  • Dental phobia is the most prevalent fear in all age groups, across gender, and in all countries. One of the primary identified sources is the fear of dental injections in the dental phobia or the high dental fear and anxiety groups. The purpose of this study was to clinically evaluate the computer controlled anesthetic device and to compare it with traditional methods of dental anesthetic delivery. Fifty(mean age : 25.6 yrs) systemically and periodontally healthy volunteers participated in this study. The subjects were given contralateral buccal and palatal injections. One side was injected with the computer-controlled anesthetic device with a microprocessor and an electric motor to precisely regulate flow rate during administration : The experimental group. The control side was injected with a standard manual syringe, in which flow rate and pressure are operator-dependent and can't be controlled accurately : The control group. The subjects described their perceived pain experiences with two subjective scales. The results of this study were as follows: 1. The computer-controlled anesthetic device was significantly less painful than conventional syringe injection 2. The female subjects reported more pain than the male subjects. But, there were no statistical differences. 3. The anesthetic effect of both methods did not show any difference. In this study, it may be concluded that pain levels decreased significantly when the computer-controlled anesthetic device was used.

  • Purpose: To evaluate clinical and radiologic outcomes of transphyseal anterior cruciate ligament (ACL) reconstruction in patients with open physes who were selected with authors' new operative indications. Materials and Methods: We evaluated 15 patients with open physes who underwent a transtibial ACL reconstruction and were followed up for 4~6 years after surgery. Our operative indications involved 1) choronologic age of ${\geq}$ 16 in male and ${\geq}$ 14 in female, 2) open physes of ${\leq}$ 2 mm width, and 3) Risser sign and Tanner stage of ${\geq}$ 3. Tibialis anterior tendon allograft was used in all patients, and endobutton and bioscrew were used for femoral and tibial fixations, respectively. Functional outcomes were evaluated using Lysholm Knee Scoring scale, Tegner activity scale, and International Knee Documentation Committee (IKDC) 2000 subjective score. Physical examinations to evaluate stability involved Lachman and pivot shift tests. For radiographic results, we evaluated side to side differences of anterior displacement in stress views. In addition, with use of scannograms taken at last follow-up, we examined side to side differences of femorotibial angles, anatomical and mechanical lateral distal femoral angles, mechanical medial proximal tibial angles and leg lengths. Results: The mean Lysholm Knee score was 51(40-61) points preoperatively and 97(94-100) points at last follow up. The mean Tegner activity score was 2.6 points preoperatively and 7.1 points at last follow up. The mean IKDC score was 32.6 points preoperatively and 88.3 points at last follow up. The mean anterior displacement of the tibia was improved from 6.7(${\pm}1.0$) mm to 1.9(${\pm}0.9$) mm. There were no leg length discrepancies over 5 mm and no statistically significant differences in all the radiographic variables representing growth disturbance. Conclusion: This study suggests that patients with open physes who selected by authors' new indication would safely undergo transphyseal ACL reconstruction with successful outcomes.

  • In this research, the selection of rail tour items of importance and achievement of service quality will be different 'analysis under the assumption that the relationships between them, were studied. Through this research, managers' subjective judgments of the customer rather than by a judge, ie, the true value to our customers think of the rail service quality for tourism products is crucial to develop the important elements. In addition, market-oriented thinking is to present the basic data of the transition. To summarize the conclusions of this paper is as follows. First, what tourists are seeking, constant research and improvement of tourism services should be looking for ways. Secondly, the focus on service quality aspects of products and the urgent need to identify the side of part "Concentrate Here" and "Possible Overkill" by reducing the effort to improve the efficiency of management is considered to be accelerated. Finally, most point in terms of focus and means of transport for the tourism sector as well as the railway tourism products, using the railway as a tourism product now on tour items should be changed.

  • The present clinical trial was performed to evaluate the anthelminthic effect and tolerance of Oxantel pamoate, a new antiwhipworm agent, in the treatment of T. trichiurus infection. Oxantel pamoate oral suspension was administered as a sing1e dose of 10 mg per kg. body weight to a total of 46 T. trichiurus infected cases selected out of 64 positives, by examining 89 fecal specimens collected from the soldiers and in-patients of xx Armed Forces General Hospital. For the evaluation, the negative conversion rate (cure rate) and the egg reduction rate were assessed 22 days after the treatment. The result obtained were as follows: 1. The prevalence rate of T. trichiurus infection in this study was 71.9%, while the mean E.P.G. of 46 treated cases was 126. 2. The egg negative conversion rate was 91.3%, and 98.4% of egg reduction rate was obtained. 3. From the observation of 107 worms expelled during 3 consecutive days after the treatment, it was known that sex ratio of male and female was about 1 : 2 and that average worm burden per capita was 2.3. 4. In all treated cases, the medication was readily accepted and well tolerated. On the hemograms, urinalysis and liver function tests, no significant differences were noted before and after the drug administration. There was also no detectable objective and subjective side effect.

  • Objective : A prospective, randomized, controlled clinical study is performed to verify the effectiveness of epidural dexamethasone-soaked gelatin sponges to reduce postoperative pain following lumbar microdiscectomy. Methods : Twenty-three patients (10 men and 13 women) undergoing lumbar microdiscectomy were included. Five pieces of gelatin sponge measuring $1{\times}1cm$ [Gelfoam; Pharmacia & Upjohn. Kalamazoo, MI, USA], soaked with either 5mg dexamethasone or an equal amount [2mL] of saline, were left on the decompressed nerve root after unilateral hemilami-nectomy, flavectomy and discectomy. Results : Subjective visual analog scale[VAS] scores of leg pain in the dexamethasone group on the first, third and first postoperative days [2.5, 2.5, 1.7, respectively] were significantly lower than in the control group [5.0, 4.8, 3.6][P<0.05]. No side effects related to the dexamethasone-soaked gelatin sponges were observed. Conclusion : The intraoperative application of dexamethasone-soaked gelatin sponges during lumber microdiscetomy can provide effective-postoperative analgesia without complications.

  • Among the sympathetic blocks used to treat various symptoms and diseases, including sympathetic dysfunction in pain clinics, a stellate ganglion block (SGB) is one of the easiest to apply. However, it is difficult to evaluate the effects of SGB due to the subjective nature of patient-reported data. A 26-year-old female, who presented with symptoms of coldness and sweating on both hands, received SGB 25 times on each side over a 2-month period. The effects of SGB were followed up using a cold stress test with thermography. Although the symptoms appeared again after 6 months, the recovery rate of the palmar temperature after the cold stress test improved significantly and was maintained for 18 months.

  • Purpose: This study was aimed to investigate the effects of gum-chewing on the recovery of bowel movement and oral cavity discomfort after abdominal surgery. Method: The nonequivalent control group, non-syncronized repeated treatment design was used. A total of 99 patients were participated in the study. The 44 patients were in the experimental group and the rest in the control group. The experimental group chewed gum three times a day until they passed gas. As the patient reported gas-passing, bowel movement time, subjective symptoms of oral cavity, and oral status were recorded precisely. The frequency, percentage, $x^2$-test and t-test were analyzed by SPSS PC 12.0. Results: There were significant differences in bowel movement, gas passing, oral cavity symptoms, and oral status scores between the experimental and the control group. Conclusion: The findings of the study demonstrated the fact that gum-chewing helps early recovery from post-operative ileus and thirst. It is because gum chewing stimulates bowel mobility and secretion of saliva. Thus gum-chewing seems to be an effective nursing intervention in reducing post-operative side effects for patients with abdominal surgery.

  • In Wyner-Ziv coding, video signals are reconstructed by correcting side information generated by block-based motion estimation/compensation at the decoder. The correction is not always accurate due to the limited number of parity bits and early stopping of low-density parity check accumulate (LDPCA) decoding in distributed video coding, or due to the limited number of measurements in distributed compressive video sensing. The blocking artifacts caused by block-based processing are usually conspicuous in smooth areas and degrade the perceptual quality of the reconstructed video. Conventional deblocking filters try to remove the artifacts by treating both sides of the block boundary equally; however, coding errors generated by block-based processing are not necessarily the same on both sides of the block boundaries. Such a block-wise difference is exploited in this paper to improve deblocking for Wyner-Ziv frameworks by designing a filter where the deblocking strength at each block can be non-identical, depending on the reliability of the reconstructed pixels. Test results show that the proposed filter not only improves subjective quality by reducing the coding artifacts considerably, but also gains rate distortion performance.

  • In low bit-rate video transmission, the payload of a single packet can often contain a whole coded frame due to the high compression ratio in both spatial and temporal domains of most modern video coders. Thus, the loss of a single packet not only causes the loss of a whole frame, but also produces error propagation into subsequent frames. In this paper, we propose a novel whole frame error concealment algorithm which reconstructs the first of the subsequent frames instead of the current lost frame to suppress the effects of error propagation. In the proposed algorithm, we impose a constraint which uses side match distortion (SMD) and overlapped region difference (ORD) to estimate motion vectors between the target reconstructed frame and its reference frame. SMD measures the spatial smoothness connection between a block and its neighboring blocks. ORD is defined as the difference between the correlated pixels which are predicted from one reference pixel. Experimental results show that the proposed algorithm effectively suppresses error propagation and significantly outperforms other conventional techniques in terms of both peak signal-to-noise ratio performance and subjective visual quality.
