한국인의 Straight Wire Bracket의 형태에 관한 연구 (A MORPHOLOGIC STUDY ON STRAIGHT WIRE BRACKET FOR KOREAN)

  • The purpose of this study was to obtain the norm of the crown shape (tip, torque, in/out) and arch form, and to provide basic data for fabricating straight wire bracket and ideal arch wire for Korean. 100 subjects aged from 17 to 26 (50 females, 50 males) were selected with a normal occlusion. By measuring the size, angulation, inclination, arch width, facial prominance of the teeth and the molar offset, the following results were obtained. 1. Average, standard deviation, minimum, maximum of each measuring item for each teeth were obtained. 2. Intermolar width (${\underline{6}}$ to ${\underline{6}}$) of upper arch before occlusal surface cutting and intermolar width of upper arch (${\underline{6}}$ to ${\underline{6}}$, ${\underline{7}}$ to ${\underline{7}}$) after occlusal surface cutting showed statistical difference. There was no difference between sexes in any other measuring items. 3. Arch form and specification of straight wire bracket for Korean who have normal occlusion was obtained.

Continuous straight archwire에 의하여 발생하는 force system의 분석 (Analysis of force system developed by continuous straight archwire)

  • Edgewise bracket에 continuous straight archwire를 장착 했을 때 각 bracket에 발생 하는 force system은 statically indeterminate system이기 때문에 임상적으로 측정이 어렵다. 이에 본 연구는 linear beam theory를 도입하여 full arch에 straight wire가 장착된 경우 geometry(a/b), 재질, 단면의 모양, interbracket distance에 관계없이 발생 가능한 모든 상태를 설명할 수 있는 식을 유도하고 일례의 분석을 통하여 다음과 같은 결과를 얻었다. 1. Bilateral fixed beam의 양끝에서 발생하는 force system을 알 수 있는 계산식을 얻었다. 2. 이를 이용하여 continuous straight archwire가 장착된 경우 각 치아에 발생하는 force system을 알 수 있다. 3. Continuous straight archwire장착시 영구변형의 발생여부와 발생위치를 예측 할 수 있다.

  • Recently, straight-wire appliance is widely used with great concern in clinical orthodontic field. The purpose of this study was to collect the information of the straight-wire appliance and to determine the crown inclination in clinical orthodontics. The author analyzed the study model of 78 individuals with normal occlusion. The obtained results were as follows. 1. Mean, maximum value, minimum value and standard deviation of crown inclination of upper and lower teeth were obtained. 2. The lingual crown inclination of upper tooth had constant value from first premolar through second molar, the lingual crown inclination of lower tooth progressively increased from canine through second molar. 3. As Howes' ratio was decreased, the crown inclination of upper incisors was increased. 4. Narrowing the upper arch, the crown inclination of upper incisors was increased.

Oriental Bracket

  • To design the acceptable prejusted bracket for Korean using straight wire appliance, teeth and dental arch of Korean were studied and brackets were made. It was named Oriental Bracket. The results were following. 1. The bracket position for Korean occlusion was obtained. 2. The axial inclinations of each tooth were obtained. 3. The crown torques of each tooth were obtained. 4. The thickness of each tooth was obtained. 5. The bracket (Oriental bracket) was made according to these data.

  • Detailed finishing of the occlusion is a clinical skill that has become difficult with the development of fixed appliances. Accuracy of bracket placement definitely improves with indirect technique, Several methods for the placement of orthodontic brackets on dental casts are currently used in the indirect bonding technique. These include attachment by means of bonding resins, adhesive tapes or sticky wax. This article presents the indirect procedures of our clinic, which use paste-paste chemically cured composites. Detailed laboratory and clinical procedure for dual tray method and other application of indirect bonding will be presented.

The effect of occlusogingival placement of clinical bracket points on the adaptation of a straight wire to the lingual arch form

  • Objective: The aim of this study is to compare the adaptation of a straight wire between brackets positioned at the mid-lingual surface and those placed gingivally by using a three-dimensional simulation software. Methods: This cross-sectional study was performed using OrthoAid, an in-house software. The subjects were 36 adolescents with normal Class I occlusion. For each dental cast, two bracket positioning approaches, namely the middle and gingival, were examined. In the middle group, the reference points were placed on the mid-lingual surface of each tooth, while in the gingival group, the reference points were positioned lingually on the anterior teeth. A 4th degree polynomial was adopted, and the in-plane and off-plane root mean squares (RMSs) of the distances between the reference points and the fitted polynomial curve were calculated using the software. Statistical analysis was performed using the paired-samples t-test (${\alpha}=0.05$). Results: The mean in-plane RMS of the polynomial curve to the bracket distance in the gingival group was significantly lower than that in the middle group (p < 0.001). The off-plane RMS was higher in the gingivally positioned brackets in the maxilla than in the middle group (p < 0.001). However, the off-plane RMS in mandible was not statistically significantly different between the two groups (p = 0.274). Conclusions: The results demonstrated that the gingival placement of lingual brackets on the anterior teeth could decrease the distance between a tooth and the straight wire.

Multiloop edgewise arch wire의 부위별 하중변형률 (Regional load deflection rate of multiloop edgewise archwire)

  • 본 연구는 multiloop edgewise arch wire(MEAW)의 기계적 특성을 분석하기 위해, 1) 개별 브라켓간 부위에서의 MEAW의 하중변형률을 수종의 재질로 된 동일 규격의 교정용 호선과 비교하고, 2)개별 브라켓간 부위와 multi-L-loop 부위(측절치 브라켓의 원심연과 제2대구치 튜브의 근심연간의 거리)에서의 wire stiffness를 비교하며, 3)단일 L-loop와 multi-L-loop의 하중변형률에 대한 공학적 이론식을 유도하여 MEAW의 하중변형특성을 규명하고자 시행하였다. 5가지의 서로 다른 수평길이를 지닌 L-loop와 5개의 L-loop로 구성된 상하악의 multi-L-loop를 .$016\times.022$ inch의 stainless steel 강선으로 제작하였고, .$016\times.022$ inch의 Plain stainless steel, TML NiTi를 준비하였다. Instron model 4466 만능시험기에 50N 용량의 load cell을 부착하여 crosshead의 속도 1.0min/분, 브라켓간 부위의 시험시에는 최대변위량 1.0mm로 각 브라켓간격에서 측정하였고, multi-L-loop부위의 경우는 최대변위량 10mm, 42mm의 거리에서 측정하였다. 반복된 실험에 의해 발생할 수 있는 응력에 따른 물리적 성질 변화의 가능성을 배제하기 위해 각 조건마다 동일한 5개의 시편을 사용하였다. 측정된 하중변형률과 각 실험의 브라켓간격을 이용하여 각 브라켓부위에서의 L-loop의 wire stiffness number를 계산하였고 이를 multi-L-loop의 그것과 비교하였다. 5개의 loop로 구성된 multi-L-loop를 35개의 직선구간으로 나누어 각 구간의 에너지를 계산, 총합을 낸 후 가해진 외력으로 미분하여 하중변형률의 이론식을 유도하였으며, 이를 wire stiffness로 환산하여 단일 L-loop의 wire stiffness와 비교하였다. 그 결과는 다음과 같았다. 1) 각 브라켓 간격에서의 L-loop의 하중변형률은 평균적으로 stainless steel wire의 1/5.16, NiTi의 1/l.53, TMA의 1/2.47이었다. 2) multi-L-loop부위 에서의 MEAW의 wire stiffness는 개개 브라켓간 간격에서보다 평균 1.53배 더 높았고, 같은 부위에서의 NiTi보다 1.9배 더 높았다. 3) 유도된 하중변형률의 이론식에 따르면, 부위에 따라서 wire stiffness의 차이를 보이지 않는 직선 강선과는 달리, L-loop가 부여된 경우, 개별 L-loop의 ire stiffness는 전체 multi-L-loop의 wire stiffness보다 낮은 것으로 나타났다. 이상의 연구결과로 미루어 볼 때, MEAW는 개별적인 치아이동을 허용하면서, 가해진 교정력을 효과적으로 전체 치열로 전달할 수 있는 독특한 기계적 특성을 지니고 있는 것으로 생각된다.

정상교합자의 crown angulation에 관한 연구 (A STUDY OF THE CROWN ANGULATION IN NORMAL OCCLUSION)

  • The purpose of this study was to collect the information of the straight-wire appliance and to determine the amount of second-order bends in clinical orthodontics. The author analysed the study model of 50 individuals with normal occlusion and results were obtained as follows. 1. The crown angulation was 4 degree in upper central incisor, 7 degree in upper lateral incisor, and 0 degree in lower central incisor and lateral incisor. 2. The crown angulation was 8 degree in upper cuspid and 2 degree in lower cuspid. 3. The crown angulations were 4 degree in upper first bicuspid, upper second bicuspid and lower second bicuspid and 1 degree in lower first bicuspid. 4. The crown angulation was 3 degree in upper first molar, 0 degree in upper second molar, 5 degree in lower first molar and 8 degree in lower second molar. 5. The crown angulations in lower arch were progressively increased from first premolar to second molar. 6. In upper arch, as the crown angulation of one tooth was increased, those of adjacent teeth were increased, too. 7. In the case of lower arch, the crown angulation of cuspid was increased as that of lateral incisor was increased, the crown angulation of second premolar was increased as that of first premolar was increased, and similarity the crown angulation of second molar was increased as that of first molar was increased.

Accuracy of lingual fixed retainers fabricated using a CAD/CAM bending machine

  • Objective: Lingual fixed retainers, made from 0.0175-inch 3-strand twisted stainless steel wire (TW) and 0.016 × 0.022-inch straight rectangular wire (RW), are generally used in clinical practice. This study aimed to calculate their accuracy by comparing the discrepancy between computer-aided customized retainers made from these two types of wires. Methods: Eleven orthodontic patients were selected, resulting in 22 maxillary and mandibular three-dimensional printing dental models. Two types of lingual fixed retainers were bonded from canine to canine. To determine the accuracy, five points were chosen for each model, resulting in 110 selected points. The absolute values of the distances on the x-, y-, and z-axes were measured to compare the accuracy of the two types of computer-aided retainers. Results: The accuracy of the two types of retainers did not differ significantly in the x- and z-axes, but only in the y-axis (P < 0.01), where RW-fixed retainers exhibited a slightly but significantly increased distance compared to the TW. Conclusions: Both types of retainers showed high accuracy; however, RW had a slight but statistically significant difference along the y-axis compared with TW. This type of computer-aided design/computer-aided manufacturing bending machine is limited to two dimensions, and the dental arch is curved. Therefore, RW may require slight manual adjustment by the practitioner after manufacturing.
