Analeptic Effects of Yohimbine on Muscle Relaxation of Succinylcholine in Dogs (Succinylcholine Chloride의 근이완에 대한 Yohimbine의 회복효과)

  • This study was carried to get detailed information about the analeptic effect of yohimbine on succinylcholine muscle relaxant. Succinylcholine was administered intra- venously at a dose rate of 0.05 mg per kg of body weight and then ten minutes after the succinylcholine injection, yohimbine was administered intravenously at a dose rate of 0.1 mg per kg of body weight. The results obtained were as follows. 1. Induction time of muscle relaxation was fast and favourable as 38 seconds. 2. Mean arousal time and mean walk time were significantly shortened by yohimbine administration in the dogs immobilized with succinylcholine (p<0.01). 3. Heart rate was slightly increased after succinylcholine administration, and increased significantly after yohimbine administration compared to the control group (p<0.05). 4. Glucose concentration was slightly increased after succinylcholine administration, and decreased after yohimbine administration compared to the control group.

  • This study was performed to evaluate the effects of doxapram after succinylcholine treatment. Succinylcholine was administered intravenously at a dose rate of 0.07 mg per kg of body weight and then ten minutes after the injection of succinylcholine doxapram was administered intravenously at a dose rate of 2 mg per kg of body weight. The results obtained were as follows : 1. Recovery time in dog given doxapram after succinylcholine treatment was shortened comparing with control group. 2. The changes in respiratory rate revealed a maximal increase immediately after the injection of doxapram. Thereafter respiratory rate gradually decreased, and revealed normal levels 20 minutes after the injection of doxapram. 3. The changes in heart rate revealed a maximal increase immediately after the injection of doxapram. Thereafter heart rate gradually decreased, but remained above the levels of control group. 4. Although arrhysthmias were observed after treatment of succinylcholine, these were disappeared after doxapram treatment. And there was no another change on electrocardiograms.

  • This study was performed to evaluate the effect of succinylcholine chloride as a muscular relaxant in deers. Succinylcholine chloride was administered for the harvesting of velvet antler to 62 sika deers which were farmed in the surburb of Seoul. The results obtained were as follows. 1. The more the dose of succinylcholine the earlier the time to immobilization and the slower the time to recover (p<0.05). 2. The optimal intramuscular dose of succinylcholine was found to be 0.11~0.14mg per kg of body weight for sika deer. 3. The time to immobilization revealed the longest An 4~6 years group. The more the age the longer the time to recover (p<0.05).

  • We estimated the changes of ECG and electrolytes in serum after intravenous administration of Succinylcholine Chloride (SCC), 0.15 mg/kg in ten normal mongrel dogs (mean 13 kg). Hyperkalemia was observed in the highest level by 6.46$\pm$0.8 mEq/L at ten minutes after the administration of SCC. The ECG appeared temporary the most severe changes as the increased T wave, the disappeared P wave, the prolonged of conduction times (PR, QTc intervals and QRS complex), and arrhythmia as ventricular premature contraction at 3 and 5 minutes after the administration SCC. Therefore, the changes of ECG after administration of SCC were suggested to specific and independent from hyperkalemic changes. Because these changes were observed to differ from ECG by hyperkalemia, and the highest period of $K^+$value in serum differ from the appearance period of severe changes of ECG and arrhythmia by SCC.

A Clinical Study of Succinylcholine-Atracurium for Cesarian Section (제왕절개환자에 있어서 Succinylcholine-Atracurium의 임상적 고찰)

  • Atracurium was used in 10 parturients undergoing elective cesarian section under general Anesthesia. An initial bolus of 0.4 mg/kg of the atracurium was injected after recovery from succinylcholine block. Complete neuromuscular block resulted in all cases. Muscular relaxation was maintained by incremental dose of 0.2mg/kg of atracurium. Administration of atracurium did not cause significant change in blood pressure, pulse rate and infants Apgar score. The residual neuromuscular block could be antagonized at the end of the procedure by mixture of 0.005mg/kg glycopyrrolate and 0.03 mg/kg of neostigmine. In all parturients, antagonism was adequate as evidenced by respiratory response and head lift test. Atracurium may be advantageous in parturients undergoing cesarian section under general anesthesia cause it maintained cardiovascular stability, is non-cumulative, is readily antagonized by neostigmine and has no deleterious effect on the newborn.

  • Background: This prospective study was designed to compare the cardiovascular response to endotracheal insertion of either an orotracheal tube or a nasotracheal tube Methods: 120 ASA physical status I and II surgical patients requiring general anesthesia and tracheal intubation were studied and assigned to two groups: orotracheal intubation group (n = 60) and nasotracheal intubation group (n = 60). Patients were premedicated with midazolam 0.05 mg/kg and glycopyrrolate 0.005 mg/kg intramuscularly and anesthesia was induced with thiopental sodium 5 mg/kg and succinylcholine 0.1 mg/kg intravenously. Systolic blood pressure (SBP), diastolic blood pressure (DBP). mean arterial pressure (MAP) and heart rate (HR) were assessed noninvasively before induction of anesthesia and immediately after intubation, 1 min, 2 min, 3 min, and 5 min after intubation. Results: Cardiovascular responses such as SBP, DBP, MAP and HR were similar for both techniques and no significant differences between two groups were observed until 5 min after intubation. Conclusions: In healthy ASA I and II patients with normal blood pressure, induction doses of thiopental sodium 5 mg/kg and succinylcholine 0.1 mg/kg didn't attenuated the cardiovascular response to laryngoscopy and tracheal intubation. Insertion of an endotracheal tube may be the most invasive stimulus during intubation procedures. (JKDSA 2001; 1: 10-15)

  • Malignant hyperthermia has been reported by many authors since Denborough [1960] first described concerning anesthetic death in a family. Malignant hyperthermia is characterized by a hypermetabolic state [tachycardia, tachypnea, hypercarbia, hypoxia, cyanosis, hypotension, high fever and muscle rigidity] and is related to a hereditary defect of skeletal muscle. In susceptible individuals, it is triggered by potent inhalational anesthetics, depolarizing muscle relaxant [Succinylcholine], amide type local anesthetics [prototype lidocaine] and occasionally by stress due to emotional and environmental factors. Unrecognized and untreated malignant hyperthermia is associated with a very high mortality rate. Recently authors have experienced malignant hyperthermia in 5 year old male child who was diagnosed to have patent ductus arteriosus and interatrial septal defect associated with congenital physical deformities such as short stature, hypotrophic muscles and genu valgus deformity of lower extremity, indirect inguinal hernia and Ramphant caries.

Airway obstruction by dissection of the inner layer of a reinforced endotracheal tube in a patient with Ludwig's angina: A case report

  • Intraoperative airway obstruction is perplexing to anesthesiologists because the patient may fall into danger rapidly. A 74-year-old woman underwent an emergency incision and drainage for a deep neck infection of dental origin. She was orally intubated with a 6. 0 mm internal diameter reinforced endotracheal tube by video laryngoscope using volatile induction and maintenance anesthesia (VIMA) with sevoflurane, fentanyl ($100{\mu}g$), and succinylcholine (75 mg). During surgery, peak inspiratory pressure increased from 22 to $38cmH_2O$ and plateau pressure increased from 20 to $28cmH_2O$. We maintained anesthesia because we were unable to access the airway, which was covered with surgical drapes, and tidal volume was delivered. At the end of surgery, we found a longitudinal fold inside the tube with a fiberoptic bronchoscope. The patient was reintubated with another tube and ventilation immediately improved. We recognized that the tube was obstructed due to dissection of the inner layer.

  • Malignant hyperthermia is a catastrophic, hypermetabolic syndrome that arises in susceptible individuals when they are exposed to certain inhalational anesthetics or muscle relaxants. It is characterized by hyperthermia, tachycardia, acidosis, and muscle rigidity. It has been noted that the majority of cases of malignant hyperthermia are fatal unless early diagnosis and treatment are performed. We experienced a 24 year old male Malignant hyperthermia presented for orthognathic surgery under $O_2-N_2O$-sevoflurane anesthesia without succinylcholine. Two half hours after induction, tachycardia developed and was followed by unstable blood pressure and hyperpyrexia. Anesthesia was terminated and vigorous emergency treatment was attempted. The patient was treated by the intravenous administration of dantrolene sodium. The diagnosis of an acute malignant hyperthermia reaction by clinical criteria can be difficult because of the nonspecific nature and variable incidence of many of the clinical signs and laboratory findings. So the malignant hyperthermia clinical grading scale is recommended for use as an aid to the objective definition of this disease. This clinical grading system provides a new and comprehensive clinical case definition for the malignant hyperthermia syndrome. We recently encountered a case of delayed malignant hyperthermia during sevoflurane anesthesia that was successfully treated by the intravenous administration of dantrolene sodium. In conclusion, exposure to sevoflurane should be avoided in patients thought to be susceprible to malignant hyperthermia.

Dental Treatment for Handicapped Patients in Day Care Units: 2 Years' Experience at Seoul National University Dental Hospital (서울대학교 치과병원 장애인 진료실의 외래마취 하 치과치료)

  • 연구배경: 장애인의 치과치료는 일반인의 치과치료와는 달리 많은 문제점들을 가지고 있다. 이러한 장애인 환자의 치과치료를 위하여 서울대학교 치과병원에서는 치과마취과를 중심으로 임상 각과가 협진하여 외래마취 기반 하에 1999년부터 장애인 진료실을 운영하고 있다. 이번 연구에서는 서울대학교 치과병원 장애인 진료실의 치료방법을 소개하고 그 동안 치료받은 장애인 환자들의 자료를 바탕으로 마취과적인 특징을 분석하였다. 방법: 2001년 1월부터 2002년 5월까지 치료받은 총 54명의 장애인 환자들의 치료기록지와 마취, 그리고 회복실 기록지를 검토하였다. 환자의 성별, 나이, 전신질환, 수술 전 시행된 검사, 치료에 참여한 협진과, 마취 유도제와 유지제, 총 마취시간과 회복실 체류시간, 그리고 회복실에서 관찰된 합병증을 조사하였다. 결과: 총 54명의 환자들을 대상으로 58건 치과치료가 시행되었다. 한 건의 의식진정 법을 제외하고는 모두 전신마취 하에서 치과치료가 시행되었다. 마취유도제는 thiopental sodium, 근이완제는 vecuronium과 succinylcholine가 가장 많이 사용되었으며 마취 유지를 위해서는 enflurane과 isoflurane의 흡입마취제를 이용하거나 TCI를 이용한 propofol 지속정주를 이용하였다. 총 마취지속시간은 $160.2{\pm}55.5$분이었으며 회복실에서의 체류시간은 $132.8{\pm}50.1$분, 그리고 완전한 회복을 확인하기 위해 병동에 단기 입원시킨 소아환자에서는 $58.7{\pm}16.8$분이었다. 회복실에서 심각한 합병증은 발생하지 않았다. 지속적인 감시를 위하여 4명의 환자가 입원을 권유받았으나 특별한 합병증 없이 다음 날 퇴원하였다. 결론: 서울대학교 치과병원 장애인 진료실은 외래마취에 대한 기존의 연구들보다 긴 마취시간과 회복실 체류시간을 보였다. 그러나 별다른 합병증 없이 일회 방문으로 장애인 환자들에게 필요한 치과치료를 모두 제공할 수 있었다.
