Comparative Analysis of Fall-Related Physicopsychological according to Virtual Exercise and Lumbar Stabilization Exercise in the Patient with Stroke (가상현실 체험형 운동과 요부안정화운동에 따른 뇌졸중환자의 낙상관련 신체·심리적 비교 분석)

  • This study conducted the following experiment to examine change of physicopsychological function on lumbar stabilization exercise(LSE) and virtual reality game training(Nintendo Wii Sport-NWS) to stroke patients subject for fall prevention. Psychological function was measured by falls efficacy with stroke patients and physical function was measured by static and dynamic balance on comparative analysis of pre, post exercise and each groups in 30 stroke patient subject. Static balance was measured by BBS, FRT, dynamic were measured by TUG, 10m walking test and falls efficacy with stroke patients was measured index of falls efficacy. These result lead us to the conclusion that each group were statistically improved at all physicopsychological test, but BBS, FRT, 10m walking test were more statistically improved at LSE group and falls efficacy with stroke patients were more improved at virtual reality game training group. Consequently, virtual reality game training would be lead to positive increment of physicopsychological function on stroke patient.

  • Neuropsychiatric disorder is one of the symptoms developed after traumatic brain injury. The main symptoms of neuropsychiatric disorder are delirium, anxiety, depression and so on. We treated the patient with herbal medicine, acupuncture, moxibustion and other appropriate traditional Korean medical treatment. The patient only appeared neuropsychiatric disorder, not another neurologic symptoms. We consider it is the rare case in traditional Korean medicine hospital. So we report the patient with the review of literatures regarding neuropsychiatric disorder induced by traumatic brain injury.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with tremor. ■ Methods A patient with tremor of one side hand diagnosed with Parkinson's disease was treated with herbal medication, acupuncture, electro-acupuncture(EA), and herbal medical injection. Then we evaluated the improvement by Unified Parkinson's Disease Rating Scale(UPDRS), Parkinson's disease Quality of Life(PDQL) and Visual Analog Scale(VAS). ■ Results Decrease of UPDRS, VAS score and improvement of PDQL score were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on tremor due to Parkinson's disease.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(TKM) on a patient with forcible closure of the eyelids due to blepharospasm. ■ Methods A patient with forcible closure of the eyelids diagnosed as blepharospasm was treated with herbal medication, acupuncture, electro-acupuncture, cupping and moxibustion. Then we evaluated the improvement with length of eye opening and Blepharospasm Disability Index. ■ Results Increase of length of eye opening and Blepharospasm Disability Index were observed after the TKM treatment. ■ Conclusion This study proved the effect of TKM treatment on forcible closure of the eyelids due to blepharospasm.

  • When we prescribed Shiryungtang to a 65 years-old male patient with ARF after stroke, we could observe that his symptoms, I/O balance and laboratory findings were all improved. Therefore, we suggest that shiryungtang might be useful for ARF in the patient with stroke.

The Effect of Virtual Reality Based Rehabilitation Program on Balance of Patient with Stroke: A Meta-analysis of Studies in Korea (가상현실기반 재활프로그램이 뇌졸중환자의 균형에 미치는 영향 : 국내연구에 대한 메타분석)

  • Purpose : The aims of this study was to conduct a systematic literature review with meta-analysis to investigate the effect of virtual reality based rehabilitation program on balance of patient with stroke in Korean studies. Method : The studies for analysis were searched in electronic databases(Research Information Sharing Service; RISS, Korean Studies Information Service; KISS, DBpia, e-articles, National Assembly Library). The key words for search were 'virtual reality', 'stroke', and 'balance' and only randomized controlled trials and clinical controlled trials were included. Of 40 studies identified in the search, 20 studies met the criteria of this study and included in this meta-analysis. Result : The results were as follows: 1) The overall effect size of virtual reality based rehabilitation program was 0.557(95% critical interval; 0.340~0.774). 2) In the analysis of sub-categorical variables, effect size was as follows; the commercial game type(0.621) > virtual environment type(0.335); the dynamic balance measurement(0.750) > static balance measurement(0.226); randomized controlled trial(0.653) > clinical controlled trial(0.275); and thesis type(0.706) > article of journal type(0.339). 3) In the analysis of sub-continuous variables, as time of program(per session) increased, the balance increased(p<0.05). Conclusion : The results of this study showed that virtual reality based rehabilitation program moderately improves the balance of stroke patient. Further studies are recommended to investigate the effect of sub-variables related to virtual reality programs on motor functions of patient with stroke.

A case study of post-stroke insomnia patient using conservative Korean medical treatment (불면을 호소하는 뇌실질내출혈 환자 치험 1례)

  • A case of a 46-year-old Korean male with post-stroke insomnia that induced by intracranial hemorrhage was presented. He was treated with Transcutaneous electric acupoint stimulation (TEAS) at BL62 and KI6 once per a day for 17 days. We recorded the patient's sleep time and time of asleep, and used PSQI-K to assess severity of patient's insomnia symptom. After using TEAS, there was improvement in patient's insomnia severity and the time of sleep. From the result of this case, electrical stimulation at acupoint can may be effective in treating post-stroke insomnia.

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with poststroke urinary retention ■ Methods A stroke patient with urinary retention was treated with herbal medication(Paljungsan), acupuncture, electro-acupuncture and moxibustion. To evaluate the effect of TKM on post-stroke urinary retention, we observed voiding pattern and residual urine after voiding. ■ Results Maintenance of regular self voiding and decrease of residual urine after voiding were observed after the TKM treatment. ■ Conclusion This study showed the effect of TKM treatment on Urinary retention of stroke patient.

  • ■ Objectives The goal of this pilot study is to observe the change of gait pattern in a patient after peroneal nerve electrical stimulation(PNST). ■ Methods We analyzed the gait pattern of stroke patient using treadmill gait analysis system before and after PNST for seven weeks. The PNST was carried out for 20minutes every day except Sunday. In addition, the measurement was carried out every Saturday. At the fifth week, the PNST was not carried out to confirm whether the effect of PNST was disappeared immediately when PNST was not applied. ■ Results After PNST, while heel contact time and heel max force increased and forefoot and midfoot max force decreased, the gait parameters such as cadence, velocity, swing phase, stance phase, total double support, step length, stride length, step time, stride time and forefoot contact time, were not changed. ■ Conclusion Gait of a patient with cerebral stroke was changed positively after PNST.

The Effect of a Stepwise PNF Pattern Therapy in Weight-Bearing Positions on the Balance and Walking Functions of a Patient with Subacute Stroke -A Single Case Study- (아급성기 뇌졸중 환자의 균형 및 보행 기능에 대한 체중지지 자세에서의 단계적 PNF 패턴 치료의 효과 - 단일사례연구 -)

  • Purpose: The purpose of this study was to demonstrate the effects of a stepwise proprioceptive neuromuscular facilitation (PNF) pattern therapy in weightbearing positions on the balance and walking functions of a patient with subacute stroke. Methods: The patient was a 78-year-old man with right post-stroke hemiparesis who had decreased balance and gait function. During the baseline and withdrawal phases, no intervention was applied; however, in the intervention phase, the patient received a stepwise PNF pattern therapy in weight-bearing positions. Results: During the intervention phase, the LOS improved by 296.51% (from $2482.13mm^2$ to $626mm^2$), and walking speed improved by 18.70% (from 0.75 m/s to 0.64 m/s). The LOS and 10MWT values appeared to be clinically significantly improved after the intervention. In addition, the scores of the BBS and ABC scales improved by 100% (from 36 points to 18 points) and 56.52% (from 720 points to 460 points), respectively. Conclusion: These findings suggest that a stepwise PNF pattern therapy may be helpful in enhancing the balance and walking function of a patient with subacute stroke. Further studies are required to validate the results of this study.
