에센셜 오일 적용이 알레르기 비염 대학생의 비염증상, 호산구 및 비염 관련 삶의 질에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of using aroma essential oils on allergic symptoms, eosinophils and quality of life related to rhinitis for university students with allergic rhinitis. Methods: A randomized controlled experimental design was used. There were 37 participants in the experimental group, and 30 in the control group. The experimental group applied the essential oils four times a day in the nasal area as well as using inhalation of the oils each night for four weeks. Rhinoconjunctivitis quality of life questionnaire was composed of 28 questions regarding seven areas associated with allergic reactions. The data were analyzed with SPSS/WIN 21.0 program. Results: Rhinitis symptom index, nose itching (p<.001), sneezing (p<.001), watery rhinorrhea (p<.001), and nose congestion (p<.001) all had significant difference for the participants in the treatment group. Further there were significant differences in eosinophils both for measured time and interaction (p<.001). For the quality of life related to rhinitis, the results showed significant difference (p<.001). Conclusion: Using aroma therapy by applying essential oils turned out to be an effective nursing intervention that can be utilized in the easing of allergic symptom and the decrease of eosinophils, causing improvement in the quality of life related to rhinitis.

Clinical Observation of Changes in Subjective Evaluation Index and Electrogastrography Parameters of Patients with Functional Dyspepsia before and after Oriental Medical Interventions (기능성 소화불량 환자의 한방치료 전후의 주관적 평가지표와 Electrogastrography Parameters를 이용한 객관적 지표의 변화 관찰)

  • Objectives : Functional dyspepsia is a type of disease characterized by bothersome postprandial fullness, early satiation, epigastric pain and burning without structural gastrointestinal disease. The aim of this study was to analyze the characteristics of functional dyspepsia patients and evaluate changes in symptoms, quality of life, and electrogastrography parameters before and after oriental medical interventions. Methods : We recruited forty-six functional dyspepsia patients who visited gastroenterology clinic in the oriental medicine hospital of Kyung Hee University between November 2009 and February 2011. Patients were assessed for their frequency of dyspepsia (based on short form-Leeds Dyspepsia Questionnaire: SF-LDQ), quality of life (based on functional dyspepsia-related quality of life questionnaire: FD-QoL), gastric motility (based on electrogastrography: EGG) on the first visit. Then, the effect of oriental medical interventions was evaluated using EGG on the second visit. Results : The majority of patients had symptoms of nausea and indigestion. The largest decrease in EGG parameters was found in the indigestion group. The frequency of regurgitation and postprandial EGG power % bradygastria showed a significant correlation. Also, significant correlations were found between some items of FD-QoL and some EGG parameters. Compared to the EGG parameters before oriental medical interventions, some parameters after treatment had positive results, implying the improvement of gastric motility disorder. We also found improvement of EGG parameters in both digestant medicinal group and digestant combined with qi-tonifying medicinal group. Conclusions : The results of this study suggest that clinical application of EGG can be an objective diagnostic tool in functional dyspepsia patients visiting oriental medical hospital.
