Golf participation after rotator cuff repair: functional outcomes, rate of return and factors associated with return to play

  • Background: Golf is a popular sport involving overhead activity and engagement of the rotator cuff (RC). This study aimed to determine to what level golfers were able to return to golf following RC repair, the barriers to them returning to golf and factors associated with their failure to return to golf. Methods: Patients preoperatively identifying as golfers undergoing RC repair at the study centre from 2012 to 2020 were retrospectively followed up with to assess their golf-playing status, performance and frequency of play and functional and quality of life (QoL) outcomes. Results: Forty-seven golfers (40 men [85.1%] and 7 women [14.9%]) with a mean age of 56.8 years met the inclusion criteria, and 80.1% were followed up with at a mean of 27.1 months postoperatively. Twenty-nine patients (76.3%) had returned to golf with a mean handicap change of +1.0 (P=0.291). Golf frequency decreased from a mean of 1.8 rounds per week preinjury to 1.5 rounds per week postoperatively (P=0.052). The EuroQol 5-dimension 5-level (EQ-5D-5L) index and visual analog scale (EQ-VAS) score were significantly greater in those returning to golf (P=0.024 and P=0.002), although functional outcome measures were not significantly different. The primary barriers to return were ipsilateral shoulder dysfunction (78%) and loss of the habit of play (22%). Conclusions: Golfers were likely (76%) to return to golf following RC repair, including mostly to their premorbid performance level with little residual symptomatology. Return to golf was associated with a greater QoL. Persistent subjective shoulder dysfunction (78%) was the most common barrier to returning to golf.

Effects of a High-Intensity Interval Physical Exercise Program on Cognition, Physical Performance, and Electroencephalogram Patterns in Korean Elderly People: A Pilot Study

  • Background and Purpose: The effects of high-intensity interval training (HIIT) interventions on functional brain changes in older adults remain unclear. This preliminary study aimed to explore the effect of physical exercise intervention (PEI), including HIIT, on cognitive function, physical performance, and electroencephalogram patterns in Korean elderly people. Methods: We enrolled six non-dementia participants aged >65 years from a community health center. PEI was conducted at the community health center for 4 weeks, three times/week, and 50 min/day. PEI, including HIIT, involved aerobic exercise, resistance training (muscle strength), flexibility, and balance. Wilcoxon signed rank test was used for data analysis. Results: After the PEI, there was improvement in the 30-second sit-to-stand test result (16.2±7.0 times vs. 24.8±5.5 times, p=0.027), 2-minute stationary march result (98.3±27.2 times vs. 143.7±36.9 times, p=0.027), T-wall response time (104.2±55.8 seconds vs.71.0±19.4 seconds, p=0.028), memory score (89.6±21.6 vs. 111.0±19.1, p=0.028), executive function score (33.3±5.3 vs. 37.0±5.1, p=0.046), and total Literacy Independent Cognitive Assessment score (214.6±30.6 vs. 241.6±22.8, p=0.028). Electroencephalography demonstrated that the beta power in the frontal region was increased, while the theta power in the temporal region was decreased (all p<0.05). Conclusions: Our HIIT PEI program effectively improved cognitive function, physical fitness, and electroencephalographic markers in elderly individuals; thus, it could be beneficial for improving functional brain activity in this population.

  • objective to ascertain whether the concept of the therapeutic side is associated with changes in the blind sport mapping that represents the brain function. design Physiological blind spot maps were used as an integer of brain activity before and after acupuncture needling on the meridian point Hapkok(合谷) and Techung(太衝) in the unilateral side decided by double-blind controlled study(20 subjects). setting outpatient clinic participants: adult volunteers intervention twenty subjects were divided into two comparative groups and underwent specific acupuncture therapy on the unilateral side. Blinded examiners obtained reproducible pre and post-acupuncture cortical maps, which were subjected to statistical analysis. main outcome measures Brain activity was demonstrated by reproducible circumferential measurements of cortical hemispheric blind spot maps before and after acupuncture on the unilateral side. in case of acupuncture needling on the ipsilateral side of an enlarged side of bilnd spot, there were reduction of blind spot in 7 cases of 10 subjects, and enlargement in 3 cases. in case of acupuncture needling on the contralateral side of the enlarged side of blind spot, there were enlargement of blind spot in 6 cases of 10 subjects, and reduction in 4 cases. results the significant changes in the blind spots before and after acupuncture were observed Acupuncture needlings on the ipsilateral or contralateral side of an enlarged cortical map were associated with the concept of the therapeutic side traditionally accepted in the oriental medical society. Acupuncture needling on the ipsilateral side of an enlarged blind spot map is associated with the reduction of map, and increaed contralateral cortical activity. Acupuncture needling on the side opposite an enlarged blind spot map is associated with the enlargement of map, and decreased cortical activity. conclusion Reproducible maps of cortical responses can be used to measure the neurological consequences of acupuncture needling. Acupuncture can affect the somatic sensory informations that reach to the contralateral thalamus, and so affect thalamic integration. we found that acupuncture therapy may be associated with an increase or a decrease in brain function depending on the side of acupuncture needling. thus, the traditional concept of the contralateral therapeutic theory in acupuncture approach has the clinical significance in the view of brain function.

  • The purpose of this review was to investigate the relationship between aerobic exercise and cognitive function as well as synthesize the effect of aerobic exercise intervention studies centered on psychiatric symptoms associated with general cognitive deficit. Prospective studies on aerobic exercise and improvement of cognitive function were reviewed and synthesized. In addition, this literature review provides significant positive results on the relationship between aerobic exercise and general cognitive deficit associated with psychiatric symptoms. Review of this literature suggests that there is a positive relationship between participation in aerobic exercise and cognitive function. In this text, there are at least three general pathways by which aerobic exercise may facilitate executive function in children : 1) cognitive demands inherent in engaging in physical activity, 2) cognitive demands inherent in cooperation of complex motor tasks, and 3) physiological changes resulting from aerobic exercise. Another main finding of this review is that physical activity has a stronger influence on cognitive deficit, including attention-deficit hyperactivity disorder, depression, and anxiety. Development of cognitive function is the most important factor for children and youth. Therefore, future research should prove relationship between physical activity and cognitive function using a more scientific and quantitative approach design.

Systematic Review of Effects of Physical Activity on Health-related Physical Fitness Among Children and Youth with Disabilities (장애 아동청소년의 건강관련 체력에 대한 신체활동의 효과성 고찰: 체계적 문헌조사)

  • The purpose of the study was to identify available evidence related to physical activity interventions that were effective to improve health-related physical fitness levels of children and youth with disabilities. Studies were accumulated from multiple databases (i.e., ERIC, Child Development & Adolescent Studies, MEDLINE, SPORTDiscus with Full Text, and CINAHL Plus with Full Text). From the initial process, 1195 articles were identified and of those, 13 studies (n = 13) were analyzed. Based on the results, community-based physical activities were the most common environment of interventions followed by school-based and home-based. Further, the most improved health-related physical fitness outcomes were muscular strength, muscular endurance, and cardiorespiratory endurance while limited evidence on weight reduction was available. There is a need for more research on physical activity intervention which particularly improves health related physical fitness of children and youth with disabilities.

Arabinoxylan Rice Bran and Endurance Exercise Training on the TLR4 Signaling-mediated Protein Expression in LPS-treated Rats (유색미 겨 아라비녹실레인과 지구성 운동트레이닝이 LPS 처치된 흰쥐의 TLR4 Signaling 단백질 발현에 미치는 영향)

  • The purpose of this study is to investigate the effects of arabinoxylan rice bran and endurance exercise training on TLR4 mediated protein expression in LPS-treated rats. The results showed that TLR4 as an important protein in the inflammatory response against lipopolysaccharide was shown to be significantly lower in both arabinoxylan supplement with exercise group and exercise group, thus the arabinoxylan rice bran had a higher inhibitory activity than arabinoxylan supplement group. However, $NF-{\kappa}B$ and MyD88 protein expression was not changed in arabinoxylan supplement with exercise training group, whereas $NF-{\kappa}B$ significantly decreased in 4 weeks of exercise training group. These results suggest that the supplement of arabinoxylan rice bran with exercise is likely to contribute to inflammation response and the arabinoxylan rice bran can be used as a possible safe alternative to the immunotherapeutic intervention.

  • Root tear of the posterior horn of the medial meniscus can occur from trauma or chronic degeneration, leading to meniscus extrusion, articular cartilage loss, osteophyte formation, and medial joint space narrowing. It is common on middle age with or without minor trauma. We experienced a case of medial meniscus posterior horn root tear in 13 years old boy during baseball game. We performed 1 direct suture anchor repair for medial meniscus posterior horn root tear in adolescent and report clinical result.

  • The base of leisure air activities are consistently expanding and its demand is expected to increase attributed to GDP growth and people's interest in its activity. Utilizing Visual Flight Rule, light sport aircrafts and ultra-light aircrafts are not under the effect of air traffic control center, which resulted in passenger injury due to emergency landing for adverse weather conditions and technical issues after pulling into mountain area, ocean and even urban area. Such events encouraged safety consciousness toward leisure aircraft activities and developing a measure to prevent a recurrence of the accident. This research focuses on suggesting compositive system for preventive safety management system by providing user based Flight information service and operating effective system, necessary for leisure aircraft activities.

  • Tendoachilles rupture has recently seen an increase in frequency, accounting for up to 40% of all tendon ruptures. However, an acute segmental rupture of the Achilles tendon is very rare with only one case caused by predisposing factors, such as steroid injection. In this report, we highlight an unusual clinical presentation of a segmental rupture of the Achilles tendon without any underling predisposing factor and without direct trauma. Herein, we discuss its mechanism. Twelve months after surgical repair with open technique, this patient became fully functional again in daily activity.

Spatial Distribution Pattern of Users by the Types of Use Activity at Lawn Areas in Park (공원잔디공간의 이용활동유형별 공간분포특성에 관한 연구)

  • This study was carried out to investigate the characteristics of spatial distributon patterns of users by each type of recreational activities at lawn areas in the parks. Study sites were lawn areas at Kyungju World and Seorabul Plaza in Bomun Resort Complex in Kyungju, Korea. Space types by each recreation activities are Repose Type, Sports Type, Play Type and Strolling Type. Main results are as follows ; 1. Sport Type activities were mainly distributed at the central areas of each lawn areas. 2.The density of Sports Type activities were affected by the form of lawn areas. For natural control of excess dynamic activities, Complex Form has more advantage in the distribution of dynamic activities. So, as the form of lawn areas in the parks, complex form is more desirable. 3.Repose Typo activities were distributed at marginal areas of lawns and surrounding woods. These, the static activities, were affected by Edge Effect in spatial distribution of activities. 4.Play Typo activities also were distributed at marginal areas of lawns. It was also affected by Edge Effect in spatial distribution. 5.Strolling Type activities were distributed evenly over all lawn areas.
