A COMPARISON OF THE DIAGNOSTIC ABILITY BETWEEN WATERS′ RADIOGRAPH AND COMPUTED TOMOGRAPHY IN THE DIAGNOSIS OF MIDFACIAL FRACTURE (안면중앙부 골절 진단시 전산화단층사진과 Waters 방사선사진의 진단능 비교)

  • This study was intended to compare the diagnostic ability between Waters' radiograph and CT in the diagnosis of midfacial fracture. The study group included 44 patients(male:32, female :12, age range :16-74 years old) with facial injury who underwent surgery. Waters' radiograph and both axial and coronal scanning were performed before surgery. Three oral radiologists and three non-oral radiologist interpreted 44 Waters' radiographs and 88 CT in three ways; 1) interpretation using Waters' radiograph only, 2) interpretation using CT only, 3) interpretation using Waters' radiograph and CT. The interpretation sites were confined to the walls of maxillary sinus; anterior, posterior, medial, lateral and superior wall. ROC curves were made with the findings during surgery as a gold standard except the posterior wall, where the joint evaluation of Waters' radiograph and CT by other three oral radiologists was used. ROC areas were compared according to the interpreting methods, the interpretation sites, and groups (R group ; oral radiologist group, N group ; non-oral radiologist group). The obtained results were as followed : 1. The diagnostic ability of CT only and Waters' radiograph and CT was higher than Waters' radiograph only in both groups(P<0.05). But there was no difference between CT only and Waters' radiograph and CT. 2. Generally, the diagnostic ability for the lateral antral wall was the highest and that for the posterior antral wall was the lowest in both groups(P<0.05). 3. In R group, for the anterior antral wall the diagnostic ability using CT only was increased but for the medial, lateral and superior antral walls the diagnostic ability was increased in only using Waters' radiograph and CT. 4. In N group, for the anterior and medial walls the diagnostic ability using CT only was increased. But for the posterior, lateral and superior antral walls there were no difference among three interpreting methods. 5. The diagnostic ability of R group was higher than N group in all interpreting methods.

  • Descending necrotizing mediastinitis is a life-threatening infection originating in the head or the neck and descends into the mediastinum. Even in the era of antibiotics, mortality rate has been reported to be 25 ∼ 40%. Prompt diagnosis and treatment is mandatory for delayed diagnosis and inappropriate drainage of the mediastinum are the main causes of high mortality Surgical management ranges from cervical drainage to routin thoracotomy:however, the optimal management still needs to be defined particularly in respect to effective mediastinal drainage. Although posterolateral thoracotomy incision has been considered as a standard approach, potential disadvantages including postoperative pain, risk of wound complication and delayed recovery remain to be concerned. Thoracoscopic approach is an attractive treatment modality as it can provide an excellent exposure with minimal incision and can complete drainage from the mediastinum and the neck in one-staged manner We describe here two cases of descending necrotizing mediastinitis successfully managed by thoracoscopic drainage.

  • Proposed equation is used many time in calculation of concrete compressive strength using the non-destructive testing at precision safety diagnosis. Most of proposed equation is suggested in abroad and have an error to estimate concrete compressive strength in the domestic. Therefor, proposed equation is low reliability to estimate concrete compressive and it has a significant effect in reliability of precision safety diagnosis. Nevertheless, It is possible to increase the reliability through a number of experiments from this problem that occurs in some localized part. This paper is proposed assessment formula of reliability related core compressive strength to increase the reliability. It is verified that reliability of proposed assessment formula is useful by probabilistic techniques. It is compared with each graphs of concrete compressive strength of proposed equation. It has been found that the present methods are very efficient.

Development of Optimum Nutrient Solution for Dill (Anethum graveolens L.) in Hydroponics (딜의 수경재배에 적합한 배양액 개발)

  • This experiment was conducted to determine the mineral absorption character and develop the optimum composition of nutrient solution for dill(Anethum graveolens L.) in hydroponics. Dill(Anethum graveolens L.) plants were grown in nutrient film technique(NFT) supplied with 1/4, 1/2, and 1 strength of the nutrient solution developed by National Research Station in Japan(HRS). Plants grown in 1/2 strength showed the best growth in plant height, fresh weight, and dry weight compared with those grown in 1/4 or 1 strength. In 1/2 strength solution, pH and EC changed little and proper nutrient contents were observed in the leaves as compared to plant nutrient diagnosis standard. Based on these results, optimum macronutrients were composed by nutrient- water absorption rate(n/w) with 1/2 strength: NO$_2$―N 8.85, NH$_4$―N 0.55, P 2.1, K 6.2, Ca 2.8, and Mg 1.7 me L$^{-1}$ To examine the suitability of the nutrient solution developed(SCU) , dill plants were grown in NFT supplied with two different kinds of solution and concentration. 1/2, 1, 3/2 and 25 of SCU and 1/2S of HRS. Changes of pH and EC were not distinct in 1S, but a significant change of pH was shown in low concentrations-HRS 1/2S and SCU 1/2S. Shoot fresh and dry weight were much higher in the plants grown in SCU IS as compared with HRS 1/2S. There were no significant differences in growth of plants grown in SCU IS, 3/2S, and 25. In addition, nutrient contents in the leaves grown by SCU 1S were in proper levels as compared with plant nutrient diagnosis standard. SCU 1S developed in this experiment was found to be optimum for dill in hydroponics.

  • In patients with thyroid nodules, ultrasonography (US) has been established as a primary diagnostic imaging method and is essential for treatment decision. The Korean Thyroid Imaging Reporting and Data System (K-TIRADS) is a pattern-based, US malignancy risk stratification system that can easily diagnose nodules during real-time ultrasound examinations. The 2021 K-TIRADS clarified the US criteria for nodule classification and revised the size thresholds for nodule biopsy, thereby reducing unnecessary biopsies for benign nodules while maintaining the appropriate sensitivity to detect malignant tumors in patients without feature of high risk thyroid cancer. Thyroid radiology practice has an important clinical role in the diagnosis and non-surgical treatment of patients with thyroid nodules, and should be performed according to standard practice guidelines for proper and effective clinical care.

Clinical Microscopy: Performance, Maintenance and Laser Safety (임상에서의 현미경: 작동, 유지보수 및 레이저 안전)

  • A microscope is the fundamental research and diagnostic apparatus for clinical investigation of signaling transduction, morphological changes and physiological tracking of cells and intact tissues from patients in the biomedical laboratory science. Proper use, care and maintenance of microscope with comprehensive understanding in mechanism are fully requested for reliable image data and accurate interpretation for diagnosis in the clinical laboratory. The standard operating procedure (SOP) for light microscopes includes performance procedure, brief information of all mechanical parts of microscopes with systematic troubleshooting mechanism depending on the laboratory capacity. Maintenance program encompasses cleaning objective, ocular lenses and inner optics; replacement and calibration of light source; XY sample stage management; point spread function (PSF) measurement for confocal laser scanning microscope (CLSM); quality control (QC) program in fluorescent microscopy; and systematic troubleshooting. Laser safety is one of the concern for medical technologists engaged in CLSM laboratory. Laser safety guideline based on the laser classification and risk level, and advisory lab wear for CLSM users are also expatiated in this overview. Since acquired image data presents a wide range of information at the moment of acquisition, well-maintained microscopes with proper microscopic maintenance program are impulsive for its interpretation and diagnosis in the clinical laboratory.

  • The mammary gland is made up of remarkably sensitive tissue, which has the capability of producing a large volume of secretion, milk, under normal or healthy conditions. When bacteria enter the gland and establish an infection (mastitis), inflammation is initiated accompanied by an influx of white cells from the blood stream, by altered secretory function, and changes in the volume and composition of secretion. Cell numbers in milk are closely associated with inflammation and udder health. These somatic cell counts (SCC) are accepted as the international standard measurement of milk quality in dairy and for mastitis diagnosis. NIR Spectra of unhomogenized composite milk samples from 14 cows (healthy and mastitic), 7days after parturition and during the next 30 days of lactation were measured. Different multivariate analysis techniques were used to diagnose the disease at very early stage and determine how the spectral properties of milk vary with its composition and animal health. PLS model for prediction of somatic cell count (SCC) based on NIR milk spectra was made. The best accuracy of determination for the 1100-2500nm range was found using smoothed absorbance data and 10 PLS factors. The standard error of prediction for independent validation set of samples was 0.382, correlation coefficient 0.854 and the variation coefficient 7.63%. It has been found that SCC determination by NIR milk spectra was indirect and based on the related changes in milk composition. From the spectral changes, we learned that when mastitis occurred, the most significant factors that simultaneously influenced milk spectra were alteration of milk proteins and changes in ionic concentration of milk. It was consistent with the results we obtained further when applied 2DCOS. Two-dimensional correlation analysis of NIR milk spectra was done to assess the changes in milk composition, which occur when somatic cell count (SCC) levels vary. The synchronous correlation map revealed that when SCC increases, protein levels increase while water and lactose levels decrease. Results from the analysis of the asynchronous plot indicated that changes in water and fat absorptions occur before other milk components. In addition, the technique was used to assess the changes in milk during a period when SCC levels do not vary appreciably. Results indicated that milk components are in equilibrium and no appreciable change in a given component was seen with respect to another. This was found in both healthy and mastitic animals. However, milk components were found to vary with SCC content regardless of the range considered. This important finding demonstrates that 2-D correlation analysis may be used to track even subtle changes in milk composition in individual cows. To find out the right threshold for SCC when used for mastitis diagnosis at cow level, classification of milk samples was performed using soft independent modeling of class analogy (SIMCA) and different spectral data pretreatment. Two levels of SCC - 200 000 cells/$m\ell$ and 300 000 cells/$m\ell$, respectively, were set up and compared as thresholds to discriminate between healthy and mastitic cows. The best detection accuracy was found with 200 000 cells/$m\ell$ as threshold for mastitis and smoothed absorbance data: - 98% of the milk samples in the calibration set and 87% of the samples in the independent test set were correctly classified. When the spectral information was studied it was found that the successful mastitis diagnosis was based on reviling the spectral changes related to the corresponding changes in milk composition. NIRS combined with different ways of spectral data ruining can provide faster and nondestructive alternative to current methods for mastitis diagnosis and a new inside into disease understanding at molecular level.

The Significance and Limitation of MR Volumetry: Comparison between Normal Adults and the Patients with Epilepsy and Hippocampal Sclerosis (MR 부피측정의 의의와 한계: 정상성인과 해마경화증 간질 환자의 비교)

  • Purpose : Hippocampal atrophy is one of the characteristic pathologic findings of hippocampal sclerosis, for which MR imaging of the hippocampus is essential in the evaluation of hippocampal sclerosis. The purpose of this study is to present the normal MR volumetric data of the hippocampus in normal adult Korean and to compare those with MR volumetric data of hippocampus in patients with hippocampal s-clerosis, providing the diagnostic volume criteria of the hippocampal atrophy. Materials and methods : MR volumetry was performed in 30 normal adults and 28 patients with temporal lobe epilepsy whose final diagnosis was hippocampal sclerosis. The volumetric data were compared between sexes, right and left sides, and normal and abnormal hippocampus, and the volume criteria for the diagnosis of hippocampal atrophy was determined. Results : The mean $volumes({\pm}standard$ deviation) of normal Korean adult were $2.20{\pm}0.73\textrm{cm}^3$ (right) and $2.17{\pm}0.72\textrm{cm}^3$ (left) in male and $2.27{\pm}0.47{\;}\textrm{cm}^3$ (right) and $2.23{\pm}0.48\textrm{cm}^3$ (left) in female. The mean right-left differences were $0.14{\pm}0.11\textrm{cm}^3$ and $0.19{\pm}0.13\textrm{cm}^3$ in male and female, respectively. The MR volumetry showed no significant statistical differences between sexes and between right and left. The mean volume and standard deviation of the hippocampus in hippocampal sclerosis patients was $1.46{\pm}0.60{\;}\textrm{cm}^3$, and the right-left difference was $0.51{\pm}0.41\textrm{cm}^3$, In comparison of two volume distributions between normal adult group and hippocampal sclerosis patients group, the reasonable diagnostic volume criteria was $0.4{\;}\textrm{cm}^3$ as right-left volume difference, in which the sensitivity and specificity are 0.61 and 0.90. In all patients with right-left volume difference more than $0.4{\;}\textrm{cm}^3$, visual determination of unilateral hippocampal atrophy was possible. Conclusion : The MR-based hippocampal volumetry is a useful add-on of visual MR diagnosis, only when visual diagnosis of hippocampal sclerosis is difficult.

  • In the medical field where artificial intelligence technology is introduced, research related to clinical decision support system(CDSS) in relation to diagnosis and prediction is actively being conducted. In particular, medical imaging-based disease diagnosis area applied AI technologies at various products. However, medical imaging data consists of inconsistent data, and it is a reality that it takes considerable time to prepare and use it for research. This paper describes a one-stop AI learning platform for converting to medical image standard R_CDM(Radiology Common Data Model) and supporting AI algorithm development research based on the dataset. To this, the focus is on linking with the existing CDM(common data model) and model the system, including the schema of the medical imaging standard model and report information for multi-center research based on DICOM(Digital Imaging and Communications in Medicine) tag information. And also, we show the execution results based on generated datasets through the AI learning platform. As a proposed platform, it is expected to be used for various image-based artificial intelligence researches.

Study of the Indicators of Each Pattern Identification Based on Korean Standard Differentiation of the Symptoms and Signs for Stroke (한국형 중풍 변증 표준안-Ⅱ의 변증별 변증지표의 분포 및 타당도에 관한 연구)

  • The purpose of this study is to investigate clinical frequency and priority of five pattern identification settled by Korean Standard Differentiation of the Symptoms and Sign for Stroke. The present study was done over 177 hospitalized patient with stroke in the Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Stroke patients had been interviewed by residents and specialists who studied standard operation procedures in Fundamental study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. In Heat-transformation pattern group, men showed significantly high frequency and in Deficiency of Gi pattern group, women showed significantly high frequency. In Heat-transformation pattern group, the indicators such as 'aversion to heat during fever', 'flushed face', 'full and rapid pulse', 'dryness of the eyes, 'yellow coating of the tongue', 'feverishness of the limbs', 'dryness of the eyes' showed significantly high frequency. In Dampphlegm syndrome group, the indicators such as 'white coating of the tongue', 'yellowish face', 'thick coating of the tongue', 'wheezing in the throat with sputum', 'swollen tongue', 'slippery pulse' showed significantly high frequency. In Deficiency of Gi pattern group, the indicators such as 'pale tongue', 'lassitude', 'pale face', 'weakness pulse' showed significantly high frequency. In Deficiency of Eum group, the indicators such as 'short and rapid pulse', 'mirror-like tongue' showed significantly high frequency. For more sensitive Korean Standard Differentiation of the Symptoms and Signs for Stroke, Large scale study is to be done, giving weight on the important indicators.
