Simultaneous Repair of Secondary Anterior Chest Wall Deformity and Secundum Atrial Septal Defect -1 Case Report- (심방중격결손을 동반한 이차성 전흉벽기형의 동시교정 -1예 보고-)

  • A 13-year-old boy presented with anterior chest wall depression and dyspnea on exertion(NYHA II). He underwent Ravitch operation for pectus excavatum 7 years ago. A preoperative echocardiographic study revealed secundum atrial septal defect. He had no other abnormality of laboratory test, except FVC and FEVI were decreased into 2.03 L(7 %) and 1.82 L(71 %). He underwent repair of secondary anterior chest wall deformity and secundum atrial septal defect. We used unique method, raising sternum at right angle to secure good operative field for open heart surgery. Acute respiratory insufficiency was developed on postoperative day 1. Mechanical ventilation was applied which could be weaned on postoperative day 6 and thereafter hospital course was uneventful without any other sequale. He was discharged on postoperative day 19.

The Effects of Seat Surface Inclination on Forward Reaching in Children with Spastic Cerebral Palsy

  • Purpose: Impaired reaching movement is commonly observed in children with cerebral palsy. The purpose of this study was to determine whether the inclination of seat surface can influence the reaching movement in children with spastic diplegic cerebral palsy (CP). Methods: The subjects were 31 children, 16 children with spastic bilateral CP and 15 typically developing (TD) children. The children performed static sitting and forward reaching under three conditions: a horizontal seat surface (Horizontal $0^{\circ}$), a seat surface inclined anterior 15 degrees (Ant $15^{\circ}$), and a seat surface inclined posterior 15 degrees (Post $15^{\circ}$). A 3-axis accelerometer ('ZSTAR3') was used for analysis of the reaching movement. A 3-axis accelerometer was attached on the manubrium of the sternum, lateral epicondyle of the humerus, and styloid process of the ulna. We measured the reaction time, movement time, and data amount during reaching the 8 cm target with an index finger on the three inclined seat surfaces. Results: Reaction time and movement time for CPs showed significant delay; comparing the TD's and CP's amount was significantly greater than the TD's during reaching task (p<0.05). In particular, CP's reaction time and movement time on a seat surface inclined Ant $15^{\circ}$ was significantly more delayed compared with the other seat surfaces (p<0.05). Conclusion: Our results suggest that seat-inclination intervention may provide an ergonomic approach for children with spastic cerebral palsy.

Titanium Plate Fixation for Sternal Dehiscence in Major Cardiac Surgery

  • Background: Sternal dehiscence is one of the most troublesome complications following cardiac surgery. Treatment failure and consequent lethal outcomes are very common. The aim of this study was to evaluate titanium plate fixation as a treatment for sternal dehiscence following major cardiac surgery. Materials and Methods: Between 2010 and 2012, 17 patients underwent sternal reconstruction using horizontal titanium plating for the treatment of post-cardiac-surgery sternal dehiscence. The plates were cut and shaped, and then were fixed to corresponding costal segments using 2-3 titanium screws per each side. Results: The median age of our patients was 66 years (range, 50 to 78 years) and 9 were female. Indications for sternal reconstruction included aseptic sternal dehiscence in 3 patients and osteomyelitis in 14 patients including 6 patients who were diagnosed with mediastinitis. During the operation, sternal resection and autologous flap interposition were combined in 11 patients. One patient died due to sepsis. Two patients required additional soft tissue wound revisions. Another patient presented with a tuberculous wound infection which was resolved using anti-tuberculosis medications. The postoperative course was uncomplicated in the other 13 patients. Conclusion: Titanium plate fixation that combines appropriate debridement and flap interposition is very effective for the treatment of sternal dehiscence following major cardiac surgery.

Bio-mechanic Hypothesis of Truncus Area Based on Pyobon and Geungyul Theory (표본, 근결 이론에 나타난 체간부위에 대한 생체역학적 연구 가설)

  • Objectives : The purpose of this study is to understand truncus area conception to be explained in the pyobon and geungyul (Root-Stem and Origin-End relationships; PG) of meridian theory as to do the macro organic spatial conception in bio-mechanic system for human body. Methods : The extremity areas and truncus area to be explained in PG theories were classified spatially, and the reason was discussed that human body was classified as the extremities and truncus areas for the people of old times. It was considered the structural characteristics, exercise species and movement characteristics of truncus area in human body also. Results : The truncus area were made of cranium, vertebrae, sternum, ribs, clavicle, scapula, sacrum and the hipbone. It was the hollow structure and classified as cranial cavity, pleural cavity(thoracic cavity) and pelvic cavity. These cavities had mutually organic relationships, and because of cavity structures in the truncus and full structures in extremities would act in different direction about gravity with each other for movements. So it would be estimated that the old peoples had to need to separate the truncus and the extremities. Conclusions : In this study, the truncus area and the extremity areas in the PG of meridian theory could be recognized as the spatial conception of bio-mechanic system in human body. Although this study was the theoretical study not to be proven, it will be used to understand for the meridian theory as basic reference for spatial conception and bio-mechanic system.

Effect of Kinesiology Taping on Hyolaryngeal Complex Movement in Stroke Patient with Dysphagia

  • Background: Kinesiology taping (KT) is a method that helps immediately increase muscle activation, strength and joint stability by being attached to various skeletal muscles and structures of the body. Objectives: To investigate the effect of KT applied below the hyolaryngeal complex on the movement of the hyolaryngeal complex during swallowing in patients with dysphagia after stroke. Design: One-group, pre-post design. Methods: Twenty individuals with dysphagia after stroke participated in this study. KT was applied to the sternum and both clavicles from the hyolaryngeal complex. We analyzed the motion of the hyolaryngeal complex during swallowing with and placebo KT and KT using the Image-J software with videofluoroscopic swallowing study. In addition, a 0-to-10 numerical rating self-report scale was used to check the required effort and resistance felt during swallowing. Results: KT condition showed that the anterior and superior movement of the hyoid bone during swallowing was significantly lower than placebo KT (P<.05, all). Also, KT condition showed that the anterior and superior movement of the larynx during swallowing was significantly lower than placebo KT (P<.05, all). In result of statistical comparison between KT group and placebo KT group, the KT group showed significantly higher self-report scale score than the placebo KT group in terms of two category; the required effort and resistance felt (P<.05, all). Conclusion: This study demonstrated that KT applied below the hyolaryngeal complex inhibits the anterior and superior movement of hyoid bone and larynx during swallowing of patients with dysphagia after stroke.

  • The appearance of a tumor in the chest wall is rare compared to that in any other part of the body. It can be classified into benign and malignant types and can be located in the rib, clavicle, sternum, cartilage and soft tissues. Tumors that are metastatic are commonly located in the lung, breast, bone and pleura. But, the soft tissue mass of anterior chest wall is rarely metastasized from a distant organ that is not confined to the thoracic cavity. This and thus has rarely been described. A 68-year-old man was admitted to our hospital with a chief complaint of resting dyspnea. A huge non-tender mass of about $10{\times}15$ cm in size was visible on his left lower anterior chest wall. We pathologically confirmed that the mass was a metastatic renal cell carcinoma of clear cell type by incision biopsy. Through an incision biopsy, the mass was pathologically confirmed as a metastatic renal cell carcinoma of the clear cell type.

Diagnostic values of abdominal muscles thickness and sterno-costal angle for young adults with rounded shoulders

  • Objective: The purposes of this study was to evaluate the diagnostic values of abdominal muscles thickness and sterno-costal angle as the quantitative diagnostic indicators for young adults with rounded shoulders. Design: A observational, cross-sectional study. Methods: This study included thirty-three male participants in order to examine the relationship among thoracic kyphosis, sternocostal angle, and abdominal muscle thickness. We used ultrasound imaging to measure the muscle thickness, two gravity-dependent inclinometers to measure the kyphosis angle, and Image J to measure the sterno-costal angle. Results: There was a significant positive correlation between the amount of thoracic kyphosis angle and muscle thickness of the external oblique (EO) on the right side (r=0.931), and on the left side (r=0.432), and the transverse abdominis (TrA) (r=0.649). There was also a significant negative correlation between the thoracic kyphosis angle and the sterno-costal angle at the right side (r=-0.942) and at the left side (r=-0.860). There was a significant positive relationship with muscle thickness of the EO and TrA on the right side with the thoracic kyphosis angle, and was significant negative relationship with the sterno-costal angle on both sides. Conclusions: The results of the study suggest that the thoracic kyphosis angle is related to muscle thickness of the EO and TrA on the dominant side and the sterno-costal angle. We also suggest that future studies are needed to determine how strengthening the abdominal muscles may contribute to preventing excessive thoracic kyphosis in young adults.

  • The pulmonary metastasis and bony metastasis finally resulted from the malignant tumors as one of the inevitable problems. Among them, the bony metastasis, which frequently involved the vertebrae, pelvis, ribs, sternum, and skull, have had the frequencies over 30%. Metastasis to the distal part of the knee is rare. However, acrometastasis which occured in bones of the foot is even rarer(0.4%) and a late manifetation of a disseminated disease from the literature review. Acrometastasis should be considered in elderly patients with a history of the previous malignancy, complaint of foot pain and mass lesion. We are reporting one rectal carcinoma with acrometastasis to the second metatarsal bone.

Sibjeondaebotang and Yugmijihwangtang's Toxicological Effects on Rat's Fetus (십전대보탕과 육미지황탕이 실험동물의 태자에 미치는 독성학적 연구)

  • Purpose : To evaluate safety of Sibjeondaebotang and Yugmijihwangtang in rats' fetus Methods : Female Sprague-Dawley rats were orally administered with the Sibjeondaebotang and Yugmijihwangtang at dose of 5mg/kg/day for 20 days. Pregnant rats were sacrificed at 20th day of gestation. Approximately live fetuses in the 20th day of gestation were randomly selected and fixed in 95% ethanol. To observe skeletal malformations, fetuses were stained with alcian blue and alizarin red S. Results : Neonatal body weight and number of fetus of Sibjeondaebotang, Yugmijihwangtang group were increased to those of control group. The fetuses treated with Sibjeondaebotang, Yugmijihwangtang didn't showed external malformation. Vertebral and sternal skeletal variations were observed in Sibjeondaebotang, Yugmijihwangtang administered group, but compared to the control, those skeletal variations were insignificant. There were no significant changes in number of ribs, cervical, thoracic, lumbar, sacral and caudal vertebrae Conclusion : From these results, it can be concluded that Sibjeondaebotang, Yugmijihwangtang shows no toxicity effects on fetus body weight and number of live fetuses. Although skeletal variations were shown in vertebrate and sternum, Sibjeondaebotang, Yugmijihwangtang did not show significant changes in bone malformation.

흉부의 ANALOG-DIGITAL 영상에 대한 비교 평가

  • This paper were studied the evaluation in compare with the conventional and AMBER of analog images, PACS and CR of digital images which were collected every ten sampling chest images with the J.J.Vucicuh chest evaluated chart, and were measured the chest phantom surface dose and the density of several part in chest images. The evaluated numbers were total 22 persons who were 6 persons of the M.D., 6 of the radiotechnological professors and 10 of the radiotechnologists. The obtained results summarized as following : 1. Approaching the optimum standard density of the several part in chest images drew near at the lung round region density in PACS images, the sternum region density in CR and AMBER images, the heart region density in CR AMBER images, the diaphram region density in AMBER and conventional images. 2. The evaluation measured surface dose were appeared orderly lesser dose at the AMBER images (spine 21 mR, lung 2mR, heart 12mR, apex 6mR) than the conventional images(32 mR), CR images(38mR) and PACS images(81mR). 3. The anatomical physical evaluation marks were taken the highest points at CR images(88.3), and orderly PACS images(82), AMBER images(79.2) and conventional images(65.2). 4. It is exposured with lesser surface dose at the analog images, but analog images leaves much room for image quality improvement, and digital images demand for lesser exposure surface dose, although excellent image quality.
