Mapping Items of Functioning Questionnaires into the International Classification of Functioning, Disability and Health: Stroke

  • Purpose: The aim of the study was to investigate items of commonly used questionnaires that measure functioning status of persons with stroke and map to the International Classification of Functioning, Disability and Health (ICF). Methods: Eighty-six patients with stroke were recruited from 12 medical institutes for the study. Each item of the Modified Bathel Index (MBI), Stroke Impact Scale (SIS), Mini Mental Status Evaluation (MMSE) and SF-36 were examined and compared its concept with the ICF. Concept linking was performed by 10 health professionals independently. A field test was performed to assess its correlation between those of scales and their linked ICF category sets. Results: It was found that 11 items in MBI was linked to 14 ICF categories, whereas 27 items of MMSE had 10 categories of ICF linked. 60 items of SIS were to be linked with 35 ICF categories. Agreement between professionals in linking was found to be high: 97.5% for MBI items, 78.0%, 78.0%, and 74.8% for MMSE, SIS, and SF-36 respectively. Strong relationship was observed between measurement scales and linked ICF code sets (r=-0.76 for SIS, r=-0.78 for MBI, r=-0.47 for MMSE) whereas there was no relationship was found between SF-36 and its ICF code set (r=-0.06) from the field test. Conclusion: It was found that items of SIS, MMSE and MBI may be linked to ICF categories. Those of linking concept between clinical tools and the ICF could be helpful for clinical data standardization.

A Study on the Oral Health and Oral Health Related Quality of Life of Stroke Patients in a Rehabilitation Ward: A Pilot Study (재활병동에 입원한 뇌졸중 환자의 구강건강 및 구강건강관련 삶의 질에 관한 연구: 예비연구)

  • The aim of this study was to examine the geriatric status, oral health, and oral health related quality of life of stroke patients hospitalized at a rehabilitatio ward and assess the relationships among these factors. A total of 78 stroke patients participated in this cross-sectional study. The following oral examinations were performed by a dentist and dental hygienist: Decayed, Missing, Filled Teeth, gingival, debris, calculus, and community periodontal index (CPI). The patients' sociodemographic and geriatric status were evaluated using the Barthel index and a mini-mental state examine (MMSE) according to their medical record and a questionnaire survey. Oral health related quality of life was assessed using the Oral Health Impact Profile (OHIP)-14 questionnaire. Except for three participants, most stroke patients had a gingival index of 2 or more, which means probing results in bleeding gums. Additionally, 53.8% of participants had a CPI code of 3 or 4, which indicates a probing depth of 4 mm or more. CPI index was significantly related to debris, calculus, and gingival index. The OHIP-14 score significantly related to the Barthel index and MMSE (p<0.05). The oral health of stroke patients in the rehabilitation ward was relatively poor and related to poor oral hygiene. Oral health related quality of life was related not to oral health but to geriatric status significantly.

Effects of Each Characteristics on NOx Emission Values for Marine 4-Stroke Diesel Engine (선박용 디젤기관(4-Stroke)의 특정인자가 질소산화물에 미치는 영향 고찰)

  • It becomes necessary for engine manufactures to verify whether lots of engines on test bed during shop test are in compliance with NOx Technical Code for marine diesel engines more efficiently on the basis of engine group test concept which contains parent engine and member engines since all the engines are not needed to take NOx measurement. In addition, it becomes more obliged to consider parameters which affect NOx emission level and describe these parameters in NOx technical file as engine information and settings to define engine operation range with tolerance to make sure the engines are still in compliance with NOx emission limit on board after shop test. During preparation of engine group test for 4-stroke marine diesel engines, we evaluated NOx emission value under different engine operating conditions and found that there are certain parameters, for example, Charge air temperature and Max. cylinder pressure which have influence on NOx emission level. The NOx emission shall be satisfied with NOx technical code by means of controlling such parameters.

  • Writing a CAI program containing Chinese characters requires a common Chinese character code to share information for educational purposes. A Chinese character code setting needs to allow a mixed use of both vowel and stroke order, to represent Chinese characters in simplified Chinese as well as in Japanese version, and to have a conversion process for data exchange among different sets of Chinese codes. Waste in code area is expected when vowel order is used because heteronyms are recognized as different. However, using stroke order facilitates in data recovery preventing duplicate code generation, though it does not comply with the phonetic rule. We claim that the first and second level Chinese code area needs to be expanded as much as academic and industrial circles have demanded. Also, we assert that Unicode can be a temporary measure for an educational code system due to its interoperability, expandability, and expressivity of character sets.

Effects of Daeseungki-tang on Constipated Stroke Patients (중풍환자의 편비에 대한 대승기탕의 임상적 연구)

  • Objectives : In oriental medicine, Daeseungki-tang is one of the most useful prescriptions for the therapy of constipation. And we also frequently use this prescription for constipated stroke patients. Methods : In order to identify the effects of Daeseungki-tang on constipated stroke patients, we administrated the extract of Daeseungki-tang(code number, HH264) to constipated stroke patients. We gave the medicine in two ways, one-pack group and two-pack group by random allocation and investigated the amount of stool, the frequency of defecation, the stool-form and the changes of blood pressure and body temperature before and after defecation. We also tried to examine the difference of symptoms between defecation-group and non-defecation group. Results : 1. There were no distinct symptoms between defecation-group and non-defecation-group in one-pack group. 2. After defecation, body temperature decreased significantly(P<0.05) in one-pack group and blood pressure decreased significantly(P<0.05) in two-pack group. 3. As for the stool-form, watery-soft was 41.9% in the firstt defecation and 47.1 % in the second defecation. 4. In one-pack group(23 cases), 13 cases(57%) defecated and 10 cases(43%) didn't defecate. And in two-pack group(24 cases), 19 cases(80%) defecated and 5 cases(20%) didn't defecate. These results meant that the effect of Daeseungki-tang was dose dependent. The amount of stool and the frequency of defecation were also dose dependent.

Storing information of stroke rehabilitation patients using blockchain technology: a software study

  • Background: Stroke patients usually experience damage to multiple functions and a long rehabilitation period. Hence, there is a large volume of patient clinical information. It thus takes a long time for clinicians to identify the patient's information and essential pieces of information may be overlooked. To solve this, we stored the essential clinical information of stroke patients in a blockchain and implemented the blockchain technology using the Java programming language. Methods: We created a mini blockchain to store the medical information of patients using the Java programming language. Results: After generating a unique pair of public/private keys for identity verification, a patient's identity is verified by applying the Elliptic Curve Digital Signature Algorithm based on the generated keys. When the identity verification is complete, new medical data are stored in the transaction list and the generated transaction is verified. When verification is completed normally, the block hash value is derived using the transaction value and the hash value of the previous block. The hash value of the previous block is then stored in the generated block to interconnect the blocks. Conclusion: We demonstrated that blockchain can be used to store and deliver the patient information of stroke patients. It may be difficult to directly implement the code that we developed in the medical field, but it can serve as a starting point for the creation of a blockchain system to be used in the field.

Depth Image based Chinese Learning Machine System Using Adjusted Chain Code (깊이 영상 기반 적응적 체인 코드를 이용한 한자 학습 시스템)

  • In this paper, we propose online Chinese character learning machine with a depth camera, where a system presents a Chinese character on a screen and a user is supposed to draw the presented Chinese character by his or her hand gesture. We develop the hand tracking method and suggest the adjusted chain code to represent constituent strokes of a Chinese character. For hand tracking, a fingertip is detected and verified. The adjusted chain code is designed to contain the information on order and relative length of each constituent stroke as well as the information on the directional variation of sample points. Such information is very efficient for a real-time match process and checking incorrectly drawn parts of a stroke.

Development of Environmentally Friendly Water-Soluble Lubricant for Cold Forging (냉간단조용 친환경 수용성 윤활제의 개발)

  • Zinc phosphate coating is required in nearly all steel cold forging operations. However, the chemical byproducts of this lubricant system are difficult to dispose of and have a negative environmental impact. In order to replace zinc phosphate based lubricants partially or completely, candidate lubricant has been developed in this study. The performance evaluation of these lubricants was conducted using the double cup extrusion test and spike forging test. With the use of the commercial FEM code DEFORM, friction factor calibration curves, i.e. cup height ratio vs. punch stroke and spike height vs. punch stroke, were established for different friction factor values. By matching the cup height ratio and the punch stroke and spike height vs. punch stroke from experiment to that obtained from FE simulations, the friction factor of the lubricants was determined. Three water-soluble lubricants; namely, Mec Homat, Royalcoat, and the developed lubricant were found to perform comparable to or better than zinc phosphate.

Real-World Impact of Modern Reperfusion Therapy for Acute Ischemic Stroke : A Nationwide Population-Based Data Study in Korea

  • Objective : The treatment paradigm for acute ischemic stroke has undergone several major changes in the past decade, contributing to improved patient prognosis in clinical practice. However, the extent to which these changes have affected patient prognosis in the real-world is yet to be clarified. This study aimed to evaluate the real-world impact of modern reperfusion therapy for acute ischemic stroke using data from the National Health Insurance Service in Korea. Methods : This study included patients aged 18-80 years who were admitted via the emergency room with an I63 code between 2011 and 2020. The rates of intravenous thrombolysis use and endovascular treatment according to the year of admission were investigated. Furthermore, the rates of decompressive craniectomy and 3-month mortality were also analyzed. The 10-year observational period was divided into three periods based on the 2015 guideline change as follows : prior, 2011-2014; transitional, 2015-2016; and modern, 2017-2020. Results : A total of 307117 patients (mean age, 65.7±10.9 years) were included, and most patients were male (59.7%). The rate of endovascular treatment gradually increased during the study period from 0.71% in the prior period to 1.32% in the transitional period and finally to 1.85% in the modern period. Meanwhile, the 3-month mortality rate gradually decreased from 4.78% in the prior period to 4.03% in the transitional period and to 3.71% in the modern period. Conclusion : In Korea, the mortality rate decreased as the rate of modern reperfusion therapy increased in patients with acute ischemic stroke. Overall, technical and scientific advances in reperfusion therapy have improved the outcome of patients with acute ischemic stroke in Korea.

Development of Simulation Program of Two-Stroke Marine Diesel Engines (선박용 2행정 디젤기관의 성능시뮬레이션 프로그램 개발)

  • The requirement of high efficiency and low emission for marine diesel engines are being enforced because of air pollution and climate change on the earth. In connection with these, many new technologies are considered. But they are mainly for new building ship. It is necessary to be concerned about the improvement of engine performance for existing ship. In this paper, the simulation program for performance of marine two-stroke diesel engine was developed to predict the deteriorating performance according to elapsed time for existing ship. The result was compared with the result of the program named TOP-CODE which was used by engine maker and checked to be shown good agreement between them.
