임신 후기 임부의 임신스트레스와 우울의 관계에서 피로의 매개효과 (The Mediating Effect of Fatigue on the Relationship between Pregnancy Stress and Depression during the Third Trimester of Pregnancy)

  • Purpose: We aimed to assess the mediating effect of fatigue on the relationship between pregnancy stress and depression during the third trimester of pregnancy. Methods: We surveyed 161 pregnant women at a hospital in Gyeonggi province in Korea from October to November, 2017. Data were analyzed by the SPSS WIN 22.0 program. Results: The mean scores of pregnancy stress, fatigue and depression were $2.09{\pm}.87$ with a range of 1~4, $2.31{\pm}0.84$ with a range of 1~4 and $0.85{\pm}0.81$ with a range of 0~3, respectively. Pregnancy stress was positively correlated with fatigue (r=.60, p<.001) and depression (r=.59, p<.001) and fatigue was positively correlated with depression (r=.48, p<.001). The mediating effect of fatigue on the relationship between pregnancy stress and depression (${\beta}=.21$, p=.010) was found. Conclusion: We found the mediating effect of fatigue on the relationship between pregnancy stress and depression. Thus, healthcare professionals should assess the pregnant woman's negative emotions in the early stage and facilitate their positive transition utilizing the assesment findings. For example, a development of interventions considering physical and psychological aspects for decreasing fatigue may improve the pregnant women's well-being during pregnancy and childbirth. In conclusion, the healthcare professionals should understand the potential problems during pregnancy and strive to enhance the positive process of pregnancy and growth of the baby.

Effects of Angelicae Gigantis Radix on Noise Stress-induced c-Fos Expression in Rats

  • Previous studies reported that exposure to noise during pregnancy adversely influenced the development of the fetus and neonate. In Oriental medicine, medications based on Angelicae gigantis radix have been known to be of efficacy in the treatment of various diseases. c-Fos, an immediate early gene whose expression is sometimes used as a marker for stimulus-induced changes in the metabolic activity of neurons. In the present study, the influence of postnatal Angelicae gigantis radix administration on c-Fos expression in the each region of hippocampus of offspring rats with prenatal noise stress during pregnancy was investigated. From the present results, exposure to the prenatal stress during pregnancy enhanced c-Fox expression, whereas exposure to postnatal Angelice gigantis radix suppressed c-Fos expression in the offsprings with prenatal noise stress during pregnancy. Based on the present study, Angelicae gigantis radix may provide new therapeutic opportunities as an agent to counteract the effects of prenatal noise stress- induced hippocampal dysfunction, and may be useful in the treatment of psychiatric problems in children of mothers who have experienced noise stress during pregnancy.

Prevalence and associated factors of prenatal depression in pregnant Korean women during the COVID-19 pandemic: a cross-sectional study

  • Purpose: This study investigated the effects of prenatal education characteristics, pandemic-related pregnancy stress, and health behaviors during pregnancy on prenatal depression in pregnant women during the coronavirus disease 2019 (COVID-19) pandemic. Methods: The participants were 180 pregnant Korean women, recruited from internet communities for pregnancy preparation, childbirth, and childcare, from July 5 to 15, 2022. The collected data were analyzed using the t-test, analysis of variance, the Mann-Whitney U-test, the Kruskal-Wallis test, and multiple regression analysis. Results: The scores for pandemic-related pregnancy stress (24.50±6.37) and health behaviors during pregnancy (67.07±9.20) were high. Nearly half of the participants (n=89, 49.4%) presented with prenatal depression, with scores of 10 or greater. Prenatal depression had a positive correlation with gestational age (r=.18, p=.019) and pandemic-related pregnancy stress (r=.27, p<.001), and a negative correlation with health behaviors during pregnancy (r=-.42, p<.001). The factors associated with prenatal depression were pandemic-related pregnancy stress (t=4.70, p<.001), marital satisfaction (dissatisfied) (t=3.66, p<.001), pregnancy healthcare practice behaviors (t=-3.31, p=.001), family type (weekend couple) (t=2.84, p=.005), and gestational age (t=2.32, p=.022). The explanatory power of these variables was 38.2%. Conclusion: Since participants had a high level of prenatal depression during the pandemic, and infectious diseases such as COVID-19 may recur, strategies should be developed to improve pregnant women's mental health with consideration of the unique variables that are relevant in a pandemic. It is also necessary to develop efficient online prenatal education programs that can be implemented even in special circumstances such as social distancing, and to evaluate their effectiveness.

대전지역 임신부의 식이 섭취 및 심리적 스트레스와 신생아 체중에 관한 연구 (Dietary Intakes and Psychological Stress Pregnant Women in Taejon in Relation to Neonatal Birth Weigh)

  • Thin study measured dietary intakes in late pregnancy and psychological stress during the period of gestation and examined the roles of diet and psychological stress in pregnancy weight gain and infant birth weight. Study subjects were 98 pregnant women who delivered infants at 2 general hospitals in Taejon city. Mean weight gain during pregnancy was 14.6$\pm$4.89Kg. Mean infant birth weight was 3.39$\pm$0.62kg in males and 3.28$\pm$0.43Kg in females. Mean energy and protein intake levels were adequate, but mean iron and calcium intakes were only 61.2$\pm$14.9% and 79.1$\pm$18.2$\%$ of RDA, respectively. Fat intake which constitutes 22.0$\pm$4.3$\%$ of total energy intake, and animal protein intake which constitutes 22.0$\pm$4.3$\%$ of total energy intake, and animal protein intake which constitutes 53.7$\%$ of total protein intake were moderately high. Though mean energy, fat, animal protein, and meat protein intakes in the low psychological stress group were higher than those in the middle or high stress group, psychological stress did not significantly affect pregnancy weight gain and infant birth weight. High intakes of nutrients except for dairy protein, iron, and niacin were associated with higher pregnancy weight gain and high intakes of protein and meat protein were associated with higher infant birth weight. It is concluded that dietary intakes during pregnancy has effects on pregnancy weight gain and infant birth weight, and psychological stress has no direct effect on them.

  • This study aimed to determine the association of maternal oxidative stress and adverse pregnancy outcome with serum vitamin C concentration and a myeloperoxidase (MPO) genetic polymorphism during pregnancy. We investigated 450 pregnant women who visited Ewha Womans University Hospital for prenatal care during gestational weeks 24~28. During the second trimester, we measured serum vitamin C levels and urinary 8-hydroxyde-2'-deoxyguanosine (8-OHdG) and malondialdehyde (MDA) as an oxidative stress biomarker. We determined the presence of a maternal MPO polymorphism (G-to-A substitution at nucleotide 463) using a PCR-RFLP assay. We compared the level of oxidative stress and birth weight with the vitamin C concentration and the presence of the MPO polymorphism. The mean level of maternal oxidative stress tended to be higher and the birth weight lower for MPO type A/A than for types A/G and G/G. Vitamin C levels above the 75 percentiles were associated with reduced concentrations of urinary MDA and 8-OHdG but increased birth weight. Our data demonstrate that oxidative stress and neonatal birth weight are associated with the MPO genetic polymorphism, with the association modified by the maternal vita-min C levels.

COVID-19 팬데믹이 임부의 우울에 미치는 영향 (Impact of the COVID-19 pandemic on depression during pregnancy: a cross-sectional study)

  • Purpose: Uncertainty and restrictions on daily life have increased fear, stress, and depression during the coronavirus disease 2019 (COVID-19) pandemic. Depression is the most common mental health problem in pregnant women. The purpose of this study was to evaluate the levels of fear and stress related to COVID-19 experienced by pregnant women, as well as their levels of depression, and to examine the factors associated with depression during pregnancy. Methods: This was a cross-sectional, correlational study conducted among 153 pregnant women who visited a maternity hospital in Busan, South Korea. A self-reported questionnaire was used for data collection from December 18, 2021 to March 8, 2022. Data were analyzed using descriptive statistics, the independent t-test, one-way analysis of variance, Pearson correlation coefficients, and multiple regression. Results: Pregnant women experienced a moderate level of fear related to COVID-19, with an average score of 21.55±4.90. The average score for depression during pregnancy was 14.86±11.10, with 50.3% of the participants experiencing depression (≥13). The factors associated with depression during pregnancy were fear of COVID-19, contact with a confirmed case of COVID-19, being in the third trimester of pregnancy, high stress levels due to difficulties experienced from social distancing measures, and unintended pregnancy. These five statistically significant factors explained 35.0% of variance in depression during pregnancy. Conclusion: Considering the prevalence of depression in pregnant women during the COVID-19 pandemic, it is necessary to develop interventions to reduce anxiety by providing correct information and alleviating the stress of social distancing.

Effects of Prenatal Cnidium officinale Makino Treatment on Spatial Memory and Neurogenesis in the Hippocampus of Rat Pups Born from Maternal Rats Exposed to Noise Stress during Pregnancy

  • During the prenatal period, the development of the individual is influenced by a host of environmental factors. Exposure to noise stress during pregnancy was determined to result in the retardation of growth, a reduction in neurogenesis, and an impairment of spatial learning ability in the rat pups. In the present study, we have attempted to characterize the effects of prenatal treatment with Cnidium officinale Makino on spatial memory and neurogenesis in the hippocampus of rat pups born from maternal rats exposed to noise stress during pregnancy. Prenatal treatment with Cnidium officinale Makino was shown to increase neurogenesis and enhanced spatial learning ability in rat pups born from maternal rats exposed to noise stress. In this study, we have determined that prenatal treatment with Cnidium officinale Makino can stimulate spatial development and neurogenesis in the brain of the fetuses exposed to prenatal stresses.

임부의 임신스트레스에 영향을 미치는 요인에 관한 단면연구 (Factors influencing pregnancy stress in pregnant women in Korea: a cross-sectional study)

  • Purpose: The purpose of this study was to investigate the association between maternal knowledge and social support on pregnancy stress among pregnant women in Korea. Methods: The participants in this study were 148 pregnant women in Korea, recruited from online communities on pregnancy and/or childbirth, from June 2019 to April 2020. The collected data were analyzed using the independent t-test, one-way analysis of variance, Pearson correlation coefficient, and multiple regression. Results: Participants were at average 18.25±8.28 weeks gestation, 56% were in the second trimester, 31% had one or more health issues in the current pregnancy (e.g., hyperemesis gravidarum), and 76% were first-time mothers. Participants had moderate levels of pregnancy stress (mean, 23.09±7.11 points out of 48) and maternal knowledge (mean, 14.42±4.67 points out of 21), whereas social support was somewhat high (mean 45.88±7.81 points out of 60). Pregnancy stress was weakly negatively correlated with social support (r=-.37, p<.001). Main source of pregnancy information (β=-.21, p=.011), marital satisfaction (β=-.18, p=.036), and social support (β=-.19, p=.038) were identified as significant factors affecting pregnancy stress, and these variables had an explanatory power of 22.7% for pregnancy stress. Conclusion: Based on these findings, nurses should assess pregnancy-related stress during pregnancy and consider main source of pregnancy information and marital satisfaction when providing education or counseling. Moreover, strategies to reduce pregnancy stress through social support are needed to improve the quality of life for pregnant women.

임신후기 여성의 피로, 수면장애 및 임신 스트레스 (Relationship between Fatigue, Sleep Disturbance, and Gestational Stress among Pregnant Women in the Late Stages)

  • Purpose: The objective of this study was to investigate the relationship between fatigue, sleep disturbance, and gestational stress in women during late stage of pregnancy. Methods: This study was conducted with 113 healthy pregnant women at gestational age greater than 27 weeks who were registered at community health centers and received prenatal care. A structured questionnaire regarding fatigue, sleep disturbance, and gestational stress was used. The data was analyzed using a t-test, an ANOVA, and Pearson correlation coefficients. Results: The subjects with unplanned pregnancies and irregular exercise patterns showed a higher level of fatigue than those with planned pregnancies and regular exercise patterns. Pregnant women with caffeine intake manifested higher levels of gestational stress and sleep disturbance than those without. The levels of sleep disturbance and gestational stress increased as the fatigue levels increased. The fatigue levels increased with increased levels of sleep disturbance. Conclusion: Planned pregnancy, regular exercise patterns, and caffeine intake were related with fatigue, sleep disturbance, and gestational stress in women during late stages of pregnancy. Fatigue, sleep disturbance, and gestational stress had close associations to each other. In the future, such results should guide development of nursing intervention programs for women in late stages of pregnancy.

임신기 부부의 태아 관련 스트레스에 영향을 미치는 변인 (The variables affecting married couples' fetus-related stress during pregnancy)

  • The purpose of this study was to evaluate the relationship between fetus-related pregnancy stress and temperament, parenting attitude, parent-fetal attachment, and marital quality. Respondents were 285 pregnant women and their spouses who participated in Pregnant Education Programs. Data was collected by means of questionnaire. The results are summarized as follows: The total fetus-related pregnancy stress level of pregnant couples demonstrated a higher relationship while parent-fetal attachment level demonstrated a very significant relationship. There were significant differences between spouses with regard to fetus-related pregnancy stress, parenting attitude, parent-fetal attachment, and marital quality.
