Open Posterior Approach versus Arthroscopic Suture Fixation for Displaced Posterior Cruciate Ligament Avulsion Fractures: Systematic Review

  • Purpose: To compare the clinical outcomes between the open posterior approach and arthroscopic suture fixation for displaced posterior cruciate ligament (PCL) avulsion fractures. Methods: A literature search was performed on MEDLINE, EMBASE, and the Cochrane Library databases. The inclusion criteria were as follows: papers written in English on displaced PCL avulsion fractures, clinical trial(s) with clear description of surgical technique, adult subjects, a follow-up longer than 12 months and modified Coleman methodology score (CMS) more than 60 points. Results: Twelve studies were included with a mean CMS value of 72.4 (standard deviation, 7.6). Overall, 134 patients underwent the open posterior approach with a minimum 12-month follow-up, and 174 patients underwent arthroscopic suture fixation. At final follow-up, the range of Lysholm score was 85-100 for the open approach and 80-100 for the arthroscopic approach. Patients who were rated as normal or nearly normal in the International Knee Documentation Committee subjective knee assessment were 92%-100% for the open approach and 90%-100% for the arthroscopic approach. The range of side-to-side difference was 0-5 mm for both approaches. Conclusions: Both arthroscopic and open methods for the treatment of PCL tibial-side avulsion injuries resulted in comparably good clinical outcomes, radiological healing, and stable knees.

Hair transplantation in patients with hair loss or scar deformity in the side hairline after midface-lifting surgery

  • Background Successful aesthetic plastic surgery is devoid of both unsightly scarring and postoperative disfigurement. Patients undergoing midface-lifting surgery are very often disconcerted by an altered side hairline, including sideburns, despite considerable amelioration of facial wrinkles. This study was conducted to identify an effective means of approaching an altered hairline and the unavoidable scarring arising from midface-lifting surgery. Methods A total of 37 patients who underwent corrective surgery with hair transplantation for hair loss or scar deformity arising from midface-lifting surgery from June 2014 to June 2017, and were observed for more than 6 months thereafter, were enrolled in the study. Prior to corrective surgery, the patients were administered a multiple-choice survey regarding their dissatisfaction arising from midface-lifting surgery. Among the 37 patients, 24, 12, and one underwent donor harvesting by the strip method, non-shaven follicular unit extraction, and partial shaving follicular unit extraction, respectively. Additionally, 33 of the 37 patients underwent hair transplantation in the frontotemporal recess area along with hairline correction surgery. The average number of transplanted grafts was 1,025. Results Surgery resulted in a natural and satisfactory appearance in all patients. The average patient and physician subjective satisfaction scores were 4.6 and 4.8, respectively. No adverse events such as folliculitis occurred. Conclusions Side-hairline correction surgery by hair transplantation can be considered an effective method of realigning an altered hairline accompanied by scars following midface-lifting surgery.

Traditional Korean Medicine Treatment for Sequelae After Surgery in Prostate Cancer Patients: Two Case Report (전립선암환자의 수술 후 발생한 후유증에 대한 한방치료 2례)

  • Objectives: The purpose of this study is to report two cases of prostate cancer patients whose side effects after surgery were treated with traditional Korean medicine. Methods: Two prostate cancer patients underwent robot-assisted laparoscopic prostatectomy, and both patients were treated with traditional Korean medicine. Subjective discomfort symptoms were evaluated using the Numerical Rating Scale. Adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events, version 5.0. Daily life vitality was evaluated by the Eastern Cooperative Oncology Group. Results: After treatment with traditional Korean medicine, discomfort symptoms that occurred after surgery, such as frequent urination, nocturia, insomnia, and depression, all gradually improved, and Eastern Cooperative Oncology Group scores also improved from two points to one point. Additionally, there were no hematologic side effects from the traditional Korean medicine treatment. Conclusions: This case study suggests that traditional Korean medicine may contribute to the improvement of side effects caused by robot-assisted laparoscopic prostatectomy, quality of life, and immunity.

  • Objective: The purpose of this study is to report two cases of breast cancer patients whose side effects after Tamoxifen were treated with traditional Korean medicine. Methods: Two breast cancer patients underwent Tamoxifen after breast-conserving surgery, and both were treated with traditional Korean medicine. Subjective discomfort symptoms were evaluated using the Numerical Rating Scale. Adverse events were assessed using the National Cancer Institute Common Terminology Criteria for Adverse Events, Version 5.0. Results: After treatment with traditional Korean medicine, the discomfort symptoms that occurred after Tamoxifen, such as osteoarthritis, hot flushes, and cold sweat, all gradually improved. In addition, no hematologic, hepatic, or renal side effects were observed from the traditional Korean medicine treatment. Conclusion: This case study suggests that traditional Korean medicine may contribute to the improvement of side effects caused by Tamoxifen and enhance the quality of life of patients.

Changes in binaural beat strength to the difference of right and left hearing ability (좌우 청력 차이에 따른 바이노럴 비트 세기의 변화)

  • The binaural beat is a subjective beating phenomenon due to the frequency difference of sounds heard in both ears, sensed by the human brain. The beating magnitude physically depends on the amplitudes of the two signals with slight different frequencies. Therefore, the binaural beat strength is expected to be affected by the difference in left and right hearing ability. In this study, 34 healthy subjects without hearing loss were chosen. They hear a pure tone sound (450 Hz) at 10 different Sound Pressure Levels (SPLs) (from 50 dB to 27.4 dB) through the one side of ears, while they hear the reference sound (440 Hz, 50 dB) through the other side of ears. Their subjective assessment using a semantic differential method reveals that the binaural beat strength decreases as SPL differences between the two sides of ears increases, if the difference is greater than 2.2 dB. The result suggests that the hearing loss difference between the two sides of ears should be less than 2.2 dB to maximize binaural beating effects.

Acupuncture Treatment about Medial Meniscus Posterior Horn Rupture : A Case Report

  • Objectives : In this case, the knee joint inconvenience with deteriorating pain has been relieved by acupuncture treatment for a patient was 51-year-old male with a left medial meniscal posterior horn rupture in 2012. Methods : Twenty-four times of acupuncture treatments were performed for Twelve weeks from July 2016 to improve the disease. SP8(Jigi; 地機), BL63(Geummun; 金門), KI3(Taegye; 太谿), LI4(Hapgok; 合谷) were chosen for treatment by principles of Traditional Korean Medicine. Results : Numeric Rating Scales, which means subjective pain, decreased from 6 points to 3 points out of 10 points in total. Pressure Pain Threshold, which means sensitivity to pressure applied to the affected area, increased from 21N to 47N on the lateral-inferior side and from 19N to 50N on the lateral side. K-WOMAC, which indicates discomfort of knee-related activity, was 56 points out of 96 points in total before treatment and 4 points after treatment, 13 points after two years, and 15 points after four years. However, MRI tests conducted before and after treatment did not identify any significant changes. As a result, we confirmed that a total of 24 acupuncture treatments had resulted in the relief objective and subjective pain and functional recovery, especially in the case of the knee function, maintained until after four years later. There was no significant substrate recovery in meniscal rupture.

  • The article explores the prospects and trends for the transformation of some basic concepts of law associated with the development of artificial intelligence systems and the problems of liability for harm caused by a robot. The prospects, conditions and consequences of vesting robots with partial (quasi) or full legal personality are explored. This process should lead to a revision of the concepts of will, subjective side and legal responsibility in the direction of their greater universalization. The legally significant signs of will, legal personality, legal liability in relation to robots, artificial intelligence systems and other complex automated information systems are clarified. The author identifies the following essential factors of legal qualification of an act committed by a robot: goals, reasons for setting goals, connections between the planned result and the action taken, the actual result, the reasons for the difference between the actual result and the planned one. The article pays special attention to the preventive function of legal liability, which, when applied to robot subjects, can be expressed in the following basic procedures. 1. Accounting for legal requirements in the behavior of the robot. 2. Timely adaptation of the robot to changes in legislation and other regulatory legal acts that affect its behavior. 3. Accounting for incidents. 4. Destruction of a series of robots whose actions lead to unacceptable consequences.

  • All creatures are living in the space and time. As the space and time are prior to experience, they are preconditions for an incident to happen and preconditions for each other to coexist as well. Therefore, time can be recognized through the change of space and the space can be understood by the passage of time. In western philosophy, the space was understood as an object, place, interval, mind and etc. In oriental philosophy, even though one space is just a part of bigger space, the space may represent the universal space, and the various spaces are no more than a space. The space itself doesn't have any color, form, beginning and end, or liu-he(六合). However, it is the biggest concept that we can find everywhere. In order to understand the space, we need to find our position by expressing subjective positions like above and below, left and right, before and after, and objective positions like high and low, east and west, south and north. In oriental philosophy, the sun is the standard point in finding position; its front side is south, the backside is north, the left side is east, the right side is west, the upper side is south and the lower side north. Based on the finding position which is stated above and by taking each characteristics of he-luo-xi-wen(河洛羲文) and interrelations among them, the space can be modeled. Followings are the results obtained from this study: Tian doesn't fill in west and north. Di doesn't fill in east and south. Tian-dao(天道) turns to left, and Di-dao(地道) turns to right. There is no direct way to get to Dui-chong-fang without passing by Zhong-gong(中宮). The solid figure of eighty-one Bian-ju(變局) and sixty-four Gua-tu(卦圖).

The 20th Anniversary of the Korean Academy of Sensory Integration (KASI): PLAY inSIde Camp Report (대한감각통합치료학회 창립 20주년 학술대회: PLAY inSIde 캠프 보고)

  • Objective : This paper is a report on the PLAY inSIde camp which connects sensory integration and play; the camp was designed for celebrating the KASI's 20th anniversary. Subjective : The camp was consisted of group play activity programs based on sensory integration for children aged 5~6 years with developmental disorder. The programs consisted of the Opening Program, Group Activity Program, Post Group Activity Program, and the Closing Activity Program. During the children's programs, their parents participated in parent education. The therapists participating in the camp planned and envisioned activity programs through pre-meetings and rehearsal sessions, where there were discussions among therapists and supporters. Conclusion : Through the camp, children experienced play activities involving sensory integration with other children, therapists shared their expertise in interaction with other therapists, and the KASI could share the accumulated resources with the society. After the events designed by the KASI, social participation through the occupation based on sensory integration could be facilitated and promoted in the future.

Audio High-Band Coding based on Autoencoder with Side Information (부가 정보를 이용하는 오토 인코더 기반의 오디오 고대역 부호화 기술)

  • In this study, a new method of audio high-band coding based on autoencoder with side information is proposed. The proposed method operates in the MDCT domain, and improves the performance by using additional side information consisting of the previous and current low bands, which is different from the conventional autoencoder that only inputs information to be encoded. Moreover, the side information in a time-frequency domain enables the high-band coder to utilize temporal characteristics of the signal. In the proposed method, the encoder transmits a 4-dimensional latent vector computed by the autoencoder and a gain variable using 12 bits for each frame. The decoder reconstructs the high band by applying the decoded low bands in the previous and current frames and the transmitted information to the autoencoder. Subjective evaluation confirms that the proposed method provides equivalent performance to the SBR at approximately half the bit rate of the SBR.
