  • Pupose : This study aimed to examine the Influence of stregthening for Transverse abdominis and resistive breathing exercise to use PNF on fulmonary functions and static sitting posture in adult stroke patient. Methods : Left hemi-plegic patient was masured pulmonary function test and analyzed sitting posture of alignment at pre intervention and post-intervention in 4weeks. Results : The pulmonary functions was increased and changed good alignment of trunk on sitting position after treatments. Conclusion : We were found that stregthening for Transverse abdominis and resistive breathing exercise to use PNF could improve pulmonary functions and sitting posture for stroke patient.

  • Objective : The purpose of this study is to report treated case about patient with diplopia caused by stroke. Methods : The improvement of diplopia was observed as he was treated with acupuncture therapy and herb medicine named Bo-gan-san(保肝散). Results : Diplopia improved and disappeared gradually with acupuncture therapy and herb medicine named Bo-gan-san(保肝散). The patient was discharged with favorable recovery. Conclusion : In traditional Korean medicine, diplopia is caused by disorder of JungKi(精氣), intrusion of PoongSa(風邪) into Neoi(腦), and hollowness of Gan(肝), Shin(腎). Treating it is by expelling PoongSa(風邪) or strengthening Gan(肝), Shin(腎). The patient was diagnosed as cerebral infarction according to Brain MRI. Diplopia was improved after acupunctural therapy and intaking Bo-gan-san(保肝散), herbal prescription selected from DongYiBoGam(東醫寶鑑).

ICF Core Sets for Measuring Functional Status of Acute Stroke Patients (급성기 뇌졸중 환자의 기능상태 측정을 위한 ICF 핵심지표)

  • Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.

Essential Functions Required by Patients and Physical Therapists in the Rehabilitation Process of Stroke Patients: A Survey Study (뇌졸중 환자의 재활 과정에서 환자와 물리치료사가 요구하는 기능에 대한 조사연구)

  • Purpose: In this study, 100 stroke patients and 205 physical therapists were surveyed to determine the essential functions needed in the rehabilitation process of stroke patients. Methods: This study involved 100 stroke patients and 205 physical therapists. Sixteen functions suggested in the previous study as necessary in the rehabilitation process of stroke patients were selected, and a revised questionnaire was prepared and distributed to several institutions. A frequency analysis of the collected data was conducted to aggregate the functions required in rehabilitation, and a scoring process was used to determine their ranking among the 16 functions. Results: The functions required in the rehabilitation process, as selected by stroke patients, were ranked as follows: walking, toileting, eating, using products and technology for communication, and washing oneself. The functions required in the rehabilitation process, as selected by physical therapists, were ranked as follows: muscle power functions, maintaining body position, muscle tone functions, attention functions, and walking. Conclusion: The results of the study confirm the importance of an agreed goal between the stroke patient and the therapist regarding the functions required for the rehabilitation. This understanding plays a significant role in achieving the patient's expectations and the therapist's predicted performance, thereby providing reassurance and confidence in the impact of the research.

A Case Study of Taeeumin Patient Who Experienced Refusing Food Intake after Stroke (뇌경색 발병 후 오식증(惡食症)을 보인 태음인(太陰人) 환자 1례)

  • Objectives The purpose of this case thesis paper is to report the improvement of symptoms for a 58-year-old male patient who experienced refusing food intake 1 month after onset of stroke by treating the patient with Chungpyesagan-tang. Methods We have determined the patient to have the Taeeumin type physique with Dry Heat Syndrome and provided Chungpyesagan-tang to alleviate the patient's symptoms. The level of improvement was measured based on changes of food intake amount. Results The patient was hospitalized to receive treatment for roughly 2 weeks. The symptom of refusing food intake as well as other symptoms that simultaneously appeared including insomnia, headache, dizziness, and general weakness have improved. Conclusions In this case, the symptom of refusing food intake appeared one month after experiencing stroke, and the patient refused not only meals, but the intake of all other forms of food including water and medicine. The cause of such symptoms were deemed to be emotional rather than the types of food which generally provide the causes for Oh Shik Jeung. As the patient was deemed to have the Taeeumin type physique with Dry Heat Syndrome. Chungpyesagan-tang was provided for treatment. Acupuncture and oriental cupping therapy were also provided concurrently, and the main symptom and other accompanying conditions have largely improved after 2 weeks of hospitalized treatment.

Case Report of Treatment with Korean Medicine on a Central Post-Stroke Pain Patient who had Impingement Syndrome of the Right Shoulder (우측 어깨의 충격증후군을 동반한 뇌졸중 후 중추성 통증 환자 치험 1례)

  • Objectives: This case study examined the effects of treatment with traditional Korean medicine on a central post-stroke pain patient, who had impingement syndrome of the right shoulder. Methods: The patient received traditional Korean medical treatments, including acupuncture, warm-needling, moxibustion, and cupping, throughout the treatment period, and extracorporeal shock-wave therapy and physical therapy for part of the treatment period. The effects of treatments were measured with a manual muscle test (MMT), numerical rating scale (NRS), range of motion (ROM), and gait staging index (self-invented). Results: The clinical symptoms of the patient were improved considerably after the complete treatment. Conclusions: Traditional Korean medicine may be effective for treatment of central post-stroke pain in patients with impingement syndrome.

The Approach of Robot-assisted Gait Therapy for Locomotor Recovery of Chronic Stroke Patients: a Case Report

  • In this case report, we investigated the effects of robot-assisted gait therapy in a chronic stroke patient using motor assessment and gait analysis. A patient who suffered from the right hemiparesis following the left corona radiata and basal ganglia infarction received 30 minutes of robot-assisted gait therapy, 3 times a week for 4 weeks. Outcome was measured using Motoricity index(MI), Fugl-Meyer assessment(FMA), modified motor assessment scale(MMAS), isometric torque, body tissue composition, 10-meter gait speed and gait analysis. After robot-assisted gait therapy, the patient showed improvement in motor functions measured by MI, FMA, MMAS, isometric torque, skeletal muscle mass, 10-meter gait speed. In gait analysis, cadence, single support time, double support time, step length, walking speed improvement in after robot-assisted gait therapy. The results of this study showed that robot-assisted gait therapy is considered to facilitate locomotor recovery of the chronic hemiparetic stroke patient.

The effect of obstacle gait training on balance, gait and ADL of the stroke patient - pilot study (장애물 보행훈련이 뇌졸중 환자의 균형, 보행 및 일상생활활동에 미치는 영향 - 예비연구)

  • Background: The purpose of this study is intended to evaluate the effectiveness and analyse the change on balance, gait and ADL of the stroke patient by obstacle gait training. Design: Randomized controlled trial. Methods: Ten subjects with stroke patient are recruited from C rehablitation hospital located Chungbuck. The subjects which are divided to 2 group as experimental group (N=5) conducted the obstacle gait training and control group (N=5) conducted the general gait training. The gait (10mWT), balance (BBS) and ADL (FIM) were measured before and after intervention. Results: There were no significant differences (p>.05) in sex, age, height, weight and K-MMSE among subjects. There were significant differences (p<.05) in balance and gait between of the experimental group and control group. But there was no significant difference (p>.05) in the ADL between of the experimental group and control group. There were significant differences (p<.05) gait and balance in the experimental group. But there were no significant difference (p>.05) gait, balance and ADL in the control group. Conclusion: Obstacle gait training showed positive effects on the gait, balance of the stroke patient.

A Clinical Report about the Effects of Dansambohyul-tang on a Stroke Patient with a Gastric Ulcer (뇌경색 환자로 위장관출혈 소인이 있는 위궤양환자의 단삼보혈탕을 이용한 치험 1례)

  • After stroke, there are many cases of gastro-intestinal bleeding in patients with dysphagia who are fed by Levin tubes. Ulcers in the stomach and duodenum are occasionally produced by psychiatric stress, irritation with the Levin tube and central nerve rusturbances. Because Dansambohyul-tang has been used to treat deficiency of blood, alleviate pain, tranquilize and to treat patients differentiated with an insufficiency of the heart and the spleen in oriental medicine, we treated a 62 year-old male patient who suffered from gastric ulcers with gastro-intestinal bleeding after stroke, and who was differentiated with an insufficiency of the heart and the spleen with Dansambohyul-tang, herb complex. After 20 days of treatment with Dansambohyul-tang, we observed improvement in the gastric ulcer through gastroscopy; serum hemoglobin increased by 5.1g/dl after 2 months; and symptoms (pale complexion.dysphagia.general weekness) were alleviated. So Dansambohyul-tang shows therapeutic effects on gastric ulcers with gastro-intestinal bleeding after stroke.

The Analysis of Ultrasonography on Gastrocnemius Medialis of Patient with Stroke (뇌졸중환자 내측 비복근의 초음파 영상 분석)

  • Purpose: The purpose of this study were to analyze ultrasonography on both sides of gastrocnemius medialis of stroke patient. Methods: In fourteen subjects with stroke, the gastrocnemius medialis(GM) muscle were scanned with ultrasonography at the muscle belly at rest at ankle angles of 0 degree (neutral position) and it were measured with ultrasonography on both sides of the leg. Muscle architecture (thickness, pennation angle, fascicle length, density, white area index) was examined using real-time B-mode ultrasonography with 7.5MHz linear-array prob. Results: Thickness of non-paratic side was significantly thicker than paratic side. Pennation angle of non-paratic side was significantly larger than paratic side. Fascicle length of non-para tic side was significantly smaller than paratic side. Density of non-paratic side was significantly smaller than paratic side. White area index of non-paranc side was significantly smaller than paratic side. Conclusion: This study showed that stroke have influenced on mechanical properties and quality properties of skeletal muscle architecture.
