Total Body Modification(TBM) 기법에 관한 고찰 (The Study of Total Body Modification (TBM) Technique System)

  • Objectives : To introduce Total Body Modification(TBM) technique system developed by Dr. Victor Frank DC, DO, ND through combining chiropractic manipulation with osteopathy, acupuncture meridian system, and naturopathy based on his long-time clinical experiences and insights. Methods : After investigating the art, philosophy and science of TBM thechnique, and applying TBM practice under private clinical situation, we compared It with Chuna Korean manual medicine and oriental medicine system. Results : This system deals with correcting human body's functional physiology to potentiate in a favorable manner. TBM uses a neuromuscular reflex test and body access meridian points to tap into the body's biocomputer and read functional programs. Corrections are usually made by means of special respiratory spinal adjustment, cranial, soft tissue, or specific joint manipulation. Conclusions : We found similarities between TBM and Chuna system in the view of Korean Traditional meridian concept. The special combinations and sequences of various alarm points and associated points have clinical effects on the treatment of chronic spinal subluxation pattern. This suggests TBM has significance In the point of the future evolution of Chuna manual medicine in Korea.

Loss of hsp70.1 Decreases Functional Motor Recovery after Spinal Cord Injury in Mice

  • Heat shock proteins (HSPs) are specifically induced by various forms of stress. Hsp70.1, a member of the hsp70 family is known to play an important role in cytoprotection from stressful insults. However, the functional role of Hsp70 in motor function after spinal cord injury (SCI) is still unclear. To study the role of hsp70.1 in motor recovery following SCI, we assessed locomotor function in hsp70.1 knockout (KO) mice and their wild-type (WT) mice via the Basso, Beattie and Bresnahan (BBB) locomotor rating scale, before and after spinal hemisection at T13 level. We also examined lesion size in the spinal cord using Luxol fast blue/cresyl violet staining. One day after injury, KO and WT mice showed no significant difference in the motor function due to complete paralysis following spinal hemisection. However, when it compared to WT mice, KO mice had significantly delayed and decreased functional outcomes from 4 days up to 21 days after SCI. KO mice also showed significantly greater lesion size in the spinal cord than WT mice showed at 21 days after spinal hemisection. These results suggest that Hsp70 has a protective effect against traumatic SCI and the manipulation of the hsp70.1 gene may help improve the recovery of motor function, thereby enhancing neuroprotection after SCI.

  • 본 증례의 환자는 자가 경부 마사지로 인한 척수부신경병증으로 경추부 근력 불균형 및 그에 따른 피로를 호소하였다. 자가 마사지로 인한 일시적 신경 압박은 7일에 걸쳐 자연 회복되었으며, 턱관절균형요법을 통해 자세와 척추 구조의 균형점을 회복시켜 환자의 피로와 자세 불균형을 개선할 수 있었다.

경추부 질환에 적용하는 추나 치료에 대한 국내 연구 동향 (The Domestic Trend of Chuna Manual Therapy on Cervical Spinal Disorders)

  • Objectives: The present study examines the domestic trend of Chuna manual therapy on cervical spinal disorders in Korea. Methods: We investigated the studies on Chuna manual therapy for cervical spinal disorders via searching 6 Korean web databases. As a result, 51 research papers were found to be analyzed according to their published year, published institution, the titles of journals, the types of study, the techniques of Chuna, the instruments for assessment, the Chuna technique and the number of the treatment trials by the cases of cervical spinal disorders and ethical approvals. Results: The number of the research papers published tended to increase every year. The studies on Chuna manual therapy for cervical spinal disorders were mainly published in The Journal of Korea CHUNA Manual Medicine for Spine & Nerves. The most frequently adopted technique of Chuna in the examined studies was Chuna spine manipulation therapy - cervical spine technique. Visual analogue scale (VAS), neck disability index(NDI) were used as primary means of assessments. The ethical problems of the examined studies needed to be improved. Conclusions: Reviewing the domestic trend of studies on Chuna manual therapy for cervical spinal disorders and examining the strong and weak points of those treatments are essential for the future studies. It is anticipated that this review benefits the future in-depth study on the treatments for Chuna in Korean medicine.

추나치료 및 상지 운동치료를 통해 호전된 경추 협착증 환자 치험례 (A Case of Cervical Spinal Stenosis Improved by Combined with Chuna Manual Therapy and Exercise Treatment)

  • This study reports the effects of Korean medicine treatments including Chuna manual therapy (CMT) and exercise treatment for a patient suffering with cervical spinal stenosis. We treated patient diagnosed with cervical spinal stenosis. The patient was treated with CMT once a day for 26 days. Manual muscle testing (MMT), ratio and degree of numbness, numerical rating scale (NRS) were used as tools for evaluating the patient's progress. MMT was increased from 3+ to 5, ratio and degree of numbness were decreased from grade (Gr.) 9 to Gr. 4, NRS was decreased from 5 to 3. This study suggests that CMT and self-exercise can be considered as effective treatment for cervical spinal stenosis.

요추추간판탈출증에서의 굴곡신연기법에 대한 제언(提言) (Suggestion of Flexion Distraction Technique in Lumbar Herniated Intervertebral Disc(HIVD))

  • Objectives : This study aimed to introduce a new traction therapy and proposes that a new Chuna manual therapy technique could be implemented. Methods : The patient is placed in the prone position and their ankle is fixed to the fixing table. Next, clinicians contact on the vertebral spinal process above herniated disc region with the tissue pulled head part of scaphoid bone and the lower part of the pelvis is pulled horizontally to the foot. Conclusions : It was recently identified that traction while maintaining lumbar lordosis is more effective for lumbar disc herniation and is associated with fewer side effects. Chuna manual therapy also uses a technique of lumbar flexion-distraction manipulation to treat HIVD. further research is required.

Retroperitoneal Yolk Sac Tumor in Adult Woman Presenting as Spinal Cord Compression and Fatal Pulmonary Tumor Embolism

  • A 35-year-old woman, previously treated for systemic metastases from retroperitoneal yolk sac tumor, presented with progressive painful paraparesis. Preoperative images showed severe cord compression by the metastatic infiltration of the lumbar vertebrae and epidural mass as well as a huge retroperitoneal mass. While performing unremarkable surgery in prone position, the patient abruptly fell into hypoxic insults and circulatory arrest. Intraoperative pulmonary tumor embolism was deemed a cause of death. When planning operative procedure for this dangerous malignancy, scrupulous manipulation is mandated and the possibility of fatal pulmonary tumor embolism should also be addressed and fully discussed preoperatively.

Surgical Outcome of Spinal Cord Hemangioblastomas

  • Objective : Spinal cord hemangioblastomas are rare tumors. Despite their benign, slow-growing nature, they can cause severe neurological consequences. The purpose of this study was to evaluate variable factors, including clinical features, tumor findings, the extent of resection, and its recurrence or progression, which determine postoperative functional outcomes. Methods : This study included sixteen patients at our institute who underwent microsurgical resection for sporadic spinal intramedullary hemangioblastomas and spinal intramedullary hemangioblastomas associated with von Hippel-Lindau (VHL) disease, between June 2003 and March 2012. Results : A total of 30 operations were performed. Total resection (TR) of the tumor was achieved in 10 patients, and subtotal resection (STR) was achieved in 6. Postoperatively, the initial presenting symptoms were improved in 18.7% of the patients and were unchanged in 56.3%, but 25% were worse. Stable postoperative neurological functions were found in 83% of patients with preoperative McCormick grade I, and TR was achieved in 75% of these patients. In the STR group, poorer neurological status was observed in one patient, despite multiple operations. There were no poorer outcomes in the four cases of VHL disease. Various factors were analyzed, but only a correlation between the pre- and postoperative neurological status was verified in the TR-group patients. Conclusion : Preoperative focal neurological impairment and meticulous microsurgical manipulation may be predictors of favorable outcomes for solitary hemangioblastomas. In addition, the preservation of function is more important than the extent of resection in VHL disease.

척추도수교정이 요통에 미치는 영향 (The Effect of spinal manipulation on the Lumbar Spine with LBP patients)

  • Purpose: Many people are suffering from Low back pain due to HIVD and muscular problems, lack of joint functions on lumbar spine. In this study we compared the change of the herniation index, Oswestry LBP disability index (OLDI), visual analog scale (VAS), lumbar flexion range of motion (ROM) between the pre-experiment and after 4 weeks treatment by maitland manual therapy. Method: we selected and managed both the 15 people. They are $46.80{\pm}15.46$ years old people with HIVD and Stenosis. We treated for the people with HIVD and Stenosis by manual therapy(maitland manipulation method) during 4 weeks. And then we compared with pre-experiment and after 12 weeks through measuring the herniation index change by using computor themograpy (CT), LBP OLDI, VAS, lumbar flexion ROM. Results: The changes in the herniation index, Oswestry lumbar Disability Index, VAS, lumbar flexion ROM between the pre-experiment and after 4 weeks treatment by maitland manual therapy, there was a statistically significant difference. Although there was a significant difference after 4 weeks in OLDI, VAS, lumbar flexion ROM. But disc herniation index was no significant difference. Conclusion: Manual therapy is very effective for Lumbago due to the HIVD and spinap stenosis patients. OLDI, VAS and lumbar flexion ROM were increased. But disc herniation index was no significant difference. We suggest the combination treatment between manual exercise and spinal traction.

Effect of Combined Traditional Acupuncture, Pharmacopuncture and Applied Kinesiology on Lumbar Diseases of Resident Patients

  • Objectives: Recently several Korean medical doctors have begun practicing applied kinesiology (AK). Although the efficacy of combining traditional acupuncture (TA) and pharmacopuncture (PP) on lumbar diseases such as lumbar spinal stenosis (LSS) and lumbar herniation of intervertebral disk (LHID) has been examined, the possible benefits of combining TA, PP and AK approaches have not been examined. Therefore the aim of this study was to develop effective treatment for lumbar disorders by combining TA, PP, and AK treatments. Methods: Twenty-four patients hospitalized at Samse Korean Traditional Medicine Hospital between March and September 2018 with L5 or S1 root radiculopathy associated with LSS and LHID were included in this study. They were treated for 10 days with TA, PP and AK approaches that included category block, manipulation and strain/counterstrain treatments. The primary outcomes were mainly assessed using Japanese Orthopedic Association lumbar scores (JOALS). JOALS were determined before the start of treatment as well as five and 10 days after treatment started. Results: The treatments improved the lumbar condition of the patients based on JOA pain score. Conclusion: The combined TA, PP and AK treatments were effective in treating spinal diseases of resident patients. Prospective, controlled, and relevant protocols using multimodal strategies to define the role of TA, PP and AK are needed.
