신생아 황달의 광선 요법 투여시간에 따른 혈청 빌리루빈 하강 효과 (The Effect of Radiation hours on Serum Total Bilirubin Decrements in the Phototherapy for Neonatal Physiologic Jaundices.)

  • The purpose of this study is to define the effect of radiation hours on the physiologic jaundice infants. Since there in no full explanation of the way of phototherapy. Data was collected from February, 1993 through August, 1993 at E University Hospital. The subjects consisting of 179 normal newborn who is to receive phototherapy due to bilirubinemia in the early postnatal period. Six groups compared the effectiveness of phototherapy based on hours of radiation ; 6hrs, 9hrs, 12hrs, 15hrs, 18hrs, 21hrs. The results were as follows 1) Effectiveness of phototherapy which means serum total bilirubin decrements were significantly different in groups (F=9.812 p=.000). And follow up study was showed the subset in less than 15hrs groups and more than 15hrs groups. 2) There was no significant difference on the effect of phototherapy between aged after birth. 3) An hours of radiation and the number of stools was not revealed the relationship. The results obtained from this study suggest that 15hrs radiation per day is effective in phototherapy. In the furture, more replication of this study will be contribute for neonatal nursing care.

Deregulation of MTDH Gene Expression in Gastric Cancer

  • Aim: Gastric cancer is the third most frequent cause of cancer mortality worldwide. In Iran, it is one of the leading causes at the national level. Localized at chromosome 8q22, the human MTDH gene has been reported to be over-expressed in a spectrum of malignancies. However, since there is a lack of data concerning with expression in gastric cancer at the transcriptional level, in this study we evaluated MTDH expression in Iranian cases. Methods: Totally, thirty paired gastric samples were examined by quantitative real-time RT-PCR. Results: Although the mRNA expression was significantly elevated in 46.6% of the examined tumor tissues; its expression was low in others (36.6%). Moreover, there was only a marginal statistical difference between the MTDH gene expression of all tumor specimens compared to their paired non-tumor ones and no statistically significant association with the grades and types of the tumors. Conclusion: Taken together, our results demonstrated that expression of MTDH at the transcriptional level may be increased in gastric cancer tissue samples but with considerable heterogeneity. Due to this, it may have the potential to be used as a target for diagnostic/therapeutic purposes only in a subset of patients.

  • In this paper, the authors establish the conditions for the extinction of solutions, in finite time, of the fast diffusive p-Laplace equation $u_t=div({\mid}{\nabla}u{\mid}^{p-2}{\nabla}u)+a{\int}_{\Omega}u^q(y,t)dy$, 1 < p < 2, in a bounded domain ${\Omega}{\subset}R^N$ with $N{\geq}1$. More precisely, it is shown that if q > p-1, any solution vanishes in finite time when the initial datum or the coefficient a or the Lebesgue measure of the domain is small, and if 0 < q < p-1, there exists a solution which is positive in ${\Omega}$ for all t > 0. For the critical case q = p-1, whether the solutions vanish in finite time or not depends crucially on the value of $a{\mu}$, where ${\mu}{\int}_{\Omega}{\phi}^{p-1}(x)dx$ and ${\phi}$ is the unique positive solution of the elliptic problem -div(${\mid}{\nabla}{\phi}{\mid}^{p-2}{\nabla}{\phi}$) = 1, $x{\in}{\Omega}$; ${\phi}(x)$=0, $x{\in}{\partial}{\Omega}$. This is a main difference between equations with local and nonlocal sources.

우울증의 병태생리에 관한 동서의학적 비교 (A comparative study on physiopathology of depression by Korean medicine and conventional medicine)

  • Objective: This study aims to summarize recent findings of depression and compare the physiopathology of depression by conventional medicine and Korean medicine. Methods: A literature search of PubMed and Journal of Oriental Neuropsychiatry was conducted. This study analyzes the physiopathology of depression. Results: Despite difference of the viewpoints between conventional medicine and Korean medicine, etiology and symptoms of depression are similar. Recent findings of depression partly accord with Korean medicine theory. Conclusion: Physiopathology of depression has yet to be completely understood and current treatments remain ineffective in a large subset of patients. As the efficacy of Korean medicine against depression is demonstrating recently, understanding the viewpoints of Korean medicine will provide new perspectives and hints for further research and therapeutic development.

헤파린의 심막유착 방지효과에 관한 실험적 연구 (The Effect of Heparin on Prevention of Pericardial Adhesion - Experimental Study -)

  • The pericardial adhesions following the first open heart surgery pose a major problem that can increase the morbidity and mortality in cases of reoperation because of the danger of damaging the heart, great vessels or grafts. The purpose of this study was to determine the effect of intrapericardial infusion of 5% dextrose solution and heparin on experimental pericardial adhesions. The 15 white rabbits were divided into three treatment groups of 5 animals each: group 1 [control group], group 2[5% dextrose group] and group 3 [heparin group]. At 4 weeks, all animals were sacrificed and the pericardial adhesions were graded as follows: 1[none], ll[mild], Ill[moderate] and IV[severe]. Histological examination was performed on a subset of each group. The difference of adhesion between control and heparin group was evaluated by Chi-square analysis. The results were as follows: 1. Pericardial adhesions developed in 80% of animals in group 1 and 2. 2. Only 40%> of animals in group 3 had mild to moderate pericardial adhesions. This was significantly different from the control group [P=0.01]. 3. Histological examination showed that the pericardium with adhesion was thickened with fibrosis, while that without adhesion was thin, normal appearance. 4. In group 3, there was no evidence of hemorrhage or hemorrhagic tendency in the pericardium.

Analysis of Golf Ball Mobility and Balancing based on IoT Sports Environments

  • Recently, IoT researches using sensor data based on embedded networks in various fields including healthcare and sports have been continuously attempted. This study analyzes golf ball mobility to support IoT application in golf sports field. Generally, since the difference in density occurs due to the condition of the inner material and the abnormal state at the time of the outer skin joining during the manufacturing of the golf ball, the weight of each subset is equal for any two points with the same radius in the sphere cannot be guaranteed. For this reason, the deflected weight of the sphere has the undesirable effect of hitting the ball in a direction in which the weight of the ball is heavy. In this study, it is assumed that there is a unique center of gravity of the ball, and even if the golf ball cannot be manufactured perfectly, it wants to establish the basic principle to accurately recognize or mark the putting line based on the center of gravity. In addition, it is evaluated how the mobility of the golf ball with a deviation from the center of gravity of the golf ball affects the progress path (or movement direction) and the moving distance (or carry distance) after the golfer hits. The basic model of the mobility of the golf ball can help the golfer exercise model and the correlation analysis. A reliability-based fragility assessment method for seismic pounding between nonlinear buildings

  • Existing methods to estimate the probability of seismic pounding occurrence of adjacent buildings do not account for nonlinear behavior or only apply to simple lumped mass systems. The present study proposes an efficient method based on subset simulation for fragility and risk assessment of seismic pounding occurrence between nonlinear adjacent buildings neglecting pounding effects with application to finite element models. The proposed method is first applied to adjacent buildings modeled as elastoplastic systems with substantially different dynamic properties for different structural parameters. Seismic pounding fragility and risk of adjacent frame structures with different floor levels is then assessed, paying special attention to modeling the non-linear material behavior in finite element models. Difference in natural periods and impact location are identified to affect the pounding fragility simultaneously. The reliability levels of the minimum code-specified separation distances are also determined. In addition, the incremental dynamic analysis method is extended to assess seismic pounding fragility of the adjacent frame structures, resulting in higher fragility estimates for separation distances larger than the minimum code-specified ones in comparison with the proposed method.

Postoperative Speech Outcomes and Complications in Submucous Cleft Palate Patients

  • Background The postoperative speech outcomes of submucous cleft palate (SMCP) surgery are known to be poorer than those of other types of cleft palate. We attempted to objectively characterize the postoperative complications and speech outcomes of the surgical treatment of SMCP through a comparison with the outcomes of incomplete cleft palate (ICP). Methods This study included 53 SMCP patients and 285 ICP patients who underwent surgical repair from 1998 to 2015. The average age of the patients at the time of surgery was $3.9{\pm}1.9years$ for the SMCP patients and $1.3{\pm}0.9years$ for the ICP patients. A retrospective analysis was performed of the complications, the frequency of subsequent surgical correction for velopharyngeal dysfunction (VPD), and speech outcomes. Results In both the SMCP and ICP patients, no cases of respiratory difficulty, bleeding, or wound disruption were noted. Delayed wound healing and fistula occurred in 18.9% and 5.7% of the SMCP patients and in 14% and 3.2% of the ICP patients, respectively. However, no statistically significant difference in either delayed wound healing or fistula occurrence was observed between the two groups. The rate of surgical correction for VPD in the SMCP group was higher than in the ICP group. In the subset of 26 SMCP patients and 62 ICP patients who underwent speech evaluation, the median speech score value was 58.8 in the SMCP group and 66 in the ICP group, which was a statistically significant difference. Conclusions SMCP and ICP were found to have similar complication rates, but SMCP had significantly worse speech outcomes.

다중 심벌 검파를 이용한 트렐리스 부호화된 대역 확산 통신 시스템 (Trellis Coded Spread Spectrum with the multiple symbol detection)

  • 본 논문에서는 직접대역확산통신시스템에서 코딩 이득을 향상시키고자 다중 심벌 검파를 수행하는 트렐리스 부호화 변조를 적용하였다. $MDPSK(\pi/4 shift QPSK)$를 트렐리스 부호화된 직접대역확산시스템 적용하고 정보가 인접한 채널 신호의 위상차에 전송된다는 것을 이용하여 1차 위상차 뿐만 아니라 다중위상차를 추출한다. BER특성을 향상시키기 위해, 이러한 다중 위상차를 이용하여 $MDPSK(\pi/4 shift QPSK)$에서 다중 심벌 검파를 수행하는 트렐리스 부호화된 직접대역확산시스템의 비터비 디코더 알고리듬을 설계하여 향상된 코딩 이득을 얻고자 한다. 이러한 시스템을 직접대역확산통신시스템에 적용하였을 때 얻을 수 있는 코딩 이득은 시뮬레이션 결과, AWGN채널에서 TCM의 콘볼류션부호화기의 상태수 4, 8, 16에 따라 3-4dB 정도의 성능향상이 있으며, 레일레이 페이딩 채널에서는 4-5dB정도의 성능 향상이 있음을 알 수 있다. 일반적으로 상태수가 증가할수록 더 큰 코딩 이득을 얻을 수 있다.

기능성 소화불량증 환자와 건강인의 어제혈 색택 비교 연구 (Skin Color Measurement of LU10; Comparison between Functional Dyspepsia Patients and Healthy Controls)

  • Objectives: According to Korean Medicine theory, the skin color of LU10 serves as a diagnostic clue to dyspeptic symptoms. The aims of this study were (1) to find the difference of skin color in LU10 region between functional dyspepsia (FD) and healthy control (HC) and (2) to examine the relationship between LU10 skin color parameters and dyspeptic symptoms. Methods: 39 participants (29 FD and 10 HC) have participated in this study. They were asked to complete gastrointestinal scale (GIS), gastrointestinal symptom rating scale (GSRS), Nepean dyspepsia index (NDI), functional dyspepsia-related quality of life (FD-QoL), visual analogue scale (VAS) for dyspeptic symptoms, food retention questionnaire (FRQ) and cold heat questionnaire (CHQ). $L^*$ (luminance), $a^*$ (red-green balance) and $b^*$ (yellow-blue balance) values of LU10 region were calculated through digital images of the participant's hand. Then we evaluated test-retest reliability of $L^*$, $a^*$ and $b^*$ values of LU10 region. Additionally, we compared $L^*$, $a^*$ and $b^*$ values of LU10 between FD and HC, and examined the relationship between LU10 color parameters and seven questionnaires scores. Results: Only $L^*$ values in LU10 region were significantly higher in FD compared with HC. GIS scores and the subset scores of NDI had a positive correlation with $L^*$ values significantly. Correlation coefficients of test-retest reliability of skin color measurement of LU10 ranged from 0.871 to 0.936 representing very strongly statistically significant (P<0.001). Conclusions: We confirmed the difference of skin color in LU10 region between FD and HC, and relationship between LU10 skin color parameters and dyspeptic symptoms.
